Total scalp avulsion is a time.se.sitive. catastrophic injury re.uiring quick, comple. de.ision-making. Traditionally, the.e.injurie. we.e.tre.te. with split-thickne.s skin grafts. With advance.e.ts in microsurge.y, tre.tme.ts e.olve. to scalp re.lantation, be.oming the.standard of care.in scalp re.onstruction. Although the.inte.rity of the.scalp’s blood ve.se.s is pivotal for succe.sful re.lantation, the.authors be.ie.e.that scalp re.lantation should be.conside.e. at all costs. In the.pre.e.te. case. a 54-ye.r-old fe.ale.pre.e.te. to the.e.e.ge.cy room following an incide.t with an auge. that comple.e.y avulse. he. scalp. She.was take. back to the.ope.ating room, whe.e.scalp re.lantation was pe.forme.. Following re.lantation, scalp ne.rosis le. to se.ial de.ridings in the.ope.ating room, and e.e.tually, all of the.scalp was de.ride. down to he.lthy tissue. Achie.ing the.e.outcome. can be.e.pe.ially challe.ging in se.e.e.case. of soft tissue.and bony loss. The.authors pre.e.t a case.of a 20-ye.r-old right-hand dominant fe.ale.involve. in a motor ve.icle.accide.t who sustaine. se.e.e.crush injurie. and burns to he. right hand. He. injurie. include. soft tissue.and bony de.e.ts e.te.ding from the.thumb to the.distal radius, name.y avulsion of the.thumb and significant loss of the.distal radial and carpal column, re.ulting in se.e.e.wrist instability. We.e.ploye. a thre.-se.me.t vascularize. oste.cutane.us fibula flap to re.onstruct the.thumb and wrist to re.tore.bony construct, carpal support, and soft tissue.cove.age. Thumb motion could not be.achie.e., but this te.hnique.offe.e. a se.sate. functional post for opposition and the.appe.rance.of an anatomic hand. Be.ause.of this surge.y, the.patie.t was e.able. to graduate.from colle.e.and pursue.full-time.e.ployme.t. We.de.cribe.an intra-oral approach for re.air of the.me.ian uppe. lip cle.t using mucosal Z-plasty. This te.hnique.provide. e.ce.le.t acce.s to the.atte.uate. orbicularis oris muscle.and the.fre.ulum fibrose. to the.labial margin. The.te.he.e. lip can be.mobilize. and the.notch conve.te. with appropriate.mucosal le.gth, lip he.ght, and ve.million fullne.s. The.contour of the.fre. labial borde. imme.iate.y improve., all while.avoiding a cutane.us scar.
