Hepatic flow is an intraoperative predictor of early allograft dysfunction in whole-graft deceased donor liver transplantation: An observational cohort study

BACKGROUND Early allograft dysfunction(EAD)after liver transplantation(LT)is an important cause of morbidity and mortality.To ensure adequate graft function,a critical hepatocellular mass is required in addition to an appropriate blood supply.We hypothesized that intraoperative measurement of portal venous and hepatic arterial flow may serve as a predictor in the diagnosis of EAD.AIM To study whether hepatic flow is an independent predictor of EAD following LT.METHODS This is an observational cohort study in a single institution.Hepatic arterial blood flow and portal venous blood flow were measured intraoperatively by transit flow.EAD was defined using the Olthoff criteria.Univariate and multivariate analyses were used to determine the intraoperative predictors of EAD.Survival analysis and prognostic factor analysis were performed using the Kaplan-Meier and Cox regression models.RESULTS A total of 195 liver transplant procedures were performed between January 2008 and December 2014 in 188 patients.A total of 54(27.7%)patients developed EAD.The median follow-up was 39 mo.Portal venous flow,hepatic arterial flow(HAF)and total hepatic arterial flow were associated with EAD in both the univariate and multivariate analyses.HAF is an independent prognostic factor for 30-d patient mortality.CONCLUSION Intraoperative measurement of blood flow after reperfusion appears to be a predictor of EAD;Moreover,HAF should be considered a predictor of 30-d patient mortality.

In a recent article published in Cancer Discovery,Guirguis et al.demonstrated that enzymatic inhibitors of the m6 A methyltransferase METTL3 induced a formation of immunostimulatory doublestranded mRNAs(dsRNAs)in cancer cells,which triggered a profound tumor-cell-intrinsic interferon response leading to an enhanced antigen presentation and thereby increased killing of tumor cells by cytotoxic CD8+T-lymphocytes.The inhibition of METTL3 synergized with anti-PD1 immunotherapy,highlighting its potential for improving cancer treatment.

Mechanical upper bowel obstruction caused by a large trichobezoar in a young woman: A very unusual case report

BACKGROUND Bezoars usually compile human fibers and debris.A special form of bezoar in case of psychologically altered individuals is the trichobezoar.It consists of voluntarily swallowed hair bulks and is normally removed via gastroscopy.Trichobezoars leading to ileus have rarely been reported.CASE SUMmarY A 24-year-old female patient presented to the emergency room with abdominal pain,nausea,and vomiting for 3 d.Her previous medical and psychiatric history was unremarkable.Laboratory analysis showed iron deficiency anemia,leukocytosis,and elevated liver enzymes.An abdominal CT scan revealed a dense structure in the patients’stomach which turned out to be a huge trichobezoar completely obstructing the pylorus.The trichobezoar had to be removed surgi-cally.During her postoperative course,a subcutaneous seroma formed.After a single puncture,the rest of the recovery process was unremarkable,and the patient recovered fully.CONCLUSION A mechanical bowel obstruction is a potentially life-threatening event for every patient.In our case a young female was suffering from severe symptoms of an obstruction which might have resulted in serious harm without successful surgical management.

Under optimal process conditions,pyrolysis of polyolefins can yield ca.90 wt%of liquid product,i.e.,combination of light oil fraction and heavier wax.In this work,the experimental findings reported in a selected group of publications concerning the non-catalytic pyrolysis of polyolefins were collected,reviewed,and compared with the ones obtained in a continuously operated bench-scale pyrolysis reactor.Optimized process parameters were used for the pyrolysis of waste and virgin counterparts of high-density polyethylene,low-density polyethylene,polypropylene and a defined mixture of those(i.e.,25:25:50 wt%,respectively).To mitigate temperature drops and enhance heat transfer,an increased feed intake is employed to create a hot melt plastic pool.With 1.5 g·min^(-1) feed intake,1.1 L·min^(-1) nitrogen flow rate,and a moderate pyrolysis temperature of 450℃,the formation of light hydrocarbons was favored,while wax formation was limited for polypropylene-rich mixtures.Pyrolysis of virgin plastics yielded more liquid(maximum 73.3 wt%)than that of waste plastics(maximum 66 wt%).Blending polyethylenes with polypropylene favored the production of liquids and increased the formation of gasoline-range hydrocarbons.Gas products were mainly composed of C3 hydrocarbons,and no hydrogen production was detected due to moderate pyrolysis temperature.

