Periprocedural complications and one-year outcomes after catheter ablation for treatment of atrial fibrillation in elderly patients:a nationwide Danish cohort study

OBJECTIVEs To investigate complications within 30-days following first-time ablation for atrial fibrillation(AF),including a composite of cardiac tamponade,hematoma requiring intervention,stroke or death,in patients≥75 years of age,compared to pa-tients aged 65−74 *** addition,one-year all-cause mortality and AF relapse were ***&REsULTs All patients receiving their first catheter ablation for AF between 2012 and 2016 were identified us-ing Danish nationwide *** aged 65−74 years served as the reference group for patients≥75 *** of AF within one year was defined as cardioversion following a three-month blanking period,re-ablation or confirmed relapse within *** composite complication outcome did not differ between the two age groups,with 39/1554(2.8%)in patients 65−74 years of age,versus 5/199(2.5%)in older patients(adjusted HR=0.94),95%CI:0.37−2.39,P=0.896).Patients≥75 years or older had no increased hazard of death within 30 days after the procedure,with an incidence of 3/1554(0.2%)in younger pa-tients and 2/199(1.0%)in patients≥75 years of age(adjusted HR=4.71,95%CI:0.78−28.40,P=0.091).There was no difference in relapse of AF after one year between age groups(≥75 years adjusted HR=1.00,95%CI:0.78-1.26,P=0.969).CONCLUsION In patients≥75 years of age selected for catheter ablation for AF,the incidence of periprocedural complica-tions,as well as one-year freedom from AF showed no statistical difference,when compared to patients 65−74 years of age.

MicroRNA profile in neosquamous esophageal mucosa following ablation of Barrett's esophagus

To investigate the microRNA expression profile in esophageal neosquamous epithelium from patients who had undergone ablation of Barrett’s esophagus. METHODsHigh throughput screening using TaqMansup>®sup> Array Human MicroRNA quantitative PCR was used to determine expression levels of 754 microRNAs in distal esophageal mucosa (1 cm above the gastro-esophageal junction) from 16 patients who had undergone ablation of non-dysplastic Barrett’s esophagus using argon plasma coagulation vs pretreatment mucosa, post-treatment proximal normal non-treated esophageal mucosa, and esophageal mucosal biopsies from 10 controls without Barrett’s esophagus. Biopsies of squamous mucosa were also taken from 5 cm above the pre-ablation squamo-columnar junction. Predicted mRNA target pathway analysis was used to investigate the functional involvement of differentially expressed microRNAs. REsULTsForty-four microRNAs were differentially expressed between control squamous mucosa vs post-ablation neosquamous mucosa. Nineteen microRNAs were differentially expressed between post-ablation neosquamous and post-ablation squamous mucosa obtained from the more proximal non-treated esophageal segment. Twelve microRNAs were differentially expressed in both neosquamous vs matched proximal squamous mucosa and neosquamous vs squamous mucosa from healthy patients. Nine microRNAs (miR-424-5p, miR-127-3p, miR-98-5p, miR-187-3p, miR-495-3p, miR-34c-5p, miR-223-5p, miR-539-5p, miR-376a-3p, miR-409-3p) were expressed at higher levels in post-ablation neosquamous mucosa than in matched proximal squamous and healthy squamous mucosa. These microRNAs were also more highly expressed in Barrett’s esophagus mucosa than matched proximal squamous and squamous mucosa from controls. Target prediction and pathway analysis suggests that these microRNAs may be involved in the regulation of cell survival signalling pathways. Objective:Anoctamin 7(ANO7)is a calcium2+-dependent chloride ion channel *** expression is restricted to prostate epithelial *** exact function is *** study aimed to analyze ANO7 expression and its clinical significance in prostate cancer(PCa).Methods:ANO7 expression was assessed by immunohistochemistry in 17,747 clinical PCa ***:ANO7 was strongly expressed in normal prostate glandular cells but often less abundant in cancer ***7 staining was interpretable in 13,594 cancer tissues and considered strong in 34.4%,moderate in 48.7%,weak in 9.3%,and negative in 7.6%.Reduced staining was tightly linked to adverse tumor features[high classical and quantitative Gleason grade,lymph node metastasis,advanced tumor stage,high Ki67 labeling index,positive surgical margin,and early biochemical recurrence(Pstate-specific antigen(PsA)recurrence after prostatectomy in patients with negative *** ANO7 expression was 2.98(95%confidence interval 2.61–3.38).The prognostic impact was independent of established pre-or postoperatively available parameters(Psis of annotated molecular data showed that low ANO7 expression was linked to TMPRss2:ERG fusions(Pssion(Ps well as presence of 9 of 11 chromosomal deletions(Pstrong association of low ANO7 expression with phosphatase and tensin homolog(PTEN)deletion may indicate a functional relationship with the PTEN/AKT ***:These data identify reduced ANO7 protein expression as a strong and independent predictor of poor prognosis in ***7 measurement,either alone or in combination,might provide clinically useful prognostic information in PCa.

