Circulating insulin-like growth factor-binding protein 3 as prognostic biomarker in liver cirrhosis

AIM: To investigate the prognostic significance of insulin-like growth factor-binding protein 3(IGFBP-3) in patients with ***: Prospective study that included two cohorts: outpatients with stable cirrhosis(n = 138) and patients hospitalized for acute decompensation(n = 189). Development of complications, mortality or liver transplantation was assessed by periodical phone calls and during outpatient visits. The cohort of stable cirrhosis also underwent clinical and laboratory evaluation yearly(2013 and 2014) in predefined study visits. In patients with stable cirrhosis, IGFBP-3 levels were measured at baseline(2012) and at second re-evaluation(2014). In hospitalized subjects, IGFBP-3 levels were measured in serum samples collected in the first and in the third day after admission and stored at-80 ℃. IGFBP-3 levels were measured by ***: IGFBP-3 levels were lower in hospitalized patients as compared to outpatients(0.94 mcg/mL vs 1.69 mcg/m L, P ciated with lower IGFBP-3 levels(1.44 mcg/mL vs 1.74 mcg/m L, P = 0.027). The Kaplan-Meier transplant-free survival probability was 88.6% in patients with IGFBP-3 ≥ 1.67 mcg/mL and 72.1% for those with IGFBP3 ciated with creatinine, INR, SpO_2/FiO_2 ratio and IGFBP-3 levels in the logistic regression. The 90-d transplant-free survival probability was 80.4% in patients with IGFBP-3 ≥ 0.86 mcg/mL and 56.1% for those with IGFBP3 ciated with worse outcomes in patients with cirrhosis, and might represent a promising prognostic tool that can be incorporated in clinical practice.

Decomposition and stabilization of organic matter in an old-growth tropical riparian forest:effects of soil properties and vegetation structure

Haplotypic Variation of Chrysoperla externa （Hagen）（Neuroptera： Chrysopidae） in Different Geographic Locations

Chrysoperla externa is found in environment, their populations may experience several Brazilian agroecosystems and feeds on many agricultural pests. In each different environmental pressures that can favor the emergence of genetic mutations and interfere in insect＇s ecological plasticity. Such genetic phenomena can be observed through the use of molecular markers, in particular mitochondrial genes, which are easy to handle and extract. The aim of this work was to study haplotype diversity of Chrysoperla externa in the cities of Jaboticabal （SP）, Brotas （SP）, Rifaina （SP）, Sao Carlos （SP）, Sao Jose dos Campos （SP） and Sao Sebastiao do Paraiso （MG）, and evaluate the degree of genetic differentiation between these populations. Two mitochondrial genes were sequenced to compare these populations. It was found that there were eight haplotypes, six mutations for cytochrome c oxidase subunit 1 （COl） gene and a greater haplotype diversity was found in Brotas and Sao Sebastiao do Paraiso. There were four mutations and six haplotypes for 16S gene and the highest haplotype diversity was found in the city of San Sebastiao do Paraiso. No pattern of haplotype distribution for the populations analyzed was found. The genetic distance between populations of C. externa was not significant, showing that they are genetically compatible.

Cutaneous leishmaniasis in an indigenous infant with Down’s syndrome: A case report

Rationale: Leishmaniasis, caused by the protozoan Leishmania and transmitted by sandflies, is endemic to the tropical and subtropical areas of Brazil. It is one of the neglected tropical diseases in the world. Patient concerns: A 20-month-old indigenous infant with severe malnutrition and Down’s syndrome presented with a facial ulcer for 5 months. Diagnosis: Giemsa staining of scraped ulcer tissues indicated the presence of the amastigote form of Leishmania sp., and positive Montenegro’s intradermal test helped in diagnosing the condition as cutaneous leishmaniasis. Interventions: The child was hospitalized and received intravenous treatment for cutaneous leishmaniasis(1 mg/kg/day of amphotericin B). Outcomes: The condition was cured with amphotericin B(1 mg/kg/day for 14 d). Lessons: Because of infanticide practices in indigenous cultures, indigenous infants with Down’s syndrome rarely survive. Thus, no similar case has been reported in the literature.

