Prevalence and risk factors of acute-on-chronic liver failure in a single center from Argentina

AIMTo study the prevalence, characteristics, risk factors and mortality at 28 d of acute-on-chronic liver failure (AcLF). METHODSA total of 100 cirrhotic patients admitted to our hospital for more than one day were included during the period between June 2013 and December 2015. We used the European Association for the Study of the Liver-chronic Liver Failure-consortium diagnostic criteria for AcLF, considering it as the acute decompensation of cirrhosis associated with the presence of one or more organ failure. For the diagnosis of organic failure the chronic Liver Failure-Sequential Organ Failure Assessment score was used. Our population was divided into patients with and without AcLF. clinical characteristics, presence of precipitating events, potential risk factors for developing AcLF and causes of mortality were analyzed. Mortality at 28 d was evaluated. RESULTSTwenty-nine patients (29%) developed AcLF criteria. Alcoholism, detected in 58 patients (58%), was the major etiological agent of cirrhosis. Bacterial infections were recognized as a precipitating event in 41.3% of cases and gastrointestinal bleeding in 27.5%. No precipitating event was identifiable in 27.5% of patients with AcLF. comparing patients with and without AcLF, statistically significant risk factors were: child Pugh score 10.2 ± 2.1 vs 8.4 ± 1.6 (P ˂ 0.0001), MELD score 20.7 ± 8.5 vs 12.3 ± 4 (P ˂ 0.0001), presence of ascites 27 (93%) vs 43 (60.5%) (P = 0.001), leukocytosis 15300 ± 8033 per cubic millimeter vs 10770 ± 5601 per cubic millimeter (P ˂ 0.0001), and high plasma levels of c reactive protein values 50.9 ± 46.4 mg/L vs 28.6 ± 23.4 mg/L (P ˂ 0.0019). Mortality rate was 62% (18 patients) vs 5.6% (4 patients), respectively (P cONcLUSIONWe observed that the AcLF is a frequent entity in this group of patients and has a significantly higher mortality rate.

We prove the existence of solitons （finite energy solitary wave） for a Boussi- nesq system that arise in the study of the evolution of small amplitude long water waves including surface tension. This Boussinesq system reduces to the generalized Benney-Luke equation and to the generalized Kadomtsev-Petviashivili equation in appropriate limits. The existence of solitons follows by a variational approach involving the Mountain Pass Theorem without the Palais-Smale condition. For surface tension sufficiently strong, we show that a suitable renormalized family of solitons of this model converges to a nontrivial soliton for the generalized KP-I equation.

The presented paper puts in discussion the dilemma of the linear supports of formal teaching and learning processes of regular school, coexisting with an inter and transdisciplinarity of the process mentioned above. In this perspective, the author wonders about the subjectivity of the students＇ apprenticeship and about the way they can change, according to the methodological variants, as well as the context. So, he intents to explore the c.P.M.P.S （creative and Participative Method of Problem Solution） in the development of Portuguese written language competence, through the art subject. In this paper, we can see the increase of different variables from the perspective of students and teachers which validates in some manner of the teaching methodology.

Background: Increased peak oxygen consumption(VO2 peak) can reduce cardiovascular risks associated with obesity. Our aim was to analyze the effect of a weight loss program on cardiovascular fitness in overweight(W) and obese(O) ***: One hundred and sixty-seven subjects(77 males and 90 females), aged 18–50 years, performed a modified Bruce protocol before(pre)and after(post) a weight loss program of 24 weeks. This program combined physical training(strength, S; endurance, E; combined strength + endurance, SE; or physical activity recommendation, PA) 3 times per week, with a 25%–30% caloric restriction ***: VO2 peak improved in overweight and obese males(pre and post values in L/min, respectively; W = 3.2 ± 0.6 vs. 3.7 ± 0.5, p creased for all 4 interventions in males(p cept for S in the obese group(1.6 ± 0.2 vs. 1.7 ± 0.3, p = 0.141). In females, it increased in E(0.9 ± 0.2 vs. 1.4 ± 0.3, p cts except for females in PA group(15.7 ± 0.3 min vs. 15.9 ± 0.3 min, p = 0.495).conclusion: Our results suggest that all methods, including the recommendation of physical activity, can improve cardiovascular fitness in overweight subjects and obese males.

Arginine-glycine-aspartic acid （RGD） dendrimer-based nanopatterns on poly（L- lactic acid） were used as bioactive substrates to evaluate the impact of the RGD local surface density on the chondrogenic induction of adult human mesenchymal stem cells. During chondrogenic commitment, active extracellular matrix （EcM） remodeling takes place, playing an instructive role in the differentiation process. Although three-dimensional environments such as pellet or micromass cultures are commonly used for in vitro chondrogenic differentiation, these cultures are rather limited with respect to their ability to interrogate cells in celI-EcM interactions. In the present study, the nanopatterns of the tunable RGD surface density were obtained as a function of the initial dendrimer concentration. The local RGD surface density was quantified through probability contour plots for the minimum interparticle distance, constructed from the corresponding atomic force microscopy images, and correlated with the cell adhesion and differentiation response. The results revealed that the local RGD surface density at the nanoscale acts as a regulator of chondrogenic commitment, and that intermediate adhesiveness of cells to the substrates favors mesenchymal cell condensation and early chondrogenic differentiation.

After a criticism on today’s model for electrical noise in resistors, we pass to use a Quantum-compliant model based on the discreteness of electrical charge in a complex Admittance. From this new model we show that carrier drift viewed as charged particle motion in response to an electric field is unlike to occur in bulk regions of Solid-State devices where carriers react as dipoles against this field. The absence of the shot noise that charges drifting in resistors should produce and the evolution of the Phase Noise with the active power existing in the resonators of L-c oscillators, are two effects added in proof for this conduction model without carrier drift where the resistance of any two-terminal device becomes discrete and has a minimum value per carrier that is the Quantum Hall resistance Rk=h/q2

Luminescent hollow micro-and nanocrystals have been successfully obtained taking advantage of the self-assembly behavior and the aggregation-induced emission enhancement properties of several bispyrazolate Pt(II)metallomesogens decorated with four terminal alkyl ***-in-water droplets have been used to confine the Pt(II)compounds and drive them to be self-assembled via intermolecular Pt…Pt interactions into spherical aggregates of about 200 or 50 *** of the oil phase generates highly-stable aqueous dispersions of nanocrystals that emit a bright orange light as a result of the existence of^(3)MMLcT excited *** methods and conditions have been tested for studying the effect of several parameters such as the temperature and the stirring speed in the final particle size and in the polydispersity ***,the micro-and nanocrystals are able to entrap hydrophobic drugs between the alkyl chains of the compounds,forming stable dispersions of drug-loaded capsules in *** droplet method is applied in the area of metallomesogens for the first time to synthesize self-assembled Pt(II)nanocapsules,which opens a new field of study that could allow the use of these liquid crystal materials in biomedical applications.
