Multiple sclerosis （MS） is a chronic inflammatory disease of the central nervous system （CNS）. essential charac- teristics are demyelination, inflammation and neurode- generation. This process affects the white and grey matter in the CNS. MS patients experience various progression subtypes in association with the cerebral or spinal, acute inflammatory or glial sclerotic lesions （Mtil.er, 2009）. Most patients end up in a progressive, smoul.ering, chronic inflammatory process （MUl.er, 2009）. Current predominantly used 1.5 respectively 3 Tesla MRI with Gadolinium~ application visualize the various old and acute lesions. They serve as a biological marker in com- bination with standardised assessment of brain atrophy, black holes, etc. However, MRI with a stronger magnetic 7 Tesla field with better sensitivity gave hints on an on- going, acute inflammatory, smoul.ering process even with Gadolinium~ enhancing acute lesions in the brain and the spinal cord in progressive, so-cal.ed relapse free MS patients （Mtil.er, 2009; Sinnecker et al., 2012）. Ad- ditionally, progress of MS is determined with subjective standardised clinical ratings （Sinnecker et al., 2012）. Both methods are used for the evaluation of the efficacy of relapse rate reducing drugs. These compounds, i.e., in- terferons, teriflunamide, glatiramer acetate, fingolimod, fumarate or monoclonal antibodies, preponderantly weaken the malfunction of the peripheral immune system in relapse remitting MS patients. These MS drugs share one common disadvantage. They do not stop progression or improve MS within a framework of a regenerative process. They do not enable reversal of symptoms, for in- stance functional deficits or spasticity （Mtil.er, 2009）.

Background and Aims:As practice patterns and hepatitis C virus(HCV)genotypes(GT)vary geographically,a global real-world study from both east and West covering all GTs can help inform practice policy toward the 2030 HCV elimination *** study aimed to assess the effectiveness and tolerability of DAA treatment in routine clinical practice in a multinational cohort for patients infected with all HCV GTs,focusing on GT3 and ***:We analyzed the sustained virological response(SVR12)of 15,849 chronic hepatitis C patients from 39 Real-World evidence from the Asia liver Consortium for HCV clinical sites in Asia Pacific,North America,and europe between 07/01/2014–07/01/***:The mean age was 62±13 years,with 49.6%*** demographic breakdown was 91.1%Asian(52.9%Japanese,25.7%Chinese/Taiwan residents,5.4%Korean,3.3%Malaysian,and 2.9%Vietnamese),6.4%White,1.3%Hispanic/latino,and 1%Black/***,34.8%had cirrhosis,8.6%had hepatocellular carcinoma(HCC),and 24.9%were treatment-experienced(20.7%with interferon,4.3%with direct-acting antivirals).The largest group was GT1(10,246[64.6%]),followed by GT2(3,686[23.2%]),GT3(1,151[7.2%]),GT6(457[2.8%]),GT4(47[0.3%]),GT5(1[0.006%]),and untyped GTs(261[1.6%]).The overall SVR12 was 96.9%,with rates over 95%for GT1/2/3/6 but 91.5%for ***12 for GT3 was 95.1%overall,98.2%for GT3a,and 94.0%for ***12 was 98.3%overall for GT6,lower for patients with cirrhosis and treatment-experienced(Te)(93.8%)but≥97.5%for tretment-naive patients regardless of cirrhosis *** multivariable analysis,advanced age,prior treatment failure,cirrhosis,active HCC,and GT3/4 were independent predictors of lower SVR12,while being Asian was a significant predictor of achieving ***:In this diverse multinational realworld cohort of patients with various GTs,the overall cure rate was 96.9%,despite large numbers of patients with cirrhosis,HCC,Te,and GT3/***12 for GT3/6 with cirrhosis and Te was lower but st

