对于一个有着强烈的平等主义社会文化的国家,人们很少使用'精英'这一术语来描述荷兰最高层级的公务员。虽然在形式上对外公开招聘人才,但是实际上中央政府的(高级)公务员是一个相当封闭的系统。在这些公务员与政治官员之间职业流动相当小,而公共部门与私营部门之间的职业流动则几乎是零。然而,为了加强这种外部流动性,最近出台了一些措施。新公共管理对荷兰行政精英的结构与功能几乎没有什么影响。对这些公务员的定位和职能更为重要的是高级执行部(the Senior Executive Service)的创立,以及这个部门内部的高级管理团队。通过在中央政府的最高行政部门创立这个职业体系,高级公务员的精英性质被强化了。

Real-world evidence (RWE) is clinical evidence on a medical product’s safety and efficacy that is generated using real-world data (RWD) resulting from routine healthcare delivery. This study evaluates the clinical efficacy of amoxicillin + clavulanic acid in children with pharyngitis, acute otitis, or acute rhinosinusitis with suspected bacterial origin under normal office and home conditions. Methods: This was a real-life, prospective, observational, pharmacovigilance study. It included children of both sexes between 2 and 12 years old, with a diagnosis of Rhinopharyngitis (tonsillitis), Acute Otitis Media and Rhinosinusitis. The main effectiveness variable evaluated was reduction and time to resolution of symptoms. All patients received Amoxicillin/Clavulanic Acid suspension 600 mg/42.9 mg/5 mL at a dose of 90 mg/Kg/day in two doses, every 12 hours for 7 days. The evaluations were carried out at the beginning, at 72 hours (3rd day) and at 7 days. All patients underwent culture and antibiogram. Results: The majority of cultures were negative for pathogenic germs, suspecting unidentifiable germs, or viral etiology despite the rigorous selection of subjects following validated scores. The most frequently isolated germ was Staphylococcus aureus;growth of gram-negative bacteria was reported in 33.33% of the cultures. There was a significant improvement in symptoms in children with tonsillitis and rhinosinusitis from the first 72 hours of treatment, persisting until the 7 days. In the otitis media group, returning to normal by the tenth day. During the conduction of this investigation, no adverse effects associated with the prescribed therapy were reported.

We report the discovery of KMT-2020-BLG-0414 Lb,with a planet-to-host mass ratio q2=0.9-1.2×10-5=3-4 q⊕at 1σ,which is the lowest mass-ratio microlensing planet to *** with two other recent discoveries(4?q/q⊕?6),it fills out the previous empty sector at the bottom of the triangular(log s,log q)diagram,where s is the planet-host separation in units of the angular Einstein radiusθ***,these discoveries call into question the existence,or at least the strength,of the break in the mass-ratio function that was previously suggested to account for the paucity of very low-q *** to the extreme magnification of the event,Amax~1450 for the underlying single-lens event,its light curve revealed a second companion with q3~0.05 and|log s3|~1,i.e.,a factor~10 closer to or farther from the host in *** measurements of the microlens parallaxπE and the angular Einstein radiusθE allow estimates of the host,planet and second companion masses,(M1,M2,M3)~(0.3 M⊙,1.0 M⊕,17 MJ),the planet and second companion projected separations,(a⊥,2,a⊥,3)~(1.5,0.15 or 15)au,and system distance DL~1 *** lens could account for most or all of the blended light(I~19.3)and so can be studied immediately with high-resolution photometric and spectroscopic observations that can further clarify the nature of the *** planet was found as part of a new program of high-cadence follow-up observations of high-magnification *** detection of this planet,despite the considerable difficulties imposed by COVID-19(two KMT sites and OGLE were shut down),illustrates the potential utility of this program.

Background: The hand is an essential component for performing our daily tasks;as a result, injuries to the fingertip are frequently seen. Accurate evaluation and treatment of fingertip injuries are essential to avoiding long-term disability, job loss and psychological issues. No matter the injury classification system, maintaining finger length, regaining sensation, and improving appearance are the three main objectives of treatment. The objective of this research was to outline the epidemiology of fingertip injuries treated between 2021 and 2022 at a highly specialized referral hospital in Mexico City. Methodology: This investigation is cross-sectional, retrospective, and observational. From January 2021 to December 2022, 60 patients with a diagnosis of fingertip injury from the South Central Hospital “Pemex” were included. The descriptive statistical analysis was performed using SPSS29. Results: 60 patients were included, 27 (45%) were women and 33 (55%) were men. Allen’s classification was able to classify only 19 patients, of which 18 (94.7%) had a type 1 classification and one (5.26%) had a type 2 classification. 10% of patients experienced complications, which included 2 patients with surgical site infections (3.3%), 3 patients with persistent pain (5%), and 1 patient (1.6%) with graft rejection. Conclusions: There are currently insufficient epidemiological data on hand and wrist injuries and its effects on labor in Mexico. Fingertip injuries require particular care in order to prevent complications and long-term damage. The creation of national registries could help with the creation of reference manuals for the care of hand and wrist injuries in our nation.

The severe acute respiratory syndrome coronavirus 2(SARS-CovV-2)infection is associated with a hyperinflammatory state and lymphocytopenia,a hallmark that appears as both signature and prognosis of disease severity *** cytokine storm and a sustained inflammatory state are commonly associated with immune cell depletion,it is still unclear whether direct SARS-Cov-2 infection of immune cells could also play a role in this scenario by harboring viral *** found that monocytes,as well as both B and T lymphocytes,were susceptible to SARS-Cov-2 infection in vitro,accumulating double-stranded RNA consistent with viral RNA replication and ultimately leading to expressive T cell *** addition,flow cytometry and immunofluorescence analysis revealed that SARS-Cov-2 was frequently detected in monocytes and B lymphocytes from coronavirus disease 2019(CoVID-19)*** rates of SARS-Cov-2-infected monocytes in peripheral blood mononuclear cells from CoVID-19 patients increased over time from symptom onset,with SARS-CoV-2-positive monocytes,B cells,and CD4+T lymphocytes also detected in postmortem lung *** results indicated that SARS-CoV-2 infection of blood-circulating leukocytes in covID-19 patients might have important implications for disease pathogenesis and progression,immune dysfunction,and virus spread within the host.

Fabry’s disease is an X-linked lysosomal storage disorder. α-Galactosidase deficiency leads to accumulation of globotriaosylceramide mainly in endothelial and smooth muscle cells. Cerebrovascular symptoms with predominant affection of the vertebrobasilar circulation are one of the major sources of morbidity in Fabry’s disease. We present a Hungarian family with Fabry’s disease caused by a new mutation in the α-galactosidase A gene (GLA), and describe a variant expression of the disease. Megadolichoba- silar anomaly was diagnosed in two male patients in the family who died of thrombosis. In another female patient who had suffered from disturbance of the vertebrobasilar circulation, a strongly dilated basilar artery without thrombosis was found at autopsy. Another three family members had basilar strokes and large and elongated basilar arteries on MRI. Genetic analysis disclosed a c.47T→C missense mutation resulting in L16P in the amino acid sequence of the α-galactosidase protein. This report suggests that megadolichobasilar anomaly is potentially life-threaten- ing, and that L16P is a disease-causing mutation in patients with Fabry’s disease. Early enzyme replacement therapy may prevent the development of these irreversible cerebrovascular complications.