Limitations and possibilities on health assessment to women in violence situation

The aim was to know how health assessment to women in violence situation is developed. Literature review on LILACS and MEDLINE databases was conducted in April 2013 with the descriptors: “domestic violence” and “women’s health”, on a 1994-2012 timeframe. Statistics characterization and content theme analysis of the scientific production were developed. Results showed that the assessment is permeated by institutional limitation and an approach strictly clinical that makes identification and diagnosis of violence difficult, reinforcing invisibility in health care. Professional assessment is influenced by socio-cultural and the naturalization of the phenomena, which is not considered a public health issue. Technical knowledge is insufficient, making the possibility of promoting violence cycle rupture difficult. However, we envisage reception as a possibility to assess female demands. We conclude the need to include the theme in health undergraduate courses and also the urgency of this learning experience to support a multidisciplinary and intersectoral work web. Thus, rethinking assessment as a way of (re)organizing how health care is structured in order to compose an assessment web to women and guarantee reception of their demands is needed. Else, constructing competence allied to coping public policy to the problem and guaranteeing a human and full assistance will stay only on the academic field, constituting itself as a limit on protecting life of these women and their families.

Native and exotic seed dispersal by the stone marten(Martes foina):implications for the regeneration of a relict climactic forest in central Portugal

The stone marten(Martes foina)is a carnivorous mammal that often consumes fleshy fruits,thus potentially promoting seed *** present study was developed in Bussaco National Forest,central Portugal,and aimed to assess the potential role of the stone marten in dispersing native and exotic plants in different forest landscape *** from stone marten scats and fleshy fruits were collected monthly and were thereafter identified and sowed in a nursery,following a randomized experimental *** emergence was monitored *** linear models were used to test for differences in time and success of emergence between seeds from scats and fruits of 3 native species(Rubus ulmifolius,Arbutus unedo and Celtis australis)and 1 exotic plant species with invasive behavior(Prunus laurocerasus).Fruit consumption by the stone martens significantly increased and accelerated the germination of the native *** but had no effect on the other 2 native species or on the invasive *** suggested that stone martens contribute to gene flow and forest regeneration by dispersing native plant ***,although the germination was not enhanced in the invasive species,the preference of stone martens for these fruits may potentially contribute to the proliferation of *** study represents a contribution to better understanding the fauna and flora interactions,enabling for a more conscious and effective decision-making in forest management.

Fecal marker levels as predictors of need for endoscopic balloon dilation in Crohn's disease patients with anastomotic strictures

AIM To evaluate the accuracy and best cut-off value of fecal calprotectin(FC) and fecal lactoferrin(FL) to predict disease recurrence in asymptomatic patients presenting with anastomotic strictures. METHODS This was a longitudinal single tertiary center study based on prospectively collected data(recorded in a clinical database created for this purpose) performed between March 2010 and November 2014. Crohn's disease(CD) patients with anastomotic stricture who submitted to postoperative endoscopic evaluation were included. Stools were collected on the day before bowel cleaning for FC and FL. Endoscopic balloon dilation(EBD) was performed if the patient presented an anastomotic stricture not traversed by the colonoscope, regardless of patients' symptoms. Successful dilation was defined as passage of the colonoscope through the dilated stricture into the neotermimal *** recurrence was defined as a modified Rutgeerts score of ≥ i2 b. RESULTS In a total of 178 patients who underwent colonoscopy, 58 presented an anastomotic stricture, 86% were asymptomatic, and 48(54% male; median age of 46.5 years) were successfully dilated. Immediate success rate was 92% and no complications were recorded. FC and FL levels correlated significantly with endoscopic recurrence(P < 0.001) with an optimal cut-off value of 90.85 μg/g(sensitivity of 95.5%, specificity of 69.2%, positive predictive value(PPV) of 72.4%, negative predictive value(NPV) of 94.7% and accuracy of 81%] for FC and of 5.6 μg/g(sensitivity of 77.3%, specificity of 69.2%, PPV of 68%, NPV of 78.4% and accuracy of 72.9%) for *** Fecal markers are good predictors of CD endoscopic recurrence in patients with asymptomatic anastomotic stricture. FC and FL may guide the need for EBD in this context.

Evaluation of DNA Extraction Methods for Detection of <i>Leishmania</i>by Polymerase Chain Reaction

There are several methods used to obtain DNA from cells;however, the quantity, integrity, and purity of DNA vary among the methods, which may interfere with the polymerase chain reaction (PCR) results. The objective was to determine the most efficient and cost-effective method that provides the best DNA yield and PCR results. Three methods of DNA isolation were compared: 20% sodium dodecyl sulfate (SDS), guanidine isothiocyanate-phenol-chloroform (GTPC), and DNA extraction using a commercial kit (GE Healthcare GenomicPrep Blood DNA Isolation KitTM). Human peripheral blood samples were inoculated with 104 promastigotes of Leishmania infantum. DNA was quantified and PCR was performed with 13A/13B primers. The results showed that a higher DNA yield was obtained using the GTPC technique (214.51 ng/μL), followed by SDS (26.16 ng/μL) and the commercial kit (10.99 ng/μL). We concluded that while all of the techniques were effective for obtaining DNA, the GTPC method provided the best yield and the brightest bands.
