Risk factors of electrocoagulation syndrome after esophageal endoscopic submucosal dissection

AIM To investigate post endoscopic submucosal dissection electrocoagulation syndrome(PEECS) of the *** We analyzed 55 consecutive cases with esophageal endoscopic submucosal dissection for superficial esophageal squamous neoplasms at a tertiary referral hospital in South Korea. Esophageal PEECS was defined as "mild" meeting one of the following criteria without any obvious perforation: fever(≥ 37.8 ℃), leukocytosis(> 10800 cells/μl), or regional chest pain more than 5/10 points as rated on a numeric pain intensity scale. The grade of PEECS was determined as "severe" when meet two or more of above *** We included 51 cases without obvious complications in the analysis. The incidence of mild and severe esophageal PEECS was 47.1% and 17.6%, respectively. Risk factor analysis revealed that resected area, procedure time, and muscle layer exposure were significantly associated with PEECS. In multivariate analysis, a resected area larger than 6.0 cm^2(OR = 4.995, 95%CI: 1.110-22.489, P = 0.036) and muscle layer exposure(OR = 5.661, 95%CI: 1.422-22.534, P = 0.014) were independent predictors of esophageal PEECS. As-hot-rolled medium-entropy alloys(MEAs)with unevenly distributed grain sizes of face-centered cubic grains exhibit better yield strength without uniform elongation loss compared to cold-rolled and an-nealed *** operation of dynamic recrystallization(DRX)during several hot rolling passes leads to a wide range of grain sizes from submicrons to tens of micrometers due to the grain growth after nucleation:early recrystallized grains are coarser than recently recrystallized *** only the grain size but internal dislocation density of the recently recrystallized grain is *** the tensile deformation of the hot-rolled MEAs at-196 ℃,dislocation pile-ups in the relatively soft and fine DRX grains enhance yield stress and hetero-deformation-induced strain *** to the enhanced yield stress of the as-hot-rolled MEAs,stress-induced martensitic transformation easily ***,partially DRXed MEAs hot-rolled at 800 ℃ have lower yield stress than fully DRXed ones,hot-rolled at 900 and 1000 ℃.This is attributed to the softening effect of the stress-induced body-centered cubic martensitic transformation in unrecrystallized coarse grains prior to the yielding,which lowers the yield stress of the partially DRXed *** yielding,the martensitic transformation facilitates strain hard-ening and early necking is *** study presents a fresh outlook on the uneven distribution of grain sizes by hot rolling beneficial to mechanical responses of uniform elongation of~45%despite the as-rolled states with an advantage of simplified thermo-mechanical processes.

AIM:To evaluate the effect of prokinetic drugs on electrogastrography(EGG) parameters according to symptomatic changes in patients with functional dyspepsia(FD).METHODS:Seventy-four patients with FD were prospectively enrolled in this study between December 2006 and December *** surveyed the patients using a questionnaire on dyspeptic symptoms before and after an 8-wk course of prokinetic drug *** also measured cutaneous pre-prandial and postprandial EGG recordings including percentage of gastric waves(normogastria,bradygastria,tachygastria),dominant frequency(DF),dominant power(DP),dominant frequency instability coefficient(DFIC),dominant power instability coefficient(DPIC),and the ratio of post-prandial to fasting in DP before and after the 8-wk course of prokinetic drug ***:Fifty-two patients(70%) achieved symptomatic improvement after prokinetic drug *** who had normal gastric slow waves showed symptom improvement group after *** DF showed a downward trend in the symptom improvement group,especially in the itopride ***-prandial DP was increased regardless of symptom improvement,especially in the itopride group and mosapride ***-prandial DFIC and DPIC in the symptom improvement group were significantly increased after the *** EGG power ratio was increased after treatment in the symptom improvement group(0.50 ± 0.70 vs 0.93 ± 1.77,P = 0.002),especially in the itopride and levosulpiride ***:Prokinetics could improve the symptoms of FD by regulating gastric myoelectrical activity,and EGG could be a useful tool in evaluating the effects of various prokinetics.

