Randomized, double-blinded, placebo-controlled trial evaluating simethicone pretreatment with bowel preparation during colonoscopy

BACKGROUND The presence of small air bubbles and foam are an impediment to a successful colonoscopy. They impair an endoscopist’s view and diminish the diagnostic accuracy of the study. This has been particularly noted to be of concern with the switch to lower volume polyethylene glycol (PEG) and bisacodyl combination preparation. AIM To evaluate the effect of oral simethicone addition to bowel preparation on intraluminal bubbles reduction during colonoscopy. METHODS Described is a prospective, randomized, multi-center, double-blinded, placebocontrolled study to evaluate the use of premixed simethicone formulation with split-regimen, low-volume PEG-bisacodyl combination bowel preparation for 168 outpatients undergoing screening, surveillance, and diagnostic colonoscopies. Primary outcome includes evaluation of bubbles during colonoscopy graded using the Intraluminal Bubbles Scale. Secondary outcomes include evaluation of the Boston Bowel Preparation Scale (BBPS), total number of polyps, polyp size differentiation, polyp laterality, adenoma detection, mass detection, cecal insertion time, withdrawal time, and patient-reported adverse events. RESULTS Higher Intraluminal Bubbles grades III and IV (less than 75% of the mucosa cleared of bubbles/foam requiring intervention with simethicone infused wash) were detected in the placebo group [Simethicone n = 4/84 vs Placebo n = 20/84 (P = 0.007)]. BBPS total score was 7.42 [standard deviation (SD)=± 1.51] in the simethicone group and 7.28 (SD =± 1.44) in the placebo group (P = 0.542) from a total of 9. Significantly higher number of adenomas were detected in the simethicone group (P = 0.001). CONCLUSION The addition of simethicone to bowel preparation is well advised for its antifoaming properties. The results of this study suggest that addition of oral simethicone can improve bowel wall visibility.

It is increasingly apparent that the inclusion of mass transfer aspects,together with certain thermal conditions,in the momentum and energy equations governing MHD flows leads to a numbers of real life *** this in view,we have attempted an exact analysis of heat and mass transfer aspects in transient hydromagnetic free convective flow of an incompressible viscous fluid through a vertical pipe under an externally applied magnetic field,assuming presence of chemical reaction and heat source/*** governing PDEs,which simplify to a set of 3 linear ODEs in the physical set up considered here,have been solved using Laplace transform technique,with solutions for key physical variables presented in the term of Bessel and modified Bessel *** influence of governing non-dimensional parameters,namely,Hartmann number,Schmidt number,source/sink parameter,Prandtl number and chemical reaction parameter,has been illustrated on the developing velocity and some concentration *** important quantities of engineering interest-surface skin friction and volumetric flow rates-have been computed too and *** notable finding worth mentioning are:(a)heat source presence causes higher fluid velocity as compared to the heat sink;(b)all important surface shear stress can be suitably controlled,among others,by chemical reaction parameter and Schmidt *** key challenge of this study has been to obtain exact closed-form solutions of the field equations,including cumbersome Laplace *** study finds innovative applications in the emerging fields such as magnetic materials processing,chemical processes,solar energy systems,etc.

Oxygen evolution reaction(OER)is the key step involved both in water splitting devices and rechargeable metal-air batteries,and hence,there is an urgent need for a stable and low-cost material for efficient *** the present investigation,Co-Fe-Ga-NiZn(CFGNZ)high entropy alloy(HEA)has been utilized as a low-cost electrocatalyst for ***,after cyclic voltammetry activation,CFGNZ-nanoparticles(NPs)are covered with oxidized surface and form high entropy(oxy)hydroxides(HEOs),exhibiting a low overpotential of 370 mV to achieve a current density of 10 mA/cm^(2)with a small Tafel slope of 71 mV/*** alloy has higher electrochemical stability in comparison to state-of-the art RuO2 electrocatalyst as no degradation has been observed up to 10 h of *** electron microscopy(TEM)studies after 10 h of long-term chronoamperometry test showed no change in the crystal structure,which confirmed the high stability of *** density functional theory(DFT)based calculations show that the closeness of d(p)-band centers to the Fermi level(EF)plays a major role in determining active *** work highlights the tremendous potential of CFGNZ HEA for OER,which is the primary reaction involved in water splitting.

Axillary artery thrombosis with anteroinferior shoulder dislocation： a rare case report and review of literature

A very rare and serious complication of shoulder dislocation is a lesion to the axillary artery in the elderly population, whose vascular structures have become less flexible. Axillary artery injury secondary to anteroinferior shoulder dislocation is much rarer, especially in the young people. Proper recognition and treatment of this entity of- fers a full recovery to the patient. Present report highlights the possibility of axillary artery injury with anteroinferior shoulder dislocation. A few case reports and small case se- ries of this injury have been reviewed. And recommenda- tions for management have been brought up to date, in line with current thinking.

