Objective:While a rushed operation can omit essential procedures,prolonged operative time results in higher ***,the optimal operative time range remains *** study aimed to estimate the ideal operative time range and evaluate its applicability in laparoscopic cancer ***:A prospectively collected multicenter database of 397 patients who underwent laparoscopic distal gastrectomy were retrospectively *** ideal operative time range was statistically calculated by separately analyzing the operative time of uneventful ***,intraoperative and postoperative outcomes were compared among the shorter,ideal,and longer operative time ***:The statistically calculated ideal operative time was 135.4-165.4 *** longer operative time(LOT)group had a lower rate of uneventful,perfect surgery than the ideal or shorter operative time(IOT/SOT)group(2.8%vs.8.8%and 2.2%vs.13.4%,all P<0.05).Longer operative time increased bleeding,postoperative morbidities,and delayed diet and discharge(all P<0.05).Particularly,an uneventful,perfect surgery could not be achieved when the operative time exceeded 240 *** of ideal time range,SOT group achieved the highest percentage of uneventful surgery(13.4%),which was possible by surgeon's ability to retrieve a higher number of lymph nodes and perform≥150 gastrectomies ***:Operative time longer than the ideal time range(especially≥240 min)should be *** the essential operative procedure were faithfully conducted without compromising oncological safety,an operative time shorter than the ideal range leaded to a better *** to minimize operative time should be attempted with sufficient surgical experience.

Silica thin films synthesized sol–gel process are proposed as flexible encapsulation materials.A sol–gel process provides a dense and stable amorphous silica structure,yielding an extremely high elastic deformation limit of 4.9%and extremely low water vapor transmission rate(WVTR)of 2.90×10^(−4)g/(m^(2)∙day)at 60℃and relative humidity of 85%.The WVTR is not degraded by cyclic bending deformations for the bending radius corresponding to a tensile strain of 3.3%in the silica encapsulation film,implying that the silica thin film is robust against the formation of pinhole-type defects by cyclic bending *** organic solar cells encapsulated with the silica films operate without degradation in power conversion efficiency for 50,000 bending cycles for a bending radius of 6 mm.

AIM: To compare characteristics and prognosis of gastric cancer based on ***: A retrospective study was conducted on clinical and molecular data from patients(n =1658) with confirmed cases of gastric cancer in Seoul National University Bundang Hospital(Seoul, South Korea) from 2003 to 2010 after exclusion of patients diagnosed with lymphoma, gastrointestinal stromal tumor, and metastatic cancer in the stomach. DNA was isolated from tumor and adjacent normal tissue,and a set of five markers was amplified by polymerase chain reaction to assess microsatellite instability(MSI). MSI was categorized as high, low, or stable if ≥ 2, 1, or 0 markers, respectively, had *** was performed on tissue sections to detect levels of expression of p53, human epidermal growth factor receptor(HER)-2, and epidermal growth factor receptor. Statistical analysis of clinical and molecular data was performed to assess prognosis based on the stratification of patients by age(≤ 45 and> 45 years).RESULTS: Among the 1658 gastric cancer patients, the number of patients with an age ≤ 45 years was 202(12.2%; 38.9 ± 0.4 years) and the number of patients> 45 years was 1456(87.8%; 64.1 ± 0.3 years).Analyses revealed that females were predominant inthe younger group(P < 0.001). Gastric cancers in the younger patients exhibited more aggressive features and were at a more advanced stage than those in older patients. Precancerous lesions, such as atrophic gastritis and intestinal metaplasia, were observed less frequently in the older than in the younger group(P < 0.001). Molecular characteristics, including overexpression of p53(P < 0.001), overexpression of HER-2(P = 0.006), and MSI(P = 0.006), were less frequent in gastric cancer of younger patients. High-temperature thermal oxidation of an Fe foil produces a high-quality,crystalline hematite nanoflake suitable as a photoanode for the photoelectrochemical(PEC)water *** pre-polishing of the foil surface has a profound effect in the formation of a vertically-aligned nanoflakes of hematite phase with extended(110)planes by removing the loosely-bonded oxide *** the surface of the photoanode is modified with a ZrO_(2) passivation layer and a cobalt phosphate co-catalyst,the charge recombination at the photoanode-electrolyte interface is greatly suppressed to improve its overall PEC *** a result,the photocurrent density at 1.10 VRHE under 1 sun condition is enhanced from 0.22 mA cm^(-2) for an unmodified photoanode to 0.59 mA cm^(-2) for the fully modified photoanode,and the photocurrent onset potential is shifted cathodically by 400 ***,the photoanode demonstrates outstanding stability by showing steady production of H_(2) and O_(2) gases in the stoichiometric ratio of 2:1 in a continuous PEC operation for 10 h.

