A logarithmic rapid desensitization protocol: initial experience in carboplatin hypersensitivity reactions

Aim: Hypersensitivity reactions to carboplatin are not an infrequent adverse event in ovarian cancer patients. However, reintroduction of platinum-containing schedules is the standard of care in platinum-sensitive recurrent ovarian cancer. Rapid desensitization is a procedure for gradual reintroduction of drug. It allows a safe administration of medications that are beneficial for the management of patients after certain types of hypersensitivity reactions. It is indicated in cases in which there are no reasonable therapeutic alternatives. Methods: We performed a descriptive retrospective study of high-grade ovarian cancer patients with known carboplatin hypersensitivity reactions that were treated with a 13-steps rapid desensitization protocol with 3 different solutions and infusion rates. The procedure followed a mathematic model (gradual increases with a relationship between doses following a geometric series) which is called logarithmic rapid desensitization protocol (LRDp). The aim was to describe the safety of the LRDp in terms of number and severity of infusion reactions and the effectiveness in the rate of cycles completely administered. Results: Four different patients diagnosed with recurrent platinum-sensitive ovarian cancer with a previous infusion reaction were included. LRDp was administered in 19 different cycles. LRDp was administered safety in all 19 cycles, only 2 patients had a mild cutaneous reaction in 4 different cycles during LRDp (21.05%). The foreseen dose of carboplatin was fully administered in all ***: LRDp with carboplatin is a feasible and safe protocol in patients with previous infusion reaction to carboplatin. The protocol might allow a safe administration of drugs, that are beneficial for the management of patients, after certain types of hypersensitivity reactions, and it is indicated in cases in which there are no reasonable therapeutic alternatives.

The use of steel-making slag as catalysts for microwave-assisted dry reforming of CH4 was studied. Two carbon materials (an activated carbon and a metallurgical coke), mixtures of the carbon materials and Fe-rich slag, and mixtures of the carbon materials and Ni/Al2O3 were tested as catalysts. The mixtures of slag with carbons gave rise to higher and steadier conversions than those achieved over the carbon materials alone. In addition, the use of the metallurgical coke mixed with metal-rich catalysts gave rise to remarkable results. Thus, no CH4 and CO2 conversions were achieved when coke was used alone, whereas high conversions were obtained when it was mixed with the metal-rich catalysts.

Effect of water chemistry on the aggregation and photoluminescence behavior of carbon dots

Genotypic Assessment by RApD Markers and Ultrastructural Characteristics of a NaCI-Tolerant potato Cell Line

Salinity is a serious threat to agricultural production. potato （Solanum tuberosum） is an important food crop characterised for having low to moderate salinity tolerance. Tissue cultures may be relevant to improve salt tolerance in potato through selection of salt-tolerant cell lines and subsequent regeneration of plants. In this work, the authors used the random amplified polymorphic DNA （RApD） markers to investigate the occurrence of genetic polymorphism in a potato calli line tolerant to 150 mM NaCI. Out of 40 primers screened, eight generated polymorphic patterns that distinguished salt-tolerant line from the control. Although the macroscopic appearance was similar in both lines, ultrastructural study revealed alterations in salt-grown cells. These showed that plastids less differentiated with a lower number of grana had more and larger starch grains than control cells. In conclusion, RApD analysis revealed that NaCl-adapted line is a somaclonal variant and the ultrastructural study showed changes essentially at the plastids.

MM： To investigate whether, under the influence of pol ypectomy, the incidence of adenoma decreases with age.METHODS： Consecutive patients with colonic adenomas identified at index colonoscopy were retrospectively selected if they had undergone three or more complete colonoscopies, at least 24 mo apart. patients who had any first-degree relative with colorectal cancer were excluded. Data regarding number of adenomas at each colonoscopy, their location, size and histological classification were recorded. The monthly incidence density of adenomas after the index examination was estimated for the study population, by using the person-years method. Baseline adenomas were excluded from incidence calculations but their characteristics were correlated with recurrence at follow-up, using the X^42 test.RESULTS： One hundred and fifty-six patients were included （109 male, mean age at index colonoscopy 56.8 ± 10.3 years）, with follow-up that ranged from 48 to 232 mo. No significant correlations were observed between the number, the presence of villous component, or the size of adenomas at index colonoscopy and the presence of adenomas at subsequent colonoscopies （p = 0.49, 0.12 and 0.78, respectively）. The incidence of colonic adenomas was observed to decay from 1.4% person-months at the beginning of the study to values close to 0%, at 12 years after index colonoscopy.CONCLUSION： Our results suggest the sporadic formation of adenomas occurs within a discrete period and that, when these adenomas are removed, all neoplasia-prone clones may be extinguished.

The decomposition and CO2 reforming of methane,respectively,are promising alternatives to industrial steam methane reforming. In recent years,research has been focused on the development of catalysts that can operate without getting deactivated by carbon deposition,where,in particular,carbon catalysts have shown positive results. In this work,the role of carbon materials in heterogeneous catalysis is assessed and publications on methane decomposition and CO2 reforming of methane over carbon materials are reviewed. The influence of textural properties(BET surface area and micropore volume,etc.) and oxygen surface groups on the catalytic activity of carbon materials are discussed. In addition,this review examines how activated carbon and carbon black catalysts,which are the most commonly used carbon catalysts,are deactivated. Characteristics of the carbon deposits from methane are discussed and the influence of the reactivity to CO2 of fresh carbon and carbonaceous deposits for high and steady conversion during CO2 reforming of CH4 are also considered.

Unusual Expansive Mass in the Thyroid Cartilage

Introduction: Giant cell tumor of the larynx is an uncommon entity with only 42 reported cases in the literature, of which 32 involved the thyroid cartilage. These tumors most commonly occur in the epiphysis of the long bones of female patients in the third decade. Case Report: We present a 57-year-old man with hoarseness and a growing anterior neck mass for the past 5 months. Endoscopic examination revealed a submucosal swelling of the left supraglottic area. CT and MRI of the larynx showed a large, expansive mass at the left thyroid cartilage. The anterior neck mass was biopsied and histopathological analysis showed mononuclear cells and homogeneously distributed multinucleated giant cells. A giant cell tumor of the larynx was suspected. The patient underwent a supracricoid subtotal laryngectomy. Conclusion: Giant cell tumor of the larynx is an uncommon entity, with very few cases reported in the literature. These tumors should be included in the differential diagnosis of a patient with hoarseness and anterior neck swelling. There is no consensus regarding the management, although surgical excision is the most frequently employed treatment with excellent outcomes.
