Magnetically controlled growing instrumentation for early onset scoliosis: Caution needed when interpreting the literature

BACKGROUND Magnetically controlled growing rods(MCGR) are a novel treatment option for early onset scoliosis(EOS). Although the complication profile with MCGR use has been reviewed, these reviews do not take into account important implants modifications, termed iterations, that were made due to early on postoperative complications is not well reported or *** To assess the effect of MCGR implant iterations on post-operative complications in *** A systematic review was performed to identify studies investigating MCGR specifically for the treatment of EOS, refined to those reporting the implant iteration, specifically the incorporation of the keeper plate to the implant *** with mixed implant iteration usage were excluded. Complications following surgery were recorded as well as potential risk factors and compared between implant *** Although 20 articles were identified for inclusion, 5 included mixed implant iteration leaving a total of 271 patients identified through 15 clinical studies thatmet inclusion criteria. The average follow-up was 25.4-mo. Pre-keeper plate implants were utilized in 3 studies with a total of 49 patients. Overall, 115(42.4%)post-operative complications were identified, with 87% defined as major. The addition of the keeper plate significantly decreased the rate of post-operative complications per study(35.7% vs 80.6%, P = 0.036), and the rate of distraction failure(8.1% vs 40.8%, P = 0.02). Unplanned reoperation occurred in 69(26.7%)patients but was not different between implant iteration cohorts(25.5% without keeper plate vs 27.1% with keeper plate, P = 0.92).CONClUSION MCGR for EOS has a cumulative complication rate of 42.4% but this is significantly reduced to 35.7% when reviewing only keeper-plate enabled implants. However, 25% of published articles included mixed implant *** studies should discern between implants iterations when reporting on the usage of MCGR for EOS.

New geochronologic data from midcontinental laurentia demonstrate that emplacement of the 1476-1470 Ma Wolf River granitic batholith was not an isolated igneous event,but was accompanied by regional metamorphism,deformation,and *** for such metamorphism and deformation is best seen in siliciclastic sedimentary rocks of the Baraboo Interval,which were deposited closely following the 1.65-1.63 Ga Mazatzal *** Baraboo Interval strata,muscovite parallel to slatey cleavage,in hydrothermal veins,in quartzite breccia,and in metamorphosed paleosol yielded ^(40)Ar/^(39)Ar plateau ages of 1493-1465 *** addition,U-Th-total Pb dating of neoblastic overgrowths on detrital monazite gave an age of 1488±20 Ma,and recrystallized hematite in folded metapelite gave a mean U/Th-He age of 1411±39 ***-Baraboo,arkosic polymictic conglomerate,which contains detrital zircon with a minimum peak age of 1493 Ma,was intruded by a 1470 Ma granite porphyry at the northeastern margin of the Wolf River *** episode of magmatism,regional deformation and metamorphism,and sedimentation,which is designated herein as the Baraboo orogeny,provides a midcontinental link between the Picuris orogeny to the southwest and the Pinware orogeny to the northeast,completing the extent of early Mesoproterozoic(Calymmian)orogenesis for 5000 km along the southern margin of *** transcontinental orogen is unique among Precambrian orogenies for its great width(-1600 km),the predominance of ferroan granites derived from partial melting of lower continental crust,and the prevalence of regional high T-P metamorphism related to advective heating by granitic magmas emplaced in the middle to upper crust.

The normal direction to the normal direction to a line in Minkowski geometries generally does not give the original line. We show that in lp geometries with p>1?repeatedly finding the normal line through the origin gives sequences of lines that monotonically approach specific lines of symmetry of the unit circle. Which lines of symmetry that are approached depends upon the value of p and the slope of the initial line.

A Construction That Produces Wallis-Type Formulas

Generalizations of the geometric construction that repeatedly attaches rectangles to a square, originally given by Myerson, are presented. The initial square is replaced with a rectangle, and also the dimensionality of the construction is increased. By selecting values for the various parameters, such as the lengths of the sides of the original rectangle or rectangular box in dimensions more than two and their relationships to the size of the attached rectangles or rectangular boxes, some interesting formulas are found. Examples are Wallis-type infinite-product formulas for the areas of p-circles with p > 1.

Controlled breakdown has recently emerged as a highly accessible technique to fabricate solid-state ***,in its most common form,controlled breakdown creates a single nanopore at an arbitrary location in the ***,we introduce a new strategy whereby breakdown is performed by applying the electric field between an on-chip electrode and an electrolyte solution in contact with the opposite side of the *** demonstrate two advantages of this ***,we can independently fabricate multiple nanopores at given positions in the membrane by localising the applied field to the ***,we can create nanopores that are self-aligned with complementary nanoelectrodes by applying voltages to the on-chip electrodes to locally heat the membrane during controlled *** new controlled breakdown method provides a path towards the affordable,rapid,and automatable fabrication of arrays of nanopores self-aligned with complementary on-chip nanostructures.

