Value of Pre- and Post-Treatment Platelet Indices in Patients with Immune Thrombocytopenic Purpura

Introduction: Immune thrombocytopenic purpura (ITP) is an acquired disorder characterized by isolated thrombocytopenia. There is no “gold standard” test that can establish the diagnosis of primary ITP. Therefore, the diagnosis is one of exclusion and is based on patient history, physical examination, complete blood count, and blood smear review. Platelet indices are biomarkers of platelet activation, allowing extensive investigations focusing on the diagnostic and prognostic values in various diseases without extra-costs. Mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) were investigated aiming at evaluating the correlation between pre- and post-treatment values in ITP patients. The sensitivity and specificity of platelet indices for assessment of their predictive value for treatment response were also evaluated. Methods: Fifty newly diagnosed primary ITP patients were selected from the Hematology Clinics of Ain Shams University Hospitals and classified into pediatric and adult groups. Platelet count, MPV, PDW, PCT have been recorded for all pre and post-treatment CBC samples using Beckman Coulter LH750 analyzer. Results: In pediatrics post-treatment MPV and PDW were significantly decreased while PCT was increased when compared to their values at the time of presentation (P < 0.01). The same results were obtained in adult patients, except for PDW which did not attain any significance. A significant correlation was found between pre-treatment values of these platelet indices and treatment response in pediatrics, while no significance was found between pre-treatment PDW and treatment response in adults. Conclusion: Increased pre-treatment MPV and PDW, and decreased PCT can provide diagnostic and predictive value for treatment response in ITP patients.

为探讨玉树地震是否为超剪切破裂事件,利用Yagi的方法重新进行了此次地震震源破裂过程的反演.通过给定不同的破裂速度进行对比,发现当破裂速度为4.7km/s时,理论与实际观测结果拟合残差最小,且反演结果更符合实际.另据P波高频辐射能量包络反演,破裂传播速度在4.7～5.8km/s范围内,而该地区的剪切波速度则为3.0～ 3.6km/s,从而证明了此次地震超剪切破裂现象的存在.反演结果表明,此次地震在玉树段的NW和SE段形成了19和31km的地表破裂,而震中所在的中段由于隆宝湖拉分盆地的存在,造成了15km的未破裂区.超剪切破裂是造成SE段玉树县城遭受严重破坏的原因之一.

In this paper, a new approach is introduced for fault detection and diagnostic. The method uses integration of PCA （Principal Component Analysis） and ANFIS （Adaptive Neuro -Fuzzy Inference System）. PCA is employed to reduce the recorded data dimension and yet extract informative features for fault detection purpose. The reduced data is then fed to ANFIS to discriminate the occurred fault. Resolution of the multiple ANFISs is enhanced through adequate selection of the utilized membership function （MF） numbers to compensate for the large number of possible created rules. This approach naturally removes extra pressure on each ANFIS to yield good responses only on close neighborhood of faulty data in training process. The combination of boundary models in the extra number of MFs provides fault isolation of the faulty plant section even when novel faults. The key point of this approach is the ability to detect and diagnose any novel fault with the same time-response pattern but different severities. The efficacy of the proposed FDD approach has been demonstrated via extensive conducted tests in a distillation column benchmark.

