Successful laparoscopic management for cholecystoenteric fistula

AIM： Since 1987, laparoscopic cholecystectomy （LC） has been widely used as the favored treatment for gallbladder lesions. Cholecystoenteric fistula （CF） is an uncommon complication of the gallbladder disease, which has been one of the reasons for the conversion from LC to open cholecystectomy. Here, we have reported four cases of CF managed successfully by laparoscopic approach without conversion to open cholecystectomy. METHODS： During the 4-year period from 2000 to 2004, the medical records of the four patients with CF treated successfully with laparoscopic management at the Chang Gung Memorial HospitaI-Taipei were retrospectively reviewed. RESULTS： The study comprised two male and two female patients with ages ranging from 36 to 74 years （median： 53.5 years）. All the four patients had right upper quadrant pain. Two of the four patients were detected with pneumobilia by abdominal ultrasonography. One patient was diagnosed with cholecystocolic fistula preoperatively correctly by endoscopic retrograde cholangiopancreatography and the other one was diagnosed as cholecystoduodenal fistula by magnetic resonance cholangiopancreatography. Correct preoperative diagnosis of CF was made in two of the four patients with 50% preoperative diagnostic rate. All the four patients underwent LC and closure of the fistula was carried out by using Endo-GIA successfully with uneventful postoperative courses. The hospital stay of the four patients ranged from 7 to 10 d （median, 8 d）. CONCLUSION： CF is a known complication of chronic gallbladder disease that is traditionally considered as a contraindication to LC. Correct preoperative diagnosis of CF demands high index of suspicion and determines the success of laparoscopic management for the subset of patients. The difficult laparoscopic repair is safe and effective in the experienced hands of laparoscopic surgeons.

Design of Digital Linear Power Supply with Low Current Ripple

A linear power supply is a circuit architecture directly driven by AC. Multiple high-voltage LEDs (light-emitting diodes) are connected in series to withstand the input voltage. The LEDs are divided into multiple segments through a segment switching mechanism to adapt to the pulsating DC changes after bridge rectification. Due to imperfect switching between control signals, there may be signal overlap or misalignment, which reduces circuit performance and component lifespan. Firstly, signal overlap occurs when the next segment opens before the previous segment is about to close. This state causes overlapping intervals where two segments of current combine, resulting in a high combined current that can lead to significant power losses and generate heat, potentially reducing the lifespan of the components. Secondly, signal misalignment occurs when the next segment prepares to open after the previous segment has completely closed. In this state, there is a time gap with no current flow, resulting in reduced output power and exacerbating LED flickering. Both of these undesirable phenomena introduce ripple into the LED output current. In this paper, a digitalized design is proposed to precisely compensate for the timing between alternating signals, significantly reducing output current ripple. This approach helps enhance power supply performance, extend component lifespan, and reduce LED index flicker.

This note is considered as a sequel of Yeh [1]. Here, we present a generalized (vertex) distance labeling (labeling vertices under constraints depending the on distance between vertices) of a graph. Instead of assigning a number (label) to each vertex, we assign a set of numbers to each vertex under given conditions. Some basic results are given in the first part of the note. Then we study a particular class of this type of labelings on several classes of graphs.

Background Low-back pain(LBP)in nurses is a major health concern that affects their quality of life and ability to work,with consequences for their economic *** This study evaluates the effect of low-level laser acupuncture combined with auricular acupressure(LAA)on pain intensity,pain interference and quality of life in nurses with ***,setting,participants and interventions This randomized controlled trial recruited a convenience sample of hospital-based nurses from one teaching hospital in Taiwan,*** were randomly assigned to the LAA group(n=38)receiving low-level laser acupuncture and auricular acupressure for 4 weeks,and the control group(n=38)receiving only sham laser acupuncture treatment without laser energy *** outcome measures Data were collected for the primary pain outcome using the Short Form of the Brief Pain Inventory,while the secondary outcome,quality of life,was evaluated using the Roland-Morris Disability *** primary and secondary outcomes were scored before the intervention,and after 2-week and 4-week *** rate of LBP recurrence was evaluated at the 4th week and 8th week after the end of *** After controlling for prior pain,the result of linear mixed model analysis showed trends in significant between-group differences in the level of current pain occurring in week 4(P<0.001),worst pain in week 2(P<0.001)and week 4(P<0.001),least pain in week 2(P=0.032)and week 4(P<0.001),pain interference in week 2(P=0.009)and week 4(P<0.001),and in the life dysfunction in week 2(P<0.001)and week 4(P<0.001).Recurrence rates of LBP at the 4th and 8th weeks after the end of intervention were 0%and 36.89%in the LAA group,and 69.44%and 36.11%in the control *** This study shows that 4-week LAA intervention reduced pain intensity and pain interference,and improved quality of life for hospital-based nurses with *** effects were maintained continuously for at least 4

Risk factors and metabolic abnormality of patients with non-alcoholic fatty liver disease: Either non-obese or obese Chinese population

