Elevated preoperative neutrophil-tolymphocyte ratio may be associated with decreased overall survival in patients with metastatic clear cell renal cell carcinoma undergoing cytoreductive nephrectomy

Objective:Inflammatory serum markers have proven to be a powerful predictive tool of patient prognosis in cancer treatment for a wide variety of solid organ malignancies,predominantly in the context of localized *** this study we evaluated the preoperative neutrophil-to-lymphocyte ratio(NLR)as a predictive tool in patients with metastatic clear cell renal cell carcinoma(RCC).Methods:Sixty-four patients with metastatic clear cell RCC undergoing nephrectomy were *** patients with preoperative NLR were included for survival *** were categorized into high and low NLR score determined by plotting the NLR ROC *** analysis was ***:Median age was 60.8 years(38.2-81.2).Median follow-up time was 8.1 months(0.1-106.3).Fuhrman grade distribution was:2(3.1%)grade 1,6(9.4%)grade 2,24(37.5%)grade 3 and 32(50.0%)grade *** NLR score was 3.5(1.4-31.0).NLR4 was associated with decreased overall survival compared toNLRhowed NLR4 as an independent predictor of survival(hazard ratio(hR)2.41,95%CI 1.05-5.50,p=0.03).Conclusion:Elevated preoperative NLR is associatedwith poor prognosis in patients withmetastatic kidney *** NLR is a useful tool,which can predict prognosis,stratify patients for postoperative surveillance,and help guide decisions for therapy.

Objective:To examine the prognostic value of tumor major histocompatibility complex I(MhCI)expression on survival and recurrence in patients with clear cell renal cell carcinoma(RCC).Methods:Fifty-three patients that underwent nephrectomy at our institution for clear cell RCC(T1eT3)with4 years of follow-up were queried from our nephrectomy *** staining for MhCI was performed on tumor specimens and MhCI expression was quantified with an automated image analysis *** were divided into high and low MhCI expression groups in order to study the relationship between MhCI expression and prognosis using the KaplaneMeier method and log-rank ***:Overall survival and recurrence free survival were increased in the high MhCI expression group compared to the low MhCI expression group(log-rank,p=0.036 and p=0.028,respectively).Patients alive at the end of the study had higher MhCI expression(mean positivity score 0.82)than those that died of disease(mean positivity score 0.76,t test,p=0.030).Patients that did not develop recurrence during the study period had higher MhCI expression(mean positivity score 0.83)than those that did develop recurrence(mean positivity score 0.78),but this difference was not significant(t test,p =0.079).

A thermodynamic perspective on electrode poisoning in solid oxide fuel cells

A critical challenge to the commercialization of clean and high-efficiency solid oxide fuel cell(SOFC)technology is the insuf-ficient stack lifespan caused by a variety of degradation mechanisms,which are associated with cell components and chemical *** components related degradation refers to thermal/chemical/electrochemical deterioration of cell materials under operating conditions,whereas the latter regards impurities in feedstocks of oxidant(air)and reductant(fuel).This article provides a thermodynamic perspective on the understanding of the impurities-induced degradation mechanisms in *** discussion focuses on using thermodynamic ana-lysis to elucidate poisoning mechanisms in cathodes by impurity species such as Cr,CO_(2),h_(2)O,and SO_(2) and in the anode by species such as S(or h_(2)S),SiO_(2),and P_(2)(or Ph_(3)).The author hopes the presented fundamental insights can provide a theoretical foundation for search-ing for better technical solutions to address the critical degradation challenges.

A theory for three-dimensional response of micropolar plates

Through combined applications of the transfer-matrix method and asymptotic expansion technique, we formulate a theory to predict the three-dimensional response of micropolar plates. No ad hoc assumptions regarding through-thickness assumptions of the field variables are made, and the governing equations are two-dimensional, with the displacements and microrotations of the mid-plane as the unknowns. Once the deformation of the mid-plane is solved, a three-dimensional micropolar elastic field within the plate is generated, which is exact up to the second order except in the boundary region close to the plate edge. As an illustrative example, the bending of a clamped infinitely long plate caused by a uniformly distributed transverse force is analyzed and discussed in detail.

Distributed Nash Equilibrium Seeking on Compact Action Sets over Jointly Strongly Connected Switching Networks

Nowadays manufacturers are facing fierce *** from the products,providing customers with multiple maintenance options in the service contract becomes more popular,since it can help to improve customer satisfaction,and ultimately promote sales and maximize profit for the *** considering the combinations of corrective maintenance and preventive maintenance,totally three types of maintenance service contracts are ***,attractive incentive and penalty mechanisms are adopted in the *** this basis,Nash non-cooperative game is applied to analyze the revenue for both the manufacturer and customers,and so as to optimize the pricing mechanism of maintenance service contract and achieve a win-win *** experiments are *** results show that by taking into account the incentive and penalty mechanisms,the revenue can be improved for both the customers and ***,with the increase of repair rate and improvement factor in the preventive maintenance,the revenue will increase gradually for both the parties.
