Aim: To assess the effects of early oral hydration after elective caesarian se ction. Methods: Hundred women were selected from the maternity wards of LHMC and SSK Hospital, and alternately as signed into control and study groups of 50 each. Women with medical complication s and antepartum haemorrhage were excluded. In the study group, oral hydration was started 6-h postextubation irrespective of presence of bowel sounds. Solid f ood was started after bowel sounds appeared. In the control group, traditional r egime of oral hydration after the appearance of bowel sounds and then a gradual shift to the solids was adopted. The return of bowel activity, time of ambulatin g and complications were compared. Results: Bowel sounds appeared in a significa ntly shorter duration of time in study group, the mean being 7.4 h as compared t o 11.5 h in the control group. Passage of flatus and bowel evacuation was earlie r in the study group (9.14 and 23.7 h, respectively) than in the control (19.9 a nd 32.3 h, respectively). Women ambulated faster in the study group than the con trol group (15 h versus 25 h, respectively). Mean oral fluid intake was much mor e and return to soft and then full diet was faster in the study group. Sixty per cent women preferred early feeding to the traditional one. Conclusion: Early ora l hydration in the postoperative period helps in the faster recovery of the pati ent by means of quicker return to normal feeding habits and early ambulation, th e two main concerns of any surgeon before discharging the woman after caesarian section.

Conservative management of spinal pathology with autologous conditioned serum: A systematic review of the literature

BACKGROUnD Chronic inflammatory pain is associated with increased expression of interleukin(IL)-1,an inflammatory cytokine,and activity on its receptor(IL-1R).In response,the body produces IL-1R antagonist(IL-1Ra)to reduce this *** conditioned serum(ACS)is the only biologic therapy for spinal pathologies that enhances the action of endogenous IL-1Ra reserves to improve *** systematic review investigates the effectiveness of ACS in treating pain and dis-ability caused by spinal *** To evaluate the use of ACS as a conservative management option for spinal *** A systematic review of PubMed/Medline was performed to identify studies inve-stigating administration of ACS for treatment of any spinal *** Six articles were included,comprising 684 patients treated with epidural(n=133)or transforaminal(n=551)ACS *** had an average age of 54.0 years with slight female predominance(53.2%).The lumbar spine was most com-monly treated,with 567 patients(82.9%)receiving injections for lumbar radicu-lopathy(n=67),degenerative disc disease(DDD)(n=372),or spinal stenosis(n=128);cervical injections were performed in 109 patients(15.9%).Mean(SD)follow-up was 21.7(4.8)weeks from first ACS *** studies investigating mecha-nical lumbar and lumbar or cervical radicular pain reported significant pain re-duction at final follow-up compared to *** achieved comparable or su-perior results to lumbar epidural steroid *** events were reported in 21 patients(3.1%),with no serious adverse *** ACS injection is a safe and effective intervention for pain reduction in many spinal pathologies,including cervical and lumbar radiculopathies.

Objective: To explore the difference in rates and reasons for disclosure of either conventional or complementary medicine(CM) to healthcare practitioners between people living with sleep disorders(SDs) and those without ***: A cross-sectional survey(n = 2019 adults) that measured sociodemographic characteristics,health status, health service utilisation, health literacy and health communication(medicine disclosure)of a representative sample of Australians was conducted. Data from participants reporting an SD(n = 265)were compared to those not reporting an SD to assess measures of health communication and disclosure about medicine ***: Overall, rates of medicine disclosure to both conventional and CM practitioners were high, in respondents with(70%) and without an SD(57%). Those reporting an SD had higher expectations of their conventional health practitioner’s knowledge of CM, associated clinical decision-making skills, and approval of CM use, and held a higher degree of concern about drug interactions(P n reasons cited for disclosing CM use to conventional health practitioners and conventional medicine use to CM practitioners were “I want them to fully understand my health status” and“I was concerned about drug interactions with the CMs used.”Conclusion: The high rate of medicine use disclosure by people with SDs is driven by an intention to inform their healthcare practitioner about their health status and concerns about potential medicine interactions. Therefore, research about the expectations that people with an SD have of their conventional healthcare practitioners’ knowledge of CM and CM-drug interactions requires further ***, further examination of CM practitioner’s conventional medicine knowledge is encouraged.

Prostate cancer, one of the most frequently occurring cancers in men, is a heterogeneous disease involving multiple cell types within tumors. This tumor heterogeneity at least partly results from genomic instability leading to sub-clonal cellular differentiation. The differentiated cell populations originate from a small subset of cells with tumor-initiating and stem-like properties. These cells, termed prostate cancer stem cells(PCSCs), play crucial roles in disease progression, drug resistance, and relapse. This review discusses the origin, hierarchy, and plasticity of PCSCs;methods for isolation and enrichment of PCSCs;and various cellular and metabolic signaling pathways involved in PCSC induction and maintenance, as well as therapeutic targeting.

