Objective:To assess and characterize online ratings and comments on laryngologists and determine factors that correlate with higher ratings.Methods:All the American Laryngological Association(ALA)members were queried across several online platforms.Ratings were normalized for comparison on a five-point Likert scale.Ratings were categorized based on context and for positive/negative aspects.Results:Of the 331 ALA members,256(77%)were rated on at least one online platform.Across all platforms,the average overall rating was 4.39±0.61(range:1.00-5.00).Specific positive ratings including“bedside manners,”“diagnostic accuracy,”“adequate time spent with patient,”“appropriate follow-up,”and“physician timeliness”had significant positive correlations to overall ratings,by Pearson's correlation(P<0.001).Long wait times had significant negative correlations to overall ratings(P<0.001).Conclusion:Online ratings and comments for laryngologists are significantly influenced by patient perceptions of bedside manner,physician competence,and time spent with the patient.

Background:B cells represent a crucial component of adaptive immunity that ensures long-term protection from infection by generating pathogen-specific immunoglobulins.Exercise alters B cell counts and immunoglobulin levels,but evidence-based conclusions on potential benefits for adaptive immunity are lacking.This systematic review assessed current literatures on the impact of acute exercise and exercise training on B cells,immunoglobulins,and markers of secretory immunity in human biofluids.Methods:According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines,MEDLINE,Web of Science,and Embase were searched on March 8,2023.Non-randomized controlled trials and crossover trials investigating the impact of acute exercise or exercise training on B cell counts and proportions,immunoglobulin levels,salivary flow rate,or secretory immunoglobulin A secretion rate were included.Quality and reporting of exercise training studies were assessed using the Tool for the Assessment of Study Quality and reporting in Exercise.Study characteristics,outcome measures,and statistically significant changes were summarized tabularly.Results:Of the 67 eligible studies,22 applied acute exercise and 45 applied exercise training.All included outcomes revealed significant alterations over time in acute exercise and exercise training context,but only a few investigations showed significant differences compared to control conditions.Secretory and plasma immunoglobulin A levels were most consistently increased in response to exercise training.Conclusion:B cell-related outcomes are altered by acute exercise and exercise training,but evidence-based conclusions cannot be drawn with high confidence due to the large heterogeneity in populations and exercise modalities.Well-designed trials with large sample sizes are needed to clarify how exercise shapes B cell-related immunity.

Introduction: Psoriasis may be triggered by several factors, and its pathophysiology is related to inflammatory cellular processes.Case presentation: A 67-year-old man who had been diagnosed with psoriasis 14 years previously developed an outbreak of small erythematous desquamative plaques distributed symmetrically on his arms and trunk. Multiple spiny whitish papules had developed over his previously established psoriasis lesions. The appearance of the papules was consistent with viral warts. The patient was treated with systemic methotrexate injected subcutaneously and oral folic acid, both dosed weekly. This treatment produced good results.Discussion: A remarkable feature of this case is that psoriasis was the first diagnosis and that human papillomavirus (HPV) appeared over widespread erythematous squamous plaques. The pathophysiology of psoriasis is related to inflammatory cellular processes. Some authors have reported that HPV proteins may stimulate keratinocyte proliferation. Others have reported that HPV infection mostly stimulates systemic interleukin 17 production; consequently, HPV infection could provide a better environment for interleukin 17 secretion and facilitate new onset of psoriasis. One report indicated that methotrexate was discontinued in a patient who presented with both pathologies, although we had very good results.Conclusion: Psoriasis and HPV may be pathophysiologically related. HPV may stimulate keratinocyte proliferation and systemic interleukin 17 production, resulting in a better environment for psoriasis. Systemic methotrexate injected subcutaneously resulted in a good outcome in our patient.

Globally,sub-optimal use of nitrogen (N) fertiliser and elevated N irrigation groundwater have led to high leached nitrate (NO_(3)^(–)) losses from protected vegetable field systems.Optimising fertiliser and irrigation management in different soil types is crucial to reduce future N loads from such systems.The present 4-year study examined leached N loads from lysimeter monitoring arrays set up across 18 protected vegetable system sites encompassing the dominant soil types of northern China.The treatments applied at each field site were:1) a high N and high irrigation input treatment (HNHI);2) a low N but high irrigation input treatment (LNHI) and 3) a low N with low irrigation input treatment (LNLI).Results showed that the mean annual leached total nitrogen loads from the HNHI,LNHI and LNLI treatments were 325,294 and 257 kg N ha^(–1) in the fluvo-aquic soil,114,100 and 78 kg N ha^(–1) in the cinnamon soil and 79,68 and 57 kg N ha^(–1) in the black soil,respectively.The N dissolved in irrigation water in the fluvo-aquic soil areas was 8.26-fold higher than in the cinnamon areas.A structural equation model showed that N fertiliser inputs and leaching water amounts explained 14.7 and 81.8%of the variation of leached N loads,respectively.Correspondingly,reducing irrigation water by 21.5%decreased leached N loads by 20.9%,while reducing manure N and chemical N inputs by 22 and 25%decreased leached N loads by only 9.5%. This study highlights that protected vegetable fields dominated by fluvo-aquic soil need management to curtail leached N losses in northern China.

