Endoscopic ultrasound sedation in the United Kingdom: Is life without propofol tolerable?

There is compelling evidence to support the quality,cost effectiveness and safety profile of non-anesthesiologist-administered propofol for endoscopic ultrasound (EUS). However in the United Kingdom, it is recommended that the administration and monitoring of propofol sedation for endoscopic procedures should be the responsibility of a dedicated and appropriately trained anaesthetist only. The majority of United Kingdom EUS procedures are performed with opiate and benzodiazepine sedation rather than anaesthetist led propofol lists due to anaesthetist resource availability. We sought to prospectively determine the tolerability and safety of EUS with benzodiazepine and opiate sedation in single United Kingdom centre. Two hundred consecutive patients undergoing either EUS or oesophago-gastroduodenoscopy (OGD) with conscious sedation were prospectively recruited with a 1:1 enrolment ratio. Patients completed questionnaires pre and post procedure detailing anticipated and actual pain experienced on a 1-10 visual analogue scale. Demographics, procedure duration, sedation doses and willingness to repeat the procedure were also recorded. EUS procedures lasted significantly longer than OGDs(15 min vs 6 min, P < 0.0001), however, there was no difference in anticipated pain scores between the groups(EUS 3.37/10 vs OGD 3.47/10, P = 0.46). Pain scores indicated EUS was better tolerated than OGD(1.16/10 vs 1.88/10, P = 0.03) although higher doses of sedation were used for EUS procedures. There were no complications identified in either group. We feel our study demonstrates that the tolerability of EUS with opiate and benzodiazepine sedation is acceptable.

Are there sex-based disparities in cataract surgery?

AIM: To develop a short,enhanced functional ability Quality of Vision(faVIQ) instrument based on previous questionnaires employing comprehensive modern statistical techniques to ensure the use of an appropriate response scale,items and scoring of the visual related difficulties experienced by patients with visual impairment. · METHODS: Items in current quality-of-life questionnaires for the visually impaired were refined by a multi-professional group and visually impaired focus groups. The resulting 76 items were completed by 293 visually impaired patients with stable vision on two occasions separated by a month. The faVIQ scores of 75 patients with no ocular pathology were compared to 75 age and gender matched patients with visual impairment. ·RESULTS: Rasch analysis reduced the faVIQ items to 27. Correlation to standard visual metrics was moderate(r =0.32-0.46) and to the NEI-VFQ was 0.48. The faVIQ was able to clearly discriminate between age and gender matched populations with no ocular pathology and visual impairment with an index of 0.983 and 95% sensitivity and 95% specificity using a cut off of 29.·CONCLUSION: The faVIQ allows sensitive assessment of quality-of-life in the visually impaired and should support studies which evaluate the effectiveness of low vision rehabilitation services.

Effects of Media Composition and Auxins on Adventitious Rooting of Bienertia sinuspersici Cuttings

An efficient in vitro method for rapid vegetative propagation of Bienertia sinuspersici, one of four terrestrial species of family Chenopodiaceae capable of performing C4 photosynthesis within a single cell, was developed. Cuttings of B. sinuspersici were used to examine the effects of Murashige and Skoog (MS) media strength and auxins on adventitious root formation. Half-strength MS medium was determined to be ideal for adventitious root formation in Bienertia cuttings. Although cuttings cultured in medium containing 5.0 mg/L α-naphthalene acetic acid (NAA) promoted the highest number of adventitious roots, cuttings cultured in medium supplemented with 1.0 mg/L indole-3-butyric acid (IBA) produced the longest adventitious roots and had the highest survival rate upon transplanting to soil. Histological analysis revealed variations in the root anatomy generated by the various auxins which may affect adventitious root formation and subsequent establishment of cuttings in soil. Overall, the established procedure provides a simple and cost-effective means for the rapid propagation of the single-cell C4 species B. sinuspersici.

A comparison of perinatal and infant mortality rates in British Columbia and Finland:Similarities and differences

Introduction: Despite similarities, perinatal and infant mortality rates between British Columbia, (BC) Canada and Finland differ. Key variables that may influence stillbirth, early neonatal, perinatal and infant mortality rates in BC and Finland were studied. Methods: After standardizing definitions, data for all births between 2001 and 2009 from provincial and national registries were used to compare perinatal outcomes between BC and Finland. Annual change was evaluated with regression analyses. Results: Births before 22 weeks gestation were excluded. All mortality rates per 1000 were lower in Finland vs BC (perinatal: 5.1 vs 6.2, stillbirth: 3.4 vs 3.9, early neonatal 1.7 vs 2.4, infant 2.9 vs 4.0;all p Higher multiple birth and preterm birth rates in BC are affecting mortality rates. Finland’s policy of single embryo transfer is a potential explanation. It is possible to have good perinatal outcomes and low caesarean section rates. Conclusions: The Finnish health care system may suggest possible solutions for improved perinatal outcomes. Lower per capita health care expenditures in Finland do not appear to have adversely affected perinatal outcomes.

Iris manipulation during phacoemulsification:intraoperative and postoperative complications

AIM:To quantify intraoperative and postoperative complications in complex phacoemulsification cataract extraction(phacoemulsification)with iris manipulation compared to non-complex and complex phacoemulsification without iris ***:All phacoemulsification cases at the University of Colorado between January 1,2014,and June 30,2017 were *** criteria for the primary outcome of intraoperative complications were planned combination surgery and eyes with less than 28 d *** criteria for the secondary outcomes of postoperative complications were unplanned additional surgery,and chronic steroid eye drop use prior to *** including sex,race/ethnicity,surgery length,visual acuity,intraoperative and postoperative complications,and intraocular pressures(IOP)were collected and analyzed utilizing general linear and Logistic regression ***:The medical records of 5772 eyes were reviewed(500 complex without iris manipulation,367 with iris manipulation).The number of any intraoperative complication in the complex with iris manipulation and complex without iris manipulation groups was 15(4.1%)and 26(5.2%),respectively,compared to 41(0.8%)in the non-complex *** inflammation was found in 135(2.8%)non-complex cases,20(4.1%)complex cases without iris manipulation,and 20(5.6%)complex cases with iris *** adjusted odds ratio of postoperative inflammation in phacoemulsification with iris manipulation compared to non-complex was 2.3(95%CI:1.3-4.0,P=0.005).The rate of IOP spikes>10 mm Hg was significantly greater in cases with iris manipulation(P=0.001).CONCLUSION:Complex cases have more intraoperative ***,only complex cases with iris manipulation led to increase rates of postoperative inflammation and IOP spikes>10 mm Hg.

Despite the availability of an effective measles virus(MeV)vaccine and efforts to increase vaccine coverage by the WHO,UNICEF,and their partners,MeV has not been eradicated,and the estimated global measles death rose from 89,780 in 2016 to 207,500 in 2019.1 Because there is an effective measles vaccine,antiviral development for measles has not been prioritized,but recent outbreaks have highlighted the need for drugs to prevent transmission in unvaccinated populations and to protect and treat immunocompromised *** identified several neutralizing mouse monoclonal antibodies(mAbs)that target the MeV fusion(F)protein in its prefusion state2 and inhibit fusion and viral *** engineered a single-chain variable fragment(scFv)from the most potent anti-MeV F *** scFv retains the ability to inhibit fusion and prevents infection in vitro,and intranasal administration of the scFv antibody construct prevents infection in vivo.
