Evaluation of the sperm DNA fragmentation index in infertile Japanese men by in-house flow cytometric analysis

Semen analysis has long been used to evaluate male fertility.Recently,several sperm function tests have been developed.Of those,the sperm DNA fragmentation index(DFI),which describes the status of the sperm DNA,is thought to be a suitable parameter for evaluating male fertility.However,there have been no large-scale studies on the sperm DFI of Japanese men.Therefore,we investigated the feasibility of using an in-house flow cytometry-based sperm DFI analysis based on the sperm DNA fragmentation test of sperm chromatin structure assay(SCSA)to assess male fertility in Japan.This study enrolled 743 infertile and 20 fertile Japanese men.To evaluate reproducibility,inter-and intraobserver precision was analyzed.A receiver operating characteristic curve analysis was used to set a cutoff value for the sperm DFI to identify men who could father children by timed intercourse or intrauterine insemination.The variability of the sperm DFI among fertile volunteers was determined.The relationship between semen parameters and the sperm DFI was assessed by Spearman’s rho test.A precision analysis revealed good reproducibility of the sperm DFI.The cutoff value of sperm DNA fragmentation in infertile men was 24.0%.Semen volume had no relationship with the sperm DFI.Sperm concentration,sperm motility,total motile sperm count,and percentage of normal-shaped sperm were significantly and negatively correlated with the sperm DFI.The median sperm DFI was smaller in fertile volunteers(7.7%)than that in infertile men(19.4%).Sperm DNA fragmentation analysis can be used to assess sperm functions that cannot be evaluated by ordinary semen analysis.

We discuss a linear seesaw model with a field content as minimum as possible,introducing a modular S_(4)using gauged U(1)_(B−L) symmetries.Owing to the rank two neutrino mass matrix,we obtain a vanishing neutrino mass eigenvalue,and only the normal mass hierarchy of neutrinos is favored via the modular S_(4)symmetry.In our numericalΔχ^(2)analysis,we especially determine a relatively sharp prediction on the sum of neutrino masses to be approximately 60 meV,in addition to other predictions.

We consider a model with multi-charged particles,including vector-like fermions,and a charged scalar under a local U(1)_(μ−τ) symmetry.We search for an allowed parameter region explaining muon anomalous magnetic moment(muon g−2)and b→sℓ^(+)ℓ^(−) anomalies,satisfying constraints from the lepton flavor violations,Z boson decays,meson anti-meson mixing,and collider experiments.Via numerical analysis,we explore the typical size of the muon g−2 and Wilson coefficients to explain the b→sℓ^(+)ℓ^(−) anomalies in our model when all other experimental constraints are satisfied.Subsequently,we discuss the collider physics of the multicharged vectorlike fermions,considering a number of benchmark points in the allowed parameter space.

We propose a new model with flavor-dependent gauged U(1)B-L1×U(1)B-L2-L3symmetry in addition to the flavor-blind symmetry in the Standard Model. The model contains three right-handed neutrinos to cancel gauge anomalies and several Higgs bosons to construct the measured fermion masses. We show the generic features of the model and explore its phenomenology. In particular, we discuss the current bounds on the extra gauge bosons from the K and B meson mixings as well as the LEP and LHC data, and focus on their contributions to the lepton flavor violating processes of ?i+1→?iγ(i=1,2).

This study aimed to ascertain the current status of Japanese sperm banking for young cancer patients. During 2015, we mailed the directors of 695 institutes where sperm cryopreservation might be performed with questionnaires requesting information on the number of patients, age, precryopreservation chemotherapy, semen analyses results and diagnoses, cryopreservation success rate, and causes of unsuccessful cryopreservation. Of these 695 institutes, 92 had cryopreserved sperm before chemotherapy within the study period. In all, 820 cancer patients （237 testicular, 383 hematological, 46 bone and soft tissue, 20 brain, and 134 other malignancy） consulted the responding institutes for sperm cryopreservation. Except for testicular tumor, the number of patients whose sperm was preserved before cancer treatment was low compared to that of young cancer patients. Approximately 20% of patients with malignancies other than testicular tumor underwent chemotherapy before cryopreservation. The success rate of cryopreservation in hematological malignancy was 82.5%, significantly lower than that of both the testicular cancer （93.6%） and other malignancy groups （95.6%）（P 〈 0.05）. The primary reasons for preservation failure were azoospermia and poor semen quality. Patients with hematological malignancies had a higher rate of unsuccessful cryopreservation compared to those in other groups, possibly due to the large number of patients requesting sperm cryopreservation after chemotherapy induction. In Japan, information regarding sperm banking prior to cancer treatment appears to be lacking. Information regarding sperm preservation before chemotherapy should be provided to all Japanese oncologists.

Dear Editor,Approximately 20% of men with nonobstructive azoospermia （NOA） are diagnosed with infertility caused by genetic defects.1 These include chromosomal abnormalities, Y-chromosome microdeletions, and several specific gene mutations/deletions, such as in DAZ, RBMY, USP9Y, SYCP3, HSF2, PLK4, and TEX11.1,2 Several histones have been detected in mammalian testes, and testis-specific variants are specifically and highly expressed during spermatogenesis^3.

