AIM:To report a one-year clinical outcomes of low-dose laser cycloplasty(LCP)among malignant glaucoma ***:In this prospective,multicenter,noncomparative clinical study,participants with malignant glaucoma were recruited and underwent LCP at eight ophthalmic centers in *** were followed up at 1wk,1,3,6,and *** pressure(IOP),number of glaucoma medications,anterior chamber depth(ACD),and complications were *** success was defined as the reformation of the anterior chamber based on slit-lamp *** was defined by the presence of a shallow orﬂat anterior chamber after initial recovery from ***:A total of 34 eyes received *** IOP and medications decreased from 36.1±11.5 mm Hg with 3.3±1.5 glaucoma medications pre-treatment to 20.9±9.8 mm Hg(P<0.001)with 2.9±1.6 medications(P=0.046)at 1d,and 17.4±6.7 mm Hg(P<0.001)with 1.3±1.7 medications(P<0.001)at *** ACD increased from 1.1±0.8 mm at baseline to 1.7±1.0 mm and to 2.0±0.5 mm at 1d and 12mo,respectively.A total of 32(94.1%)eyes achieved initial anatomical *** follow-up,2(5.9%)eyes failed and 8(23.5%)eyes relapsed,yielding a 12-month anatomical success rate of 64.3%.Complications including anterior synechia(8.82%),choroidal/ciliary detachment(5.88%)and hypopyon(2.94%)were observed within ***:LCP is simple,safe,and effective in reforming the anterior chamber in malignant glaucoma.

Objective： In this study, we examine the effects of recombinant adenovirus-p53 （rAd-p53） on the pancreatic carcinoma cell line SW1990. Specifically, we determine if expression of rAd-p53 sensitizes these cells to radiation. Methods： Following transfection of SW1990 cells with rAd-p53, we measured expression of P53, P21 and Bax by immunocytochemistry. Both transfected and control cell lines were irradiated with a range of doses, and the survival fractions （SF） were calculated. Dose survival cttrves were constructed and modeled for comparison. Results： Transfection of SW1990 cells with rAd-p53 resulted in increased expression of P53, P21 and Bax in a time-dependent manner. At 96 h after transfection, 89.92% of cells expressed P53, 56.8% expressed P21, and 76.50% expressed Bax. The SF following radiation was lower in the rAd-p53 transfected cells compared to the control cells, suggesting that rAd-p53 sensitizes SW1990 cells to radiation （Do for the experimental and control groups was 2.199 and 2.462, respectively）. Conclusions： Use of the adenoviral vector is an effective means of transfecting SW1990 cells with wild-type P53, and this sensitizes the cell line to irradiation. This work suggests that combining rAd-p53 with radiation therapy in pancreatic cancer may be therapeutically beneficial.

Re-irradiation Using Intensity-Modulated Radiation Therapy of Locally Recurrent Nasopharyngeal Carcinoma

Objectives:To investigate the efficacy of high-dose radiation therapy(RT) to the primary and regional disease in combination with systemic chemotherapy and local treatment to metastatic foci in patients with newly diagnosed metastatic nasopharyngeal carcinoma(NPC).Methods: One hundred and five consecutive patients with pathologically confirmed NPC with distant metastasis at diagnosis seen between 1995 and 2002 were *** were offered cisplatin-based chemotherapy, high-dose radiation therapy(>30 Gy) to the head and neck region and active treatment to the metastatic ***:Patients’ median age was 46 years,and all had a Karnofsky Performance Score(KPS) of≥***-nine patients(85%) had metastases confined to one ***-six patients(91%) received at least one cycle of chemotherapy,and 71(68%) received >65 Gy of radiation to the head and neck *** a median follow-up time of 22 months(range 2-142 months),90 patients had deceased,and the median survival time of the entire group was 25 *** 2-and 5-year estimated overall survival (OS) rates were 50%and 17%,*** dose of > 65 Gy to the primary region(p=0.05) and number of organs with metastases(single versus multiple)(p=0.002) were independent predictive factors for OS on log-rank *** moderately severe acute toxicities,such as Radiation Therapy Oncology Group(RTOG) grade 3 mucositis,skin desquamation,and leucocytopenia were *** patient experienced Grade 4 acute ***:High dose radiation therapy is indicated for local disease control in patients with metastatic nasopharyngeal carcinoma,and may improve survival when actively used with systemic chemotherapy and local treatment for metastatic *** with single organ metastases have a better prognosis as compared to those with more widespread metastases.