Centrifugal blood pumps have become popular for adult extracorporeal membrane oxygenation(ECMO)due to their superior blood handling and reduced thrombosis risk featured by their secondary flow paths that avoid stagnant ***,the high rotational speed within a centrifugal blood pump can introduce high shear stress,causing a significant shear-induced hemolysis *** Revolution pump,the Rotaflow pump,and the CentriMag pump are three of the leading centrifugal blood pumps on the *** many experimental and computational studies have focused on evaluating the hydraulic and hemolytic performances of the Rotaflow and CentriMag pumps,there are few on the Revolution ***,a thorough direct comparison of these three pumps'flow characteristics and hemolysis is not *** this study,we conducted a computational and experimental analysis to compare the hemolytic performances of the Revolution,Rotaflow,and CentriMag pumps operating under a clinically relevant condition,i.e.,the blood flow rate of 5 l/min and pump pressure head of 350 mmHg,for adult ECMO *** silico simulations were used to characterize the shear stress distributions and predict the hemolysis index,while in vitro blood loop studies experimentally determined hemolysis *** simulation results and experimental data demonstrated that the CentriMag pump caused the lowest hemolysis while the Revolution pump generated the highest hemolysis.

Background The number of mobile stroke programmes has increased with evidence,showing they expedite intravenous *** questions include whether time savings extend to patients eligible for endovascular therapy and impact clinical *** Our mobile stroke unit(MSU),based at an academic medical centre in upstate New York,launched in October *** reviewed prospective observational data sets over 26 months to identify MSU and non-MSU emergency medical service(EMS)patients who underwent intravenous thrombolysis or endovascular thrombectomy for comparison of angiographic and clinical *** Over 568 days in service,the MSU was dispatched 1489 times(2.6/day)and transported 300 patients(20%of dispatches).Intravenous tissue plasminogen activator(tPA)was administered to 57 MSU patients and the average time from 911 call-to tPA was 42.5 min(±9.2),while EMS transported 73 patients who received tPA at 99.4 min(±35.7)(plast known well and would likely have been outside the window with EMS *** thrombectomy was performed on 21 MSU patients with an average 911 call-to groin puncture time of 99.9 min(±18.1),while EMS transported 54 patients who underwent endovascular thrombectomy(ET)at 133.0 min(±37.0)(p=0.0002).There was no difference between MSU and traditional EMS in modified Rankin score at 90-day clinic follow-up for patients undergoing intravenous thrombolysis or endovascular thrombectomy,whether assessed as a dichotomous or ordinal *** Mobile stroke care expedited both intravenous thrombolysis and endovascular *** is an ongoing need to show improved functional outcomes with MSU care.

English professional football players concussion knowledge and attitude

Background： Concussions are a common pathology in football and multiple misconceptions exist amongst the players and managers. To address these misconceptions, and potentially reduce concussion associated sequela, effective educational interventions need to be developed. However, the current knowledge and attitude status must be ascertained to appropriately develop these interventions. The purpose of this study was to assess the concussion knowledge and attitude of English professional footballers. Methods： Twenty-six participants from one English Football league Championship club completed the study. A mixed methods approach included the Rosenbaum Concussion Knowledge and Attitudes Survey （RoCKAS） and a semi-structured interview. The RoCKAS contains separate knowledge （055） and attitude （15-75） scores and was followed by a semi-structured interview consisting of concussion knowledge, attitude, and behavior related questions. Results： The mean score on the *** knowledge was 16.4± 2.9 （range 11-22） and the attitude score was 59.6 ± 8.5 （range 41-71）. The interview responses identified inconsistencies between the RoCKAS and the intended behaviors, endorsing multiple concussion misconceptions, and revealed barriers to concussion reporting. Conclusion： The results of this study suggest that Championship level English footballers have moderate concussion knowledge, safe attitudes, and good concussion symptom recognition when assessed with pen and paper questionnaires. However, within the semi-structured interview many respondents reported unsafe concussion behaviors despite accurately identifying the potential risks. Further, multiple barriers to concussion reporting were identified which included perceived severity of the injury, game situations, and the substitution rule. These findings can help form the foundation of educational interventions to potentially improve concussion reporting behaviors amongst professional footballers.
