The Use of Intensity Modulated Radiotherapy as a Mean of Reducing Dose to Bone Marrow for Patients with Cancer Cervix Treated at Nci, Cairo, Egypt. Dosimetric Study

Purpose: To test the use of Intensity Modulated Radiotherapy (IMRT) to spare the bone marrow (BM) in patients treated with cancer cervix through using the bone marrow an organ at risk. Patients and methods: Thirteen patients with stage (IB2-IIIB) intact cervix cancer were included;C-T simulation was done with contrast and full bladder with slice thickness 2.5 mm. Clinical Target Volume (CTV) included the cervix, uterus, upper half of the vagina, parametrium and regional lymph nodes. The CTV was expanded by 1 cm to form the Planning Target Volume (PTV). The organ at risk (OAR) included the bladder and rectum, the external contour of the pelvic bones to define the Pelvic Bone Marrow (PBM). Four plans were done for every patient with anteroposterior-posteroanterior (AP/PA), three dimensional conformal radiotherapy (3DCRT), IMRT, bone marrow-sparing intensity-modulated pelvic radiotherapy (BMS-IMRT). Results: BMS-IMRT reduced the V20, V30, V40 and V45 of the BM in comparison to 3DCRT and IMRT plans. Reduction in V20 with BMS-IMRT plan compared to 3DCRT (p 0.03). The PBM volume receiving 5, 10 and 20 Gy was lower AP/PA than BMS-IMRT (p < 0.01, p 0.001 and p 0.04 respectively). The volumes of the rectum and bladder receiving dose of 40 and 45 Gy were lower in BMS-IMRT plan compared to AP/PA and 3DCRT plans with p = 0.01 for both of them. PTV cover was better in the BMS-IMRT and IMRT plans. Conclusion: BMS-IMRT decreased the irradiated BM volume compared to other techniques. Thus using BMS-IMRT is recommended to decrease hematological toxicity and avoid treatment interruption.

Objective: To determine the possible therapeutic gain of using three-dimensional conformal radiotherapy (3D-CRT) as a treatment option for portal vein tumor thrombus (PVTT) in patients with hepatocellular carcinoma (HCC) and to evaluate the tolerance and toxicity of using such treatment. Materials and methods: Sixty two patients were enrolled in this prospective study between June 2013 and August 2015. The clinical target volume (CTV) was the PVTT and the prescribed dose was 50 Gy/25 fractions. The median follow-up time was 7.4 months. Results: The thrombus crude response rate was 40.4% and the only significant prognostic factor for response was the thrombus size. Responders had significant better survival compared to non-responders with a median survival of 12.5 and 8 months respectively (P Conclusions: The results of this study suggest that radiotherapy should be considered as a safe treatment option for HCC patients with PVTT. It is effective not only for PVTT local control but also for survival, although prospective randomized trials are needed to confirm these results.

Background Acute kidney injury(AKI)is common in critically ill children with significant mortality and *** creatinine is an insensitive and late biomarker compared to newly proposed AKI *** Prospective study in pediatric intensive care unit(PICU)over three months to compare between serum cystatin-C(s-Cys-C)and urinary neutrophil gelatinase-associated lipocalin(uNGAL)as AKI biomarkers at multiple time points with pediatric risk,injury,failure,loss,end-stage renal disease(pRIFLE)classification in diagnosing *** Forty children were *** these 40 children,22 developed AKI according to pRIFLE *** was no significant difference between AKI and non-AKI in age(P=0.29).Post cardiac surgery,renal insult was the main cause of AKI(27.3%).There was a twofold increased risk of incident AKI in those patients with high baseline uNGAL at PICU admission and almost a fourfold increased risk in patients with high baseline s-Cys-C at PICU *** levels were highly predictive of AKI during the follow-up period[area under the curve(AUC)=0.76,95%confidence interval(CI)0.61-0.92].The cutoff point with the highest correctly classified proportion was 223 ng/mL(≥12 centiles)which correctly predict 80.0%patients with AKI,with a corresponding sensitivity of 72.7%and a specificity of 89.9%.AUC for s-Cys-C was 0.86(95%CI 0.75-0.97),and the highest correctly classified proportion was 1009μg/L(≥13 centiles);75%of patients with AKI,with a corresponding sensitivity of 63.6%and a specificity of 88.9%.Conclusion uNGAL and s-Cys-C predicts AKI early in critically ill children.

