Evaluation of an educational telephone intervention strategy to improve non-screening colonoscopy attendance:A randomized controlled trial

BAcKGROUND colonoscopy attendance is a key quality parameter in colorectal cancer population screening programmes.Within these programmes,educative interventions with bidirectional contact carried out by trained personnel have been proved to be an important tool for colonoscopy attendance improvement,and because of its huge clinical and economic impact,they have been widely implemented.However,outside of this population programmes,educative measures to improve colonoscopy attendance have been poorly studied and no navigation interventions are usually performed.AIM To investigate the clinical and economic impacts of an educational telephone intervention on colonoscopy attendance outside colorectal cancer screening programmes.METHODS This randomized controlled trial included consecutive patients referred to colonoscopy from primary care centres from November 2017 to May 2018.The intervention group(IG)received a telephone intervention,while the control group(cG)did not.Patients assigned to the IG received an educational telephone call 7 d before the colonoscopy appointment.The intervention was carried out by two nurses with deep endoscopic knowledge who were previously trained for a telephone educational intervention for colonoscopy.The impact on patient compliance with preparedness protocols related to bowel cleansing,antithrombotic management,and sedation scheduling was also evaluated.A second call was conducted to assess patient satisfaction.Intention-to-treat(ITT)and perprotocol(PP)analyses were performed.RESULTS A total of 738 and 746 patients were finally included in the IG and cG respectively.Six hundred thirteen(83%)patients were contacted in the IG.The non-attendance rate was lower in the IG,both in the ITT analysis(IG 8.4%vs cG 14.3%,Pcontrol group increased the risk of nonattendance in both,the ITT analysis(OR 1.81,95%cI:1.27 to 2.58,P=0.001)and the PP analysis(OR 3.56,95%cI:2.25 to 5.64,P<0.001).There was als

Workers'task specialization and division of labor are critical features of social insects'ecological success.It has been proposed that the division of labor relies on response threshold models:individuals varying their sensitivity(and responsiveness)to biologically relevant stimuli and performing a specific task when a stimulus exceeds an internal threshold.In this work,we study carbohydrate and protein responsiveness and their relation to worker task specialization in Vespula germanica,an invasive social wasp.The sucrose and peptone responsiveness of two different subcastes,preforagers and foragers,was determined by stimulating the antenna of the wasps with increasing concentrations of the solution and quantifying whether each concentration elicited a licking response.We studied responsiveness in five different ways:(1)response threshold,(2)concentration 50(concentration to which at least 50%of wasps responded),(3)maximum response,(4)mean scores and(5)median scores.Our results suggest that V germanica foragers are more sensitive to sucrose(lower thresholds)than preforager workers.However,we found no differences for peptone thresholds(i.e.,a protein resource).Nonetheless,this is the first study to investigate response thresholds for protein resources.The intercaste variation in sucrose responsiveness shown in our work contributes to the existing knowledge about response threshold theory as a mechanism for task specialization observed in V germanica.

BAcKGROUND Total patellectomy is currently reserved for exceptional cases,such as recalcitrant patellofemoral instability and comminuted fractures,due to its demonstrated negative impact on knee biomechanics.Therefore,managing patellectomy is crucial to mitigate its inherent deleterious effects.Various techniques have been described,including autologous or allogeneic bone grafts for reconstruction and soft tissue realignment to enhance the extensor mechanism.cASE SUMMARY A 73-year-old male underwent a patellectomy due to a comminuted fracture,subsequently developing osteoarthritis and experiencing a decline in functional status.concurrent with total knee replacement,we conducted a patellar reconstruction,incorporating routine bone cuts and utilizing bone chips to fashion a new patella.This intervention resulted in the restoration of full extension and improvement of knee function.cONcLUSION Patellar reconstruction demonstrates benefits on knee mechanics and stabilization,contributing to enhanced outcomes and satisfaction following knee replacement.We present an affordable technique for managing patellectomized patients undergoing total knee replacement.

Background Pediatric cancer survivors are at increased risk of muscle weakness and low areal bone mineral density(aBMD).However,the prevalence of muscle strength deficits is not well documented,and the associations of muscle strength with aBMD are unknown in this population.Therefore,this study aimed to investigate the prevalence of upper-and lower-body muscle strength deficits and to examine the associations of upper-and lower-body muscle strength with age-,sex,and race-specific aBMD Z-scores at the total body,total hip,femoral neck,and lumbar spine.Methods This cross-sectional study included 116 pediatric cancer survivors(12.1±3.3 years old,mean±SD;42.2%female).Upper-and lower-body muscle strength were assessed by handgrip and standing long jump test,respectively.Dual‑energy X‑ray absorptiometry was used to measure aBMD(g/cm2).Associations between muscle strength and aBMD were evaluated in multivariable linear regression models.Logistic regression was used to evaluate the contribution of muscle strength(1-decile lower)to the odds of having low aBMD(Z-score≤1.0).All analyses were adjusted for time from treatment completion,radiotherapy exposure,and body mass index.Results More than one-half of survivors were within the 2 lowest deciles for upper-(56.9%)and lower-body muscle strength(60.0%)in comparison to age-and sex-specific reference values.Muscle strength deficits were associated with lower aBMD Z-scores at all sites(B=0.133–0.258,p=0.001–0.032).Each 1-decile lower in upper-body muscle strength was associated with 30%–95%higher odds of having low aBMD Z-scores at all sites.Each 1-decile lower in lower-body muscle strength was associated with 35%–70%higher odds of having low aBMD Z-scores at total body,total hip,and femoral neck.conclusion Muscle strength deficits are prevalent in young pediatric cancer survivors,and such deficits are associated with lower aBMD Z-scores at all sites.These results suggest that interventions designed to improve muscle strength in this vulnerable population may have the added ben