本文提出了一个研究时尚服装冲动购买决策的框架.时尚服装对我们非常重要,服装是我们日常生活的一部分。没有很多研究考察时尚服装的参与,特别是从消费者购买决定的角度来看。本研究关注三类因素之间的关系:消费者特征(唯物主义,性别和个人价值),产品特征(产品价格)和情境因素(他人的存在)以及它们与冲动行为是否一致购买。该研究还分析了购后评估,购买行为之后冲动性购买决定产生的后果(顾客对该过程感到失望,或者感到高兴,或者如果他或她对刚刚购买的产品感到后悔。我使用罗马尼亚Cluj-Napoca两家购物中心的消费者样本进行了一项实证调查,通过调查问卷收集数据,然后用统计软件IBM SPSS统计数据进行分析.根据数据分析结果,罗马尼亚消费者在购物时冲动。然而,性别分析显示,女性的冲动购买倾向水平与男性的冲动购买倾向水平几乎没有差别。所有这三类因素影响冲动购买行为.个人价值比唯物主义更能影响冲动购买行为。罗马尼亚的消费者从购物中获得乐趣,他们喜欢拥有他们渴望的物品。尽管消费者表示他们确实接受了朋友和家人的建议,他们倾向于花费比原先计划时多的钱,但作为主持人的其他人的存在并不影响消费者特征与冲动购买之间的关系。因此,在购物中找到快乐和自我实现的高度物质消费者在购物时不会受到朋友或家人的影响。产品价格本身对冲动行为有很大影响。大多数参与者同意他们喜欢在商店销售时购物。然而,调查结果显示,高度物质化和时尚化的消费者会不顾产品促销而冲动购买产品。即使大多数参与者在大部分时间都对他们所做的冲动购买感到满意,但他们确实相信他们购买的某些产品是无用的,并希望他们可以用另一种产品取而代之。这项研究可以帮助营销人员在他们的策略中了解罗马尼亚消费者从克卢日纳波卡的行为,他们倾向购买冲动的倾向以及最终影响这位消费者做出即兴购买的因素,而不考虑后果。

Prevalence and clinical significance of antiphospholipid antibodies among hospitalized COVID-19 patients

Objective:To describe the prevalence of antiphospholipid antibodies in coronavirus disease-19(COVID-19)and to find potential associations between antiphospholipid antibody positivity and clinical ***:From September to November 2020,clinical and laboratory data were collected from 50 COVID-19 patients hospitalized at Saiful Anwar General Hospital in Malang,*** antibodies were measured by finding Ig M anti-β2 glycoprotein,lupus anticoagulant,and Ig M/Ig G *** characteristics,thrombotic events,ICU admission,and mortality during hospitalization were *** severity was defined by the Guidelines for the Prevention and Control of COVID-19,***:Among 50 patients,5 patients(10.0%)were positive for antiphospholipid antibodies:4 patients(80.0%)had Ig M anti-β2 glycoprotein and 1 patient had Ig G anti-cardiolipin(20.0%)and Ig M anti-cardiolipin(20.0%),none of lupus anticoagulant was *** antibodies were associated with anosmia(OR 8.1;95%CI 1.1-57.9;P=0.018),nausea and vomiting(OR 12.4;95%CI 1.2-122.6;P=0.010),diarrhea(OR 9.8;95%CI 1.3-70.9;P=0.010),cardiovascular disease(OR 1.4;95%CI 1.0-1.9;P=0.001),chronic kidney disease(OR 12.0;95%CI 1.6-90.1;P=0.05),acute coronary syndrome(OR 29.3;95%CI 2.0-423.7;P=0.001),moderate(OR 0.11;95%CI 0.01-1.10;P=0.031)and severe(OR 18.5;95%CI 1.8-188.4;P=0.002)disease severity,and in-hospital mortality(OR 8.1;95%CI 1.1-57.9;P=0.018).However,there is no correlation between the presence of antiphospholipid antibody and ICU ***:In summary,the prevalence of antiphospholipid antibodies in COVID-19 patients is low,mainly against Ig M anticardiolipin,and is associated with an acute coronary syndrome,gastrointestinal manifestations,moderate and severe disease severity,and increased risk of mortality.

