Sheathless and fluoroscopy-free retrograde intrarenal surgery:An attractive way of renal stone management in high-volume stone centers

Objective:To evaluate the efficacy and safety of sheathless and fluoroscopy-free flexible ureterorenoscopic laser lithotripsy(FURSL)for treating renal ***:Between May 2015 and May 2017,135 patients with renal stones treated with sheathless and fluoroscopy-free FURSL were prospectively *** technique involved a semirigid ureteroscopic assessment of the ureter,and the guidewire was left in situ to railroad the flexible ureteroscope.A holmium laser was used to fragment and dust the stones;fragments were neither grasped nor ***:The study population consisted of 135 patients including 85 males(62.96%)and 50 females(37.04%)with a mean age of 40.65 years(range:3-70 years)were *** mean stone size was 17.23 mm(range:8-41 mm).Complete stone-free status was achieved in 122(90.37%)patients and clinically insignificant residual fragments(CIRF)in two(1.48%),while residual stones were still present in 11(8.15%)*** complications occurred in 23(17.4%)cases and were mostly minor,including fever in 17(12.6%),pyelonephritis in four(3.0%),subcapsular hematoma in one(0.7%)and steinstrasse in one(0.7%).These complications were Clavien Ⅰ-Ⅱ,GⅠ in 17(12.6%)patients,GⅡ in five(3.7%),and Clavien Ⅲb in one(0.7%).No major complications were *** size2 cm,operative time ≥30 min,and lasing time ≥20 min were significantly associated with a higher rate of complications and lower stone-free rates upon univariate analysis(p<0.05).Conclusion:Sheathless and fluoroscopy-free FURSL are effective and safe for renal stone management,especially for stones under 2 cm in *** process is a feasible option for avoiding sheath complications,which can protect surgeons from the negative effects of radiation.

Application of multi-criteria decision-making methods to identification of soil moisture monitoring sites in an urban catchment in South Australia

When choosing sites for monitoring of soil moisture for hydrological purposes,a suitable process that considers the factors influencing soil moisture level should be *** this study,two multi-criteria decision-making(MCDM)methods,the multi-influencing factor(MIF)method and the analytical hierarchy process(AHP)method,were used to identify the optimal soil moisture monitoring(SMM)sites in the Dry Creek Catchment in South *** most representative areas for nine SMM sites were obtained using the MIF method,considering the factors of rainfall,soil type,land use,catchment slope,elevation,and upslope accumulated area(UAA).The AHP method was used to select the optimal sites using the site-specific criteria.30.3%of the catchment area in the Australian Water Resources Assessment Landscape(AWRA-L)Grid_DC2 can be considered acceptable as representative area with the MIF *** potential sites were evaluated for each AWRA-L grid using the relative weights of the site-specific criteria with the AHP *** Grid_DC2 required two sites that had the highest overall weight chosen with the AHP *** procedure was repeated for the remaining four AWRA-L grids within the study area to select the required SMM sites.

Sociodemographic and clinical characteristics of SARS-CoV-2 infected population during the second and third epidemiological waves in Sri Lanka

Objective:To analyze data on socio-demographic and clinical characteristics of SARS-CoV-2 infected population whose samples were received from Medical Research Institute,Sri ***:Laboratory based retrospective study was done on patient samples which were tested positive for SARS-CoV-2 by National Reference Virology Laboratory at the Medical Research Institute,Sri Lanka,from November,2020 to November,*** on sociodemographic characteristics and clinical presentation of 13126 patients were ***:The mean age of the study population was(36.0±7.2)years and the majority were men(64.0%).The highest number of positive cases were found in the 21-30 years-of-age *** distinct peaks were noted in the incidence of SARS-CoV-2 positive *** addition,42.5%of the positive samples tested positive(42.5%)were from Medical Officer of Health collection ***,60.6%(7951)of the infected subjects were asymptomatic whereas the remaining were *** highest percentage of symptomatic patients were observed in the 91-100 years-of-age group while the highest asymptomatic subjects were found in the 31-40 years-of-age *** percentage of asymptomatic children(65.3%)was significantly(P<0.05)higher than that of adults(43.4%).Conclusions:The findings of this study aid decision makers to focus on the vulnerable groups,and geographic and temporal distribution of patients in the public health strategies that aim at preventing the spread of the disease and reducinig its mortalities.

