new therapeutic strategy with extracorporeal membrane oxygenation for refractory hepatopulmonary syndrome after liver transplant: A case report

BACKGROUnD Due to the lack of published literature about treatment of refractory hepatopulmonary syndrome(HPS)after liver transplant(LT),this case adds information and experience on this issue along with a treatment with positive *** is a complication of end-stage liver disease,with a 10%-30%incidence in cirrhotic *** can reverse the physiopathology of this process and restore normal ***,in some cases,refractory hypoxemia persists,and extracorporeal membrane oxygenation(ECMO)can be used as a rescue therapy with good *** SUMMARY A 59-year-old patient with alcohol-related liver cirrhosis and portal hypertension was included in the LT waiting list for *** had good liver function(Model for End-Stage Liver Disease score 12,Child-Pugh class B7).He had pulmonary fibrosis and a mild restrictive respiratory pattern with a basal oxygen saturation of 82%.The macroaggregated albumin test result was>*** demonstrated a forced expiratory volume in one second(FEV1)of 78%,forced vital capacity(FVC)of 74%,FEV1/FVC ratio of 81%,diffusion capacity for carbon monoxide of 42%,and carbon monoxide transfer coefficient of 57%.He required domiciliary oxygen at 2 L/min(16 h/d).The patient was admitted to the intensive care unit(ICU)and extubated in the first 24 h,needing high-flow therapy and non-invasive ventilation and inhaled nitric oxide *** was needed after 72 *** to the non-response to supportive therapies,installation of ECMO was decided with progressive recovery after 9 *** was possible on the tenth day,maintaining a high-flow nasal cannula and de-escalating to conventional oxygen therapy after 48 *** was discharged from ICU on postoperative day(POD)20 with a 90%-92%oxygen *** recycling was needed twice for acute *** patient was discharged from hospital on POD 27 with no symptoms,with an 89%-90%oxygen *** Due to the favorable results observed,ECMO could bec

To explain the presence and spatial distribution of nO_(3)^(−)and n-nH_(3)in the Aquifer of the Metropolitan Area of Mexico City(AMAMC),a hydrogeochemical and isotopic analysis using^(13)C DIC(as well as the stable isotopes^(18)O and^(2)H)in groundwater was *** aquifer is located in an old closed lacustrine volcano-sedimentary basin;some wells hosted in the semi-confined zone contain high n-nH_(3)concentrations,while others present nO_(3)^(−)contents in the recharge zones(hosted in an oxidizing environment).In this study,a change in the isotopic signature(primarily in^(18)O and^(2)H)was observed from the recharge zones to the basin center in some of the wells with high nO_(3)^(−)concentrations,this behavior can be attributed to evaporation during the incorporation of recently infiltrated *** addition,the results for^(13)C(along with ^(2) H)in wells with the highest n-nH_(3)concentrations exhibited an atypically broad range of *** indicated the occurrence of hydrogeochemical and/or biochemical processes in the aquifer(in an oxidizing or reducing environment),such as organic degradation,bacterial decomposition(primarily in the ancient Lake Texcoco and which acts as a natural sink for carbon,nitrogen,sulfur,and phosphorus),besides rock weathering and dissolution,which may be responsible for a very marked isotopic modification of the^(13)C(and,to a lesser extent,2 H).Methanotrophic bacterial activity and methanogenic activity may be related to n-nH_(3)removal processes by oxidation and residual water incorporation respectively,whereas the increase in the nO_(3)^(−)content in some wells is due to the recent contribution of poor-quality water due to contamination.

