Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. Influence of tool deflection on micro channel pattern of 6:4 brass with rectangular tool

Machining experiment of micro channel structure with 6:4 brass was carried out by shaping process using a single crystal diamond tool. FEM simulation using solid cantilever beam model was analyzed. In result of experiment, tool deflection is observed as machining characteristics through result of experiments such as surface roughness, cutting force and burr formations. And the influence of tool deflection is experimentally proved.

Effects of design parameters and tension on behavior of a coil using finite element analysis

During the thin strip coiling process, it is necessary to use a sleeve with a mandrel to prevent excessive deformation of the strip. Stress distribution in the sleeve and in the strip is an important factor that determines the quality of the coil. However, owing to the accumulation of high pressure, it is difficult to determine the stress distribution through experimentation. Thus, stress analysis of the strip coiling process was conducted. Finite element analysis was used to investigate the effects of the weight of the strip and the mandrel on the stress distribution and stress concentration near the starting point of the coil. The radial stress was predicted for a coil with a stacked thickness of 384 mm, which corresponds to a strip length of 1486 m, using the stress analysis model developed in a preceding research. A method was presented to reduce the weight and radial stress of a strip coil. It was found that the deformation of the sleeve can be reduced by decreasing the gap between the mandrel segments. The thickness of the sleeve can be reduced from 120 to 106 mm using the stress analysis results. Furthermore, coiling tension can be reduced by 44% compared to the existing value considering the interlayer slip of the strip coil.

Effect of CaF2 content in refining slag on formation behavior of spinel inclusion in Ni-Cr-V-alloyed special steel

The influence of CaF2 content on the formation behavior of spinel inclusions in the Ni-Cr-V-alloyed special steel was presented. The spinel was not formed at CaF2 content lower than 10 wt%. However, it was formed at early stage when the CaF2 content was greater than 30 wt%, followed by a modification to the aluminosilicate-type inclusions. Because the slag was saturated by MgO, the activity of MgO was unity irrespective of CaF2 content in the slag. Thus, Mg was transferred from slag to metal phase. Mg transferred to molten steel reacted with Al2O3-rich inclusions to form ***2O3 spinel. However, the spinel inclusion was modified to aluminosilicate-type inclusions by the reaction with Si and Ca transferred from slag to molten steel about 2-3 h later.

Light-stabilized FHA2 suppresses miRNA biogenesis through interactions with DCL1 and HYL1

The precise regulation of microRNA(miRNA)biogenesis is crucial for plant development,which requires core microprocessors and many fine tuners to coordinate their miRNA processing activity/specificity in fluctuating cellular *** de-etiolation,light triggers a dramatic accumulation of core microprocessors and primary miRNAs(pri-miRNAs)but decreases pri-miRNA processing activity,resulting in relatively constant miRNA *** mechanisms underlying these seemingly contradictory regulatory changes remain *** this study,we identified forkhead-associated domain 2(FHA2)as a light-stabilized suppressor of miRNA *** found that FHA2 deficiency increased the level of mature miRNAs,accompanied by a reduction in pri-miRNAs and target *** assays showed that FHA2 associates with the core microprocessors DCL1,HYL1,and SE,forming a complex to suppress their pri-miRNA processing *** analyses revealed that FHA2 promotes HYL1 binding but inhibits the binding of DCL1-PAZ-RNase-RNA-binding domains(DCL1-PRR)to miRNAs,whereas FHA2 does not directly bind to these ***,we found that FHA2 protein is unstable in the dark but stabilized by light during ***,disruption of FH A led to defects in light-triggered changes in miRNA expression and reduced the survival rate of deetiolated seedlings after prolonged light ***,these data suggest that FHA2 is a novel light-stabilized suppressor of miRNA biogenesis and plays a role in fine-tuning miRNA processing during de-etiolation.

Anti-obesity Effects of Ginsenosides in High-Fat Diet-Fed Rats

Objective: To examine the anti-obesity effects of ginsenosides in Korea Red Ginseng(KRG, Panax ginseng) in rats fed with a high-fat diet(HFD). Methods: Twenty-five 4-week-old obesity rats after receiving an HFD for 5 weeks;subsequently, they were additionally treated with ginsenosides Rb1, Rd, Rg1, or Re(10 mg/kg, intraperitoneal injection) for a further 3 weeks(n=5 in each group). The control rats were fed a normal diet. The food consumption, body weight, locomotor activity, serum lipids, adipose tissues, nitric oxide(NO) expression, leptin, neuropeptide Y(NPY), cholecystokinin(CCK) in the brains were measured. Results: In the HFD-fed rats, body weight, body fat mass, serum levels of leptin and NO were significantly higher than in the control rats(Pgulating signals.

