食管基底细胞样鳞状细胞癌临床病理分析

目的　探讨食管基底细胞样鳞癌 (BSC)的临床病理特点。方法　回顾分析 10年间食管癌手术病理档案 ,对符合基底细胞样鳞癌的病例行免疫组化标记。结果　 2 960例食管癌中 10例为食管基底细胞样鳞癌 ,占 0 .3 %。其中 8例曾误诊为低分化鳞癌、腺鳞癌、腺样囊性癌和小细胞癌。肿瘤主要由核大浆少的基底细胞样癌组织与多少不等的鳞癌或(和 )原位鳞癌构成 ,两者间无移行。 1例部分细胞免疫组化癌胚抗原阳性 ,2例局部癌组织突出素、嗜铬素阳性 ,1例灶性角蛋白 17、角蛋白 8阳性 ;无一例表达S10 0和肌动蛋白。结论　食管基底细胞样鳞癌主要由基底样细胞组成 ,并具有鳞癌的变异型 ,具有高侵袭性又易在病理上误诊。

MET基因扩增和/或蛋白过表达是非小细胞肺癌(non-small cell lung cancer,NSCLC)接受表皮生长因子受体-酪氨酸激酶抑制剂(EGFR-TKI)治疗后耐药的主要机制之一,也是潜在重要治疗靶点。多项临床研究结果显示,伴有MET基因扩增和/或蛋白过表达的TKI耐药患者能从MET抑制剂治疗中获益。在NSCLC患者中,MET蛋白过表达比例高于MET基因扩增比例。现有研究提示,免疫组织化学(immunohistochemistry,IHC)检测MET蛋白过表达可提高EGFR-TKI耐药后抗MET靶向治疗的潜在获益人群比例。目前,用于MET蛋白IHC检测的抗体种类繁多、检测流程不同,且尚无统一的判读标准。为提高检测结果准确性且精确筛选潜在获益患者,达成NSCLC患者的MET IHC检测和判读标准共识十分重要。本共识主要对MET IHC检测流程、判读标准及检测报告形式进行了规范,以期进行高质量的MET IHC检测,提高判读结果的准确性和一致性,从而指导临床开展相关临床研究,使更多EGFR-TKI耐药后的NSCLC患者在后续靶向治疗中获益。

The purpose of this study is to observe the effects of HBx on the apoptosis of hepatoma cells induced by TNF-related apoptosis-inducing ligand (TRAIL) and to study prelimi- nary molecular mechanisms for its effects. In order to set up a model in vitro, BEL7402-HBx cell line, stably expressing HBx mRNA, was established by stable transfection of pcDNA-HBx, which contains HBx gene, into hepatoma cell line BEL7402. Control cell line BEL7402-cDNA3, stably transfected with pcDNA3, was set up simultaneously as a control. Trypan blue exclusion test, caspase 3 activity detection and TUNEL assay were performed to detect the apoptosis of BEL7402, BEL7402-cDNA3, BEL7402-HBx induced by TRAIL. The expression of TRAIL recep- tors in three groups was analyzed by Flow cytometry. In addition, phosphorothioated antisense oligonucleotide against the translation initial region of HBx gene (PS-asODNs/HBx) was used to block the expression of HBx in HepG2.2.15 cells and to further confirm the effects of HBx on TRAIL-induced apoptosis. Trypan blue exclusion test indicated that TRAIL had a dose-dependent cytotoxicity on BEL7402, BEL7402-cDNA3 and BEL7402-HBx cells. Under treatment of the same concentration of TRAIL, BEL7402-HBx had a higher apoptosis rate and a higher level of Caspase 3 activation than BEL7402 and BEL7402-cDNA3. TUENL assay showed that the apoptosis rate of BEL7402-HBx induced by 10 μg/L TRAIL was 41.4%±7.2%, signifi- cantly higher than that of BEL7402 and BEL7402-cDNA3 cells. Blockade of HBx expression in Hep G2.2.15 cells partly inhibited the apoptosis induced by TRAIL. The introduction or blockade of HBx did not change the expression pattern of TRAIL receptors. The present study firstly con- firms the effects of HBx on TRAIL- induced apoptosis from two different points and it is not re- lated with the expression level of TRAIL receptors. Objective: The aim of this study was to investigate the clinicopathologic and prognostic factors of the partial subtypes of lung adenocarcinoma, including pure bronchioloalveolar carcinoma (BAC), adenocarcinoma (AC) with BAC component and AC without BAC component. Methods: One hundred and six adenocarcinoma specimens which were followed up completely for 3 years, were obtained from 106 patients (45 men and 61 women) who underwent surgical resection for pathologically confirmed pulmonary adenocarcinoma in the Cancer Hospital of Tianjin Medical University, from June 2004 to December 2005. According to the recent 2004 World Health Organization (WHO) pathological classification criteria of lung cancer, lung adenocarcinomas were divided into three subgroups: pure BAC, AC with BAC component and AC without BAC component. The clinical data were retrospectively analyzed based on statistical methods. All data were analyzed using SPSS statistics software and Kaplan-Meier survival curves were constructed, meanwhile, we conducted a Log-rank test. Results: The statistical analysis showed that no significant association was found among the three groups in gender and age; however, smoke index, tumor size, N stage, TNM stage, postoperative recurrence and metastasis had a statistically significant correlation among three groups (P < 0.01). The 3-year survival rates of the three groups were 96.4%, 61.0% and 40.5% respectively, which had a statistically significant difference. And the 3-year survival rate was significantly higher in the patients with pure BAC than in the patients with other types of lung adenocarcinomas (P < 0.01). In contrast to the other two groups (pure BAC and AC with BAC component), we found the evidence that the 3-year prognosis of lung adenocarcinoma without BAC component was worse than the two formers. 骨桥蛋白RNA干扰对人U251胶质瘤细胞在裸鼠体内生长的影响