Present and future of new systemic therapies for early and intermediate stages of hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is a high mortality neoplasm which usually appears on a cirrhotic *** therapeutic arsenal and subsequent prognostic outlook are intrinsically linked to the HCC stage at *** the current deployment of treatments with curative intent(liver resection/local ablation and liver transplantation)in early and intermediate stages,a high rate of HCC recurrence persists,underscoring a pivotal clinical *** systemic therapies(ST),particularly immunotherapy,have demonstrate promising outcomes in terms of increase overall survival,but they are currently bound to the advanced stage of *** review provides a comprehensive analysis of the literature,encompassing studies up to March 10,2024,evaluating the impact of novel ST in the early and intermediate HCC stages,specially focusing on the findings of neoadjuvant and adjuvant regimens,aimed at increasing significantly overall survival and recurrence-free survival after a treatment with curative *** also investigate the potential role of ST in enhancing the downstaging rate for the intermediate-stage HCC initially deemed ineligible for treatment with curative ***,we critically discuss about the current relevance of the results of these studies and the encouraging future implications of ST in the treatment schedules of early and intermediate HCC stages.

Association between lifestyle factors and thyroid function in young euthyroid adults

Purpose:The present work examines the associations of dietary habits,sedentarism,physical activity(PA)levels and sleep habits,with thyroid function in young euthyroid ***:A total of 105 young euthyroid adults participated in this cross-sectional *** function was determined in fasting conditions(>6 h).Dietary habits were measured by a food frequency questionnaire and three non-consecutive 24 h recalls,and different dietary intake and patterns were then *** time spent in sedentary,PA levels and sleep habits were objectively measured using a wrist-worn ***:Energy and carbohydrate intake were positively associated with thyroid stimulating hormone(TSH)(β=0.222;R^(2)=0.102;P=0.022 andβ=0.425;R^(2)=0.129;P=0.007,respectively)whereas fat intake was negatively associated with TSH(β=-0.428;R^(2)=0.137;P=0.004).Energy intake was also positively associated with free triiodothyronine(β=0.277;R^(2)=0.137;P=0.004).Further,adherence to the Mediterranean diet was negatively related to TSH and free thyroxine(FT4)(β=-0.221;R^(2)=0.113;P=0.020 andβ=-0.268;R^(2)=0.071;P=0.007,respectively).Vigorous-intensity and overall PA were negatively associated with FT4(β=-0.227;R^(2)=0.052;P=0.022 andβ=-0.204;R^(2)=0.042;P=0.041,respectively).In contrast,no associations were found between sleep parameters and thyroid ***:Lifestyle factors such as dietary intake and PA levels seems to be related to thyroid function even in young euthyroid adults.

盾构隧道环间剪切效应模拟分析-以隧道开孔为例

盾构隧道处于不对称荷载下或结构体系发生变化时（如隧道联络通道开孔等），环缝剪切对盾构隧道的结构响应影响增大，环间剪切效应将进一步影响结构的极限承载力。然而现有的模型对环缝接头进行了不同程度的简化，这给在此过程中对隧道内环间剪切分布进行全面细致的分析带来了困难。为此，以接触原理为基础，建立一种“壳-接触”精细化模型，并与机械法联络通道主隧道开孔足尺试验结果对比，探明该过程的盾构隧道的环间剪切效应机理。研究结果表明：壳-接触模型采用“软接触”的方式，在保证可以模拟接头转动的前提下，也可以实现接头位置压力与剪切的耦合模拟，与实际环缝力学性质相符；在机械法联络通道开孔案例中，考虑摩擦损失的精细化模型结果对足尺实验结果模拟较好；开孔管片的环间剪切以径向剪切力为主；结构体系发生变化时，环缝的剪切力不再满足自平衡现象；且环缝剪切力的突变位置均为纵缝的位置；环间的最大径向剪切力发生位置揭示了盾构隧道开孔后的“环间传力”方向：从开孔位置逐渐向顶部靠近并最终消失；在实际工程中，应关注环间最大剪切力移动方向，经济高效的对环间剪切进行加固，确保有效环间传力。研究成果补充了机械法联络通道盾构隧道中开孔后的环间剪切效应，可为易发生环间错台的盾构隧道工程给出指导建议。

Osteotomy Protocol for Implant Insertion of One Day Biotech: One Drill, One Implant

