Castleman's disease (CD) of the pancreas/peripancreas is extremely rare. The recently introduced, endoscopic ultrasonography (EUS)-guided trucut biopsy (TCB) is a useful diagnostic modality for obtaining tissue samples from peripancreatic lesions. However, its role in diagnosing CD remains unknown. We report a case of localized, peripancreatic, hyaline-vascular CD biopsied using EUS. The pathology results were initially interpreted as an extranodal, marginal-zone B-cell lymphoma. However, polymerase chain reaction (PCR) study for the IgH gene rearrangement revealed a polyclonal pattern. We also reviewed the relevant literature. To our knowledge, this is the first illustrated report on EUS-TCB findings of CD with its pathology results of EUS-TCB mimicked a B-cell lymphoma.

Objective:To examine the antiplatelet and antithrombotic activity of Rumex acetosella extract.Methods:Standard light aggregometry was used for platelet aggregation,intracellular calcium mobilization assessed using Fura-2/AM,granule secretion(ATP release)by luminometer,and fibrinogen binding to integrin α_(Ⅱb)β_(3) detected using flow cytometry.Western blotting is carried out to determine the phosphorylation of mitogen-activated protein kinase(MAPK)and phosphoinositide 3-kinase(PI3K)/Akt signaling.Results:Rumex acetosella displayed the ability to inhibit platelet aggregation,calcium mobilization,granule secretion,and fibrinogen binding to integrin α_(Ⅱb)β_(3).Rumex acetosella has also down-regulated MAPK and PI3K/Akt phosphorylation(all P<0.01).Conclusion:Rumex acetosella extract exhibits antiplatelet activity via modulating GPVI signaling,and it may protect against the development of platelet-related cardiovascular diseases.

Utility of a deep learning model and a clinical model for predicting bleeding after endoscopic submucosal dissection in patients with early gastric cancer

BACKGROUND Bleeding is one of the major complications after endoscopic submucosal dissection(ESD)in early gastric cancer(EGC)patients.There are limited studies on estimating the bleeding risk after ESD using an artificial intelligence system.AIM To derivate and verify the performance of the deep learning model and the clinical model for predicting bleeding risk after ESD in EGC patients.METHODS Patients with EGC who underwent ESD between January 2010 and June 2020 at the Samsung Medical Center were enrolled,and post-ESD bleeding(PEB)was investigated retrospectively.We split the entire cohort into a development set(80%)and a validation set(20%).The deep learning and clinical model were built on the development set and tested in the validation set.The performance of the deep learning model and the clinical model were compared using the area under the curve and the stratification of bleeding risk after ESD.RESULTS A total of 5629 patients were included,and PEB occurred in 325 patients.The area under the curve for predicting PEB was 0.71(95%confidence interval:0.63-0.78)in the deep learning model and 0.70(95%confidence interval:0.62-0.77)in the clinical model,without significant difference(P=0.730).The patients expected to the low-(<5%),intermediate-(≥5%,<9%),and high-risk(≥9%)categories were observed with actual bleeding rate of 2.2%,3.9%,and 11.6%,respectively,in the deep learning model;4.0%,8.8%,and 18.2%,respectively,in the clinical model.CONCLUSION A deep learning model can predict and stratify the bleeding risk after ESD in patients with EGC.

The bulk Al-12 Si eutectic composites were fabricated through a conventional liquid metal casting route, especially with the help of ultrafine ceramic powders made by self-propagating high-temperature synthesis (SHS) process. The SHS powders were fabricated by the chemical reaction between micro-sized SiC and Al particles at very high combustion temperatures, producing the coarse Al particles (several tens of microns) containing ultrafine SiC ceramic particles. Microstructural observation revealed that the addition of ultrafine SiC particles has a crumbling tendency of Si eutectic phase. It is suggested that the casting method combined with SHS process is promising for fabricating the Al-based MMC with ultrafine ceramic particles.

Hydroxyapatite,an essential mineral in human bones composed mainly of calcium and phosphorus,is widely used to coat bone graft and implant surfaces for enhanced biocompatibility and bone formation.For a strong implant-bone bond,the bone-forming cells must not only adhere to the implant surface but also move to the surface requiring bone formation.However,strong adhesion tends to inhibit cell migration on the surface of hydroxyapatite.Herein,a cell migration highway pattern that can promote cell migration was prepared using a nanosecond laser on hydroxyapatite coating.The developed surface promoted bone-forming cell movement compared with the unpatterned hydroxyapatite surface,and the cell adhesion and movement speed could be controlled by adjusting the pattern width.Live-cell microscopy,cell tracking,and serum protein analysis revealed the fundamental principle of this phenomenon.These findings are applicable to hydroxyapatite-coated biomaterials and can be implemented easily by laser patterning without complicated processes.The cell migration highway can promote and control cell movement while maintaining the existing advantages of hydroxyapatite coatings.Furthermore,it can be applied to the surface treatment of not only implant materials directly bonded to bone but also various implanted biomaterials implanted that require cell movement control.

