AIM To prospectively evaluate the efficacy of submucosal injection of platelet-rich plasma(PRP) on endoscopic resection of large sessile *** Eleven patients were submitted to endoscopic mucosal resection(EMR) with prior injection of PRP, obtained at the time of endoscopy. Patients were followed during 1 mo. The incidence of adverse events(delayed bleeding or perforation) and the percentage of mucosal healing(MHR) after 4 wk were registered. RESULTS EMR was performed in 11 lesions(46.4 mm ± 4 mm, range 40-70 mm). Delayed bleeding or perforation was not observed in any patient. Mean ulcerated area atbaseline was 22.7 cm^2 ± 11.7 cm^2 whereas at week 4 were 2.9 cm^2 ± 1.5 cm^2. Patients treated with PRP showed a very high MHR after 4 wk(87.5%). COnCLUSIOn PRP is an easy-to-obtain solution with proven and favourable biological activities that could be used in advanced endoscopic resection.

在众多类型的喷头中,液力式雾化喷头因其雾化量大、能耗低、设备简单、经济性好而得到更广泛应用。研究液力式喷头的雾化与沉积特性对改善农药喷施效果具有重要的指导意义。为明确液力式喷头的类型和喷雾参数对其雾化和沉积特性的影响,本文对6种类型的液力式喷头(标准扇形雾XR 110-015、防飘扇形雾DG 110-015、射流扇形雾AIXR 110-015、射流扇形雾AI 110-015、空心圆锥雾TXA 80-015和射流空心圆锥雾AITXA 80-015)的雾滴粒径、雾滴沉降速度以及沉积分布进行了测试。结果表明:6种供试喷头的雾滴粒径均随喷雾压力的增大而减小,不同喷雾压力下细雾滴(XR 110-015)和中等雾滴喷头(DG 110-015)的体积中值直径(D_(v0.5))差异较小,而粗雾滴(AIXR 110-015)和超粗雾滴喷头(AI 110-015和AITXA 80-015)对喷雾压力的敏感性更强,其雾滴的D_(v0.5)值随着喷雾压力的增加而急剧下降。雾滴沉降速度与分布特征主要由喷头类型决定,喷雾压力对雾滴沉降速度的影响较小,同一喷头在不同喷雾压力下的雾滴沉降速度分布趋势相似。相同喷雾压力下,细雾滴喷头的平均沉降速度(Vavg)高于其体积中值沉降速度(V_(v0.5)),但随着喷头雾滴的增大,中雾滴喷头和粗雾滴喷头的V_(v0.5)超过其Vavg,且雾滴的粒径越大两者差值越明显。雾滴的沉积结果显示,不同喷雾压力下各喷头的喷雾液流失率均在7.82%~9.75%范围内,各喷头在不同喷雾压力下的流量相似,且略低于其名义流量。喷雾压力对DG 110-015、XR 110-015和AIXR 110-015喷头的雾滴沉积分布均匀性无显著影响,但较高的喷雾压力可提高AITXA 80-015和TXA 80-015喷头的雾滴沉积分布均匀性。本研究可为田间施药时液力式喷头的选择和作业参数的优化提供理论指导和技术参考。

Introduction: Diabetes mellitus, a metabolic disease, is now an important public health problem across the world. Our aim was to study the epidemiological, clinical, paraclinical, and evolutionary aspects of acute metabolic complications (AMC) of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a descriptive cross-sectional study with data collection based on the medical records of patients presenting with acute metabolic complications of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital during the 3 years study period (from January 2020 to December 2022). Results: Over 788 patients hospitalized during the study period, 157 had an acute metabolic complication of diabetes, which is a hospital prevalence of 19.9%. Among these 157 cases, 140 were suitable for analysis and therefore constituted our study sample. The mean age of the patients was 49.9 ± 14.7 years, with 19 as minimum and 90 years as maximum. Most of them were women (52.4%). Hypertensive diabetics accounted for 54.9% of cases. Ketoacidosis was the most common complication (62.1%), followed by hyperosmolar hyperglycemic syndrome (23.6%) and hypoglycemia (14.3%). Infection (69.3%) and therapeutic noncompliance (22.9%) were the most common triggers. The average length period of their hospital stay was 7.1 ± 5.1 days, and the outcome was satisfactory in 87.9% of the cases. Unfortunately, there were 3.6% deaths during hospitalization. Conclusion: Acute metabolic complications of diabetes were dominated by ketoacidosis. Infection was the dominant factor in decompensation. Therapeutic education of diabetic patients as well as clinical and biological monitoring must be more stringent and rigorous.