Background:In vivo cell tracking after transplantation in regenerative medicine remains an unmet challenge and limits current understanding of the wound healing mechanism through cell-based *** study investigated tracking of human Wharton’s jelly stem cells(hWJSCs)seeded onto an acellular dermal matrix(ADM)and labeled with superparamagnetic iron oxide nanoparti-cles(SPIONs)by magnetic resonance imaging(MRI)in burn ***:The hWJSCs were characterized and assessed for growth kinetics.A total of 30 rats were enrolled in three equal *** 1 underwent scald burn injury left without treatment,the group 2 was treated by an ADM that was prepared from cosmetic surgery skin samples and the group 3 received hWJSCs labeled with SPIONs seeded onto an *** strength was evaluated before and after interventions,real time PCR assessed apoptosis,and Prussian blue staining,scanning electron microscopy(SEM)and MRI were used for the tracking of labeled ***:The hWJSCs exhibited mesenchymal stem cell *** doubling time was 40.1 *** did not show any toxic *** hWJSCs seeded onto an ADM decreased Bax and increased Bcl-2 gene *** of SPIONs within hWJSCs was confirmed by Prussian blue staining,SEM and MRI until day *** was a significant difference between the Young’s moduli of normal skin and the group receiving hWJSCs seeded onto an *** observations and SEM imaging confirmed that MRI is an accurate method to track SPION-labeled hWJSCs in ***:This study showed that SPION labeling coupled with MRI can be used to further understand the fate of stem cells after transplantation in a burn model.

FEAR: Fuzzy-Based Energy Aware Routing Protocol for Wireless Sensor Networks

Wireless Sensor Networks (WSNs) are used in different civilian, military, and industrial applications. Recently, many routing protocols have been proposed attempting to find suitable routes to transmit data. In this paper we propose a Fuzzy Energy Aware tree-based Routing (FEAR) protocol that aims to enhance existing tree-based routing protocols and prolong the network’s life time by considering sensors’ limited energy. The design and implementation of the new protocol is based on cross-layer structure where information from different layers are utilized to achieve the best power saving. Each node maintains a list of its neighbors in order to use neighbors’links in addition to the parent-child links. The protocol is tested and compared with other tree-based protocols and the simulation results show that FEAR protocol is more energy-efficient than comparable protocols. According to the results FEAR protocol saves up to 70.5% in the number of generated control messages and up to 55.08% in the consumed power.

Sporadic ganglioneuromatosis of esophagogastric junction in a patient with gastro-esophageal reflux disorder and intestinal metaplasia

A 58-year-old female with a recurrent history of upper abdominal pain and intermittent dysphagia underwent endoscopic evaluation that demonstrated an irregular and nodular esophago-gastric (EG) junction and gradeⅠ erosive esophagitis. Biopsies showed prominent intestinal metaplasia of Barrett’s type without dysplasia, chronic inflammation and multiple aggregates of large cells within the mucosal lamina propria, some with spindle shaped nuclei. Immunohistochemistry stains for keratins AE-1/ AE-3 were negative, while S-100 and NSE were positive. This, together with routine stains, was diagnostic for mucosal ganglioneuromatosis. The background of chronic inflammation with intestinal type metaplasia was consistent with long-term reflux esophagitis. No evidence of achalasia was seen. Biopsies of gastric antrum and fundus were unremarkable, without ganglioneural proliferation. Colonoscopy was unremarkable. No genetic syndromes were identified in the patient including familial adenomatous polyposis and multiple endocrine neoplasia type Ⅱb (MEN Ⅱb). Iansoprazole (Prevacid) was started by oral administration each day with partial relief of symptoms. Subsequent esophagogastroscopy repeated at 4 mo showed normal appearing EG junction. Esophageal manometry revealed a mild non- specific lower esophageal motility disorder. Mild motor dysfunction is seen with gastro-esophageal reflux disease (GERD) and we feel that the demonstration of localized ganglioneuromatosis was not likely related etiologically. In the absence of findings that might suggest neural hypertrophy, such as achalasia, the nodular mucosal irregularity seen with this instance of ganglioneuromatosis may, however, have exacerbated the patient’s reflux.

Base Width Variations and its Effects on Frequency Response of Double Hetero-structure Long Wavelength Transistor Laser

In this paper we investigate the effects of base width variation on performance of long wavelength transistor laser. In our structure with increasing the base width, the cut off frequency increases until 367 nm with 24.5 GHz and then abruptly fall. In 100 nm base width, we have 17.5 GHz cut off frequency, and overall ac performances become optimized, although, other parameters like optical losses and threshold current density are not optimized.