Fe alloy composites reinforced with in-situ titanium carbide （TIC） particles were fabricated by reactive sintering using different reactant C/Ti ratios of 0.8, 0.9, 1 and 1.1 to investigate the microstructure and mechanical properties ofin-situ TiC/Fe alloy composites. The microstructure showed that the in-situ syn- thesized TiC particles were spherical with a size of 1-3 }~m, irrespective of C/Ti ratio. The stoichiometry of in-situ TiC increased from 0.85 to 0.88 with increasing C/Ti ratio from 0.8 to 0.9, but remained almost unchanged for C/Ti ratios between 0.9 and 1.1 due to the same driving force for carbon diffusion in TiCx at the common sintering temperature. The in-situ TiC/Fe alloy composite with C[Ti ~ 0.9 showed improved mechanical properties compared with other C/Ti ratios because the presence of excess carbon （C/Ti = 1 and 1.1） resulted in unreacted carbon within the Fe alloy matrix, while insufficient carbon （C/Ti = 0.8） caused the depletion of carbon from the Fe alloy matrix, leading to a significant decrease in hardness. This study presents that the maximized hardness and superior strength of in-situ TiC/Fe alloy composites can be achieved by microstructure control and stoichiometric analysis of the in-situ synthesized TiC par- ticles, while maintaining the ductility of the composites, compared to those of the unreinforced Fe alloy. Therefore, we anticipate that the in-situ synthesized TiC/Fe alloy composites with enhanced mechanical properties have great potential in cutting tool, mold and roller material applications.

The prevalence of cardiovascular diseases(CVDs)is increasing at a rapid pace in developed countries,and CVDs are the leading cause of morbidity and mortality.Natural products and ethnomedicine have been shown to reduce the risk of CVDs.Schizonepeta(S.)tenuifolia is a medicinal plant widely used in China,Korea,and Japan and is known to exhibit anti-inflammatory,antioxidant,and immunomodulatory activities.We hypothesized that given herbal plant exhibit pharmacological activities against CVDs,we specifically explored its effects on platelet function.Platelet aggregation was evaluated using standard light transmission aggregometry.Intracellular calcium mobilization was assessed using Fura-2/AM,and granule secretion(ATP release)was measured in a luminometer.Fibrinogen binding to integrin a_(Ⅱb)β_3,was assessed using flow cytometry.Phosphorylation of mitogen-activated protein kinase(MAPK)signaling molecules and activation of the protein kinase B(Akt)was assessed using Western blot assays.S.tenuifolia,extract potently and significantly inhibited platelet aggregation,calcium mobilization,granule secretion,and fibrinogen binding to integrin a_(Ⅱb)β_3.Moreover,all extracts significantly inhibited MAPK and Akt phosphorylation.S.tenuifolia extract inhibited platelet aggregation and granule secretion,and attenuated collagen mediated GPVI downstream signaling,indicating the potential therapeutic effects of these plant extracts on the cardiovascular system and platelet function.We suggest that S.tenuifolia extract may be a potent candidate to treat platelet-related CVDs and to be used as an antiplatelet and antithrombotic agent.

The carbon steels dispersed with ultrafine TiC particles were fabricated by conventional casting method. The casting process is more economical than other available routes for metal matrix composite production, and the large sized components to be fabricated in short processing time. However, it is extremely difficult to obtain uniform dispersion of ultrafine ceramic particles in liquid metals due to the poor wettability and the specific gravity difference between the ceramic particle and metal matrix. In order to solve these problems, the mechanical milling (MM) and surface-active processes were introduced. As a result, Cu coated ultrafine TiC powders made by MM process using high energy ball milling machine were mixed with Sn powders as a surfactant to get better wettability by lowering the surface tension of carbon steel melt. The microstructural investigations by OM show that ultrafine TiC particles are distributed uniformly in carbon steel matrix. The grain sizes of the cast matrix with ultrafine TiC particles are much smaller than those without ultrafine TiC particles. This is probably due to the fact that TiC particles act as nucleation sites during solidification. The wear resistance of cast carbon steel composites added with MMed TiC/Cu-Sn powders is improved due to grain size refinement.

Epstein-Barr virus(EBV)-associated lymphoepitheliomalike gastric carcinoma(LELC) is characterized by a lower lymph node(LN) metastasis rate and a higher survival rate than other forms of gastric cancer. Although current prognosis for LELC is favorable, the most common approach is radical gastrectomy involving an extensive D2 lymph node dissection. Here, we report four cases of EBV-associated early LELC that were treated by an alternative approach, endoscopic submucosal dissection(ESD). The long-term outcome of this procedure is discussed. All patients were treated by ESD en bloc, and all ESD specimens showed tumor-free lateral resection margins. None of the lesions showed lymphovascular invasion. A pathological examination of ESD specimens revealed submucosal invasion of more than 500 μm in all four cases. One patient underwent additional radical surgery post-ESD; no residual tumor or LN metastasis was noted in the surgical specimen. The other three patients did not undergo additional surgery, either because of severe comorbidity or their refusal to undergo operation, but were subjected to medical follow-up. None of the ESD-treated patients reported local recurrence or distant metastases during the 27-32 mo of follow-up after ESD.

The microstructures of the brazed joints for commercially pure Ti and stainless steel were investigated by the applications of various filler alloys including Ag-, Ti-, Zr- and Ni-based alloys. Generally, the dissimilar joints between Ti and stainless steel were dominated by the Ti-based intermetallic compounds (IMCs), e.g. (Ti, Zr)2(Fe, Ni), TiFe, TiCu, and Ti2(Fe, Ni), due to a significant dissolution of Ti from the base metal. The (Fe-Cr) σ phase was also observed near the stainless steel due to a segregation of Cr into the interface region. This research demonstrates empirically that the brittleness of the Ti and stainless steel joint can not be avoided only by applying single braze alloy or single insert metal, and thus an introduction of additional suitable interlayer between the filler alloy and the base metal is necessary to prevent the brittleness of the joint.