The demand for industrial enzymes is continually rising,fueled by the growing need to shift towards more sustainable industrial processes.However,making efficient enzyme production strains and identifying optimal enzyme expression conditions remains a challenge.Moreover,the production of the enzymes themselves comes with unavoidable impacts,e.g.,the need to utilize secondary feedstocks.Here,we take a more holistic view of bioprocess development and report an integrative approach that allows us to rapidly identify improved enzyme expression and secretion conditions and make use of cyanobacterial waste biomass as feed for supporting Pichia pastoris fermentation.We demonstrate these capabilities by producing a phytase secreted by P.pastoris that is grown on cyanobacterium hydrolysate and buffered glycerol-complex(BMGY)medium,with genetic expression conditions identified by high-throughput screening of a randomized se-cretion library.When our best-performing strain is grown in a fed-batch fermentation on BMGY,we reach over 7000 U/mL in three days.

Background:Atherosclerosis forms the pathological basis for the development of cardiovascular disease.Since pathological processes initially develop without clinically relevant symptoms,the identification of early markers in the subclinical stage plays an important role for initiating early interventions.There is evidence that regulatory T cells(Tregs)are involved in the development of atherosclerosis.Therefore,the present study aimed to identify and investigate associations with Tregs and their subsets in a cohort of healthy elderly individuals with and without subclinical atherosclerotic plaques(SAP).In addition,various lifestyle and risk factors,such as cardiorespiratory fitness,were investigated as associated signatures.Methods:A cross-sectional study was performed in 79 participants(male:n=50;age=63.6±3.7 years;body mass index=24.9±3.1 kg/m2;mean±SD)who had no previous diagnosis of chronic disease and were not taking medication.Ultrasound of the carotids to identify SAP,cardiovascular function measurement for vascular assessment and a cardiorespiratory fitness test to determine peak oxygen uptake were performed.Additionally,tests were conducted to assess blood lipids and determine glucose levels.Immunophenotyping of Tregs and their subtypes(resting(rTregs)and effector/memory(mTregs))was performed by 8-chanel flow cytometry.Participants were categorized according to atherosclerotic plaque status.Linear and logistic regression models were used to analyze associations between parameters.Results:SAP was detected in a total of 29 participants.The participants with plaque were older(64.8±3.6 years vs.62.9±3.5 years)and had higher peripheral systolic blood pressure(133.8±14.7 mmHg vs.125.8±10.9 mmHg).The participants with SAP were characterized by a lower percentage of rTregs(28.8%±10.7%vs.34.6%±10.7%)and a higher percentage of mTregs(40.3%±14.7%vs.30.0%±11.9%).Multiple logistic regression identified age(odds ratio(OR)=1.20(95%confidence interval(95%CI):1.011.42))and mTregs(OR=1.05(95%CI:1.021.10))as independent

Effects of dietary supplementation with tropical microfungi Mucor circinelloides and Trichoderma harzianum powder on growth performance,bacterial communities,and the biomass nutritional profile of white shrimp grow-out culture(Litopenaeus vannamei)

Microfungi have been explored further as supplement constituents in shrimp culture due to their rich protein,lipid,essential amino acids,fatty acids,and various bioactive compounds.The researchers have evaluated the effects of two microfungi powders(Mucor circinelloides and Trichoderma harzianum)as feed supplements on growth performance,bacterial communities,and the biomass nutritional profile of white shrimp(Litopenaeus vannamei).The results showed that commercial/artificial feed(Feng-Li)with supplementation microfungi powder had a different effect on white shrimp growth performance than control after 62 days of the grow-out period.Supplementation with 1.5%M.circinelloides powder(M-2)could improve the growth performance of white shrimp,especially the survival rate(81.33%),final body weight(5.21 g),and shrimp biomass(211.35 g).In contrast,supplementation with 1.5%T.harzianum powder(T-2)did not improve the growth performance of white shrimp compared to control(p>0.05).Furthermore,it also affected the profile of the bacterial community in culture water as well as in shrimp intestines.Supplementation with both microfungi could reduce the total number of bacteria in culture water at the end of the experiment,where the lowest number was obtained in treatment T-2(3.9×10^(5)CFU/mL),followed by M-2(9.9×10^(5)CFU/mL)and control(4.4×10^(6)CFU/mL).In the culture water,20 isolates of culturable bacteria were found during the trial,with the dominant bacteria in M-2:Bacillus cereus,Bacillus velezensis,Gordonia polyisoprenivorans,T-2:Acinetobacter sp.,Gordonia polyisoprenivorans,Micrococcus luteus.Based on average well colour development(AWCD),the highest bacterial activity was in the M-2(0.297-0.707),followed by T-2(0.226-0.717)and control(0.094-0.694).The highest substrate group utilised in M-2 and T-2 were polymers(0.896 and 1.348)and control:amines(0.100).The highest Shannon diversity index was obtained in the M-2(2.508),followed by control(2.286)and the T-2(2.157).As many as 11 isolates of culturable bacteria were found in