Type 2 diabetic mellitus is a predominant metabolic disorder that has a direct impact on human *** scientific data are deficit,coconut jaggery has been suggested as a better alternative for cane sugar by some *** study was conducted to assess the credibility of this *** jaggery was prepared at Coconut Research Institute,Sri Lanka and nutritional composition of coconut jaggery was compared with cane sugar using standard *** higher(P23.0).The mean fasting blood glucose level and HbA1c of the subjects were(149.05±54.88)mg/dL and(9.170±2.022)%,*** was no significant difference(P>0.05)in peak blood glucose concentrations or incremental area under the curve in blood glucose response of two test ***,coconut jaggery cannot be considered as a healthy substitute for cane sugar in type 2 diabetic patients.

AIM:To determine if inflammatory bowel disease(IBD) is a risk factor for osteoporosis in adult Sr ***:We identified eligible subjects from among consecutive patients diagnosed with IBD who attended our outpatient *** included only patients aged between 20 and 70 *** who were pregnant, had significant comorbidity, or were on calcium supplements or treatment for osteoporosis within the past 6 mo, were ***, ageand sex-matched controls were also recruited, ina control to patient ratio of 3:*** groups were screened for osteoporosis using peripheral dual energy X-ray absorptiometry ***:The study population consisted of 111 IBD patients(male:female = 43:68;mean age 42.5 years) and 333 controls(male:female = 129:204;mean age 43.8 years).The occurrence of osteoporosis among IBD patients(13.5%) was significantly higher than among controls(4.5%)(P = 0.001).The frequency of osteoporosis was not significantly different between ulcerative colitis(14.45%) and Crohn's disease(10.7%).However, on multivariate analysis, only age(P = 0.001), menopause(P = 0.024) and use of systemic steroids(P < 0.001) were found to be associated independently with the occurrence of osteoporosis, while IBD, severity of disease, number of relapses, duration of illness or treatment other than systemic steroids were ***:IBD does not appear to be an independent risk factor for the occurrence of osteoporosis in this ***, the use of systemic steroids was a risk factor.

The energy levels, transition energy, B （E2） values,intrinsic quadrupole moment Q_0 and potential energy surface for even-even ^（184）W and ^（184）Os nuclei were calculated using IBM-1. The predicted energy levels, transition energy, B（E2） values and intrinsic quadrupole moment Q_0 results are reasonably consistent with the experimental data. A contour plot of the potential energy surfaces shows that two interesting nuclei are deformed and have rotational characters.

Soma hewa教授出生于斯里兰卡,在斯里兰卡获得学士学位,1983年在加拿大曼尼托巴大学获得社会学硕士学位,1984年至1985年在德国慕尼黑大学做客座学生,1987年在加拿大阿尔伯塔大学获得社会学博士学位,1997年在意大利博洛尼亚大学担任客座教授。他曾在加拿大不列颠哥伦比亚大学、阿尔伯塔大学等多个研究机构从事教学和研究工作,现为加拿大女王大学政策研究学院教授,加拿大魁北克人口健康与非营利部门研究所首席研究员,并担任广州大学客座教授。他在加拿大从事教学30多年,主要研究健康社会学、人口健康和社会资本、公共健康、慈善组织等领域,是国际学术界健康社会学领域的知名学者,多次获得美国洛克菲勒大学洛克菲勒档案中心、福特基金会和亚洲基金会的奖学金。

Pattern of Cardiac Trauma in Sulaimani Province of Southern Kurdistan: 5 Years’ Experience

Introduction: Cardiac injuries are one of the important causes of death in young population. With aggressive resuscitative therapy and emergency room thoracotomy, the salvage rate of these patients can reach 35%. In this case series the types of presentation, methods of resuscitation, surgical approaches, operative and postoperative outcomes are discussed. Patients and Methods: From January 2009 to January 2014 there were 3157 patients treated for thoracic injuries at Sulaimani university hospital, 14 patients had cardiac injuries. All admitted cases with central chest trauma were submitted to a thorough clinical examination, ECG and eFAST (extended Focused assessment with sonography for trauma) and/or transthoracic echocardiography. Results: Total of 14 cardiac injuries from 3157 causalities were identified, which is 0.44% of the total admissions. Male gender was predominant (85.7% vs. 14.28%) for females. Mechanism of injury was mostly penetrating (85.71%) among which stabs were majority (57.14%) while bullet and shrapnel each constituted (14.28%). Mean time of interval between the accident and our intervention was 2.96 hours. No diagnostic test was 100% specific and sensitive. Discussion: Cardiac injury regarded as a crucial injury because of its high fatality. It is reported that 10.3% of emergency surgical operations are thoracic type and about 1% of them are associated with cardiac injury. Although any penetrating injury to the thorax may injure the heart but those within the box are more suspicious. We conclude that cardiac trauma is a fatal injury but still if the facilities are available the mortality can be minimized.