AIM:To evaluate whether weekend or nighttime admission affects prognosis of peptic ulcer bleeding despite early ***:Retrospective data collection from four referral centers,all of which had a formal out-of-hours emergency endoscopy service,even at weekends.A total of 388 patients with bleeding peptic ulcers who were admitted via the emergency room between January 2007 and December 2009 were *** parameters included time from patients' arrival until endoscopy,mortality,rebleeding,need for surgery and length of hospital ***:The weekday and weekend admission groups comprised 326 and 62 patients,*** were no significant differences in baseline characteristics between the two groups,except for younger age in the weekend *** patients (97%) had undergone early endoscopy,which resulted in a low mortality rate regardless of point of presentation (1.8% overall vs 1.6% on the weekend).The only outcome that was worse in the weekend group was a higher rate of rebleeding (12% vs 21%,P = 0.030).However,multivariate analysis revealed nighttime admission and a high Rockall score (≥ 6) as significant independent risk factors for rebleeding,rather than weekend ***:Early endoscopy for peptic ulcer bleeding can prevent the weekend effect,and nighttime admission was identified as a novel risk factor for rebleeding,namely the nighttime effect.

AIM： To investigate endoscopic and clinicopathologic characteristics of early gastric cancer （EGC） according to microsatellite instability phenotype. METHODS： Data were retrospectively collected from a single tertiary referral center. Of 981 EGC patients surgically treated between December 2003 and October 2007, 73 consecutive EGC patients with two or more microsatellite instability （MSI） mutation [high MSI （MSI-H）] and 146 consecutive EGC patients with one or no MSI mutation （non-MSI-H） were selected. The endoscopic and clinicopathologic features were compared between the MSI-H and non-MSI-H EGC ***： In terms of endoscopic characteristics, MSI-H EGCs more frequently presented with elevated pattern （OR 4.38, 95% Cl： 2.40-8.01, P 〈 0.001）, moderate- to-severe atrophy in the surrounding mucosa （OR 1.91, 95% CI： 1.05-3.47, P = 0.033）, antral location （OR 3.99, 95% CI： 2.12-7.52, P 〈 0.001） and synchronous le- sions, compared to non-MSI-H EGCs （OR 2.65, 95% CI： 1.16-6.07, P = 0.021）. Other significant clinicopatholog- ic characteristics of MSI-H EGC included predominance of female sex （OR 2.77, 95% CI： 1.53-4.99, P 〈 0.001）, older age （〉 70 years）（OR 3.30, 95% CI： 1.57-6.92, P = 0.002）, better histologic differentiation （OR 2.35, 95% Cl： 1.27-4.34, P = 0.007）, intestinal type by Lau- ren classification （OR 2.34, 95% CI： 1.15-4.76, P = 0.019）, absence of a signet ring cell component （OR 2.44, 95% CI： 1.02-5.86, P = 0.046）, presence of mu- cinous component （OR 5.06, 95% Cl： 1.27-20.17, P = 0.022）, moderate-to-severe lymphoid stromal reaction （OR 3.95, 95% CI： 1.59-9.80, P = 0.003）, and co-exist- ing underlying adenoma （OR 2.66, 95% CI： 1.43-4.95, P = 0.002）. CONCLUSION： MSI-H EGC is associated with unique endoscopic and clinicopathologic characteristics includ- ing frequent presentation in protruded type, co-exist- ing underlying adenoma, and synchronous lesions.

