Sarcopenia is highly prevalent in community dwelling older adults in many countries;however, the prevalence of sarcopenia in nursing home older residents is not well characterized. The aim of this study is to assess the prevalence of sarcopenia in nursing home older residents in Cairo. Cross sectional study was performed among 150 nursing homes residents in Cairo. The European Working Group on Sarcopenia in Older People (EWGSOP) recommendation was used for diagnosing sarcopenia. The study showed that the prevalence of sarcopenia in nursing home older residents in Cairo was 17.3%;22.6% in elderly men and 13.6% in elderly women. Age, underweight and lack of physical activity were found to be associated with sarcopenia among studied participants. The study concluded that sarcopenia is an emerging health problem in nursing home older residents in Cairo.

Throughout 3000 years ago, over the New Kingdom in the Pharaonic period, the Ram-headed Sphinxes Avenue connected Karnak and the Temples of Luxor, a processional avenue was lined on both sides by 1200 statues of sphinxes. The lining of the avenue was erased. Centuries over centuries this avenue has been buried with its statues under about 2 m of silt and sand, and urban development covered it with housing, asphaltic streets, and other structures, obscuring its route and interrupting this dramatic connection. This paper focuses on the discovery of some of these Sphinx statuses and remains at a suggested part of the avenue using both near-surface magnetic and shallow seismic refraction methods. A gradiometer survey was conducted in an area that amounted 576 m2 as (48 m?× 12 m) to measure the vertical magnetic gradient with a high resolution instrument with 0.25 m sampling interval. A superior detection was accomplished by using the analytic signal and Euler deconvolution techniques. The shallow seismic refraction survey was done in the same area to illustrate the lithology of layers material with 1 m interval;both P and S waves were measured to calculate the geotechnical properties of the area to sustain the sketch of structures’ boundaries. We have lucratively detected six main structures;they can be the pedestal of these Ram-headed Sphinx statues. Mining a small part of the study area has proven the reliability of, both the magnetic and shallow seismic refraction discoveries, and the shallowness and composition of the detected features.

In this paper, a numerical study is reported based on the Finite Element Method (FEM) and the Finite Integral Technique (FIT) of Ansoft’s HFSS and CST Microwave Studio (MWS) formulations, respectively, applied to a Bended Complementary Split-Ring Resonator (BCSRR) of rejection band extending from 4.1 GHz to 4.6 GHz. The proposed BCSRR structure is combined with the design of a circularly polarized cylindrical antenna array of square patches with trimmed opposite corners. The performance of the cylindrical antenna array is characterized and compared to that of the flat profile. It is found that the proposed BCSRR reduces the mutual coupling to -15 dB between two patches with a separation of only 1/11th free-space wavelength for applications involving Multi Input Multi Output (MIMO) system.

Objective: To study the prevalence of silent cerebral infarction in Type II diabetes mellitus. Patients and Methods: The study was a prospective on 80 patients recruited from neurology, endocrine outpatient clinics. Patients were type II diabetics as described by the American Diabetes Association, 2011 criteria. All patients underwent full metabolic profiles to diagnose diabetes mellitus and MRI brain scans to detect cerebral infarction. Results: Silent cerebral infarctions were detected in 60% of patients (48/80 patients) predominately along periventricular white matter area and subcortical areas (Basal ganglia, Thalamus). Conclusion: Asymptomatic Type II diabetes mellitus patients could have vascular cerebral changes without neurological symptoms. MRI brain scans could be recommended as routine diagnosis (if possible) for early cerebral infarct detection in type II diabetic patients.