AIM to evaluate gender differences in the aspect of ghrelin,nociception-related genes and psychological aspects and the quality of life(Qo L) in Korean functional dyspepsia(FD) *** Total of 191 persons were prospectively enrolled between March 2013 and May 2016 in Seoul National Bundang Hospital,and classified into control and FD group based on ROME Ⅲ criteria. Questionnaire included assessment for dyspepsia symptoms,Qo L and anxiety or depression. Preproghrelin and nociception genes in the gastric mucosa and plasma acyl/des-acyl ghrelin were measured. RESULTS Lower level of plasma acyl ghrelin in FD patients compared to control was significant only in male(15.9 fmol/m L vs 10.4 fmol/m L,P = 0.017). Significantly higher m RNA expressions of nerve growth factor and transient receptor potential vanilloid receptor 1 were observed in male(P = 0.002 and P = 0.014,respectively) than in female. In contrast,female FD patients had a higher anxiety and depression score than male FD(P = 0.029),and anxiety score was correlated with epigastric pain only in female FD patients(female: Spearman rho = 0.420,P = 0.037). The impairment of overall Qo L was more prominent in female FD patients than male patients(5.4 ± 0.3 vs 6.5 ± 0.3,P = 0.020). CONCLUSION Gender differences of ghrelin and nociception-related genes in male and psychological factors in female underlie FD symptoms. More careful assessment of psychological or emotional status is required particularly for the female FD patients.

Age-related cataract （ARC） is the most common cause of visual impairment and blindness worldwide. A previous study reported that genetic factors could explain approximately 50% of the heritability of cataract. However, a genetic predisposition to ARC and the contributing factors have not yet been elucidated in the Korean population. In this study, we assessed the influence of genetic polymorphisms on the risk of ARC in Koreans, including 156 cataract cases and 138 healthy adults. We conducted an exome-wide association study using Illumina Human Exome-12v1.2 platform to screen 244,770 single nucleotide polymorphisms （SNPs）. No SNPs reached exome-wide significance level of association （P 〈 l x 10 6）. B3GNT4 rs7136356 showed the most significant association with ARC （P = 6.54x 10 5）. Two loci （MUC16 and P2RY2） among the top 20 ARC-associated SNPs were recognized as probably linked to cata- ractogenesis. Functions of these genes were potentially related to regulating dehydration or homeostasis of the eyes, and showed a potential association with dry eye disease. This finding suggests that mucin- and dry eye disease-related genes may play a significant role in cataractogenesis. Our study provides insight into the genetic predisposition of ARC in Koreans. Additional studies with larger sample sizes are required to confirm the results of this study.

Air jet loom, as one of the shuttleless looms, transports a yam into warps using viscosity and kinetic energy of an air jet. Performance of this picking system depends on the ability of instantaneous inhalation/exhaust, configuration of nozzle, operation characteristics of a check valve, etc. In the recent past, many studies have been reported on the air jet discharged from a nozzle exit, but studies for understanding the flow field characteristics associated with shear layer and shock wave/boundary layer interaction in the nozzle were not conducted enough. In this paper, a computational study was performed to explain the flow field in the air jet nozzle with an acceleration tube and validated with previous experimental data available. The results obtained from the computational study show that, in the supersonic flow regime, the flow field depends significantly on the length of acceleration tube. As nozzle pressure ratio increases, drag force acting on the string also increases. For a longer acceleration tube, the total pressure loss is large, owing to the frictional loss.
