Objective:To assess the pharmaceutical quality of different brands of metronidazole manufactured,available and prescribed by the doctors of Peshawar,***:Different film-coated metronidazole products manufactured by multinational and national pharmaceutical industries and prescribed by the doctors were collected from Peshawar,Khyber Pakhtunkhwa,*** hysico-chemical properties were assessed,which included friability,hardness,disintegration,dissolution and weight variation tests according to the the United States Pharmacopoeia(USP)***:All the brands showed a average weight of 5.74 g for 13 tablets(products A-M).Product J had the highest weight and products D and E failed the hardness,friability and disintegration time according to USP *** M,K and G,L failed disintegration test and dissolution test,*** variation of all the products proved statistically that they were in accordance with the standards of ***:The study suggested that products A,F,H and J passed all the tests according to the USP standards while the rest of them did not fulfill the standards specification.

BACKGROUND:A perennial challenge faced by clinicians and made even more relevant with the global obesity epidemic,difficult intravenous access(DIVA)adversely impacts patient outcomes by causing significant downstream delays with many aspects of diagnoses and *** most published DIVA strategies are limited to various point-of-care ultrasound techniques while other“tricks-of-the-trade”and pearls for overcoming DIVA are mostly relegated to informal nonpublished material,this article seeks to provide a narrative qualitative review of the iterature on DIVA and consolidate these strategies into a practical ***:We conducted a literature search on PubMed using the keywords“difficult intravenous access”,“peripheral vascular access”and“peripheral venous access”and searched emergency medicine and anaesthesiology resources for relevant *** strategies were then categorized and incorporated into a DIVA ***:We propose a Vortex approach to DIVA that is modelled after the Difficult Airway Vortex concept:starting off with standard peripheral intravenous cannulation(PIVC)techniques,progressing sequentially on to ultrasound-guided cannulation and central venous cannulation and finally escalating to the most invasive intraosseous access should the patient be in extremis or should best efforts with the other lifelines ***:DIVA is a perennial problem that healthcare providers across various disciplines will be increasingly challenged *** is crucial to have a systematic stepwise approach such as the DIVA Vortex when managing such patients and have at hand a wide repertoire of techniques to draw upon.

Training of emergency physicians through an emergency medicine residency program takes 5 years,that is,3 years in junior residency and 2 years in senior *** this period,residents will be exposed to a variety of educational methodologies and meet a spectrum of faculty,supervisors,and teachers,who will have different personalities,styles,and approaches to teaching and nurturing *** is important to ensure the maintenance of psychological safety for these residents throughout their training journey and into the future years of practice s an emergency ***,interactions(which involve questioning),and presentations will be an important part of this training *** article looks at two modes of questioning:advocacy inquiry and circular questioning,which can be applied as *** two methods are examples in which faculty may consider adopting in their many interactions,follow-up,feedback,tutorials,facilitation,partnerships,and counseling sessions with *** two techniques offer options to maintain psychological safety,which can facilitate learners sharing and opening *** can be included in the faculty’s armamentarium of questioning techniques and applied where applicable.

From the early past to the present, biotechnologies have produced the ability to genetically transform a wide variety of plant species. The plant transformation technologies have changed the face of agriculture and plant biology. Plant genetic transformation is one of the key technologies for crop improvement in addition to emerging approach for producing recombinant proteins in plants. Both plastid genomes and plant nuclear can be genetically modified. Until now, essential functional differences between the prokaryotic-like genome of the plastid and the eukaryotic genome of the plant cell nucleus will have an impact on characteristics of transgenic organism. Thus, the main goals are to generate transgenic plants with the traits of interest as well as minimizing the amount of transgenic DNA in plants while maximizing stability of gene expression and trait performance. In this review, two broad groups of gene delivery methods will be discussed namely, (bilogical and physical methods) and subsequently there applications for improving disease resistance will be discussed.

Objective： This study evaluates the phytochemical constituents, antioxidant and anti-inflammatory activity, cytotoxicity, and inhibitory activity against carbohydrate metabolism of extracts from Ocotea bullata stem bark. Methods： Hexane, ethyl acetate, methanol and water were used to extract the air-dried sample. The phytochemical investigation and antioxidant assays were carried out on the extracts using standard procedures. The antidiabetic and anti-inflammatory potentials were evaluated using α-amylase, α-glucosidase and 5-1ipoxygenase enzymes respectively. Vero cells were employed to determine the cytotoxicity of the extracts. Results： The ethyl acetate extract showed higher phenolic contents （8.97 mg/g gallic acid） while metha- nol displayed higher flavonoid （36.06 mg/g quercetin） and flavonol （153.44 mg/g rutin） contents than other extracts. Hexane extract had the greatest capacity to scavenge 1,1-diphenyl-2-picryl-hydrazyl （0.19mg/mL）, hydroxyl （25.77mg/mL） and 2,2-azino-bisC3-ethylbenzothiazoline）-6-sulfonic acid （0.07 mg/mL） radicals, while ethyl acetate extract exhibited stronger inhibition （P 〈 0.05） against superoxide anion （0.41 mg/mL） and ferric ion-reducing power （2.36 mg/mL） compared to other extracts and standards. Aqueous extract （27.02 mg/mL） exhibited strong metal-chelating activity （P 〈 0.05） compared to other extracts and gallic acid. The aqueous extract demonstrated the greatest inhibition of α-glucosidase （1.45 mg/mL） and α-amylase （2.43 mg/mL） compared to other extracts and acarbose. There were no significant differences （P 〈 0.05） in half-maximum inhibitory concentration （ICso） values of all tested extracts and indomethacin in the inhibition of 5-lipoxygenase activity. The aqueous extract was nontoxic to Vero cells with an IC50 value of 0.38 mg/mL. Conclusion： O. bullata stem bark contains active phytochemicals with diverse pharmacological potentials that could be beneficial in managing diabetes and inflammation.