Background: Intra-biliary rupture of hydatid cyst [IBRH] is not a rare complication of hydatid disease of the liver and surgery until recently is the only definitive treatment. With the introduction of ERCP preoperatively or postoperatively there was a great reduction in the operative complications. There were reports in which the ERCP was the sole treatment of this condition. Objective: To evaluate role of ERCP in the management of cases of IBRH. Design: A retrospective study of 7 cases of IBRH and prospective study of 5 cases of IBRH managed by ERCP. Setting: The ERCP Unit of Kurditan Center for Gastroenterology & Hepatology, Asulaimaneyah-Iraqi, Kurdistan, Iraq. Main Outcome Measurements: Improvement in the symptoms, obstructive liver functions pattern and ultrasonic findings in these patients following ERCP management. Methods: This is a study of 12 cases of IBRH managed by ERCP in the Kurdistan Center of Gastroenterology (KCGE) in Asulaimaneyah-Iraq, from 2007-2010. Seven cases of these were studied retrospectively from the available information on the center computers and on follow-up of these patients, while the other five patients were studied prospectively during the study period. We collected the available information: laboratory investigations, trans-abdominal ultrasonography (TAUS), computed tomography (CT) or magnetic resonance images (when available), before and after ERCP. We depended on the liver functions, trans-abdominal ultrasound and the clinical presentation before and after the ERCP;some patients were contacted via mobile phone for follow-up. Results: Cases were more males than females (58% vs 42%);most were from the ages between 30 - 50 years. The clinical presentation was fever, jaundice, itching and right hypochondrial pain in most patients. The liver function tests were obstructive pattern with elevated direct bilirubin and alkhaline phosphates in most patients. Increasing demands for ultrahigh-strength steels in commercial as well as military applications have raised interest in finding alternatives to the high-cost high-alloyed steel and super-alloys currently used, e.g. the use of economic low-alloy compositions processed via low-cost air induction melting and electroslag refining (ESR). In this work the yield of alloying elements and the removal of the impurities nitrogen, sulphur and phosphorus as a result of electroslag refining (ESR) in a newly developed CrNiMoWMnV ultrahigh-strength steel (UHSS) have been studied in relation to their activities in the molten metal pool. Six experimental heats of CrNiMoWMnV UHSS with different chemical compositions were designed, melted in an induction furnace (IF) and refined using ESR. This was followed by hot forging of the ingots at 1100°C to 950°C. ESR using a CaF2-CaO-Al2O3 slag system led to a high yield in Cr, Ni, Mo, W, Mn and V, while the yield of Si is low. The desulphurization of all six UHSS grades was pronounced with most of the sulphur removed either to the slag or by gas reactions. The degree of dephosphorization was only 5% irrespective of the steel composition. On the other hand, denitrification (removal of nitrogen) was achieved. It ranged from 8% to 63% depending on the steel composition. The yield of the alloying elements and removal of impurities from the steel during ESR depends on the chemical and physical properties of the ESR slag and the activity of the elements in the molten state, taking into account elemental interactions.

Matrix-assisted laser desorption/ionization (MALDI) is a preferred and widely used mass spectrometric technique for the analysis of macromolecules. Limited UV-LDI matrices are available for the analysis of biomolecules due to the restricted structural features to serve in the laser desorption/ionization mechanism with a problem of background signals appearing in the low mass region. This paper describes the application of Schiff base derivatives of acylhydrazide and isatin as alternate UV-LDI matrices for the analysis of peptides with significantly low background signals. Thirty one compounds have been successfully employed as matrices for the analysis of low molecular weight (LMW) peptides (α-Cyano-4-hydroxycinnamic acid (HCCA), a preferred choice for peptide analysis.

The Surface Reactivity of Pure and Monohydrogenated Nanocones Formed from Graphene Sheets