The histological analysis of peripheral nerve regeneration is one of the most used methods to demonstrate the success of the regeneration through nerve conduits. nowadays, it is possible to evaluate different parameters of nerve regeneration by using histological, histochemical, immunohistochemical and ultrastructural techniques. The histochemical methods are very sensible and are useful tools to evaluate the extracellular matrix remodeling and the myelin sheath, but they are poorly specific. In contrast, the immunohistochemical methods are highly specific and are frequently used for the identification of the regenerated axons, Sehwann cells and proteins associated to nerve regeneration or neural linage. The ultrastructural techniques offer the possibility to perform a high resolution morphological and quantitative analysis of the nerve regeneration. However, the use of a single histological method may not be enough to assess the degree of regeneration, and the combination of different histological techniques could be necessary.

Corticosteroids and immunomodulators have been the mainstay therapies for Crohn’s disease. Corticosteroids are highly effective to control symptoms in the short- term, but they are not effective in maintaining remission, they heal the mucosa in a reduced proportion of cases, and long-time exposure is associated with an increased risk of infections and mortality. Immunomodulators, azathioprine and methotrexate, heal the mucosa in a higher proportion of patients that corticosteroids but their onset of action is slow and they benefit less than half of patients with Crohn’s disease. In the last decade, medical therapy for Crohn’s disease has experienced a remarkable change due to the introduction of biologic therapy, and particularly the use of anti-tumour necrosis factor-alpha agents. Infliximab, adalimumab, and certolizumab pegol have demonstrated efficacy for induction and maintenance of remission in active Crohn’s disease. These agents have raised the bar for what is a suitable symptomatic response in Crohn’s disease and modification of the natural history of the disease has become a major goal in the treatment of Crohn’s disease. There are several data in the literature that suggest that early use of biologic therapy and achievement of mucosal healing contribute to disease course modification. However, many questions on early biological therapy for Crohn’s disease remain still unanswered.

This paper presents an improved understanding of coupled hydro-thermo-mechanical(HTM) hydraulic fracturing of quasi-brittle rock using the bonded particle model(BPM) within the discrete element method(DEM). BPM has been recently extended by the authors to account for coupled convective econductive heat flow and transport, and to enable full hydro-thermal fluidesolid coupled *** application of the work is on enhanced geothermal systems(EGSs), and hydraulic fracturing of hot dry rock(HDR) is studied in terms of the impact of temperature difference between rock and a flowing fracturing fluid. Micro-mechanical investigation of temperature and fracturing fluid effects on hydraulic fracturing damage in rocks is presented. It was found that fracture is shorter with pronounced secondary microcracking along the main fracture for the case when the convectiveeconductive thermal heat exchange is considered. First, the convection heat exchange during low-viscosity fluid infiltration in permeable rock around the wellbore causes significant rock cooling, where a finger-like fluid infiltration was observed. Second, fluid infiltration inhibits pressure rise during pumping and delays fracture initiation and propagation. Additionally, thermal damage occurs in the whole area around the wellbore due to rock cooling and cold fluid infiltration. The size of a damaged area around the wellbore increases with decreasing fluid dynamic viscosity. Fluid and rock compressibility ratio was found to have significant effect on the fracture propagation velocity.