＜正＞Wheat plants with dwarf symptom were collected from Baoding and preserved in greenhouse. The virus was transmitted to maize by vector Laodelphax striatellus,which appeared dwarfed with white tumors along the veins on the abaxial leaf surfaces,the typical symptom of maize rough dwarf disease. Viral double-stranded RNAs were isolated and the complete genomic sequences were determined, about a total of 29,142 base pairs（bp）.Sequence analyses showed that this isolate had similar genome organizations with the isolate from Hubei in maize and Zhejiang in *** comparisons showed each pair of the corresponding segment in this isolate shared sequence identities of 93.58%～99.06%and 95.62%～100%with the isolate from Zhejiang（the GenBank accession number AJ294757.1,AJ409145.1,AJ293984.1,AJ409146.1,AJ409147.1,AJ409148.1,AJ297427.1, AJ297431.1,AJ297430.1 and AJ297433.1 of SI- S10） at nucleotide and amino acid levels.

研究下调骨桥蛋白(osteopontin,OPN)对人U251胶质瘤细胞在裸鼠体内生长的影响并探讨其对胶质瘤生长、侵袭的可能机制.应用RNA干扰技术,将OPN基因的慢病毒干扰载体LV-OPNshRNA感染U251细胞.将对照和试验组U251细胞分别接种裸鼠,建立裸鼠荷瘤模型.3周后测量肿瘤的体积、瘤重并做肿瘤组织病理切片分析;利用RT-PCR和免疫印迹法检测OPN、尿激酶型纤维蛋白酶原激活物(uPA)、基质金属蛋白酶(MMP-2、MMP-9)的mRNA和蛋白表达;免疫组化法检测肿瘤组织微血管密度和血管内皮生长因子(VEGF)表达情况.经OPN的RNA干扰后,能显著降低肿瘤组织OPN mRNA水平及蛋白表达,有效抑制肿瘤细胞生长及侵袭能力,肿瘤体积及重量的减小有统计学意义(P<0.05).感染组uPA、MMP-2和MMP-9的mRNA和蛋白表达明显减少,肿瘤组织的MVD值和VEGF的表达均显著降低.上述结果表明,抑制OPN的表达能明显抑制人U251胶质瘤细胞在裸鼠体内的生长和侵袭,OPN可能通过激活uPA、MMP-2和MMP-9等蛋白酶降解细胞外基质和促进肿瘤血管生成,参与胶质瘤的生长.
