Background: Post-hepatectomy liver failure(PHLF) is a leading cause of postoperative mortality after liver surgery. Due to its significant impact, it is imperative to understand the risk stratification and preventative strategies for PHLF. The main objective of this review is to highlight the role of these strategies in a timeline centered way around curative resection. Data sources: This review includes studies on both humans and animals, where they addressed PHLF. A literature search was conducted across the Cochrane Library, Embase, medLINE/Pub med, and Web of Knowledge electronic databases for English language studies published between July 1997 and June 2020. Studies presented in other languages were equally considered. The quality of included publications was assessed using Downs and Black’s checklist. The results were presented in qualitative summaries owing to the lack of studies qualifying for quantitative analysis. Results: This systematic review with 245 studies, provides insight into the current prediction, prevention, diagnosis, and management options for PHLF. This review highlighted that liver volume manipulation is the most frequently studied preventive measure against PHLF in clinical practice, with modest improvement in the treatment strategies over the past decade. Conclusions: Remnant liver volume manipulation is the most consistent preventive measure against PHLF.

BACKGROUND Small-for-size grafts (SFSGs) in living donor liver transplantation (LDLT) could optimize donor postoperative outcomes and also expand the potential donor pool. Evidence on whether SFSGs would affect medium-term and long-term recipient graft survival is lacking. AIM To evaluate the impact of small-for-size liver grafts on medium-term and longterm graft survival in adult to adult LDLT. METHODS A systematic review and meta-analysis were performed by searching eligible studies published before January 24, 2019 on Pubmed, EMBASE, and Web of Science databases. The primary outcomes were 3-year and 5-year graft survival. Incidence of small-for-size syndrome and short term mortality were also extracted. RESULTS This meta-analysis is reported according to the guidelines of the PRISMA 2009 Statement. Seven retrospective observational studies with a total of 1821 LDLT recipients were included in the meta-analysis. SFSG is associated with significantly poorer medium-term graft survival. The pooled odds ratio for 3-year graft survival was 1.58 [95% confidence interval 1.10-2.29, P = 0.014]. On the other hand, pooled results of the studies showed that SFSG had no significant discriminatory effect on 5-year graft survival with an odds ratio of 1.31 (95% confidence interval 0.87-1.97, P = 0.199). Furthermore, incidence of small-for-size syndrome detected in recipients of SFSG ranged from 0-11.4% in the included studies. CONCLUSION SFSG is associated with inferior medium-term but not long-term graft survival. Comparable long-term graft survival based on liver graft size shows that smaller grafts could be accepted for LDLT with appropriate flow modulatory measures. Close follow-up for graft function is warranted within 3 years after liver transplantation.

AIM: To evaluate the applicability of nonbismuth concomitant quadruple therapy for Helicobacter pylori(H. pylori) eradication in Chinese ***: A systematic review and meta-analysis of randomized controlled trials was performed to evaluate the efficacy of nonbismuth concomitant quadruple therapy between sequential therapy or triple therapy for H. pylori eradication in Chinese regions. The defined Chinese regions include China, Hong Kong, Taiwan, and Singapore. The primary outcome was the H. pylori eradication rate; the secondary outcome was the compliance with therapy. The Pub med, Embase, Scopus, and Cochrane databases were searched for studies published in the period up to March 2016 with no language ***: We reviewed six randomized controlled trials and 1616 patients. In 3 trials comparing concomitant quadruple therapy with triple therapy, the H. pylori eradication rate was significantly higher for 7-d nonbismuth concomitant quadruple therapy than for 7-d triple therapy(91.2% vs 77.9%, risk ratio = 1.17, 95%CI: 1.09-1.25). In 3 trials comparing quadruple therapy with sequential therapy, the eradication rate was not significant between groups(86.9% vs 86.0%). However, higher compliance was achieved with concomitant therapy than with sequential ***: The H. pylori eradication rate was higher for nonbismuth concomitant quadruple therapy than for triple therapy. Moreover, higher compliance was achieved with nonbismuth concomitant quadruple therapy than with sequential therapy. Thus, nonbismuth concomitant quadruple therapy should be the first-line treatment in Chinese regions.