The in-situ measurements of the ionospheric plasma that we use come from two instruments of the scientific payload of the satellite DEMETER; the plasma analyser IAP （Instrument d＇analyse du plasma） and the Langmuir probe ISL （Instrument Sonde de Langmuir）. DEMETER is a micro-satellite realized by the CNES（Centre National d＇Etudes Spatiales, France） with a principal objective to seek a possible influence of the seismic activity on the electromagnetic waves in the ionosphere and on the ionospheric plasma. The satellite was placed on June 29, 2004, in a circular and quasi helio-synchronous orbit at -710 km altitude. The experiments function primarily at mid-latitudes （from ＋60° to -60°）. The IAP data were analysed to deduce the ion population （densities of the dominant ions, i.e. generally O^＋, H^＋ and He^＋） therefore the total ion density. The use of data IAP thus requires some precaution to make sure that the electric equilibrium conditions of the satellite, such as the satellite potential （Фsat）, are obtained during the treatment of routine, does not induce an error of measurement. When this potential is negative, the minority light ions H^＋ and He^＋ can be measured in a reliable way when their proportion is above 3% to 5% of that of O^＋. The critical limitation is： under certain conditions, the satellite potential becomes positive and reach a value about -0.5 V so that it becomes impossible to measure H^＋ ions. This is likely to involve a significant error on the composition and the density of the plasma. Therefore we carried out a calibration to estimate the missing density. The ISL experiment （Langmuir probe） provided the collected current/polarized tension characteristics of a cylindrical probe from which both electron density Ne and temperature Te were obtained. Tablets are one of the most comm only used solid dosage forms taken by *** preparati on of high-quality tablets requires an understanding of the preparation process and elucidation of how the physical and mechanical properties change as a function of the preparation *** work aims to investigate the impact of microwave irradiation drying and conventional drying methods(including freeze,convective and vacuum drying)on the hardness,tensile strength and friability of tablets made from a multi-component formulation containing naproxen sodium,microcrystalline cellulose,and *** results show that tablets subjected to microwave drying had the secondhighest tensile strength and hardness of 1.296 MPa and 67 N,*** tablets subjected to vacuum drying had the lowest tensile strength and hardness of 1.21 MPa and 64 N,*** friability index values for the tablets derived from the microwave and freeze-drying methods werel%.Microwave drying was observed to be an efficient method to produce naproxen sodium-containing tablets with satisfactory mechanical *** findings confirm that the drying method plays an essential role in the improvement or degradation of the mechanical properties of tablets.

Crack potential and hydration processes of the cement pastes were monitored using an upto-date eccentric steel cracking frame(ESCF), associated with the non-contact electrical resistivity apparatus, independently. The objective of employing the ESCF is to give a new method determining cracks of concrete at early age. The findings indicate that the lowest water-cement ratio paste reveals highest resistivity values, compasses an earlier inflection point and obtained higher stress. The eccentric restrained cracking test exhibited that lower water-cement ratio paste cracked at the earliest time, accordingly confirms cracking tendency is the highest. Tensile strength test and stresses utilizing ABAQUS simulation was performed. The crack initiation ages obtained are consistent with the experimental program results, which indicates that ABAQUS numerical analysis can well be utilized to predict the crack tendency of cement.

Named Entity Recognition(NER)is one of the fundamental tasks in Natural Language Processing(NLP),which aims to locate,extract,and classify named entities into a predefined category such as person,organization and *** of the earlier research for identifying named entities relied on using handcrafted features and very large knowledge resources,which is time consuming and not adequate for resource-scarce languages such as ***,deep learning achieved state-of-the-art performance on many NLP tasks including NER without requiring hand-crafted *** addition,transfer learning has also proven its efficiency in several NLP tasks by exploiting pretrained language models that are used to transfer knowledge learned from large-scale datasets to domain-specific *** Encoder Representation from Transformer(BERT)is a contextual language model that generates the semantic vectors dynamically according to the context of the *** architecture relay on multi-head attention that allows it to capture global dependencies between *** this paper,we propose a deep learning-based model by fine-tuning BERT model to recognize and classify Arabic named *** pre-trained BERT context embeddings were used as input features to a Bidirectional Gated Recurrent Unit(BGRU)and were fine-tuned using two annotated Arabic Named Entity Recognition(ANER)*** results demonstrate that the proposed model outperformed state-of-the-art ANER models achieving 92.28%and 90.68%F-measure values on the ANERCorp dataset and the merged ANERCorp and AQMAR dataset,respectively.