Retinitis pigmentosa is a hereditary retinal disease that affects rod and cone photoreceptors,leading to progressive photoreceptor loss.Previous research supports the beneficial effect of electrical stimulation on photoreceptor survival.This study aims to identify the most effective electrical stimulation parameters and functional advantages of transcorneal electrical stimulation(tcES)in mice affected by inherited retinal degeneration.Additionally,the study seeked to analyze the electric field that reaches the retina in both eyes in mice and post-mortem humans.In this study,we recorded waveforms and voltages directed to the retina during transcorneal electrical stimulation in C57BL/6J mice using an intraocular needle probe with rectangular,sine,and ramp waveforms.To investigate the functional effects of electrical stimulation on photoreceptors,we used human retinal explant cultures and rhodopsin knockout(Rho^(-/-))mice,demonstrating progressive photoreceptor degeneration with age.Human retinal explants isolated from the donors’eyes were then subjected to electrical stimulation and cultured for 48 hours to simulate the neurodegenerative environment in vitro.Photoreceptor density was evaluated by rhodopsin immunolabeling.In vivo Rho^(-/-)mice were subjected to two 5-day series of daily transcorneal electrical stimulation using rectangular and ramp waveforms.Retinal function and visual perception of mice were evaluated by electroretinography and optomotor response(OMR),respectively.Immunolabeling was used to assess the morphological and biochemical changes of the photoreceptor and bipolar cells in mouse retinas.Oscilloscope recordings indicated effective delivery of rectangular,sine,and ramp waveforms to the retina by transcorneal electrical stimulation,of which the ramp waveform required the lowest voltage.Evaluation of the total conductive resistance of the post-mortem human compared to the mouse eyes indicated higher cornea-to-retina resistance in human eyes.The temperature recordings during and after electrical

Background: Research exploring the influence of glaucoma on quality of life has gained momentum in recent years. Numerous studies have investigated the multifaceted aspects of quality of life in glaucoma patients, utilizing various assessment tools and methodologies. Objective: To determine the influence that glaucoma has on the quality of life among adult patients. Study site: kitwe teaching eye hospital, Zambia. Method: It was a cross-sectional study that was conducted from 30th June 2022 to 17th April 2023. The study included 106 diagnosed glaucoma patients who were above the age of 55 years. Data was collected using validated 25-item national eye institute visual function Questionnaire (NEI VFQ-25). The data was analyzed using SPSS version 26 and Microsoft excel to present frequencies and percentages that generated graphical presentation. Results: 62% of the participants worry about their eye sight most of the time, 26% sometimes and 12% a little time. 9% of the participants had little difficulty to read ordinary prints, 14% moderate difficulties and 58% extremely difficult. 40% of the participants could drive despite glaucoma damaging their eyes while 60% could not. Majority of the patients said it was difficult noticing things around them while walking or sitted in one place. There was a significant difference between effects of glaucoma on patients and the existing relationship between glaucoma and quality of life (ANOVA, p = 0.019 and ANOVA, p = 0.023). Conclusion: This study has demonstrated the magnitude of impact that glaucoma has on the Quality of Life (QoL) in diagnosed patients and the relationship between glaucoma and the Quality of Life. Due to the progressive visual loss, the amount of activities these patients would perform is negatively affected especially where social and economic activities are concerned which also affects their monthly earnings for those living in middle to low-income countries. AIM:To investigate sex-based differences in the occurrence of intra-operative and post-operative complications and associated visual outcomes following cataract surgery.METHODS:This was a retrospective study of patients who had phacoemulsification cataract surgery at the University of Colorado School of Medicine.Data collected included the patient’s health history,ocular comorbidities,operative and post-operative complications,and the post-operative best corrected visual acuity(BCVA).The data were analyzed using univariate and multivariable logistic regression with generalized estimating equations to account for the correlation of some patients having two eyes included in the study.RESULTS:A total of 11977 eyes from 7253 patients were included in the study.Ocular comorbidities differed by sex,with males having significantly higher percentages of traumatic cataracts(males 0.7%vs females 0.1%),prior ocular surgery(6.7%vs 5.5%),and mature cataracts(2.8%vs 1.9%).Conversely,females had significantly higher rates of pseudoexfoliation(2.0%vs 3.2%).In unadjusted analysis,males had higher rates of posterior capsular rupture(0.8%vs 0.4%)and vitreous loss(1.0%vs 0.6%),but this difference was not significant after adjustment for confounders.Males had a significantly increased risk of post-operative retinal detachment,but in multivariable analysis this was no longer significant.Males were significantly less likely to undergo post-operative neodymium-doped yttrium aluminum garnet(Nd:YAG)laser capsulotomy for posterior capsule opacification(OR=0.8,95%CI=0.7-0.9,P=0.0005).The BCVA was slightly worse for males pre-operatively;but post-operatively,both sexes exhibited similar visual acuity of Snellen equivalent 20/25.CONCLUSION:The study finds that in a cohort of patients presenting for cataract surgery,sex differences exist in pre-operative comorbidities and surgical characteristics that contribute to higher rates of some complications for males.However,observed surgical complication rates exhibit almost no difference by sex afte

Interferon-gamma(IFN-γ)plays a dual role in cancer;it is both a pro-and an antitumorigenic cytokine,depending on the type of cancer.The deregulation of the IFN-γcanonic pathway is associated with several disorders,including vulner-ability to viral infections,inflammation,and cancer progression.In particular,the interplay between lung adenocarcinoma(LUAD)and viral infections appears to exist in association with the deregulation of IFN-γsignaling.In this mini-review,we investigated the status of the IFN-γsignaling pathway and the expression level of its components in LUAD.Interestingly,a reduction in IFNGR1 expression seems to be associated with LUAD progression,affecting defenses against viruses such as severe acute respiratory syndrome coronavirus 2.In addition,alterations in the expression of IFNGR1 may inhibit the antiproliferative action of IFN-γsignaling in LUAD.