In recent years,pure iron(Fe)has attracted significant attention as a promising biodegradable orthopedic implant material due to its excellent mechanical and biological ***,in physiological conditions,Fe has an extremely slow degradation rate with localized and irregular degradation,which is problematic for practical *** this study,we developed a novel combination of a nanostructured surface topography and galvanic reaction to achieve uniform and accelerated degradation of an Fe *** target-ion induced plasma sputtering(TIPS)technique was applied on the Fe implant to introduce biologically compatible and electrochemically noble tantalum(Ta)onto its surface and develop surface nano-galvanic *** tests revealed that the uniformly distributed nano-galvanic corrosion cells of the TIPS-treated sample(nano Ta-Fe)led to relatively uniform and accelerated surface degradation compared to that of bare ***,the mechanical properties of nano Ta-Fe remained almost constant during a long-term in vitro immersion test(~40 weeks).Biocompatibility was also assessed on surfaces of bare Fe and nano Ta-Fe using in vitro osteoblast responses through direct and indirect contact assays and an in vivo rabbit femur medullary cavity implantation *** results revealed that nano Ta-Fe not only enhanced cell adhesion and spreading on its surface,but also exhibited no signs of cellular or tissue *** results demonstrate the immense potential of Ta-implanted surface nanostructures as an effective solution for the practical application of Fe-based orthopedic implants,ensuring long-term biosafety and clinical efficacy.

Assessment of nutrient distributions in Lake Champlain using satellite remote sensing

The introduction of nutrients to lakes causing eutrophic conditions is a major problem around the world. Proper monitoring and modeling are important to effectively manage eutrophication in lake waters. The goal is to develop remote sensing models for nutrients, total phosphorus and total nitrogen, in Lake Champlain. The remote sensing models were created using multivariate linear regression with the unique band combinations of Landsat Enhanced Thematic Mapper Plus （ETM＋） imagery based on the empirical relationship with the field observations. The resulting models successfully showed nutrient distributions in the most eutrophic part of Lake Champlain, Missisquoi Bay, with reasonable adjusted coefficient of determination values （R2 = 0.81 and 0.75 for total phosphorus and total nitrogen, respectively）. The results show the feasibility and the utility of satellite imagery to detect spatial distributions of lake water quality constituents, which can be used to better understand nutrient distributions in Lake Champlain. This approach can be applicable to other lakes experiencing eutrophication assisting decision making when implementing Best Management Practices and other mitigation techniques to lakes.

Effects of Korean Red Ginseng on Semen Parameters in Male Infertility Patients:A Randomized,Placebo-Controlled,Double-Blind Clinical Study

Objective： To investigate the effects of Korean red ginseng （KRG） on semen parameters in male infertility patients in a randomized, double-blind, placebo-controlled study. Methods： A total of 80 male infertility patients with varicocele were recruited from April 2011 to February 2012. The subjects were then divided into the following four groups： non-varicocelectomy （V）＋placebo （P） group, V＋P group, non-V＋KRG group （1.5-g KRG daily）, and V＋KGR group （1.5-g KRG daily）. Semen analysis was performed and hormonal levels were measured in each treatment arm after 12 weeks. Results： All groups but not the non-V＋P group, showed significant improvements in sperm concentrations, motility, morphology, and viability at the end of the study. However, there were no significant differences in serum follicle-stimulating hormone, luteinizing hormone, and testosterone among groups. The incidence of adverse events was low, and all events were assumed to be unrelated to the treatments administered. Conclusions： Although the exact mechanism by which KRG improves spermatogenesis remains unclear, KRG may be a useful agent for the treatment of male infertility. Nevertheless, additional studies to evaluate the optimal dose and duration of treatment are needed.

Investigation on static routing and resource assignment of elastic all-optical switched intra-datacenter networks

In this paper, we explore the issue of static routing and spectrum/IT resource assignment(RSIA)of elastic all-optical switched intra-datacenter networks(intra-DCNs) by proposing anycast- and manycastbased integer linear programming(ILP) models. The objective is to jointly optimize the DCN resources, i.e.,network transmission bandwidth and IT resources, under different situations. First, for given service-request matrices with unknown network transmission bandwidth and IT resources, we propose anycast and manycast ILP models to minimize the maximum numbers of required network and IT resources to accommodate all the service requests. For anycast RSIA issue, we proposed two different ILP models that are based on node-arc and link-path methods, respectively. Node-arc based manycast ILP model is also proposed for the first time to our knowledge. Second, for given network transmission bandwidth and IT resources and known servicerequest matrices, we propose node-arc based anycast ILP models to maximize the total number of successfully served service requests. To evaluate the efficiency of anycast and manycast models, all proposed ILP models are evaluated and compared with unicast ILP models. Simulation results show that anycast and manycast ILP models perform much better in efficiently using DCN resources and successfully accommodating more service requests when compared to unicast ILP models under the same network conditions.