Portal vein embolization failure:Current strategies and future perspectives to improve liver hypertrophy before major oncological liver resection

＜正＞Purpose:Local recurrences of nasopharyngeal carcinoma（NPC） may be salvaged by re-irradiation using conventional technique,but with significant *** modulated radiation therapy（IMRT） provides potentially improvement in therapeutic ratio by reducing dose to normal *** aim of this study is to address the efficacy and toxicity profile of IMRT for a cohort of patients with locally recurrent NPC. Methods and Materials:Between August 2003 and June 2009,seventy patients with histologically proven locally recurrent NPC were treated with *** time to recurrence was 30 months after the completion of conventional radiation to definitive ***-seven percent of the cases had rT3-4 *** minimal planned dose was 59.4-60 Gy in 1.8-2 Gy per daily fractions to the gross disease with margins,with or without chemotherapy. Moderate to severe late toxicities were noted in 25 patients（35.7%）.Eleven patients（15.7%） had posterior nasal space ulceration,17（24.3%） developed cranial nerve palsies,12（17.1%） had trismus,and 12（17.1%） suffered *** grade 5 toxicity *** disease-free interval and advanced T-classification at presentation were adverse prognostic factors. Conclusion:Re-irradiation with IMRT provides reasonable long-term control in patients with locally recurrent NPC with acceptable profile of adverse-effects.

Retrorectal, developmental tail gut cysts, include dermoid cysts, rectal duplication cysts and retrorectal cyst-hamartomas. Retrorectal cyst-hamartomas (RCH) are derived from remnants of the tail gut, the most caudal part of the embryonic hind gut, which normally involutes by the 8th wk of embryonic development (3-8 mm stage). They have specific radiological and histopathological features that distinguish them from other similar formations (dermoid cysts, enteric duplication cysts and teratomas). We report a patient with adenosquamous carcinoma arising within RCH, who underwent complete resection of the cyst through anterior laparotomy, and reached complete (recurrencefree for 14 mo, so far) functional recovery. The cyst was incidentally discovered during hysterectomy 12 years ***, therapeutic and histopathological aspects of this rare case are discussed. The mentioned period between diagnosis and surgical treatment suggests that RCH, given enough time, can develop malignant degeneration, and should be resected at the time of diagnosis.

AIM: To evaluate the effect of antiviral agents on intrahepatic HBV DNA in HBeAg-positive chronic hepatitis B patients. METHODS: Seventy-one patients received treatment with lamivudine,interferon alpha (IFN-α2b) or sequential therapy with lamivudine-IFN-α2b for 48 wk. All subjects were followed up for 24 wk. Serum and intrahepatic HBV DNA were measured quantitatively by PCR. HBV genotypes were analyzed by PCR-RFLP. RESULTS: At the end of treatment,the intrahepatic HBV DNA level in 71 patients decreased from a mean of (6.1 ± 1.0) log10 to (4.9 ± 1.4) log10. Further,a larger decrease was seen in the intrahepatic HBV DNA level in patients with HBeAg seroconversion. Intrahepatic HBV DNA level (before and after treatment) was not significantly affected by the patients' HBV genotype,or by the probability of virological flare after treatment. CONCLUSION: Intrahepatic HBV DNA can be effectively lowered by antiviral agents and is a significant marker for monitoring antivirus treatment. Low intrahepatic HBV DNA level may achieve better efficacy of antivirus treatment.