Background: Four factors determine the quality of an implantology kit: 1) Heat generated by the drills;2) Morphology of the osteotomy according to the diameter of the implant;3) Efficiency of collecting autologous bone;and 4) Osteotomy execution time. Materials and Methods: This article examines the heat produced by drills during osteotomy, focusing on the effect of the following factors: drilling technique;volume of autologous bone harvested;drilling time;implant primary stability;and the percentage of osseointegrated implants after primary healing. Discussion: The four factors mentioned above are analyzed based on the data obtained for sequential, biological, and One Drill milling techniques. Conclusions: 1) One Drill is the fastest technique for performing the osteotomy;2) All techniques stay within the biological temperature range of living bone, with the lowest increase in temperature achieved using One Drill with irrigation;3) The bone harvested showed no statistically significant differences between biological milling and the One Drill technique, both far superior to the sequential technique;and 4) There is no statistically significant difference in the number of osseointegrated implants among the three techniques analyzed.

Grouping tree species to estimate basal area increment in temperate multispecies forests in Durango,Mexico

Multispecies forests have received increased scientific attention,driven by the hypothesis that biodiversity improves ecological ***,a greater species diversity presents challenges for forest management and *** study aims to develop basal area growth models for tree species *** analysis is based on a dataset of 423 permanent plots(2,500 m^(2))located in temperate forests in Durango,***,we define tree species cohorts based on individual and neighborhood-based variables using a combination of principal component and cluster ***,we estimate the basal area increment of each cohort through the generalized additive model to describe the effect of tree size,competition,stand density and site *** principal component and cluster analyses assign a total of 37 tree species to eight cohorts that differed primarily with regard to the distribution of tree size and vertical position within the *** generalized additive models provide satisfactory estimates of tree growth for the species cohorts,explaining between 19 and 53 percent of the total variation of basal area increment,and highlight the following results:i)most cohorts show a"rise-and-fall"effect of tree size on tree growth;ii)surprisingly,the competition index"basal area of larger trees"had showed a positive effect in four of the eight cohorts;iii)stand density had a negative effect on basal area increment,though the effect was minor in medium-and high-density stands,and iv)basal area growth was positively correlated with site quality except for an oak *** developed species cohorts and growth models provide insight into their particular ecological features and growth patterns that may support the development of sustainable management strategies for temperate multispecies forests.

随着人们对绿色能源和环境友好型社会需求的日益增长,先进能量转换装置或系统的研发亟待加速,而燃料电池系统被认为是最有希望的未来发电装置。在不同种类的燃料电池中,固体氧化物燃料电池(SOFCs)是一种清洁环保的发电装置,因其独特的多燃料运行能力及能量转化效率高等优点而前景广阔。然而,传统使用镍-金属陶瓷阳极的SOFCs,在碳氢化合物燃料下运行时,由于燃料的不完全氧化会使阳极产生积碳,从而导致电池性能下降。讨论了碳基SOFC中碳沉积机理和解决阳极积碳的策略,重点介绍了目前广泛研究的具有混合离子-电子导体的双金属-金属陶瓷材料的种类以及研究进展,并总结了双金属-金属陶瓷材料的抗积碳机理,主要表现为降低阳极表面的碳沉积速率以及加速积碳的去除。还介绍了在还原性气氛中,钙钛矿材料中纳米粒子原位偏析的原理,并讨论了原位偏析所形成的纳米颗粒以及其与钙钛矿主体所形成的金属氧化物异质结构在抗积碳阳极中的应用。此外,还探究了多层阳极和单原子催化剂阳极在中温SOFCs中的应用。在燃料电池技术巨大进步的推动下,人们对可用于固定式发电的集成煤气化的燃料电池发电系统(IGFCs)产生了新的兴趣,该系统对于实现煤炭的清洁高效利用具有重要意义,该技术的关键依托于SOFCs的成熟度。最后,总结并指出了碳基SOFCs未来的一些研究方向。
