The serious pandemic of of HCV infection which ultimately will kill millions of peoples all over the world is the dramatic disastrous wave of viral infectious *** Egypt the situation is quite worse,HCV is a leading cause of liver cancer and ***, genotype 4a represents over 90% of cases in *** has higher rates of HCV than neighboring countries as well as other countries in the world with comparable socioeconomic conditions and hygienic *** strong homogeneity of HCV subtypes found in Egypt(mostly 4a)suggests an epidemic spread of HCV which explain the high prevalence of HCV in Egypt due to the past practice of parenteral therapy for *** has a population of 75 million and contains the highest HCV prevalence worldwide which is estimated by the Egyptian Ministry of Public Health(MOHP) to be 12 %(9 million people).90% of persons with chronic HCV infection were infected through unsterilized injection equipment and infusion of inadequately screened blood and blood products or via improperly sterilized medical or dental equipment,needles or syringes,hemodialysis equipment, oral hygiene instruments,and through tattooing, piercing *** Egypt the sexual transmission of HCV is considered to be rare due to the very low incidence of the risk of factors as heterosexual relations and rarity of AIDS *** treatment used in Egypt which is authorized by the Egyptian ministry of public Health is the combination therapy of pegylated interferon alpha and the antiviral drug ribavirin for a period of 24 or 48 ***,being this treatment is very expensive,relatively toxic, and effective in only less than half of the treated patients in the resistant genotype 4a with evidences of relapse in even more than half of the ***,anti-HCV drug-trials are viramidine, which is a prodrug of ribavirin that may be more effective against hepatitis C,is in phase Ⅲ experimental *** will be used in c

背景脑外伤后出现的高血糖与预后不良相关，本实验研究了脑外伤儿童围手术期发生高血糖的概率及危险因素。方法以1994年至2004年间，港景医学中心（成人与儿童一级创伤中心）开展脑外伤（TBI）急诊开颅术的13岁以下儿童做回顾性群组研究。分别在手术前（从到达急诊室至开始全身麻醉）、手术中（全身麻醉期间）及手术后早期（手术后首个24小时）测试每例患者的血糖值，计算高血糖（血糖2200mg/dl）及低血糖（血糖〈60mg／d1）的发生率。持续性高血糖定义为在3个实验阶段中（手术前、手术中、手术后早期）有任意两阶段出现血糖升高，而暂时性高血糖是指有任意一阶段出现血糖升高。采用多元10~stic归分析来推测围手术期高血糖的独立预测因子，数据表示为经修正的比值比（AOR）（95％可信区间），P〈0．05认为有统计学意义。结果在每个实验阶段中至少记录1次血糖值：手术前（86[82％]），手术中（94[89％]），和手术后（101[97％]）。在麻醉期间，有64％儿童的血糖测定次数不足每小时1次。47例（45％）儿童在3个实验阶段中至少有1次血糖升高，有29例儿童（28％）发生了暂时性高血糖，18例儿童（17％）发生了持续性高血糖。围手术期高血糖的独立预测因素为年龄〈4岁（AOR[95％CI]；3．5[1．2～10．6]），格拉斯哥昏迷评分≤8（AOR[95％CI]；7．2[2．4～21．5]）以及合并有硬膜下血肿的复合损伤（AOR[95％CI]；34．7[2．3—525．5]）。6例儿童使用胰岛素进行治疗，两例儿童出现了非胰岛素相关性低血糖。结论围手术期高血糖是常见的，而手术中低血糖也并不少见。为了有效监测手术中高血糖及低血糖的发生概率，需要增加手术中血糖测定频率。年龄〈4岁、严重的脑外伤和合并硬膜下血肿的复合损伤是围手术期高血糖发生的危险因素。

Tuberculosis(TB)remains a leading cause of death among infectious diseases,particularly in poor *** infections,multidrug-resistant and ex-tensively drug-resistant TB strains,as well as the coexistence of chronic illnesses such as diabetes mellitus(dm)greatly aggravate TB morbidity and ***[particularly type 2 dm(T2dm)]and TB have converged making their control even more *** contemporary global epidemics,TB-dm behaves like a syndemic,a synergistic confluence of two highly prevalent diseases.T2dm is a risk factor for developing more severe forms of multi-drug resistant-TB and TB recurrence after preventive *** a bidirectional relationship exists between TB and dm,it is necessary to concurrently treat both,and promote recommendations for the joint management of both *** are also some drug-drug interactions resulting in adverse treatment outcomes in TB-dm patients including treatment failure,and *** addition,autophagy may play a role in these ***,the TB-dm comorbidities present several health challenges,requiring a focus on multidisciplinary collaboration and integrated strategies,to effectively deal with this double *** effectively manage the comorbidity,further screening in affected countries,more suitable drugs,and better treatment strategies are required.

Background: The aim of the current study was to evaluate the anthropometric and demographic factors and their correlation with type 2 diabetes mellitus (T2dm) in Bangladesh. Methods: One hundred fourteen patients (70 males and 44 females) between 30 and 75 years of age from various areas of Bangladesh were screened for T2dm. Fasting blood sugar (FBS) was analyzed by using laboratory kits and spectrophotometric technique. Anthropometric and socio-demographic data were collected using a structured questionnaire. Body mass index (BMI) was calculated from weight (kg) and height (m) of the individual respondents. Physical activity was categorized based on activity during daily work. Economic condition is defined by respective family income and education level is categorized into 3 levels: illiterate, 0 - 12 years of education and graduate or above. Results: According to the current study results, half of the patients were from the middle-class family with low physical activity and their age was within the range of 30 - 45 years. The male and female ratio of the study population was 60:40. Most of the patients were found to be obese and educated. Urban populations were more prone to have dm than the rural population. Age, education, the area of residence (urban and rural), physical activity and co-morbid diseases were significantly correlated with T2dm in Bangladesh (P Conclusion: Our study shows that different socio-demographic factors have a significant correlation with T2dm in Bangladesh. Diabetes awareness, early diagnosis, patient education and life-style modification can be initiated to manage T2dm efficiently.

Background: Very limited data are available regarding the association of vitamin D with glycemic status among adults with newly detected type 2 diabetes mellitus (T2DM) in Bangladesh. Objectives: To determine vitamin D status and its association with glycemic status in Bangladeshi adults with newly detected T2DM. Materials and Methods: This cross-sectional study was carried out in 102 newly detected T2DM diagnosed on the basis of the American Diabetes Association 2017 criteria (age: 42.95 ± 10.68 yrs.;m/f: 44/58) and equal number of age and sex matched controls (age: 40.43 ± 11.04 years) recruited consecutively from the Department of Endocrinology, BSMMU to measure serum vitamin D by high performance liquid chromatography method. Results: Both vitamin D deficiency (p p p 1c (r = -0.249, p = 0.011) in patients with T2dm. HbA1c was linearly associated with vitamin D (β = -0.26, p = 0.009) and severe vitamin D deficiency by binary (OR = 1.37, p = 0.003) and multinomial logistic regression (HbA1c ≥ 10%: OR = 4.25, p = 0.04) in people with T2dm after adjustment for age and BMI.
