During the period 1955-1959, approximately 4000 people in southeast Anatolia developed porphyria due to the ingestion of hexachlorobenzene (HCB), a fungicide added to wheat seedlings. These HCB exposures subsequently led to the development of bullae on sun-exposed areas, hyperpigmentation, hypertrichosis, and porphyrinuria. The condition was called kara yara or 'black sore.' Many of the breast-fed children under the age of 2 years whose mothers had ingested HCB-treated grain died from a disease known as pembe yara or 'pink sore.' In this follow-up study of 252 patients, 20-30 years postexposure, there were 162 males and 90 females, with an average current age of 35.7 years, an average of onset of 7.6 years, and a duration of 2.2 years. Many patients had dermatologic, neurologic, and orthopedic symptoms and signs. The observed clinical findings include scarring of the face and hands (83.7％), hyperpigmentation (65％), hypertrichosis (44.8％), pinched facies (40.1％), painless arthritis (70.2％), small hands (66.6％), sensory shading (60.6％), myotonia (37.9％), cogwheeling (41.9％), enlarged thyroid (34.9％), and enlarged liver (4.8％). Urine and stool porphyrin levels were determined in all patients, and 17 have at least one of the porphyrins elevated. A total of 56 specimens of human milk obtained from mothers with porphyria were analyzed for HCB. The average value was 0.51 ppm in HCB-exposed patients compared to 0.07 ppm in unexposed controls. Offspring of mothers with three decades of HCB-induced porphyria appear normal. 1989 Academic Press, Inc.

Background: The rising prevalence of diabetes mellitus imposes great burden on health worldwide. The burden is enormous in terms of pressure on existing human and material health resources. Diabetes is insidious in onset and has many patterns of presentation which often militates against early detection and diagnosis. Thus most patients present late with the exacerbation of symptoms and complications. Objective: To determine the prevalence and pattern of presentation of diabetes mellitus among patients attending Imo State University Teaching Hospital (IMSUTH) Orlu and Imo State Specialist Hospital (IMSSH) Umuguma Owerri, both in Imo State, Nigeria. Methodology: A retrospective study of the prevalence and pattern of presentation of diabetes mellitus. A total of 2028 patients had diabetes mellitus. Using data collection proforma, information relating to their pattern of presentation such as symptoms and complications, as well as their sociodemography was collected from the case folders. Data were analyzed using SPSS version 17 and results presented in frequency tables and pie charts. Result: A total of 18,912 patients attended the designated hospitals within the period under review, out of which 2028 were diabetic giving a prevalence of 107.2 per 1000 patients (10.7%). Out of these 2028 diabetic patients studied, 75% (1521) had onset of the diseases at the age of 40 years and above, while the remaining 25% (507) had the onset of the diseases before the age of 40 years. The most affected group was in the age range of 51 - 60 years, 585 (28.7%), while the least affected age group was 1 - 10 years, 6 (0.31%). The commonest symptom at presentation was polyuria 2028 (100%), followed closely by polydypsia 1939 (95.6%), while the least presenting symptoms were vaginal discharge and erectile dysfunction 27 (1.35%) respectively. AIM: To investigate the effects of hypertonic saline in the neurocritical care ***: We retrospectively reviewed our hospital's use of hypertonic saline(HS) since March of 2005, and prospectively since October 2010. Comparisons were made between admission diagnoses, creatinine change(Cr), and HS formulation(3% Na Cl, 3% Na Cl/sodium acetate mix, and 23.4% Na Cl) to patients receiving normal saline or lactated ringers. The patients(n = 1329) of the retrospective portion were identified. The data presented represents the first 230 patients with data. RESULTS: Significant differences in Acute Physiology and Chronic Health Evaluation Ⅱ scores and GlasgowComa Scale scores occurred between different saline formulations. No significant correlation of Cl- or Na+ with Cr, nor with saline types, occurred. When dichotomized by diagnosis, significant correlations appear. Traumatic brain injury(TBI) patients demonstrated moderate correlation between Na+ and Cr of 0.45. Stroke patients demonstrated weak correlations between Na+ and Cr, and Cl- and Cr(0.19 for both). Patients receiving HS and not diagnosed with intracerebral hemorrhage, stroke, subarachnoid hemorrhage, or TBI demonstrated a weak but significant correlation between Cl- and Cr at ***: Cr directly correlates with Na+ or Cl- in stroke, Na+ in TBI, and Cl- in other populations. Prospective comparison of HS and renal function is needed.

The first search for the doubly heavy Ωbc0baryon and a search for the Ξbc0baryon are performed using pp collision data collected via the LHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 *** baryons are reconstructed via their decays to ∧c+π-andΞc+π-.No significant excess is found for invariant masses between 6700 and 7300 MeV/c2,in a rapidity range from2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ωbc0andΞbc0production cross-section times the branching fraction to ∧c+π-（Ξc+π-） relative to that of the ∧b0（Ξb0） baryon,for different lifetime hypotheses,at 95% confidence *** upper limits range from 0.5×10-4to 2.5×10-4for theΩbc0→∧c+π-(Ξbc0→∧c+π-) decay,and from 1.4×10-3to 6.9×10-3for the Ωbc0→Ξc+π-(Ξbc0→Ξc+π-) decay,depending on the considered mass and lifetime of the Ωbc0(Ξbc0) baryon.