BACKGROUND:This study aimed to review bicycle-related injuries during the COVID-19 pandemic to assist with reinforcement or implementation of new policies for injury ***:This is a retrospective descriptive analysis of injuries sustained during cycling for patients 18 years old and above who presented to Singapore General Hospital from January to June *** records were reviewed and *** analyses were used to summarize patient characteristics,and differences in characteristics subgrouped by triage acuity and discharge status were ***:The study included 272 patients with a mean age of 43 years and a male predominance(71.7%).Most presented without referrals(88.2%)and were not conveyed by ambulances(70.6%).Based on acuity category,there were 24(8.8%)Priority 1(P1)patients with 7 trauma activations,174(64.0%)and 74(27.2%)P2 and P3 patients *** most common injuries were fractures(34.2%),followed by superficial abrasion/contusion(29.4%)and laceration/wound(19.1%).Thirteen(4.8%)patients experienced head injury and 85 patients(31.3%)were documented to be wearing a *** majority occurred on the roads as traffic accidents(32.7%).Forty-two patients(15.4%)were admitted with a mean length of stay of 4.1 d and 17(6.3%)undergone surgical *** of 214(78.7%)discharged patients,no re-attendances or mortality were *** the subgroup analysis,higher acuity patients were generally older,with higher proportions of head injuries leading to ***:Our study highlights significant morbidities in bicycle-related *** is also a high proportion of fractures in the young healthy male *** prevention is paramount and we propose emphasizing helmet use and road user safety.

How consultation-relational empathy and demographics influence satisfaction with primary antenatal health care:evidence from rural Nigeria

Objectives:This study aims to explore the role of consultation and relational empathy(CARE)and socio-demographic profile as predictors of patients'satisfaction among pregnant women attending antenatal care(ANC)in selected public primary health facilities in rural sub-districts of Saki,Oyo State,***:This cross-sectional study recruited a total of 582 pregnant women in four primary health *** mothers responded to a questionnaire covering demographic characteristics,clinic visitation,CARE,and patients'*** data was analyzed using descriptive,univariate and multivariate statistical tools.P<0.05 level was considered statistically ***:Findings revealed that the participants had a mean age of 23.15±5.23 *** pattern of patients'satisfaction revealed that 13.40%(78/582)were highly satisfied with the health care received compared to 86.60%(504/582)with moderate to low satisfaction ratings with *** satisfied patients placed higher value on technical quality,interpersonal relation,communication,financial aspects,time spent and accessibility convenience(P=0.001).Multiple linear regression model indicated that relational empathy(β=0.28,P<0.001),2nd trimester of pregnancy(β=0.13,P=0.01),age categories of 30-<40 years(β=-0.12,P=0.02),and 40-<50 years(β=-0.11,P=0.04),religious affiliation(β=0.10,P=0.03)predicted satisfaction with ***:Low patients'satisfaction with antenatal healthcare services resulting from hostile attitudes from health workers has implication for the delivery and acceptability of services offered to eradicate maternal mor-tality ***,regular training and re-training of health care personnel in frequent contact and interaction with patients will go a long way in reducing untoward work attitude and maternal mortality in rural ANC centers globally.

Assessment of Knowledge and Attitude in a Sample of Patients with Rheumatoid Arthritis and Its Association with Disease Activity and Severity: A Cross-Sectional Study

Background: Rheumatoid arthritis (RA) is a chronic and disabling disease that has a major impact on the lives of patients. Objectives: To test the patients’ knowledge about their disease, its treatment, its complications, and if this affects severity of the disease as measured by the disease activity index (DAS 28). Patients and Methods: A sample of 100 patients with RA who met the Criteria of American College of Rheumatology for RA agreed to participate in this study over the period September 2011-March 2012. Patients’ data were obtained by personal interview. The questionnaire included demographic characteristics and the patients’ knowledge about their disease. The disease activity was measured using standard (DAS28). Results: Thirty three percent of the patients didn’t have an idea about their disease, 20% didn’t know the reason for the investigations, 49% didn’t know the treatment and 40% didn’t know the side effects of their medications. Most patients had a high disease activity index, and there was a poor correlation between patients’ educational level and the disease activity. Conclusions: Neither the educational level nor the frequency of hospital admissions had effects on the knowledge about this disease. As most patients had a high disease activity, a better knowledge may improve disease control and prevent complications.
