Single Positive Core Prostate Cancer Has Less Aggressive Pathologic Features than Multiple Positive Core Prostate Cancer but Should It Still Be Considered an Indolent Tumor?

Introduction: A single positive core (SPC) in a prostate biopsy is usually associated with indolent prostate cancer (PCa), and is considered one active surveillance criteria. To determine if a SPC should qualify a patient for surveillance, we compared the pathological findings for SPC and multiple positive core in a matched population who underwent radical prostatectomy (RP). Material and Methods: We evaluated 373 SPC patients who underwent RP (Group 1) and 375 consecutive cases with multiple positive core (Group 2) who were matched according to age, prostate weight, PSA level and clinical stage. In addition to preoperative data and epidemiological characteristics, we compared the rates of positive surgical margins (PSMs), extraprostatic extension (EPE) and seminal vesicle invasion (SVI) according to the Gleason scores (GSs) of the biopsies. Results: Both groups were similar according age, PSA level, prostate weight and clinical stage. Group 1 had a lower PSM rate (20.9% vs 37.6%, p < 0.001), less EPE (10% vs 26%, p < 0.001) and SVI (6% vs 13.3%, p = 0.006). The PSM, EPE and SVI rates increased with increasing GS in both groups. In Group 1, a patient with a GS ≥ 8 was 3.5 times more likely to have a PSM than a patient with a GS ≤ 6 (p = 0.03);with no difference in Group 2 (p = 0.162). There were no correlations between the EPE and SVI rates and the GS in Group 1 (p = 0.273 and p = 0.95, respectively). However, in Group 2, we observed a higher rate of EPE among GS 7 than among GS ≤ 6, OR = 3.1 (p < 0.001). We also observed a higher rate of SVI among GS 7 than among GS ≤ 6 in Group 2, OR = 3.1 (p = 0.004). Conclusion: SPC PCa have reduced rates of PSM, EPE and SVI relative to multiple positive core. However, these pathologic findings were observed in 6.0% - 20.9% of SPC, especially in undifferentiated tumors. Management of Phylloid Sarcomas: A Retrospective Study of 12 Cases

The certification of a theoretical model for quantum measurements is of vital importance in quantum information tasks. However, such certification usually relies on extra assumptions on the state-generating devices. Recently, the device-independent（DI） way of testing quantum measurements has been introduced and the set of input-output correlation compatible with any qubit measurement has been fully characterized. Here we implement two kinds of important measurements in quantum information science, the mutually unbiased bases（MUBs） and symmetric informationally complete（SIC） measurements, with practical photonic apparatus, and experimentally test them in a DI way with the theoretical hypotheses and their reconstructed measurements using conventional quantum detector tomography. In particular, we develop and demonstrate a novel way of characterizing quantum measurements motivated by the DI test, relaxing assumptions on the preparation apparatus. Our results open a new avenue in the fundamental issue of characterizing quantum measurements, and pave the way to novel tools for quantum information science and applications.

Background: Phylloid sarcomas are rare. There is not enough data to codify the management. Objectives: The objective was to study the clinical and therapeutic aspects and the fate of patients after a follow-up of at least 4 years. Thus contributing to the limited body of knowledge on these tumors. Methods: a retrospective analysis of the files from 2013 to 2017 was carried out and patients were followed up until 2021at Hassan II Hospital. Epidemiological, clinical and therapeutic aspects were studied. Survival was calculated using the Kaplan-Meier method. Results: We collected 12 charts of patients treated for phyllodeal sarcoma from 2013 to 2017. The median age was 43 years. The circumstance of discovery was marked by the presence of nodule in all patients. The coupled echo-mammography examination classified the nodules, ACR 4 in 7 patients and ACR 3 in 3 and ACR 5 in 2 patients. Histological examination revealed a phylloid sarcoma in 11 patients and a borderline phylloid tumor in 1 patient. All patients had radical surgery with positive margins in 2 patients, 16.66%. One patient had revision surgery. Histological examination of the surgical specimens showed phylloid sarcoma on all specimens. All patients had adjuvant radiotherapy with doses of 50 Gy in 25 fractions of 2 Gy and a boost of 10 Gy was done in one patient. The median spread of radiotherapy was 37 days. Grade 1 and 2 skin toxicities were noted in 5 and 3 patients respectively. The median time from surgery to radiotherapy was 2.95 months. 3 patients relapsed after 13.6 months of follow-up. Background/Purpose: Neurologically impaired (NI) children have an increased incidence of gastroesophageal reflux and many will require surgery. Methods: The case notes of 50 NI children who underwent total oesophagogastric dissociation (TOGD) were reviewed. Thirty-four were done as a primary procedure,and 16 were rescues for failed fundoplications. Results: There was no operative mortality. Morbidity consisted of 1 subphrenic collection,1 oesophagojejunal dehiscence and 2 stenoses that responded to dilatation,and 2 bowel obstructions. In 1 case,partial gastric resection was needed because of transhiatal herniation of stomach. Gastrostomy feeding was established by 3 to 5 days. The mean hospital stay was 10.9 days. At 4 months to11 years of follow-up,there was no recurrence of reflux. Children who could swallow enjoyed oral feeds. Their general health and weight SD scores improved. Food aspiration,chest infections,and hospitalizations were reduced,with an improvement in quality of life. There were 5 late deaths in the “ primary” and 7 in the “ rescue” group from deterioration in their original condition. Conclusion: Total oesophagogastric dissociation is a safe and versatile procedure without immediate mortality and limited surgery-related morbidity. Review of our practice suggests TOGD should be considered as a primary procedure in severely NI children with gastroesophageal reflux and significant oropharyngeal incoordination and dependence on enteral tube feeding. Rescue TOGD carries a greater morbidity because of previous surgery with consequent difficult dissection,poor oesophageal tissue,and higher incidence of vagal nerve injury.