Background: Non-alcoholic fatty liver disease(NAFLD) occurs not only in obese individuals but also in non-obese ones. The aim of this study was to focus on the association between NAFLD and metabolic events in a non-obese or obese Chinese ***: Data collected from subjects registered at Taichung Veterans General Hospital from January to December 2009 were analyzed. The exclusion criteria were alcoholics, chronic hepatitis B or C. Patients included in analyses were assigned to four groups according to sonography of their liver(normal or NAFLD), and body mass index(BMI) levels(non-obese if BMI < 25 kg/m~2 or obese if BMI ≥ 25 kg/m~2).Results: There were 745, 208, 770 and 285 patients enrolled in four groups labeled non-obese normal liver(group A), non-obese NAFLD(group B), obese normal liver(group C) and obese NAFLD(group D),respectively. The highest ratio of metabolic syndrome existed in the group B(26.9%), followed by group A(11.7%), group D(10.9%) and finally the group C(5.2%). The positive association with NAFLD in non-obese individuals was significant in triglyceride(OR = 1.01; 95% CI: 1.01–1.02) and glucose(OR = 1.02; 95% CI:1.01–1.03), while the positive association with NAFLD in obese subjects was only significant in triglyceride(OR = 1.01; 95% CI: 1.01–1.02). The positive association was most significant in all cases(adjusted OR = 2.41; 95% CI: 1.78–3.24), especially in non-obese individuals(OR = 2.81; 95% CI: 1.92–4.12).Conclusions: Non-obese NAFLD subjects displayed a higher proportion of metabolic abnormality. Hyperlipidemia and hyperglycemia had the most positive strength association with NAFLD.

Our understanding of psoriatic arthritis has evolved as new knowledge of the disease has emerged. However, the exact prevalence of psoriatic arthritis is unknown, and its pathogenesis has not been fully elucidated. Genetic, environmental, and immunologic factors have all been implicated in disease development. Early diagnosis and treatment have become primary objectives in clinical rheumatology. Psoriatic arthritis not only causes functional impairment, but also increases mortality risk of patients. The advent of new therapeutic agents capable of arresting the progression of joint damage is expected. However, early psoriatic arthritis assessment remains limited. The objectives of this article are to outline the epidemiology, diagnosis, and treatment of psoriatic arthritis and to suggest a paradigm for identifying early psoriatic arthritis patients.

Traditional Chinese Medicine-facilitated switch from methadone to buprenorphine-naloxone for treatment of heroin dependence: a case report

The switch from methadone to buprenorphine-naloxone for individuals with heroin dependence is associated with several obstacles and *** patients may experience discomfort from discontinuing methadone, precipitated withdrawal symptoms induced by buprenorphine-naloxone,and poor psychosocial adjustments such as anticipatory anxiety regarding severe opioid *** herein describe a 46-year-old man with a history of heroin dependence who underwent Traditional Chinese Medicine(TCM)-facilitated switching from methadone to buprenorphine-naloxone. No precipitated withdrawal was induced by buprenorphine-naloxone. The drug-switching process was successful and smooth. He maintained abstinence from heroin for the following year. In this case, we applied TCM for enhancement of methadone metabolism and detoxification, analgesic effects, and anxiolytic and hypnotic effects during the drug switch. We observed that TCM effectively facilitated the switch from methadone to buprenorphine-naloxone in our case. Further studies regarding TCM-facilitated treatment for heroin dependence should be conducted.

The association of non-alcoholic fatty liver disease and metabolic syndrome in a Chinese population

BACKGROUND： Non-alcoholic fatty liver disease（NAFLD） is associated with features of metabolic syndrome. The aim of this study was to investigate the association between NAFLD and metabolic syndrome in a Chinese population. METHODS： Data from subjects were retrospectively collected from 2006 to 2009. The exclusion criteria included significant consumption of alcohol and chronic hepatitis B and C. The patients were assigned to two groups according to ultrasound findings： normal group and fatty liver group. The liver function of patients was determined by assessing serum alanine aminotransferase（ALT）. Metabolic syndrome was diagnosed based on the 2005 International Diabetes Federation criteria. RESULTS： A total of 7568 subjects were enrolled and 5736（75.8%） and 1832（24.2%） patients were assigned to the normal and fatty liver groups, respectively. The fatty liver group had significant male predominance（69.7% vs 56.0%）, higher body mass index（mean, 26.67 vs 23.55 kg/m^2） compared with the normal group. There were 441（7.7%） and 377（20.6%） cases with metabolic syndrome in the normal and fatty liver groups, respectively, with significant difference（P=0.001）, and the subgroup of 385 cases with fatty liver and elevated ALT had higher prevalence（28.8%） of metabolic syndrome. The strongest association of an individual component of metabolic syndrome with NAFLD was hyperlipidemia（adjusted OR=2.55, 95% CI： 2.22-2.94）.CONCLUSION： The individuals with NAFLD had a higher ratio of metabolic syndrome. Hyperlipidemia had the strongest positive association with NAFLD.

The Y‑chromosome has been widely used in ancestry inference based on its region‑specific haplogroup ***,there is always a debate on how informative such a single marker is for inferring an individual’s genetic ***,we compared genetic ancestry inferences at continental level made by Y‑chromosomal haplogroups to those made by autosomal single‑nucleotide polymorphisms in 1230 samples of Affymetrix Human Origins *** highest ancestry proportions of a majority of individuals match the highest average continental‑ancestry proportions in haplogroups A,B,D,H,I,K,L,T,O,and *** high consistencies have not been observed in haplogroups E,C,G,J,N,Q,and R,but in some of their sublineages,such as E1a,E1b1a1,E1b1b1b1a,E2b1a,J1a2b,Q1a1a1,Q1a2a1a1,R1b1a2a1a,and *** the consistencies of Y‑chromosomal and autosomal continental ancestry vary among haplogroups,Y‑chromosome could provide valuable clues for individual’s continental ancestry.