Leg Length Discrepancy in Cementless Total Hip Arthroplasty

The use of cementless total hip arthroplasty (THA) is on the increase. In order to achieve rotational and axial stability larger implants may be required than originally templated for. This could potentially result in a lar-ger leg length inequality. Our objective was to determine whether there is greater inequality in leg length post-operatively in cementless THA as compared to cemented implants. 136 consecutive patients undergoing elective THA between June 2007 and May 2008 were included. Post-operative digital radiographs were ex-amined to determine leg length. Twenty seven patients (20%) underwent a cemented procedure and 109 (80%) a cementless procedure. In the cemented group the mean leg length discrepancy was 7.3 mm (range 19 mm short to 21 mm long). In the cementless group the mean measured leg length discrepancy was 6.3 mm (range 18 mm short to 23 mm long). There was no significant difference between the two groups (P = 0.443). This study shows that with accurate pre-operative templating, both cemented and cementless proce-dures produce comparable and acceptable leg length discrepancies.

Sitagliptin in patients with non-alcoholic steatohepatitis: A randomized, placebo-controlled trial

AIMTo evaluate the effect of sitagliptin vs placebo on histologic and non-histologic parameters of non-alcoholic steatohepatitis (nASH).METHODSTwelve patients with biopsy-proven nASH were randomized to sitagliptin (100 mg daily) (n = 6) or placebo (n = 6) for 24 wk. The primary outcome was improvement in liver fibrosis after 24 wk. Secondary outcomes included evaluation of changes in nAFLD activity score (nAS), individual components of nAS (hepatocyte ballooning, lobular inflammation, and steatosis), glycemic control and insulin resistance [including measurements of glycated hemoglobin (HbA1C) and adipocytokines], lipid profile including free fatty acids, adipose distribution measured using magnetic resonance imaging (MRI), and thrombosis markers (platelet aggregation and plasminogen activator inhibitor 1 levels). We also sought to determine the correlation between changes in hepatic fat fraction (%) [as measured using the Iterative Decomposition of water and fat with Echo Asymmetry and Least-squares estimation (IDEAL) MRI technique] and changes in hepatic steatosis on liver *** was not significantly better than placebo at reducing liver fibrosis score as measured on liver biopsy (mean difference between sitagliptin and placebo arms, 0.40, P = 0.82). There were no significant improvements evident with the use of sitagliptin vs placebo for the secondary histologic outcomes of nAS total score as well as for the individual components of nAS. Compared to baseline, those patients who received sitagliptin demonstrated improved HbA1C (6.7% ± 0.4% vs 7.9% ± 1.0%, P = 0.02), and trended towards improved adiponectin levels (4.7 ± 3.5 μg/mL vs 3.9 ± 2.7 μg/mL, P = 0.06) and triglyceride levels (1.26 ± 0.43 mmol/L vs 2.80 ± 1.64 mmol/L, P = 0.08). However, when compared with placebo, sitagliptin did not cause a statistically significant improvement in HbA1C (mean difference, -0.7%, P = 0.19) n

目的 :为了证实前列腺叶切除术后前列腺叶是否的确再生 ,如果再生 ,再生的程度如何。研究的其他问题还有 :①是否再生叶在形态上与从胎儿到成年的正常发育相同 ?②部分叶切除后对对侧叶的影响以及性激素对再生过程的影响。方法 :在叶切除术后的不同时间间隔 ,研究手术摘除腹侧前列腺叶之一对对侧叶的影响。结果 :手术摘除大鼠腹侧前列腺叶的再生在叶切除术后的 8~ 16周达到最大值。然而 ,再生叶明显地小于原先的大小。在叶切除术后的早期 (2周 ) ,对侧叶明显肥大。之后随切除叶的不断再生 ,对侧叶恢复到正常大小。叶切除术的同时行睾丸切除引起保留叶的急剧缩小 ,但能被外源性雄激素治疗阻止。接受雌激素处理的动物其保留叶部分地而不是全部萎缩。不过 ,雌激素不能支持手术切除叶的再生 ,再生需要雄激素。结论 :本研究证明了手术摘除腹侧前列腺的一叶导致了一个再生过程 ,然而再生叶不能达到正常大小。

The concise building of the spiroketal core of acortatarin-type alkaloids as potential therapeutic agents in diabetic nephropathy was established in four steps,through a tandem n-alkylation/hemiacetalization between pyrrole units and the corresponding halo alcohols generated by convenient halomethylation of chiral lactones from natural aldoses.