Plant Nutrient Status during Boll Development and Seed Cotton Yield as Affected by Foliar Application of Different Sources of Potassium

Bt cotton hybrids require large supply of metabolites to support their greater boll load and commonly suffer from premature leaf senescence. A field experiment was conducted to study the nutritional status of Bt cotton leaves during boll development stage and to evaluate the most profitable source of foliar fertilizers. Treatments included basal application of 0 and 60 kg·K2O·ha-1 as muriate of potash (MOP) in main plots and foliar spray treatments viz: 4 & 6 sprays of 2% potassium nitrate (Multi-K, 13-00-45), 4 & 6 sprays of NPK Blend (Polyfed, 19-19-19), 4 sprays of MOP, 4 sprays of MOP + urea (to supply same amount of N & K as in potassium nitrate) and unsprayed control in sub plots. The results revealed that only N and K contents of premature senesced leaves were below the sufficiency range for cotton sufficient levels of P, Fe, Mn, Zn and Cu were observed. Though the concentrations of N and K in both the petiole and leaf blade initially improved with foliar spray, N content declined below the unsprayed control at later stages. Basal application of MOP increased seed cotton yield by 19%. Four foliar sprays of KNO3, NPK, MOP and MOP + urea recorded yield increase in seed cotton yield of 22.8%, 22.4%, 18.5% and 24.5%, respectively over unsprayed control. Six sprays of KNO3 and NPK had no yield advantage over four sprays and rather proved economically less viable.

Right Radical Nephrectomy with Type-IV IVC Tumor Thrombectomy Using Venovenous Bypass Instead of Cardiopulmonary Bypass—A Case Report

A renal mass with level Level IV IVC thrombus is usually managed with radical nephrectomy and IVC thrombectomy. This procedure requires the assistance of a cardiac surgeon and is usually done under complete cardiopulmonary bypass. However, the use of cardiopulmonary bypass is associated with reduced cardiac venous return and can consequently decrease cardiac output, adversely affecting haemo-dynamic stability and systemic arterial perfusion. This can lead to relative ischemia of the abdominal viscera, lower limbs and kidneys. We report a case where radical nephrectomy with IVC thrombectomy was done under venovenous bypass, thus avoiding the complications associated with the cardiopulmonary bypass.

Background: Rabies is a dreaded disease and an estimated 55,000 people die of rabies every year. Himachal Pradesh is in the North bordering China and is predominantly rural and hilly. Villages are near forests, where wild reservoirs of rabies exist. Since health facilities are not accessible easily, we need to innovate on existing schedules of rabies vaccination keeping in view the compliance of the patients and affordability so as to give them the best possible option of treatment. In the year 2006 and 2007, we, at DDU Hospital Shimla, experienced a severe shortage of rabies vaccine and patients were running from pillar to post to fetch rabies vaccine. At the same time, we learnt that some of the patients died because either they were not able to purchase the vaccine, mostly because of its high cost, $35, or they ignored the animal bites and did not seek the treatment. Since last year, we have been experiencing non-availability of rabies immunoglobulins (RIGs) in the market and have to innovate new schedules and techniques to save lives of the patients. Methods: During shortage of rabies vaccine in 2008, we contemplated to start a low cost intra-dermal (ID) clinic so as to make rabies vaccine affordable as intramuscular (IM) vaccination cost five times more than ID vaccination. But, there were three main hurdles. One hurdle was the non-availability of rabies vaccine vials having written on them “For IM/ID use” and another hurdle was only fewer animal bite patients attending the DDU Hospital, sometimes only one or two per day, which was insufficient to open a vaccine vial and distribute among them. The third problem being faced was reluctance of the hospital doctors to prescribe ID vaccine as this was not the practice at higher teaching institutions, including medical colleges. Dynamism of Antimicrobial Activity of Actinomycetes—A Case Study from Undisturbed Microbial Niche

Escalation of drug resistant microbes (bacteria) had forced researchers to search new and improved therapeutic compounds from different possible sources, including metabolites secreted by the actinomycetes. The aim of this study was to evaluate the pattern of antimicrobial actinomycetes from physiologically distinct soil of different geographical locations. Forty five soil samples were collected from 5 districts of Gujarat including two sanctuaries as source of survey for bioactive actinomycetes. Crowded plate technique was used for isolation and Agar cylinder method was employed for the antimicrobial screening. A total of 171 actinomycetes were isolated and screened against eighteen pathogens responsible for causing diseases in plants and humans. Results indicate that 79% of the isolates were active against at least one of the eighteen tested pathogens. Some of the actinomycetes strain had shown strong antibacterial and antifungal activity which may be a good source of obtaining novel antimicrobials.
