Association of time to prostate-specific antigen nadir and logarithm of prostate-specific antigen velocity after progression in metastatic prostate cancer with prior primary androgen deprivation therapy

We investigated the association of time to prostate-specific antigen nadir （TTPN） and logarithm of prostate-specific antigen velocity after progression Log（PSAVAP） in metastatic prostate cancer with prior primary androgen deprivation therapy （ADT）. All metastatic prostate cancer patients treated with primary ADT from 2000 to 2009 were reviewed. Patients who developed disease progression were included in the subsequent analyses. Patients were categorized into three groups according to their TTPN： TTPN of 〈3 months, 3-17 months, and 〉17 months. We compared the Log（PSAVAP） between the different TTPN groups using Mann-Whitney U-test and Kruskal-Wallis test. Further multiple linear regression analyses on Log（PSAVAP） were performed to adjust for other potential confounding factors. Among 419 patients who were treated with primary ADT, 306 patients developed disease progression with a median follow-up of 28 months, Longer TTPN was associated with lower Log（PSAVAP）（P = 0.008） within all subgroup analyses （TTPN of 〈3 vs 3-17 months, P = 0.020; TTPN of 3-17 vs 〉17 months, P = 0.009; and TTPN of 〈3 vs 〉17 months, P = 0.001）. Upon multiple linear regression analyses, baseline PSA （regression coefficient 0.001, P = 0.045）, PSA nadir （regression coefficient 0.002, P = 0.040）, and TTPN （regression coefficient -0.030, P = 0.001） were the three factors that were significantly associated with Log（PSAVAP）. In conclusion, a longer TTPN was associated with lower Log（PSAVAP） in metastatic prostate cancer patients following primary ADT. TTPN cut-offs at 3 months and 17 months appeared to have prognostic significance in predicting Log（PSAVAP）. TTPN may serve as a good prognostic indicator in deciding the treatment strategy in patients with disease progression.

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Psychiatric illness, such as affective disorders, anxiety disorders and schizophrenia, exerts exceptional personal burden on affected individuals. Although not physically noticeable, these disorders cost enormously on ones' family and society. Currently pharma-ceutical and psychological treatments are generally accepted as effective for psychiatric disorders, while the exact mechanisms underlying the treatment efficacy, etiology and neurobiology of the disorders remain elusive. In the past decade, "neurogenic hy-pothesis" emerged as an attempt to explain the nature of psychiatric illness. The origination of the hypothesis is based on several pre-clinical and clinical observations. First, stress, which is a common risk factor of the disorders, was found to suppress neurogenesis; second, treatment for the illnesses like antidepressants and antipsychotics were shown to improve neurogenesis and behavioral deficits simultaneously; and third, the therapeutic effect of antidepressants was abolished in animal models when neuro-genesis was blocked. Increasing efforts were invested to determine whether neurogenesis is a key to the understanding and treatment of psychiatric disorders, although contrasting results are also found and thus the importance of neurogenesis remains a matter of debate. The present chapter will discuss the recent findings about the involvement of neurogenesis in major depression, anxiety disorders and schizophrenia, and whether neurogenesis would be a potential target for development of the treatment in the future.

Objective：This study was to investigate the variables in bone marrow harvesting procedure and individual donor factors which can potentially affect the yield of mesenchymal stromal cells（MSC）.Methods：We determined the yield of MSC from bone marrow under different clinical conditions by comparing the MSC colony numbers from：（1） donors of different ages;（2） healthy donors and patients with leukemia;（3） bone marrow aspirated at different time points during marrow harvesting;（4） bone marrow harvested by different ***：During the process of harvesting,the number of MSC significantly decreased with increase number of aspiration,from 675/ml at the initial decreased to 60/ml after 100 ml bone marrow aspirated,and 50/ml after 200 ml bone marrow *** number of MSC retrieved from leukemia patients（99/ml bone marrow） was significantly lower than that of healthy donors（708/ml bone marrow）.however,there was no significant difference in growth *** was no significant age-related difference of MSC yielded from donors 55 *** there was no significant difference in MSC number between the samples from single end-holed needle and those from multiple-side-hole ***：The optimal bone marrow samples for MSC collection should be obtained earlier in the process of harvesting *** marrow from donors 55 years was equally good as MSC *** autologous MSC from leukemia patients can be utilized for in-vitro MSC expansion.

