Introduction:A retrospective study based on sentinel surveillance was conducted in 10 provinciallevel administrative divisions(PLADs)in China to enhance the understanding of the epidemiological characteristics of human parainfluenza viruses(HPIVs).Methods:From January 2019 to June 2023,respiratory specimens were collected from individuals with acute respiratory infections(ARIs)and screened for four HPIVs serotypes and other common respiratory viruses using multiplex real-time polymerase chain reaction(PCR).This study analyzed the association of HPIVs infections with seasonal patterns,geographical distribution,demographic profiles,clinical features,and co-infection ***:During the study period,a total of 12,866 ARIs were *** overall detection rate of HPIVs was 6.15%,varying from 5.04%in 2022 to 9.70%in *** median age of HPIVs-infected patients was 3 ***2 was more prevalent among individuals aged 5–17 years(42.57%),while HPIV4 was more common in those over 65 years(12.24%).HPIV3(54.16%)and HPIV1(27.18%)were the predominant serotypes,and their prevalence exhibited significant seasonal fluctuations postcoronavirus disease 2019(COVID-19)*** peak of HPIV3 shifted three months later in 2020 compared to 2019 and returned to a summer peak *** peaks of HPIV1 were observed in 2021 following the peak of ***,coinfections were frequent in HPIVs cases(overall rate:22.12%),with human rhinovirus being the most common co-infecting ***:The prevalence of HPIVs in China was predominantly due to HPIV3 and HPIV1,and their seasonal patterns were altered by pandemic ***,continuous surveillance of HPIVs is essential.

Wastewater surveillance(WWS)can leverage its wide coverage,population-based sampling,and high monitoring frequency to capture citywide pandemic trends independent of clinical *** we conducted a nine months daily WWS for severe acute respiratory syndrome coronavirus 2(SARSCoV-2)from 12 wastewater treatment plants(WWTPs),covering approximately 80%of the population,to monitor infection dynamics in Hong Kong,*** found that the SARS-CoV-2 virus concentration in wastewater was correlated with the daily number of reported cases and reached two pandemic peaks three days earlier during the study *** addition,two different methods were established to estimate the prevalence/incidence rates from wastewater *** estimated results from wastewater were consistent with findings from two independent citywide clinical surveillance programmes(rapid antigen test(RAT)surveillance and serology surveillance),but higher than the cases number reported by the Centre for Health Protection(CHP)of Hong Kong,***,the effective reproductive number(R_(t))was estimated from wastewater measurements to reflect both citywide and regional transmission *** findings demonstrate that large-scale intensive WWS from WWTPs provides cost-effective and timely public health information,especially when the clinical surveillance is inadequate and *** approach also provides insights into pandemic dynamics at higher spatiotemporal resolutions,facilitating the formulation of effective control policies and targeted resource allocation.

老年患者肺部感染病原菌分布及耐药性分析

目的探讨老年患者肺部感染病原菌分布特点、耐药性及血清降钙素原、C-反应蛋白在不同程度老年肺部感染中的诊断价值,为临床研究提供参考依据。方法选取本院接诊的133例老年肺部感染患者及同期100例中青年肺部感染患者为本次研究对象。收集电子病历资料,对比两组患者临床特征。采集患者痰液标本,进行菌种鉴定及药敏试验。依据肺炎严重程度指数判定患者感染严重程度,测定患者血清降钙素原、C-反应蛋白水平,对比中度与重度肺部感染患者血清降钙素原、C-反应蛋白水平,采用受试者工作特征(ROC)曲线分析其单独检测及联合检测对老年肺部感染的诊断价值。结果对比老年组患者与中青年组患者临床资料,两组患者性别占比差异无统计学意义(P>0.05),合并基础疾病、典型呼吸系统感染表现、病变部位为肺下野患者占比及治疗时间差异有统计学意义(P<0.05)。133例老年肺部感染患者共培养分离出病原菌139株,其中革兰阴性菌共83株(59.71%,83/139)、革兰阳性菌共46株(33.09%,46/139)、真菌共10株(7.19%,10/139)。铜绿假单胞菌、鲍曼不动杆菌、肺炎链球菌为主要致病菌,分别占比19.42%(27/139)、11.51%(16/139)、10.79%(15/139)。革兰阴性菌对哌拉西林、庆大霉素、氯霉素的耐药率较高,对头孢吡肟、美罗培南、亚胺培南、阿米卡星的耐药率较低,未产生对多粘菌素B的耐药株。革兰阳性菌对青霉素、苯唑西林、红霉素、克林霉素、氯霉素的耐药率较高,对环丙沙星、利福平的耐药率较低,未产生对万古霉素、替考拉宁的耐药株。本次研究中,105例为中度肺部感染,28例为重度肺部感染。重度肺部感染组患者血清PCT、CRP水平显著高于中度肺部感染组患者(P<0.05)。ROC曲线分析显示,PCT水平、CRP水平单独及联合检测诊断肺部感染程度的曲线下面积(AUC)分别为0.893、0.752、0.903,联合检测诊断效能高于单独检测。结论老年肺部感染患者多合并基础疾病、病变部位以肺下野为主,典型呼吸系统感染表现不明显,治疗时间较长。病原菌主要为铜绿假单胞菌、鲍曼不动杆菌、肺炎链球菌,对临床常见抗菌药物表现出较高的耐药性。血清PCT水平、CRP水平单独及联合检测对评估老年肺部感染程度具有较高的诊断价值。

目的观察桑代克三律框架下PTP激励结合物联网交互肺康复在慢阻肺患者中的应用效果。方法选择2022年1月至12月就治于南京鼓楼医院集团宿迁医院的慢阻肺患者126例为研究对象,以组间均衡特征分为试验组63例、对照组63例。对照组遵常规式肺康复法施护,试验组接受桑代克三律框架下PTP激励结合物联网交互肺康复干预,对两组患者干预后的健康心理控制源评分、肺康复依从率及肺功能测评指标进行比较。结果试验组患者健康心理控制源评分优于对照组,肺康复依从率高于对照组,肺功能测评指标优于对照组,呼吸困难评分低于对照组,差异均有统计学意义(P<0.05)。结论将桑代克三律框架下PTP激励结合物联网交互肺康复应用于慢阻肺患者护理中,利于其健康心理控制源倾向性的优化、肺康复依从度的提升,并可改善其肺功能、减低呼吸困难程度。

急性呼吸窘迫综合征(ARDS)是一种危及生命的非心源性肺水肿。尽管肺保护性通气策略是ARDS治疗的基石,但部分患者仍无法有效缓解低氧血症或减少呼吸机相关性肺损伤。近期研究发现,通过胸壁或腹部按压可以改善肺顺应性,缓解ARDS患者吸气末期肺泡过度膨胀。胸壁压迫作为一种对仰卧患者胸部腹侧施加重物的方法,具有潜在的临床获益能力,也被认为是诊断ARDS患者肺泡过度通气的方法之一。本综述从生理及临床角度分析胸壁压迫在ARDS患者通气中的应用,并探讨其机制与临床意义,旨在为ARDS患者的治疗提供新思路和依据。

严重急性呼吸系统综合征冠状病毒2引发新型冠状病毒感染全球大流行,尽管病毒感染后主要表现为呼吸道疾病,但越来越多的研究表明新型冠状病毒能够引发多种神经系统损伤,例如脑炎、脑病、吉兰-巴雷综合征和“长新冠”综合征等。新型冠状病毒引发神经系统并发症的确切机制仍不明确,本文对新型冠状病毒感染引起相关神经系统损伤的临床表现以及可能的病理机制进行综述,有助于提前识别神经症状和预防危重症发生。
