It is estimated that 20000 to 30000 new patients are diagnosed with osteonecrosis annually accounting for approximately 10% of the 250000 total hip arthroplasties done annually in the United States. Thelack of level 1 evidence in the literature makes it difficult to identify optimal treatment protocols to manage patients with pre-collapse avascular necrosis of the femoral head, and early intervention prior to collapse is critical to successful outcomes in joint preserving procedures. There have been a variety of traumatic and atraumatic factors that have been identified as risk factors for osteonecrosis, but the etiology and pathogenesis still remains unclear. Current osteonecrosis diagnosis is dependent upon plain anteroposterior and frog-leg lateral radiographs of the hip, followed by magnetic resonance imaging(mRI). Generally, the first radiographic changes seen by radiograph will be cystic and sclerotic changes in the femoral head. Although the diagnosis may be made by radiograph, plain radiographs are generally insufficient for early diagnosis, therefore mRI is considered the most accurate benchmark. Treatment options include pharmacologic agents such as bisphosphonates and statins, biophysical treatments, as well as joint-preserving and joint-replacing surgeries. the surgical treatment of osteonecrosis of the femoral head can be divided into two major branches: femoral head sparing procedures(FHSP) and femoral head replacement procedures(FHRP). In general, FHSP are indicated at pre-collapse stages with minimal symptoms whereas FHRP are preferred at post-collapse symptomatic stages. It is difficult to know whether any treatment modality changes the natural history of core decompression since the true natural history of core decompression has not been delineated.

many transportation agencies lack sufficient funds to maintain and repair roads, which results into increased pavement maintenance cost. Pavement management System （PmS） has demonstrated to be an essential tool for proper management of infrastructure and proper utilization of available funds. The University of Tennessee at Chattanooga utilized micropaver software as PmS tool to conduct a pavement management analysis of principal arterials in the City of Chattanooga. The study used the City of Chattanooga pavement database to create the current and future pavement conditions. maintenance and repair （m＆R） planning analysis was also performed in order to determine the most cost-effective treatment and suggest the optimum utilization of funds for the city. An analysis of five budget scenarios was conducted for a five-year plan using the critical pavement condition index （PCI） method （ASTm D6433）. Results show that the backlog elimination budget would be the best scenario because it increases the pavement condition and eliminates the backlog of major maintenance and repairs over the five-year period. The unlimited budget seems though ideal, it does not improve pavement condition. maintaining current condition and limited budget scenarios would increase both the backlog and the total cost of maintenance and repairs over the analysis period.

Incorporation of Biochar Carbon into Stable Soil Aggregates:The Role of Clay mineralogy and Other Soil Characteristics

Aggregation and structure play key roles in water-holding capacity and stability of *** this study,the incorporation of carbon(C) from switchgrass biochar into stable aggregate size fractions was assessed in an Aridisol(from Colorado,USA) dominated by 2:1 clays and an Alfisol(from Virginia,USA) containing weathered mixed 1:1 and 2:1 mineralogy,to evaluate the effect of biochar addition on soil *** biochar was applied at 4 levels,0,25,50,and 100 g kg^(-1),to the soils grown with wheat in a growth chamber *** changes in soil strength and water-holding capacity using water release curves were *** the Colorado soil,the proportion of soil occurring in large aggregates decreased,with concomitant increases in small size *** changes in aggregate size fractions occurred in the Virginia *** the Colorado soil,C content increased from 3.3 to 16.8 g kg^(-1),whereas in them fraction C content increased from 5.7 to 22.6 g kg^(-1) with 100 g kg^(-1)biochar *** the Virginia soil,C content within aggregate size fractions increased for each size fraction,except the>2 000 μm *** greatest increase(from 6.2 to 22.0 g kg^(-1)) occurred in the 53–250 μm *** results indicated that C was incorporated into larger aggregates in the Virginia soil,but remained largely unassociated to soil particles in the Colorado *** addition had no significant effect on water-holding capacity or strength *** biochar to more weathered soils with high native soil organic content may result in greater stabilization of incorporated C and result in less loss because of erosion and transport,compared with the soils dominated by 2:1 clays and low native soil organic content.

AIm To compare(1) demographics in urea breath test(UBT) vs endoscopy patients; and(2) the molecular detection of antibiotic resistance in stool vs biopsy *** Six hundred and sixteen adult patients undergoing endoscopy or a UBT were prospectively recruited to the study. The Geno Type Helico DR assay was used to detect Helicobacter pylori(H. pylori) and antibiotic resistance using biopsy and/or stool samples from CLOpositive endoscopy patients and stool samples from UBT-positive patients. RESULTS Infection rates were significantly higher in patients referred for a UBT than endoscopy(overall rates: 33% vs 19%; treatment-na?ve patients: 33% vs 14.7%, respectively). H. pylori-infected UBT patients were younger than H. pylori-infected endoscopy patients(41.4 vs 48.4 years, respectively, P males in the endoscopy-compared to the UBT-cohort(52.6% vs 33.3%, P = 0.03). The Geno Type Helico DR assay was more accurate at detecting H. pylori infection using biopsy samples than stool samples [98.2%(n = 54/55) vs 80.3%(n =53/66), P mples from CLO-positive endoscopy patients revealed a higher detection rate ofresistance-associated mutations using stool samples compared to biopsies. The concordance rates between stool and biopsy samples for the detection of H. pylori DNA, clarithromycin and fluoroquinolone resistance were just 85%, 53% and 35%, respectively. CONCLUSION Differences between endoscopy and UBT patients provide a rationale for non-invasive detection of H. pylori antibiotic resistance. However, the Geno Type Helico DR assay is an unsuitable approach.