Soil heterotrophic respiration and its temperature sensitivity are affected by various climatic and environmental ***,little is known about the combined effects of concurrent climatic and environmental changes,such as climatic warming,changing precipitation regimes,and increasing nitrogen(N)***,in this study,we investigated the individual and combined effects of warming,wetting,and N addition on soil heterotrophic respiration and temperature *** incubated soils collected from a temperate forest in South Korea for 60 d at two temperature levels(15 and 20℃,representing the annual mean temperature of the study site and 5℃warming,respectively),three moisture levels(10%,28%,and 50%water-filled pore space(WFPS),representing dry,moist,and wet conditions,respectively),and two N levels(without N and with N addition equivalent to 50 kg N ha^(-1)year^(-1)).On day 30,soils were distributed across five different temperatures(10,15,20,25,and 30℃)for 24 h to determine short-term changes in temperature sensitivity(Q_(10),change in respiration with 10℃increase in temperature)of soil heterotrophic *** completing the incubation on day 60,we measured substrate-induced respiration(SIR)by adding six labile substrates to the three types of *** treatment(increase from 28%to 50%WFPS)reduced SIR by 40.8%(3.77 to 2.23μg CO_(2)-C g^(-1)h^(-1)),but warming(increase from 15 to 20℃)and N addition increased SIR by 47.7%(3.77 to 5.57μg CO_(2)-C g^(-1)h^(-1))and 42.0%(3.77 to 5.35μg CO_(2)-C g^(-1)h^(-1)),respectively.A combination of any two treatments did not affect SIR,but the combination of three treatments reduced SIR by 42.4%(3.70 to 2.20μg CO_(2)-C g^(-1)h^(-1)).Wetting treatment increased Q_(10)by 25.0%(2.4 to 3.0).However,warming and N addition reduced Q_(10)by 37.5%(2.4 to 1.5)and 16.7%(2.4 to 2.0),*** coupled with wetting did not significantly change Q_(10),while warming coupled with N addition reduced Q

Most organic electrode materials(OEMs)for rechargeable batteries employ n-type redox centers,whose redox potentials are intrinsically limited<3.0 V versus Li^(+)/***,p-type materials possessing high redox potentials experience low specific capacities because they are capable of only a single redox reaction within the stable electrochemical window of typical ***,we report 5,11-diethyl-5,11-dihydroindolo[3,2-b]carbazole(DEICZ)as a novel p-type OEM,exhibiting stable plateaus at high discharge potentials of 3.44 and 4.09 V versus Li^(+)/***,the second redox potential of DEICZ is within the stable electrochemical *** mechanism of the double redox reaction is investigated using both theoretical calculations and experimental measurements,including density functional theory calculations,ex situ electron spin resonance,and X-ray photoelectron ***,hybridization with single-walled carbon nanotubes(SWCNT)improves the cycle stability and rate performance of DEICZ owing to theπ-πinteractions between the SWCNT and co-planar molecular structure of DEICZ,preventing the dissolution of active materials into the *** DEICZ/SWCNT composite electrode maintains 70.4%of its initial specific capacity at 1-C rate and also exhibits high-rate capability,even performing well at 100-C ***,we demonstrate its potential for flexible batteries after applying 1000 bending stresses to the composite electrode.

Surface electromyography(sEMG)sensors play a critical role in diagnosing muscle conditions and enabling prosthetic device control,especially for lower extremity robotic ***,challenges arise when utilizing such sensors on residual limbs within a silicon liner worn by amputees,where dynamic pressure,narrow space,and perspiration can negatively affect sensor *** commercial sEMG sensors and newly developed sensors are unsuitable due to size and thickness,or susceptible to damage in this *** this paper,our sEMG sensors are tailored for amputees wearing sockets,prioritizing breathability,durability,and reliable recording *** employing porous PDMS and Silbione substrates,our design achieves exceptional permeability and adhesive *** serpentine electrode pattern and design are optimized to improve stretchability,durability,and effective contact area,resulting in a higher signal-to-noise ratio(SNR)than conventional ***,our proposed sensors wirelessly enable to control of a robotic leg for amputees,demonstrating its practical feasibility and expecting to drive forward neuro-prosthetic control in the clinical research field near future.