According to the World Health Organization(WHO),oxidative stress(OS)is a significant contributor to male infertility.SeminalOS can be measured by a number of assays,all of which are either costly or time sensitive and/or require large semen volume andcomplex instrumentation.One less expensive alternative is to quantify the oxidation-reduction potential(ORP)with the MiOXSYS.In this international multi-center study,we assessed whether ORP levels measured by the MiOXSYS could distinguish semensamples that fall within the 2010 WHO normal reference values from those that do not.Semen samples were collected from 2092patients in 9 countries;ORP was normalized to sperm concentration(mV/10^6 sperm/ml).Only those samples with a concentration>1×10^6 sperm ml1 were in eluded.The results showed that 199 samples fell within the WHO no rmal refere nee range while theremaining 1893 samples did not meet one or more of the criteria.ORP was negatively correlated with all semen parameters(P <0.01)except volume.The area under the curve for ORP was 0.765.The ORP cut-off value(1.34 mV/10^6 sperm/ml)was able todifferentiate specimens with abnormal semen parameters with 98.1%sensitivity,40.6%specificity,94.7%positive predictivevalue(PPV)and 66.6%negative predictive value(NPV).When used as an adjunct to traditional semen analysis,ORP levels mayhelp identify altered functional status of spermatozoa caused by OS in cases of idiopathic male infertility and in male partners ofcouples sufferi ng recurre nt pregna ncy loss,and thereby directi ng these men to relevant medical therapies and lifestyle modificati ons.

Classification of rectal cancer according to recurrence types-comparison of Japanese guidelines and Western guidelines

BACKGROUND Rectal cancer is characterized by more local recurrence(LR)and lung metastasis than colon cancer.However,the diagnosis of rectal cancer is not standardized as there is no global consensus on its definition and classification.The classification of rectal cancer differs between Japanese and Western guidelines.AIM To clarify the characteristics of rectal cancer by comparing the tumor location and characteristics of rectal cancer with those of colon cancer according to each set of guidelines.METHODS A total of 958 patients with Stage II and III colorectal cancer were included in the analysis:607 with colon cancer and 351 with rectal cancer.Localization of rectal cancers was assessed by enema examination and rigid endoscopy.According to Japan guidelines,rectal cancer is classified as Rb(below the peritoneal inversion),Ra(between the inferior margin of second sacral vertebrae and Rb)or RS(between Ra and sacral promontory).RESULTS There were no significant differences between RS rectal cancer and colon cancer in the rates of liver and lung metastasis or LR.Lung metastasis and LR were significantly more common among Rb rectal cancer(in Japan)than in colon cancer(P=0.0043 and P=0.0002,respectively).Lung metastases and LR occurred at significantly higher rates in rectal cancer measuring≤12 cm and≤10 cm than in colon cancers(P=0.0117,P=0.0467,P=0.0036,P=0.0010).Finally,the rates of liver metastasis,lung metastasis,and LR in rectal cancers measuring 11 cm to 15 cm were 6.9%,2.8%,and 5.7%,respectively.These were equivalent to the rates in colon cancer.CONCLUSION High rectal cancer may be treated with the same treatment strategies as colon cancer.There was no difference in the classification of colorectal cancer between Japan and Western countries.

Effects of Lifestyle Intervention Performed by Community Pharmacists on Glycemic Control in Patients with Type 2 Diabetes: The Community Pharmacists Assist (Compass) Project, a Pragmatic Cluster Randomized Trial

Background: Community pharmacists should be involved in diabetes care, while there has been less evidence about whether a brief lifestyle intervention is effective for diabetes care in community pharmacies. Objectives: To examine the effects of brief lifestyle intervention on glycemic control in patients with type 2 diabetes mellitus (T2D) by using a coaching style, provided by community pharmacists. Methods: A prospective, cluster-randomized, controlled trial was conducted in 50 groups of community pharmacies in Japan. In all, 132 patients with T2D (age, 20 - 75 years, ≥8.0% of hemoglobin A1c (HbA1c)) were assigned to the intervention group (n = 90) or the usual care group (n = 42). The intervention group (IG) underwent brief lifestyle coaching for self-care of T2D for 6 months. The standard care group (CG) received usual care by pharmacists and was given a general newsletter. The primary outcome was changes in HbA1c levels. Results: After 6 months, the IG had significantly improved HbA1c (IG: -0.6 ± 0.9 vs. CG: -0.2% ± 0.9%;p = 0.021 using the last observation carried forward analysis). Although the number of drugs reduced from 2.3 ± 0.8 to 2.0 ± 1.2 in the IG, the number increased from 2.3 ± 1.1 to 2.5 ± 1.1 in the CG (-0.2 ± 0.9 in IG vs. 0.2 ± 0.6 in CG;p = 0.023). Conclusions: The brief lifestyle intervention by community pharmacists improved glycemic control in patients with T2D. Community pharmacists may more positively participate as lifestyle coaches for diabetes care.
