Liquid biopsy for gastric cancer:Techniques,applications,and future directions

After the study of circulating tumor cells in blood through liquid biopsy(LB),this technique has evolved to encompass the analysis of multiple materials originating from the tumor,such as nucleic acids,extracellular vesicles,tumor-educated platelets,and other metabolites.Additionally,research has extended to include the examination of samples other than blood or plasma,such as saliva,gastric juice,urine,or stool.LB techniques are diverse,intricate,and variable.They must be highly sensitive,and pre-analytical,patient,and tumor-related factors significantly influence the detection threshold,diagnostic method selection,and potential results.Consequently,the implementation of LB in clinical practice still faces several challenges.The potential applications of LB range from early cancer detection to guiding targeted therapy or immunotherapy in both early and advanced cancer cases,monitoring treatment response,early identification of relapses,or assessing patient risk.On the other hand,gastric cancer(GC)is a disease often diagnosed at advanced stages.despite recent advances in molecular understanding,the currently available treatment options have not substantially improved the prognosis for many of these patients.The application of LB in GC could be highly valuable as a non-invasive method for early diagnosis and for enhancing the management and outcomes of these patients.In this comprehensive review,from a pathologist’s perspective,we provide an overview of the main options available in LB,delve into the fundamental principles of the most studied techniques,explore the potential utility of LB application in the context of GC,and address the obstacles that need to be overcome in the future to make this innovative technique a game-changer in cancer diagnosis and treatment within clinical practice.

There is an increasing interest in restoring degraded forests,which occupy half of the forest areas.Among the forms of restoration,passive restoration,which involves the elimination of degrading factors and the free evolution of natural dynamics by applying minimal or no management,is gaining attention.Natural dynamics is difficult to predict due to the influence of multiple interacting factors such as climatic and edaphic conditions,composition and abundance of species,and the successional character of these species.Here,we study the natural dynamics of a mixed forest located in central Spain,which maintained an open forest structure,due to intensive use,until grazing and cutting were banned in the 1960s.The most frequent woody species in this forest are Fagus sylvatica,Quercus petraea,Quercus pyrenaica,Ilex aquifolium,Sorbus aucuparia,Sorbus aria and Prunus avium,with contrasting shade and drought tolerance.These species are common in temperate European deciduous forest and are found here near their southern distribution limit,except for Q.pyrenaica.In order to analyze forest dynamics and composition,three inventories were carried out in 1994,2005 and 2015.Our results show that,despite the Mediterranean influence,the natural dynamics of this forest has been mainly determined by different levels of shade tolerance.After the abandonment of grazing and cutting,Q.pyrenaica expanded rapidly due to its lower shade tolerance,whereas after canopy closure and forest densification,shade-tolerant species gained ground,particularly F.sylvatica,despite its lower drought and late-frost tolerance.If the current dynamics continue,F.sylvatica will overtake the rest of the species,which will be relegated to sites with shallow soils and steep slopes.Simultaneously,all the multi-centennial beech trees,which are undergoing a rapid mortality and decline process,will disappear.

diabetic kidney disease(dKd)is one of the complications of diabetes,affecting millions of people worldwide.The relentless progression of this condition can lead to kidney failure,requiring life-altering interventions such as dialysis or transplants.Accumulating evidence suggests that immunologic and inflammatory elements play an important role in initiating and perpetuating the damage inflicted on renal tissues,exacerbating the decline in organ function.Toll-like receptors(TLRs)are a family of receptors that play a role in the activation of the innate immune system by the recognition of pathogen-associated molecular patterns.Recent data from in vitro and in vivo studies have highlighted the critical role of TLRs,mainly TLR2 and TLR4,in the pathogenesis of dKd.In the diabetic milieu,these TLRs recognize diabetic-associated molecular signals,triggering a proinflammatory cascade that initiates and perpetuates inflammation and fibrogenesis in the diabetic kidney.Emerging non-traditional strategies targeting TLR signaling with potential therapeutic implications in dKd have been proposed.One of these approaches is the use of microRNAs,small non-coding RNAs that can regulate gene expression.This editorial comments on the results of this approach carried out in a rat model of diabetes by Wu et al,published in this issue of the World Journal of diabetes.The results of the experimental study by Wu et al shows that microRNA-630 decreased levels compared to non-diabetic rats.Additionally,microRNA-630 exerted anti-inflammatory effects in the kidneys of diabetic rats through the modulation of TLR4.These findings indicate that the microRNA-630/TLR4 axis might represent a pathological mechanism of dKd and a potential therapeutic target capable of curbing the destructive inflammation characteristic of dKd.