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(eTV)-resistant chronic hepatitis B(CHB) patients. MeTHODS: F i fty CHB pat ient s wi t h genotypic resistance to eTV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus eTV(ADV/eTV,n = 23) or ADV plus lamivudine(lMV)(ADV/lMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical.examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA l) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m l. The most commonly observed eTV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/eTV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/lAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(log10 IU/m l) and initial virologic response at 3 mo(IVR-3; HBV DNA log10 IU/m l after 3 mo) were independent predictive factors for VR. AIM: To study the mechanism of Je-Chun-Jun (JCJ)-inducing the apoptosis of the human cervical carcinoma, Hela cells. MeTHODS: The cell viability was assessed using MTT assay. The optical density was measured at 570 nm. The caspase activity was measured using 50 mmol/l of fluorogenic substrate, AC-DeVD-AMC (caspase-3), AC-VeID-AMC (caspase-8) or AC-leHD-AFC (caspase-9). To confirm the expression of proteins, Western blotting was performed. To detect the characteristic of apoptosis chromatin condensation, Hela cells were stained with Hoechst 33258 in the presence of JCJ. For the cell cycle analysis, Hela cells were incubated with Propidium iodide (PI) solution. Fluorescence intensity of cell cycle was measured using flow cytometry system. ReSUlTS: The loss of viability occurred following the exposure of 10 g/l JCJ. Cells treated with 10 g/l JCJ for 3 d exhibited the apoptotic morphology (brightly bl.e-fluorescent condensed nuclei by Hoechst 33258-staining) and the reduction of cell volume. Cells incubated

AIM: To evaluate the possibility of treatment effect monitoring using hepatic fat quantification magnetic resonance(MR) in pediatric nonalcoholic steatohepatitis(NASH).MeTHODS: We retrospectively reviewed the medical records of patients who received educational recommendations and vitamin e for NASH and underwent hepatic fat quantification MR from 2011 to *** fat fraction(%) was measured using dual- and triple-echo gradient-recal.ed-echo sequences at *** compliant and non-compliant groups were compared clinically,biochemically,and ***: Twenty seven patients(M:F = 24:3; mean age: 12 ± 2.3 years) were included(compliant group = 22,non-compliant = 5).None of the baseline findings differed between the 2 groups,except for triglyceride level(compliant vs non-compliant,167.7 mg/d l vs 74.2 mg/d l,P = 0.001).In the compliant group,high-density lipoprotein increased and all other parameters decreased after 1-year ***,there were various changes in the non-compliant *** fat fraction(-19.2% vs 4.6,P l.echo fat fraction(-13.4% vs 3.5,P lanine aminotransferase(-110.7 IU/l vs-10.6 IU/l,P = 0.047),total cholesterol(-18.1 mg/d l vs 3.8 mg/d l,P = 0.016),and triglyceride levels(-61.3 mg/d l vs 11.2 mg/d l,P = 0.013) were significantly decreased only in the compliant *** change in body mass index and dual.echo fat fraction showed a positive correlation(ρ = 0.418,P = 0.030).CONClUSION: Hepatic fat quantification MR can be a non-invasive,quantitative and useful tool for monitoring treatment effects in pediatric NASH.

MeTHODS:One hundred and sixty-eight patients during routine HCC surveillance were included in this *** the 168 patients,90(53.6%)had HCC including newly developed HCC(n=82)or recurrent HCC after treatment(n=8).Sera were obtained during their first evaluation for HCC development and at the time of HCC diagnosis before commencing HCC *** was diagnosed by histological.examination,appropriate imaging characteristics-computed tomography or magnetic resonance *** sera were col.ected from 78 patients with benign liver disease(BlD),which were obtained during routine surveillance with a suspicion of ***,AFP-l3 and PIVKA-Ⅱwere measured in the same serum by microchip capillary electrophoresis and liquid-phase binding assay on a micro-total analysis system Wako i30 auto *** performance characteristics of three tests and combined tests for the diagnosis of HCC were obtained using receiver operating characteristic curves in all populations and subgroups with AFPl. ReSUlTS:Of 90 HCC patients,38(42.2%)patients had AFPl,20(22.2%)patients had AFP 20-200 ng/ml and 32(35.6%)patients had AFP>200 ng/*** the 78 BlD patients,74(94.9%)patients had AFPent for age and HBV infection status,AFP-l3 levels were higher in HCC than in BlD among patients with low AFP levels(l)(Pl of 168 patients,areas under the curve(AUC)for HCC were 0.879,0.887,0.801 and 0.939 for AFP,AFP-l3,PIVKA-Ⅱand the combined markers,*** combined AUC for three markers showed higher val.e than the AUCs of individual marker(Pl3 had higher AUC val.e than PIVKA-Ⅱfor HCC detection in entire patients(P =0.043).With combination of AFP-l3(cut-off>5%) and PIVKA-Ⅱ(cut-off>40 AU/l),the sensitivity were 94.4%and specificity were 75.6%in all *** 112 patients
