Comparison of six human anti-transglutaminase ELISA-tests in the diagnosis of celiac disease in the Saharawi population

AIM: Celiac disease (CD) is an enteropathic disorder very prevalent in Saharawi people. Our aim was to investigate the diagnostic accuracy of six human tissue transglutaminase (tTG) based ELISA tests in Saharawi CD patients. METHODS: Fifty-two CD patients and 23 controls were selected from the Saharawi refugee camps in Tinduf. CD patients were divided into two groups according to their anti-endomysium (EmA) status: 41 EmA positive and 11 EmA negative. Sera from patients and controls were tested for human tTG using six commercial ELISA kits. We used receiver operating characteristics (ROC) curves and areas under the curve to compare the diagnostic accuracies of the six assays. RESULTS: In general, there are differences in the sensitivity and specificity of the human tTG ELISA assays used. Diagnostic accuracy of tests was significantly improved by adjusting the cut-off thresholds according to ROC plot analysis; the correction of the cut-off with the employment of the ROC curve analysis modifies the decision limit in more than 50% in five of the six kits evaluated. CONCLUSION: Some of the human tTG ELISAs used in this study have a diagnostic accuracy similar to EmA determination for diagnosis of CD in Saharawi people. However, it is necessary to select the assay with a higher sensitivity and specificity, and recalculate the cut-off threshold using samples from the referral population.

We expose a case of a woman with hypertension and hypokalemia. The differential diagnosis should include primary hyperaldosteronism, diuretics or lazantes intake, secondary hyperaldosteronism. In this patient, additional tests performed show no cause of hormonal disruption and the whole picture is due to a high intake of licorice. Glycyrrhetinic acid, the active component of licorice, inhibits renal IIbeta-hydroxisteroid dehydrogenase. This allows cortisol to stimulate mineralocorticoid receptors. Licorice ingestion should be considered in the differential diagnosis of hypertension with hypokalemia.

Purpose: Prospective study to evaluate the anti-inflammatory efficacy of isotonic seawater solution on metalloproteinase 9 (MMP-9) levels in the tears of patients with dry eye disease (DED). Methods: A total of 50 patients were included, with the following dry eye criteria in both eyes: InflammaDry? test initially positive, ocular surface disease index (OSDI) score ≥ 12 and Results: The InflammaDry test was positive in 100% of the patients (n = 50) and in 100% (n = 100) of the eyes before treatment. In 14 (28%) patients the test became negative in both eyes, and in 15 (30%) it was negative in one of the eyes. In 43% (n = 43) of the positive eyes, MMP-9 became undetectable in the tear fluid following treatment with isotonic seawater. This change was statistically significant (p Conclusions: The washes with ophthalmic isotonic seawater solution have a significant impact on inflammation of the ocular surface in dry eye disease, with an ability to make MMP-9 levels negative in 43% of cases.

Physicochemical and sensory characteristics of restructured meat of common carp （Cyprinus carpio） fortified with 0-8 g/100 g of chia seed flour （CSF） was evaluated. It had a higher nutritional value （higher fibre content and protein retention] （p 〈 0.05） and better cooking characteristics （higher cooking yield and moisture retention）（p 〈 0.05） than the control. The colour （a＊, b＊） increased; lightness and whiteness index decrease （p 〈 0.05）. Hardness increase （p 〈 0.05） occurred because of CSF addition. Differential scanning calorimetry showed that fibre fortification did not interfere with the thermal transitions of the restructured meat. No significant differences were detected with the preference test scores of 4% or 8% CSF compared with the control. Restructured （4%-8% CSF） had a higher content of fibre and fat, which could be linoleic and linolenic acid, and an increase in the content of protein compared with those of commercial products, among had 1.62 and 2.25 mg AGE/g. Therefore, the restructured properties of common carp were governed by CSF addition.

The purpose of this study was to analyze the effects of the treatment with Exenatide joint to insulin detemir. The study was carried out with obese patients with type 2 diabetes mellitus (DM2) and deficient control, despite intensive insulin therapy combined with oral antidiabetics. A longitudinal study was made with 15 patients controlled by the Endocrinology and Nutrition Section of the Reina Sofía University Hospital, Murcia. These patients were treated with Exenatide joint to insulin during 3, 6 and 12 months. Statistically meaningful differences (p < 0.05) were observed in the reduction of HbA1c in the 3 and 6 months treatment groups (initial HbA1c: 9.55% ± 1.086%, weight: 119.94 kg, insulin requirement: 0.71 IU/kg/day;after 3 months: HbA1c: 7.77% ± 1.164%, weight: 98.13 kg, insulin requirement: 0.59 IU/kg/day;after 6 months: HbA1c: 7.21%, weight: 97.93 kg, insulin requirement: 0.55 IU/kg/day). It was observed that the association of Exenatide and insulin promotes an improvement in several indicators such as weight, body mass index (BMI), systolic pressure (SP), diastolic pressure (DP), triglycerides (TG), cLDL, cHDL and total cholesterol (TC), in patients with diabetes resistant to conventional therapy. However, these improvements were statistically no significant (p < 0.05). It is possible to talk of a reduction of cardiovascular risk factors in patients with Exenatide, being thus a suitable option for this kind of individuals. It is important to emphasize the fact that our study showed bigger body-weight reductions results after 6 and 12 months, as compared to the researches quoted.

AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P < 0.0001),whereas by modified intention-to-treatment(m ITT) no were found significant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. Background: Menarche is an important indicator of pubertal development in adolescent girl. Age at menarche has declined over the years attributable to many factors. Objective: To determine the age at menarche of secondary school adolescent girls and its relationship with anthropometric values, nutritional status, social class of parents, and category of school. Subjects and Methods: Cross-sectional study of 311 secondary school adolescent girls (10-19 years) from two secondary schools (boarding school-Maria Regina secondary school Nnewi and Anglican Girls Secondary School Nnewi-day school) in Nnewi North LGA of Anambra State, Southeastern Nigeria, between May and June 2018. Semi-structured questionnaires were used to extract their data on sociodemographic characteristics and age at menarche. Height and weight were measured using WHO recommended methods. Body mass index (BMI) was determined using the Quetelet index (kg/m2). The relationship between menarche and anthropometric measurement was determined. Results: Of the 313 participants, 212 (68.2%) were post menarcheal, while 99 (31.8%) were pre-menarcheal. The mean age at menarche was 12.79 ± 1.20 years. Early menarcheal age was associated with significantly higher anthropometric values and BMI. Boarding-students had significantly lower menarcheal age (12.69 ± 1.26) compared to the day students (12.86 ± 1.15);students with parents of the high social class had lower menarcheal age (12.56 ± 1.51), compar