3-Arylisothiazoloquinols as potent ligands for the benzodiazepine site of GABA_(A) receptors

3-Arylisothiazolo[5,4-b]quinolin-4(9H)-ones and 3-arylisoxazolo[5,4-b]quinolin-4(9H)-ones were synthesized and assayed for affinity for the benzodiazepine binding site of the GABAA receptors. While the 3-arylisothiazoloquinolin-4-ones were found to be potent ligands, with affinities (expressed as the affinity Ki value) down to 1 nM, the 3-arylisoxazoloquinolin-4-ones are less potent. This is suggested to depend on sterical repulsive interaction of the 3-arylisoxazoloquinolin-4-ones with the receptor essential volume of the binding site, and a higher electron density at the nitrogen in the azole ring (N-2) as well as the carbonyl oxygen in the isothiazoloquinolin-4-ones enabling them to interact stronger with hydrogen bond donor sites at the binding site.

Determination of Crystallization Kinetics and size Distribution Parameters of Agglomerated Calcium Carbonate Nanoparticles during the Carbonation of a suspension of Lime

The reaction studied in this work is the synthesis of nanometric size calcium carbonate by carbonation of a suspension of lime, which represents the most common industrial route. The carbonation was proceeded in a pilot batch reactor. This article presents a method for the determination of nucleation and crystal growth rates of calcium carbonate by following two macroscopic parameters: the mass production rate by precipitation and the specific surface area. The results give a constant nucleation rate around 4 × 1015m-3 ·s-1 and a decreasing crystal growth rate between 0.2 and 2 × 10-10 m·s-1. It also provides the main characteristics of the monoparticle size distributions (i.e. the mean particle sizes and in situ coefficient of variation) in the agglomerates, which cannot be obtained by other known methods. For the carbonation carried out in this work, the mean mass particle size at the end of the reaction is about 300 nm and the coefficient of variation of 0.28 indicates a narrow particle size distribution of the monoparticles.

Agglomeration Mechanisms and Kinetics during the Carbonation of a suspension of Lime in a Pilot Batch Reactor

The reaction studied in this work is the synthesis of nanometric size calcium carbonate particles by carbonation of a suspension of lime, which represents the most common industrial route. It consists in bubbling carbon dioxide in a suspension of lime to obtain precipitated calcium carbonate (PCC). PCC is a mineral filler with various applications: sealants, paints, paper, ink, pharmacy, cosmetics, food etc. However, there is a challenge related to the synthesis and the use of this precipitate: the agglomeration of the monoparticles. The aim of this work is then to understand the mechanisms of this phenomenon and to study its kinetics to improve the run of the process and the control of its impact on the final product. Experiments realized with a high concentration in sodium chloride (2 M) showed that the modification of the electrostatic environment did not change the particle size distribution and the morphology of the agglomerates. This indicates that the electrostatic interactions are not responsible for the agglomeration but the formation of crystalline bridges induced by the crystal growth. Thus, thanks to an agglomeration model including the crystal growth rate, the agglomeration kernel β and the agglomeration constant β0 can be determined using a mathematical treatment of the experimental particle size distributions. Finally, by varying the experimental conditions, it appears that the agglomeration constant increases with the temperature whereas there is an optimal value regarding the shear rate.

Energetic Consequences of stranding of Juvenile Atlantic salmon(salmo salar L.)

An effect of hydropower and hydropeaking regulation in rivers is stranding of fish. Those fishes that survive stranding may experience stranding as a stressful situation. In four experimental stranding experiments (each with 6 individuals in 10 control and 10 treatment replicates), the energetic consequences of two forms of stranding (i.e. trapping and beaching) of juvenile Atlantic salmon (salmo salar) were investigated in summer and winter. Restricted food access in the experimental channels ensured that effects of hydropeaking could be revealed. Mean fish length ranged between 60 mm and 110 mm among experiments. Both during the winter and summer experiments fish did not grow in length, neither in the control nor in the treatment channels and fish lost body mass as well as body fat in all experiments (body fat in summer trapping experiment not determined). The four experiments revealed similar results: stranding did not affect growth or energy content. Despite the severity of the stranding and the resulting mortality, which was especially high during summer, no stranding related effects on fish performance could be detected.
