AIM: To review the characteristics of hematological malignancies in tropical areas, and to focus on the specific difficulties regarding their management. METHODS: This is a retrospective narrative review of cases of patients with hematological malignancies. All medical files of patients with malignant disease whose treatment was coordinated by the HematoOncology service of the Cayenne Hospital in French Guiana between the 1st of January 2010 and the 31 st of December 2012 were reviewed. Clinical data were extracted from the medical files and included: Demographic data, comorbidities, serological status for human immunodeficiency virus, human T-lymphotropic virus 1(HTLV1), hepatitis B virus and hepatitis C virusinfections, cytology and pathology diagnoses, disease extension, treatment, organization of disease management, and follow-up. The subgroup of patients with hematological malignancies and virus-related malignancies were reviewed. Cases involving patients with Kaposi sarcoma, and information on solid tumor occurrence in virus-infected patients in the whole patient population were included. Since the data were rendered anonymous, no informed consent was obtained from the patients for this retrospective analysis. Data were compiled using EXCEL® software, and the data presentation is descriptive only. The references search was guided by the nature of the data and discussion. RESULTS: In total, the clinical files of 594 patients(pts) were reviewed. Hematological malignancies were observed in 87 patients, and Kaposi sarcoma in 2 patients. In total, 70 patients had a viral infection, and 34 of these also had hematological malignancies. The hematological diagnoses were: Multiple myeloma in 27 pts, lymphoma(L) in 43 pts, myeloproliferative disorders in 17 pts and Kaposi sarcoma in two patients. AIM: To assess and compare outcomes of laparoscopic total colectomy performed for a variety of indications. METHODS: Sixty six patients underwent laparoscopic total colectomy for inflammatory bowel disease (IBD) (13) and other diseases (53). Data on demographics, pre and post-operative outcomes were collected ***: Mean operative time was 4.5 h. Conversion rate was 13.6%. Total colectomy performed for IBD was associated with a signifi cantly higher anastomotic leak rate (23.1% vs 1.9%, P < 0.05). On univariate analysis, hand sewn anastomosis and treatment with more than 20 mg of prednisolone for at least 3 mo was associated with a higher anastomotic leak rate (P < 0.05). No signifi cant difference was found in return of gut function and overall morbidity between disease groups. CONCLUSION: Laparoscopic total colectomy is feasible and outcomes are equivalent whatever the indica- tion, except for anastomotic leak rate which is higher for patients with IBD.

背景有研究证明手术前12服加巴喷丁可减轻手术后疼痛，然而加巴喷丁辅助用于区域麻醉和慢性疼痛的效果还不清楚。本研究选择颈浅丛神经阻滞（superficialcervicalplexusblock，SCPB）麻醉下行甲状腺切除术的患者，观察手术前口服加巴喷丁对手术后早期和晚期（6个月之后）疼痛的作用。方法50例签署知情同意书的患者随机分成2组，即G组和P组，分别于手术前2小时口服1．2g加巴喷丁和安慰剂。通过数字量化评分（0—6分）评估手术前焦虑程度。患者于标准全麻诱导后行颈浅丛神经阻滞，分别记录手术中、手术后在麻醉恢复室和回到病房后患者的基本情况和镇痛药消耗量。数字量化评分（0—10分）评估手术后24小时静息时和吞咽时疼痛程度。如果静息时疼痛评分超过4分（最疼为10分），则每隔6小时静注1g扑热息痛或者50mg曲马多。手术前l天和甲状腺切除术后6个月，进行神经病理性疼痛诊断问卷。结果两组患者的一般资料、手术前焦虑程度评分、手术中用药、手术时间和手术后恢复室停留时间差异没有统计学意义。2组患者手术后24小时内镇痛药应用次数相似（G组：3[0—5]剂量／24小时；P组：3[1。5]剂量／24小时；P：NS）、静息时疼痛评分（G组：2，2『0．2。3．7]；P组：2[0—6．3]；P=NS）和吞咽时疼痛评分（G组：2．8[0．4～8．9]：P组：3[1．4。6．3]；P=NS）均没有差异。手术后6个月神经病理性疼痛诊断问卷评分超过3分者有8例，而手术前仅有2例（P=0．04）。延迟性神经病理性疼痛的8例患者中7例为单纯颈丛神经阻滞，仅有1例为手术前口服加巴喷丁辅助颈丛神经阻滞的患者（P=0．01）。结论手术前口服加巴喷丁没有改变颈丛神经阻滞下甲状腺切除术患者手术后的早期疼痛，但能预防手术后6个月出现的延迟性神经病理性疼痛。

背景：目前，有关社区中动脉粥样硬化血栓形成稳定患者心血管（cardiovascular，CV）事件发生率的资料比较少，既往也无国际性队列研究对冠状动脉病（coronary artery disease，CAD）、脑血管病（cerebrovascular disease，CVD）、外周动脉病（peripheral arterial disease，PAD）确诊患者以及此类疾病高危患者不同事件的发生率进行评估。目的：在患有动脉疾病的门诊患者或存在多个动脉粥样硬化血栓形成危险因素的门诊患者中确定当今世界1年内CV事件的发生率。设计、地点及参试者：“减少动脉粥样硬化血栓形成维系健康（Reduction of Artherotllrombosis for Continued Health，REACH）”注册研究是一项国际前瞻性队列研究。研究人员于2003—2004年从44个国家、5587名医生入选68236例已确诊的动脉粥样硬化性血管病患者（CAD、PAD、CVD；n=55814）以及至少存在3个动脉粥样硬化血栓形成危险因素的患者（n=12422）。主要观测指标：CV死亡、心肌梗死（myocardial infarction，MI）及卒中的发生率。结果：截至2006年7月，共有95．22％的患者（n=64977）提供了1年内的结局。心血管死亡、MI及卒中的总体发生率为4．24％；动脉粥样硬化性血管病患者为4．69％，仅存在多个危险因素的患者为2．15％。CAD、CVD及PAD患者CV死亡、MI及卒中的总体发生率分别为4．52％、6．47％和5．35％。CAD、CVD及PAD患者发生终点事件（即CV死亡、MI、卒中）以及因动脉粥样硬化血栓形成事件入院的几率分别为15．20％、14．53％和21．14％。上述事件发生率随症状性动脉疾病病变部位数量的增加而增加，仅存在危险因素的患者为5．31％，有1处症状性动脉疾病的患者为12．58％，有2处症状性动脉疾病的患者为21．14％，有3处症状性动脉疾病的患者为26．27％（趋势P〈0．001）。结论：在这项当代大型国际研究中，患有动脉粥样硬化性血管病的门诊患者以及有动脉粥样硬化血栓形成危险的门诊患者CV事件的年发生率相对较高。存在多部位病变者CV事件的1年发生率升高。
