Value of intravascular ultrasound imaging in following up patients with replacement of the ascending aorta for acute type A aortic dissection

Background The value of intravascular ultrasound （IVUS） imaging in patients with replacement of the ascending aorta for acute type A aortic dissection （AD） is unknown. The purpose of this study was to assess the potential use of IVUS imaging in this setting. Methods From September 2002 to July 2005, IVUS imaging with a 9 MHz probe was performed in a series of 16 consecutive patients with suspected or established AD. This study focused on 5 of them with replacement of the ascending aorta for acute type A AD. Among these 5 patients, other imaging modalities including aortography, spiral computed tomography, magnetic resonance imaging and transesophageal echocardiography were performed in 5, 3, 3 and 1 patients, respectively. Results There were no complications related to IVUS imaging. For the replaced graft, as other imaging modalities, IVUS could identify all 5 grafts, the proximal and the distal anastomoses, and the ostia of the reimplanted coronary arteries. In 2 cases, IVUS detected 2 peri-graft pseudo-aneurysms （1 per case）, which were also detected by magnetic resonance imaging but omitted by aortography. For the residual dissection, IVUS had similar findings as other imaging modalities in detecting the patency （5/5）, the longitudinal and the circumferential extent, the thrombus （4/5）, the recurrent dissection （1/5） and an aneurysm distal to the graft （5 in 4 patients）. However, it detected more intimal tears and side branch involvements than other imaging modalities （15 vs 10 and 3 vs 1, respectively）. Conclusions In following-up patients with replacement of the ascending aorta for acute type A AD, IVUS imaging can provide complete information of the replaced graft and the residual dissection. So, IVUS imaging may be considered when the four current frequently used imaging modalities can not supply sufficient information or there are some discrepancies between them.

Background:Few studies have analyzed outcomes of liver transplantation(LT)when the recipient hepatic artery(HA)was not ***:We retrospectively evaluated the outcomes of LT performed using the different alternative sites to ***:Between 2002 and 2017,1,677 LT were performed in our institution among which 141(8.4%)with unusable recipient HA were *** groups were defined according to the site of anastomosis:the splenic artery(SA group,n=26),coeliac trunk(CT group,n=12),aorta using or not the donor’s vessel(Ao group,n=91)and aorta using a vascular prosthesis(Ao-P group,n=12)as *** median number of intraoperative red blood cell transfusions was significantly increased in the Ao and Ao-P groups(5,5,8.5 and 16 for SA,CT,Ao and Ao-P group respectively,P=0.002),as well as fresh frozen plasma(4.5,2.5,10,17 for the SA,CT,Ao and Ao-P groups respectively,P=0.001).Hospitalization duration was also significantly increased in the Ao and Ao-P groups(15,16,24,26.5 days for the SA,CT,Ao and Ao-P groups respectively,P<0.001).The occurrence of early allograft dysfunction(EAD)(P=0.07)or arterial complications(P=0.26)was not statistically *** of factor V,INR,bilirubin and creatinine during the 7th postoperative days(POD)was significantly improved in the SA *** difference was observed regarding graft(P=0.18)and patient(P=0.16)***:In case of unusable HA,intraoperative and postoperative outcomes are improved when using the SA or CT compared to aorta.

Experimental Study of the Evolution of Temperatures in Pavement Structures and Influence of Traffic on the RN1 in Burkina Faso in a Hot and Dry Climate

Asphalt concrete pavements in hot and dry climates deteriorate prematurely some time after their construction. This degradation is accelerated under the effect of heavy vehicle traffic pressure and high temperatures after the bitumen has softened. A study was conducted on the RN1 in Burkina Faso in order to analyze the evolution of temperature and traffic in the process of degradation of asphalt concrete pavements. It consisted on the one hand in recording the temperatures at different points inside the layers of asphalt concrete pavements and the layer of laterite. These results were compared with the values of the softening temperatures of the bitumen obtained in the laboratory. On the other hand, this study is supplemented by the daily counting of heavy goods vehicles passing through the RN1 during the month of April in order to study the influence of the evolution of heavy goods vehicle traffic on the degradation of pavements in hot and dry climatic conditions. The results obtained for temperatures and the frequency of heavy goods vehicle traffic favor pavement deterioration under certain conditions.

Modeling and numerical analysis of granite rock specimen under mechanical loading and fire

The effect of ISO 834 fire on the mechanical properties of granite rock specimen submitted to uniaxial loading is numerically investigated. Based on Newton's second law, the rate-equation model of granite rock specimen under mechanical load and fire is established. The effect of heat treatment on the mechanical performance of granite is analyzed at the center and the ends of specimen. At the free end of granite rock specimen, it is shown that from 20 C to 500 C, the internal stress and internal strain are weak; whereas above 500 C, they start to increase rapidly, announcing the imminent collapse. At the center of specimen, the analysis of the internal stress and internal strain reveals that the fi re reduces the mechanical performance of granite signi fi cantly. Moreover, it is found that after 3 min of exposure to fi re,the mechanical energy necessary to fragment the granite can be reduced up to 80%.

Diabetes Mellitus and COVID-19 at the Bouake University Hospital Center (Côte d’Ivoire)

Context: COVID-19 is a global public health problem and diabetes mellitus is a poor prognostic factor for the disease. The aim of this study was to investigate the epidemiological, clinical and evolutionary characteristics of COVID-19 in diabetic patients in Bouaké. Patients and Methods: This was a retrospective study carried out from January 2021 to December 2022. We included patients with diabetes known before COVID-19 infection and having a positive COVID-19 RT-PCR. Data were analyzed with Epi info7 software. Results: Among 1122 infected patients, 71 patients (6.3%) were diabetic, the sex ratio was 1.3 and the mean age was 58.7 years. Type 2 diabetes (97.2%) was predominant and the average seniority was 3.1 years. Arterial hypertension (46.5%) was the main comorbidity. The main clinical signs were cough (69%), dyspnea (43.7%) and fever (23.9%). Oxygen saturation was Conclusion: Diabetes mellitus remains one of the most important comorbidities of severe Covid-19. The death rate of diabetics was high. Hence the need to vaccinate people living with diabetes.