AIM To study exercise capacity and determinants of early peak oxygen consumption(VO_(2peak)) in a cohort of de novo heart transplant(HTx) recipients. METHODS To determine possible central(chronotropic responses, cardiopulmonary and hemodynamic function) and peripheral factors(muscular exercise capacity and body composition) predictive of VO_(2peak), a number of different measurements and tests were performed, as follows: Cardiopulmonary exercise testing(CPET) was performed mean 11 wk after surgery in 81 HTx recipients > 18 years and was measured with breath by breath gas exchange on a treadmill or bicycle ergometer. Metabolic/respiratory measures include VO_(2peak) and VE/VCO2 slope. Additional measures included muscle strength testing, bioelectrical impedance analysis, echocardiography, blood sampling and health-related quality of life. Based on the VO_(2peak)(mL/kg per minute) median value, the study population was divided into two groups defined as a low-capacity group and a high-capacity group. Potential predictors were analyzed using multiple regression analysis with VO_(2peak)(L/min) as the dependent *** The mean ± standard deviation(SD) age of the total study population was 49 ± 13 years, and 73% were men. This de novo HTx cohort demonstrated a median VO_(2peak) level of 19.4 mL/kg per min at 11 ± 1.8 wk postHTx. As compared with the high-capacity group, the low-capacity group exercised for a shorter time, had lower maximal ventilation, O_2 pulse, peak heart rate and heart rate reserve, while the VE/VCO_2 slope was higher. The low-capacity group had less muscle strength and muscular exercise capacity in comparison with the highcapacity group. In order of importance, O_2 pulse, heart rate reserve, muscular exercise capacity, body mass index, gender and age accounted for 84% of the variance in VO_(2peak)(L/min). Surgical resection is the only option of cure for patients with metastatic colorectal cancer(CRC). However, the risk of recurrence within 18 mo after metastasectomy is around 75% and the liver is the most frequent site of relapse. The current international guidelines recommend an adjuvant therapy after surgical resection of CRC metastases despite the lower level of evidence(based on the quality of studies in this setting). However, there is still no standard treatment and the effective role of an adjuvant therapy remains controversial. The aim of this review is to report the state-of-art of systemic chemotherapy and regional chemotherapy with hepatic arterial infusion in the management of patients after resection of metastases from CRC, with a literature review and meta-analysis of the relevant randomized controlled trials.

试验旨在研究意大利大白猪(ITLW)窝产仔数候选基因的多态性。试验选取6个农场120头经产母猪,对10个基因的10个SNPs进行了基因分型,在其他猪中这10个基因的多态性与窝产仔数性状间具有相关性,是功能和/或染色体定位的候选基因。结果表明,10个SNPs中的CLGN、pDAZL和RFN4没有发现基因分型,剩余具有多态性的7个标记的小等位基因频率在0.10(AFP)～0.48(RBP4)之间。由于所选位点中存在基因多态性,因此,AFP、BMPR1B、CXCL10、ESR2、GNRHR、MAN2B2、RBP4可作为候选基因用于意大利大白猪窝产仔数性状的相关性研究。

Urea cycle disorders:A case report of a successful treatment with liver transplant and a literature review

The urea cycle is the final pathway for nitrogen metabolism. Urea cycle disorders(UCDs) include a variety of genetic defects, which lead to inefficient urea synthesis. Elevated blood ammonium level is usually dominant in the clinical pattern and the primary manifestations affect the central nervous system. Herein, we report the case of a 17-year-old girl who was diagnosed with UCD at the age of 3. Despite a controlled diet, she was hospitalized several times for acute attacks with recurrent life risk. She came to our attention for a hyperammonemic episode. We proposed an orthotopic liver transplant(OLT) as a treatment; the patient and her family were in complete agreement. On February 28, 2007, she successfully received a transplant. Following the surgery, she has remained well, and she is currently leading a normal life. Usually for UCDs diet plays the primary therapeutic role, while OLT is often considered as a last resort. Our case report and the recent literature data on the quality of life and prognosis of traditionally treated patients vs OLT patients, support OLT as a primary intervention to prevent life-threatening acute episodes and chronic mental impairment.

Objective:To test the hypothesis that oxidative stress signaling contributes to post-transplant endothelial dysfunction and hypertension in pediatric post-transplant hypertension. Study design:This study evaluated in 16 pediatric renal transplant patients,divided in two groups based on the presence of post transplant hypertension,the oxidative stress status measuring the gene expression (reverse transcription-polymerase chain reaction)of two major oxidative stress-related proteins,p22phox and heme oxygenase-1(HO-1). Total plasma antioxidant power(ELISA)was also evaluated. Results:Mononuclear cell p22phoxgene expression was higher in hypertensive patients compared with the normotensive group (0.91 ±0.06 vs 0.79 ±0.08 densitometric units,P < 0.02),whereas HO-1 RNA production and total plasma antioxidant power were higher in the normotensive group (0.38 ±0.04 vs 0.20 ±0.11 d.u.,P < 0.006,and 1189.35±145.75 vs 772.71 ±196.03 μmol/L,P < 0.01,respectively).Conclusions:Oxidative stress is associated with post-transplant hypertension in hypertensive pediatric kidney-transplant patients,who therefore are at risk of oxidative stress-induced organ damage. They might benefit from treatments addressing not only hypertension but also oxidant-related complications.
