CO_2 insufflation for potentially diffi cult colonoscopies: Effi cacy when used by less experienced colonoscopists

AIM: To clarify the effectiveness of CO2 insufflation in potentially difficult colonoscopy cases, particularly in relation to the experience level of ***: One hundred twenty potentially difficult cases were included in this study, which involved females with a low body mass index and patients with earlier abdominal and/or pelvic open surgery or previously diagnosed left-side colon diverticulosis. Patients receiving colonoscopy examinations without sedation using a pediatric variable-stiffness colonoscope were divided into two groups based on either CO2 or standard air insufflation. Both insufflation procedures were also evaluated according to the experience level of the respective colonoscopists who were divided into an experienced colonoscopist (EC) group and a less experienced colonoscopist (LEC) group. Study measurements included a 100-mm visual analogue scale (VAS) for patient pain during and after colonoscopyexaminations, in addition to insertion to the cecum and withdrawal ***: Examination times did not differ, however, VAS scores in the CO2 group were signifi cantly better than in the air group (P < 0.001, two-way ANOVA) from immediately after the procedure and up to 2 h later. There were no significant differences between either insufflation method in the EC group (P = 0.29), however, VAS scores for CO2 insufflation were significantly better than air insufflation in the LEC group (P = 0.023) immediately after colonoscopies and up to 4 h ***: CO2 insufflation reduced patient pain after colonoscopy in potentially difficult cases when performed by LECs.

AIM:To investigate the usefulness of chromoendoscopy,using an acetic acid indigocarmine mixture(AIM),for gastric adenoma diagnosed by forceps ***:A total of 54 lesions in 45 patients diagnosed as gastric adenoma by forceps biopsy were prospectively enrolled in this study and treated by endoscopic submucosal dissection(ESD)between January 2011 and January ***-chromoendoscopy(AIMCE)was performed followed by *** solution was sprinkled and images were recorded every 30 s for *** characteristics such as tumor size(<2cm,≥2 cm),surface color in white light endoscopy(WLE)(whitish,normochromic or reddish),macroscopic appearance(flat or elevated,depressed),and reddish change in AIM-CE were selected as ***:En bloc resection was achieved in all 54 cases,with curative resection of fifty two lesions(96.3%).Twenty three lesions(42.6%)were diagnosed as welldifferentiated adenocarcinoma and the remaining 31lesions(57.4%)were gastric *** adenocarcinoma lesions were well-differentiated tubular adenocarcinomas and were restricted within the mucosal *** sensitivity of reddish color change in AIM-CE is significantly higher than that in WLE(vs tumor size≥2 cm,P=0.016,vs normochromic or reddish surface color,P=0.046,vs depressed macroscopic type,P=0.0030).On the other hand,no significant differences were found in the specificity and *** univariate analysis,normochromic or reddish surface color in WLE(OR=3.7,95%CI:1.2-12,P=0.022)and reddish change in AIM-CE(OR=14,95%CI:3.8-70,P<0.001)were significantly related to diagnosis of early gastric cancer(EGC).In multivariate analysis,only reddish change in AIM-CE(OR=11,95%CI:2.3-66,P=0.0022)was a significant factor associated with diagnosis of ***:AIM-CE may have potential for screening EGC in patients initially diagnosed as gastric adenoma by forceps biopsy.

The effects of Ni content, soldering temperature and time on the IMC thickness in Sn-3Ag-0.5Cu and Sn-3Ag-0.5Cu-0.2Co alloys were researched using uniform design method and computer programs. For each alloy, the factors were divided into three levels in the experiment. Two correlative equations are given by regression. They indicate that the effects of three factors on the function are in the mutual and quadratic forms. And the analysis of variance shows the equations are sound and meaningful. Using the equations, it is easy to search, predict and control the IMC thickness. The existence of element Co accelerates the crystallization and growing up of IMC.

In the wild, primate foraging behaviors are related to the diversity and nutritional properties of food, which are affectedby seasonal variation. The goal of environmental enrichment is to stimulate captive animals to exhibit similar foraging behaviorof their wild counterparts, e.g. to extend foraging time. We conducted a 12-month study on the foraging behavior of Japanesemacaques in a semi-naturally forested enclosure to understand how they use both provisioned foods and naturally availableplant foods and what are the nutritional criteria of their consumption of natural plants. We recorded time spent feeding on provisionedand natural plant foods and collected the plant parts ingested of their major plant food species monthly, when *** conducted nutritional analysis (crude protein, crude lipid, neutral detergent fiber-'NDF', ash) and calculated totalnon-structural carbohydrate - 'TNC' and total energy of those food items. Monkeys spent 47% of their feeding time foraging onnatural plant species. The consumption of plant parts varied significantly across seasons. We found that leaf items were consumedin months when crude protein, crude protein-to-NDF ratio, TNC and total energy were significantly higher and NDF was significantlylower, fruit/nut items in months when crude protein and TNC were significantly higher and crude lipid content was significantlylower, and bark items in months when TNC and total energy were higher and crude lipid content was lower. This preliminaryinvestigation showed that the forested enclosure allowed troop members to more fully express their species typical flexiblebehavior by challenging them to adjust their foraging behavior to seasonal changes of plant item diversity and nutritional content,also providing the possibility for individuals to nutritionally enhance their

Comparative assessment of primary and secondary infection risks in a norovirus outbreak using a household model simulation

Diarrheal diseases can be transmitted via both primary infection due to exposures to contaminated materials from the environment and secondary infection due to person- to-person contacts. Usually, the importance of secondary infection is empirically assessed by fitting mathematical models to the epidemic curves. However, these empirical models may not be applicable to other epidemic cases because they are developed only for the target epidemics and they don＇t consider the detail routes of infection. In our previous study, we developed a theoretical model taking into account the various routes of infection that commonly occur in households （e.g., shaking hands, food handling, and changing diapers）. This model was made flexible and applicable to any epidemics by means of adjusting model parameters. In this study, we proposed a new index ＂Vulnerability indicator to secondary infection （VISI）＂, which expressed a ratio of secondary infection to primary infection risks and calculated this index in a simulated norouirus （NOV） epidemic that involved I0,000 households. The results demonstrated that households composed of more than three members including infant（s） had much higher levels of VlSl （5-45） than two-member-households with VlSI （0.1-4）. These results concluded that the infants were likely to be a hub of secondary infections in highly dense families and therefore careful handling of diapers was deemed indispensible in such families to effectively control the secondary infections.

We develop magnetic metallic contaminant detectors using high-temperature superconducting quantum interference devices （HTS-SQUIDs） for industrial products. Finding ultra-small metallic contaminants is an important issue for manufacturers producing commercial products such as lithium ion batteries. If such contaminants cause damages, the manufacturer of the product suffers a big financial loss due to having to recall the faulty products. Previously, we described a system for finding such ultra-smafi particles in food. In this study, we describe further developments of the system, for the reduction of the effect of the remnant field of the products, and we test the parallel magnetization of the products to generate the remnant field only at both ends of the products. In addition, we use an SQUID gradiometer in place of the magnetometer to reduce the edge effect by measuring the magnetic field gradient. We test the performances of the system and find that tiny iron particles as small as 50 × 50 μm^2 on the electrode of a lithium ion battery could be clearly detected. This detection level is difficult to achieve when using other methods.