Temporal stability and directional change in a color cline of a marine snail from NW Spain

The evolution and maintenance of color clines is a classic topic of research in evolutionary ecology.However,studies analyzing the temporal dynamics of such clines are much less frequent,due to the difficulty of obtaining reliable data about past color distributions along environmen-tal gradients.In this article,we describe a case of decades-long temporal stability and directional change in a color cline of the marine snail Littorina saxatilis along the coastal inlet of the Ria de Vigo(NW Spain).L.saxatilis from this area shows a clear color cline with 3 distinct areas from the innermost to the more wave-exposed localities of the Ria:the inner,protected localities show an abundance of fawn-like individuals;the intermediate localities show a high diversity of colors;and the outer,wave-exposed localities show populations with a high frequency of a black and lineated morph.We compare data from the 1970s and 2022 in the same localities,showing that the cline has kept relatively stable for at least over half a century,except for some directional change and local variability in the frequency of certain morphs.Multiple regression analyses and biodiversity measures are presented to provide clues into the selective pressures that might be involved in the maintenance of this color cline.Future research avenues to properly test the explanatory power of these selective agents as well as the possible origins of the cline are discussed.

Design and selection of advanced protection schemes have become essential for reliable and secure operation of networked microgrids.Various protection schemes that allow correct operation of microgrids have been proposed for individual systems in different topologies and connections.Nevertheless,protection schemes for networked microgrids are still in devel-opment,and further research is required to design and operate advanced protection in interconnected systems.Interconnection of these microgrids in different nodes with various intercon-nection technologies increases fault occurrence and complicates protection operation.This paper aims to point out challenges in developing protection for networked microgrids,potential solutions,and research areas that need to be addressed for their development.First,this article presents a systematic analysis of different microgrid clusters proposed since 2016,including several architectures of networked microgrids,operation modes,components,and utilization of renewable sources,which have not been widely explored in previous review papers.Second,the paper presents a discussion on protection systems currently available for microgrid clusters,current challenges,and solutions that have been proposed for these systems.Finally,it discusses the trend of protection schemes in networked microgrids and presents some conclusions related to implementation.IndexTerms—Adaptive eprotection,microgrid cluster,microgrid,multiple microgrid,networked microgrid,real-time simulation,smart grid.

Anoplophora glabripennis(Asian longhorn beetle,ALB)and Anoplophora chinensis(citrus longhorn beetle,cLB)are native forest pests in china;they have become important international quarantine pests.They are found using the same Salix aureopendula host tree of cixi,Zhejiang province,china.On this host tree,we collected additional beetles that appeared to be morphologically intermediate between ALB and cLB.By using a stereoscope,we observed that there were several bumps on the base of the elytra,which was inconsistent with ALB,which typically has a smooth elytral base,but was more like cLB,which has numerous short tubercles on the elytral base.Given their sympatry and intermediate morphology,we hypothesized that these may represent ALB×cLB hybrids.We studied the genomic profiles for 46 samples(ALB,cLB,and putative hybrids)using genotyping-by-sequencing(GBS)providing a reduced representation of the entire genome.Employing principal component analyses on the 163 GBS-derived single nucleotide polymorphism data,we found putative hybrids tightly clustered with ALB,but genetically distinct from the cLB individuals.Therefore,our initial hybrid hypothesis was not supported by genomic data.Further,while mating experiments between adult ALB and cLB were successful in 4 separate years(2017,2018,2020,and 2021),and oviposition behavior was observed,no progeny was produced.Having employed population genomic analysis and biological hybridization experiments,we conclude that the putative hybrids represent newly discovered morphological variants within ALB.Our approach further confirmed the advantage of genome-wide information for Anoplophora species assignment in certain ambiguous classification cases.

Asperuloside Promotes Apoptosis of cervical cancer cells through Endoplasmic Reticulum Stress-Mitochondrial Pathway

Objective: To investigate the effects of asperuloside on cervical cancer based on endoplasmic reticulum(ER) stress and mitochondrial pathway. Methods: Different doses(12.5–800 μg/mL) of asperuloside were used to treat cervical cancer cell lines Hela and caSki to calculate the half maximal inhibitory concentration(Ic_(50)) of asperuloside. The cell proliferation was analyzed by clone formation assay. cell apoptosis, intracellular reactive oxygen species(ROS) and mitochondrial membrane potential were determined by flow cytometry. The protein expressions of cleaved-caspase-3, Bcl-2, Bax, cyt-c, cleaved-caspase-4 and glucose-regulated protein 78(GRP78) were analyzed by Western blot. And the inhibitor of ER stress, 4-phenyl butyric acid(4-PBA) was used to treat cervical cancer cells to further verify the role of ER stress in the apoptosis of cervical cancer cells induced by asperuloside. Results: Asperuloside of 325, 650, and 1300 μg/mL significantly inhibited the proliferation and promoted apoptosis of Hela and caSki cells(Pcantly increased intracellular ROS levels, reduced mitochondrial membrane potential, significantly reduced Bcl-2 protein expression level, and increased Bax, cyt-c, GRP78 and cleaved-caspase-4 expressions(Pcantly promoted cell proliferation and reduced apoptosis(Pcould reverse 4-PBA-induced increased cell proliferation, decreased apoptosis and cleaved-caspase-3,-4 and GRP78 protein expressions(Pconclusion: Our study revealed the role of asperuloside in cervical cancer, suggesting that asperuloside promotes apoptosis of cervical cancer cells through ER stress-mitochondrial pathway.