A systematic computational study of surface reactivity for pure and mono-hydrogenated carbon nanocoes (CNCs) formed from graphene sheets as functions of disclination angle, cone size and hydrogenation sites has been investigated through density functional (DFT) calculations and at the B3LYP/3-21G level of theory. Five disclination angles (60°, 120°, 180°, 240° and 300°) are applied and at any disclination angle four structures with different sizes are studied. For comparison, pure and mono-hydrogenated boron nitride nanocones (BNNCs) with disclination angles 60°, 120°, 180°, 240° and 300° are also investigated. The hydrogenation is done on three different sites, HS1 (above the first neighbor atom of the apex atoms), HS2 (above one atom of the apex atoms) and HS3 (above one atom far from the apex atoms). Our calculations show that the highest surface reactivity for pure CNCs and BNNCs at disclination angles 60°, 180° and 300° is 23.50 Debye for B41N49H10 cone and at disclination angles 120° and 240° is 15.30 Debye for C94H14 cone. For mono-hydrogenated CNCs, the highest surface reactivity is 22.17 Debye for C90H10-HS3 cone at angle 300° and for mono-hydrogenated BNNCs the highest surface reactivity is 28.97 Debye for B41N49H10-HS1 cone when the hydrogen atom is adsorbed on boron atom at cone angle 240°.

Background: Urosepsis is one of the most common infections that require empirical broad spectrum antibiotics immediately after diagnosis. This has led to development of bacterial resistance by acquiring the capability to destroy the β-lactam ring. Methodology: This is a cross-sectional hospital-based study. The study was conducted from 2019 to 2020 at Gezira Hospital for Renal diseases and surgery (GHRDS). A hundred patients were diagnosed clinically with urosepsis and the isolated organisms were Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Klebsiella pneumonia and Pseudomonas aeruginosa. The susceptibility test was conducted by Kirby Bauer disc diffusion technique according to clinical laboratory standard institute (CLSI) guidelines. Seventy eight samples of bacterial genomic DNA were confirmed by 16srRNA and multiplex PCR, were performed for genotypic blaOXA-51 and blaOXA-23 gene characterization of isolated bacteria. Then gel electrophoresis was used to identify the presence or absence of (blaOXA-51 and blaOXA-23) genes. Results: 88.5% (69/78) in 16srRNA detected. Using multiplex PCR, the frequencies of blaOXA-51 and blaOXA-23 genes were 13% and 10.1%, respectively. The percentages of isolates which yielded both blaOXA-51 and blaOXA-23 among P. aeruginosa was 25% (1/4), among K. pneumonia was 17% (1/6), and among E. coli was 8% (3/37). Only blaOXA-51 was detected in P. mirabilis 10% (1/10) and only blaOXA-23 was detected in S. aureus 5% (1/18). Conclusion: In this study, the presence of blaOXA-51 and blaOXA-23 genes was increased in the isolated bacteria.

Background: Prostate cancer (PCa) is considered one of the major health threats facing males in Sudan. Prostate-specific antigen (PSA) test is the most important laboratory test used in the diagnosis of prostate cancer, the main disadvantage of PSA is its limited specificity, which triggered a lot of interest in development, more research on other markers such as serum human kallikrein-2 (KLK-2) and free prostate specific antigen (fPSA). Objectives: To evaluate the validity of serum kallikrein-2 (KLK-2) and free prostate specific antigen (fPSA) in the early detection of prostate cancer among Sudanese patients. Method: In this study seventy men were considered as a case subject, who were diagnosed as cancer prostate at Gezira Hospital for Renal Disease and Surgery (GHRDS), Sudan during the period February 2018 to July 2019. Randomly selected sixty patients of BPH patients and forty-five apparently healthy men as control subject. KLK-2, fPSA and PSA estimations were performed from serum samples using the principle of Enzyme Linked Immunosorbent Assay (ELISA). Results: The results revealed a highly significant difference between the serum levels individual biomarkers (KLK-2, fPSA, PSA) and multiple biomarkers (fPSA/PSA, KLK-2/fPSA, KLK-2/PSA) for patients with prostate cancer when compared with the control groups. Furthermore, the fPSA/PSA ratio was lower in the patients with prostate cancer (P value = 0.00) than in the control group, the fPSA/PSA ratio showed that best accuracy to differentiate prostate cancer from control group, fPSA cut-off value was found to be more than (18 ng/ml), with sensitivity (93%), specificity (80%), and odds ratio (55). Conclusions: The use of multiple biomarkers rather than individual biomarkers especially fPSA/PSA ratio improves the specificity as well as maintenance of higher sensitivity for early diagnosis of the prostate cancer.