This paper proposes an online framework to characterize demand response(dR) over time. The proposed framework facilitates obtaining and updating the daily consumption patterns of customers. The essential concept of response profile class(RPC) is introduced for characterization and complemented by the measure of the variability in customer behavior. This paper uses a modified version of the incremental clustering by fast search and find of density peaks(CFSFdP) algorithm for daily profiles, considering the multivariate normal kernel density estimator and incremental forms of the davies-Bouldin(i dB) and Xie-Beni(i XB) validity indices. Case studies conducted using real-world and simulated daily profiles of residential and commercial Chilean end-users have demonstrated how the proposed framework can continuously characterize dR. The proposed framework is proven to achieve realistic customer models for effective energy management by estimating the customer response to price signals at the distribution system operator(dSO) level.

In rural territories, the communities use energy sources based on fossil fuels to supply themselves with electricity, which may address two main problems: greenhouse gas emissions and high fuel prices. Hence, there is an opportunity to include renewable resources in the energy mix. This paper develops an optimization model to determine the optimal sizing, the total annual investment cost in renewable generation, and other operating costs of the components of a hybrid microgrid. By running a k-means clustering algorithm on a meteorological dataset of the community under study, the hourly representative values become input parameters in the proposed optimization model. The method for the optimal design of hybrid microgrid is analyzed in six operating scenarios considering:(1) 24-hour continuous power supply;(2) load shedding percentage;(3) diesel power generator(genset) curtailment;(4) the worst meteorological conditions;(5) the use of renewable energy sources including battery energy storage systems(BESSs);and(6) the use of genset. A mathematical programming language(AMPL) tool is used to find solutions of the proposed optimization model. Results show that the total costs of microgrid in the scenarios that cover 100% of the load demand(without considering the scenario with 100% renewables) increase by over 16% compared with the scenario with genset operation limitation. For the designs with power supply restrictions, the total cost of microgrid in the scenario with load shedding is reduced by over 27% compared with that without load shedding.

BACKGROUNd The influence of Helicobacter-pylori(H.pylori)infection and the characteristics of gastric cancer(GC)on tumor-infiltrating lymphocyte(TIL)levels has not been extensively studied.Analysis of infiltrating-immune-cell subtypes as well as survival is necessary to obtain comprehensive information.AIM To determine the rates of deficient mismatch-repair(dMMR),HER2-status and H.pylori infection and their association with TIL levels in GC.METHOdS Samples from 503 resected GC tumors were included and TIL levels were evaluated following the international-TILs-working-group recommendations with assessment of the intratumoral(IT),stromal(ST)and invasive-border(IB)compartments.The density of Cd3,Cd8 and Cd163 immune cells,and dMMR and HER2-status were determined by immunohistochemistry(IHC).H.pylori infection was evaluated by routine histology and quantitative PCR(qPCR)in a subset of samples.RESULTS dMMR was found in 34.4%,HER2+in 5%and H.pylori-positive in 55.7%of samples.High IT-TIL was associated with grade-3(P=0.038),while ST-TIL with grade-1(Pd no-recurrence(P=0.003).dMMR was associated with high TIL levels in the ST(P=0.019)and IB(P=0.01)compartments,and STCd3(P=0.049)and ST-Cd8(P=0.05)densities.HER2-was associated with high IT-Cd8(P=0.009).H.pylorinegative was associated with high IT-TIL levels(P=0.009)when assessed by routine-histology,and with high TIL levels in the 3 compartments(P=0.002-0.047)and Cd8 density in the IT and ST compartments(P=0.001)when assessed by qPCR.A longer overall survival was associated with low IT-Cd163(P=0.003)and Cd8/Cd3(P=0.001 in IT and P=0.002 in ST)and high IT-Cd3(P=0.021),ST-Cd3(P=0.003)and Cd3/Cd163(P=0.002).CONCLUSION TIL levels were related to dMMR and H.pylori-negativity.Low Cd8/Cd3 and high Cd163/Cd3 were associated with lower recurrence and longer survival.

Colorectal cancer(CRC)remains one of the most commonly diagnosed and deadliest types of cancer worldwide.CRC displays a desmoplastic reaction(dR)that has been inversely associated with poor prognosis;less dR is associated with a better prognosis.This reaction generates excessive connective tissue,in which cancer-associated fibroblasts(CAFs)are critical cells that form a part of the tumor microenvironment.CAFs are directly involved in tumorigenesis through different mechanisms.However,their role in immunosuppression in CRC is not well understood,and the precise role of signal transducers and activators of transcription(STATs)in mediating CAF activity in CRC remains unclear.Among the myriad chemical and biological factors that affect CAFs,different cytokines mediate their function by activating STAT signaling pathways.Thus,the harmful effects of CAFs in favoring tumor growth and invasion may be modulated using STAT inhibitors.Here,we analyze the impact of different STATs on CAF activity and their immunoregulatory role.