背景:膝关节粘连严重影响了患者术后的日常生活,而传统的治疗方式疗效有限。降低膝关节粘连发生率是提高手术预期疗效的关键,也是临床医学上需要急待解决的重要问题。目的:对膝关节粘连的各类研究文献进行分析,总结近10年的研究成果,为防治膝关节粘连提供新的思路和方法。方法:由第一作者检索Pub med数据库,查找2006至2016年的文献,英文检索词为"tissue adhesions,arthrofibrosis,joint stiffness,prevent,treatment",共检索到1 500余篇相关文献,59篇符合纳入标准。结果与结论:(1)近年来大量学者研究发现,创伤/手术引起膝关节出血、炎症反应,造成血管内皮细胞生长因子和白细胞介素6高表达,增加了成纤维细胞的活性,导致大量胶原蛋白沉积,发生粘连;(2)改进传统治疗方式和运用微创技术治疗膝关节粘连取得了一定的疗效,但仍存在再粘连风险;(3)关节腔内用药预防膝关节粘连在基础研究方面获得了显著成果,可成为预防膝关节粘连的一种有效方案,但临床可行性存在争议,需进一步深入研究,寻找疗效确切、不良反应小的药物。

目的对火针治疗类风湿关节炎的临床随机或半随机对照试验研究进行文献质量评价。方法计算机检索CBM(1979—2015)、CNKI(1979—2015)、VIP(1989—2015)、万方数字化期刊群(1998—2015)、Pub med(1966—2015)、EMbase(1980—2015)和Cochrane Library(Issue4,2015)数据库,并辅以手工检索。纳入随机或半随机对照试验。由两名评价者独立提取资料并交叉核对,而后按照Cochrane系统评价员手册4.2.8进行质量评估,包括研究设计的方法学质量、诊断与纳入/排除标准、火针/对照组干预措施、疗效评价标准及观察时间点,不良反应报告等方面进行评价。对同质性研究进行Meta分析,并对系统结果进行GRADE分级。结果共纳入7篇随机对照试验,火针组与药物组(止痛药+抗风湿药)比较,纳入4个研究,在有效率方面,无统计学意义[RR=1.22,95%CI(0.97,1.52)]。GRADE分级为非常低级,国际公认的诊断标准及通用的评价方法尚未得到普遍应用。结论从目前的临床报道来看,火针治疗类风湿关节炎有一定疗效,但仍需相关证据支持。

肝细胞性肝癌是临床高度恶性肿瘤，病情进展迅速，治疗方法有限。从全球看在男性恶性肿瘤中占６２％，女性恶性肿瘤中占２６％。其发病率有明显的地理差异：在北欧和美国为每年／１０００００～３／１０００００；南撒哈拉非洲和南亚居民每年达１５０／１０００００。临床流行病学研究指出：肝细胞肝癌的发病率与肝炎病毒肝炎的发生率密切相关，特别是乙型肝炎病毒、丙型肝炎病毒和丁型肝炎病毒感染。肝炎病毒肝炎与肝细胞性肝癌之间存在着病因联系，肝细胞性肝癌的发生还可能是嗜酒、黄曲霉、血色素沉着症、α１－抗胰蛋白酶缺乏症和卟啉症等所致慢性肝病的后果。方法：近年用ＰＡＰ法和单克隆或多克隆抗体在肝细胞性肝癌患者的肝组织进行了乙型、丙型和丁型肝炎病毒抗原表达的研究，并与其它慢性肝病作为对照。结果：（１）乙型肝炎表面抗原和核心抗原在肝脏肿瘤和非肿瘤组织的表达：５０例肝细胞性肝癌病人用过氧化酶－抗过氧化酶技术进行研究，在非肿瘤组织内ＨＢｓＡｇ和ＨＢｃＡｇ的检出率为７６％和３０％；在肿瘤组织内为１４％和６％。这一发现与慢性肝脏疾病中的检出率有很大的相似性；（２）丁型肝炎病毒抗原在ＨＢＶ标志阳性肝细胞性肝癌肝脏组织内的表达：１０４份肝细胞性肝癌患者肝?

AIM: To review the benefits of single photon emission computed tomography(SPECT)/computed tomography(CT) hybrid imaging for diagnosis of various endocrine ***: We performed medLINE and Pub med searches using the terms: "SPECT/CT"; "functional anatomic mapping"; "transmission emission tomography"; "parathyroid adenoma"; "thyroid cancer"; "neuroendocrine tumor"; "adrenal"; "pheochromocytoma"; "paraganglioma"; in order to identify relevant articles published in English during the years 2003 to 2015. Reference lists from the articles were reviewed to identify additional pertinent articles. Retrieved manuscripts(case reports, reviews, meta-analyses and abstracts) concerning the application of SPECT/CT to endocrine imaging were analyzed to provide a descriptive synthesis of the utility of this ***: The emergence of hybrid SPECT/CT camera technology now allows simultaneous acquisition of combined multi-modality imaging, with seamless fusion of three-dimensional volume datasets. The usefulness of combining functional information to depict the biodistribution of radiotracers that map cellular processes of the endocrine system and tumors of endocrine origin, with anatomy derived from CT, has improved the diagnostic capability of scintigraphy for a range of disorders of endocrine gland function.