本文旨在为澳门特别行政区和珠海的碧海工程理念提出一个概念性框架。该理念的提出以广泛的社会学和建筑学理论与实际研究为依据。本文论述了民族特性和建筑之间的互动关系。虽然社会学和建筑学属于完全不同的两个学科,但是在这两个学科里,民族特性的表达方式是相同的,那就是土地。许多关于民族特性的理论研究都突出了领土和家园的重要性,而任一历史时期的建筑都可以反映出当时的社会发展状况以及族群的特点。这类理论的探讨主要采用对文献的研究以及借鉴梁和刘对加拿大、美国、希腊和香港特别行政区等国家和地区的许多案例分析。我们认为对于建筑而言,不可能不参考任何外界因素而完全孤立的建造,而应该充分考虑其被社会所能接受的程度,即生活在当地的族群的接受程度。同时,研究这些社会条件的学者和专业人士也不应该忽视建筑和建筑产生的环境对社会的重要贡献。因此而形成的这种社会学和建筑学之间的互动关系可以多方面为实践工作服务:有助于研究一个地方或城市的历史发展;对某个地方或城市今后的可持续发展提供预策和决策;为未来的建筑和建筑营造出来的建筑环境提出规划设想。本届会议的其他两篇文章将详细介绍我们如何在该框架内提出澳门特别行政区和珠海的碧海工程的构想。

Photocatalytic disinfection has been well-studied for water treatment. however, the post-separation of photocatalyst remains a major challenge for practical application. This study developed a novel method for photocatalytic disinfection, which used a "partition system" to retain the photocatalyst. Cadmium indium sulfate （CdIn 2 S 4 ） coupled with regenerated cellulose membrane gave the best performance. 4 log-reduction of a Gram-negative bacterium, Escherichia coli, and 5 log-reduction of a Gram-positive bacterium, Microbacterium Barkeri, （initial cell density 2×10 5 cells/mL） were achieved within 4 and 6 h, respectively. Scavenger study revealed that H 2 O 2 is the major reactive species for bacterial inactivation in the "partition system". The study provides an alternative cost-effective method for the application of photocatalysis in water disinfection.

Objective To report the experience in sperm extraction from testicular biopsies (TESE) performed from March 1996 to July 1998 in men with non obstructive azoospermia (NOA) Methods Comparisons of age, volume of both testes, serum FSH and testosterone in men, and histology of testicular samples in the first cycles between cycles with spermatozoa found and those without spermatozoa found were performed Comparisons of fertilization, cleavage and pregnancy rates between cycles with spermatozoa injected and those with spermatids injected were performed Results Spermatozoa were found in only 12 out of 26 first TESE cycles (46 2%) and other cycles had spermatids (round cells) only Age of men, history of mumps orchitis/oligozoospermia, volume of both testes and serum FSH/testosterone levels in men were not significantly different between cycles with and without spermatozoa The fertilization rate was significantly higher in cycles with spermatozoa injected than those with round cell injections (63 3% vs 23 2%, P <0 0001, Chi squared test) The pregnancy rate was 14 3% per cycle when spermatozoa were injected Conclusions TESE followed by Intracytoplasmic sperm injection (ICSI) is an effective treatment in patients with NOA Less than half of the patients undergoing TESE had spermatozoa recovered Age of men, volume of both testes and serum FSH/ testosterone levels in men were not useful in predicting successful recovery Compared to using ejaculated and epididymal spermatozoa, fertilization and pregnancy rates were achieved when testicular spermatozoa were used for ICSI

This paper examines the architectural and Landscape design strategies and intentions for green, open spaces facilities targeting stress aLLeviation for learning environments such as those of university campuses in a compact urban setting. Literature reviews provide three prevailing perspectives for physical design pedagogical operatives： healing gardens where greenery and plants produce restorative effects; flexible spaces that accommodate functional needs of different activities; and green buildings that incorporate open space as a catalyst for integrated eco-system. Corresponding design approaches （Landscape design, spatial design and green design） are scrutinized by case study. A comparison of two university campuses with different urban contexts is conducted to identify challenges and opportunities for applying these design approaches. For a compact campus, high-dense surroundings may limit the size of an open space and may handicap circulation and accessibility; on the other side, a small open space may provide its users more intimate contact with natural restorative elements and also a more controllable microclimate for physical comfort. A healthy campus should encompass diverse open spaces to satisfy different purposes. Finally, a framework that integrates the three approaches is combined to produce a sustainable design rubric.
