Int.oduct.on: Ext.acorporeal Membrane Oxygenat.on (ECMO) is used in select.d pat.ent.wit. cardiogenic and/or re- spirat.ry shock. We report.our experience wit. st.ndardized management.prot.cols and t.e applicat.on of t.e Qua- droxD oxygenat.r wit. a cent.ifugal pump t. maximize end-organ recovery and improve survival. Met.ods: t.is is an Int.rnal Review Board (IRB) approved, single inst.t.t.on ret.ospect.ve st.dy of end-organ recovery and survival in pa- t.ent. who required ECMO for cardiogenic and/or respirat.ry shock bet.een July 2010 and June 2011. Result.: Sixt.en pat.ent. (median age: 46 years) were init.at.d on eit.er Veno-Art.rial (VA) or Veno-Venous (VV) ECMO. Cardiogenic shock, acut. respirat.ry dist.ess syndrome (ARDS) and a combined respirat.ry and cardiogenic compromise were t.e primary indicat.ons for ECMO in 8 (50%), 5 (31%) and 3 (19%) pat.ent. respect.vely. t.e median t.me on ECMO was 8 days (range: 4 - 26 days). t.elve pat.ent. (75%) were successfully weaned off ECMO, of which four (25%) were bridged t. a vent.icular assist.device (VAD) and eight.(50%) were weaned t. recovery. All eight.pat.ent. (100%) t.at.were weaned t. recovery and t.o pat.ent. (50%) t.at.were bridged t. a VAD were successfully discharged from t.e hospit.l, result.ng in a discharge rat. of 63%. t.ere was an improvement.in pre- vs. post.ECMO ASt.(449 IU/L vs. 63 IU/L, p Conclusion: ECMO using t.e QuadroxD oxygenat.r and a cent.ifugal pump, coupled wit. st.ndardized management.prot.cols is beneficial in carefully select.d pat.ent.. Improvement.or main- t.nance of end-organ funct.on is associat.d wit. successful bridge t. device t.erapy and/or increased survival.

Using t.e Ecopat. mass-balance t.ophodynamic model,t.is paper analyzed t.e t.ophic levels,flows,food web st.uct.re and ecosyst.m mat.rit. of t.e East.China Sea,and ident.fied ecologically import.nt.funct.onal groups in t.e *** model is based on fishery resource surveys of t.e East.China Sea in 2000,st.dies on diet.composit.on and global dat.bases such as FishBase and t.e Sea Around Us Project.*** result. showed t.at.t.ophic levels of t.e funct.onal groups are bet.een 2.86 and 4.37,wit. an average of ***(Engraulis japonicus),small fishes and bent.ic crust.ceans such as shrimps and crabs are import.nt.groups in t.rms of t.e t.ophic st.uct.re and flow dynamics in t.e East.China *** flows of most.groups are bet.een specific t.ophic levels,except.file fish(t.amnaconus spp.),pomfret.Pampus spp.) and *** t.ansfer efficiency of levels Ⅱ,Ⅲ,IV and more t.an V are 11.8%,21.1%,17.4% and 22.1-22.5%,*** of fishery-t.e largest.‘consumer’ of t.e ecosyst.mare much st.onger t.an t.ose exert.d by biological groups in t.e *** model suggest. t.at.t.e current.fishery can furt.er reduce t.e complexit. of t.e *** of t.e syst.m indices suggest.t.at.mat.rit. of t.e ecosyst.m is *** conclusion of t.is model indicat.s t.at.it.was t.e overfishing t.at.caused t.e ecosyst.m of t.e East.China Sea declined,which should be t.ken int. account.as a crit.cal reference for fisheries management.in t.e fut.re.

AIM: Irrit.ble bowel syndrome (IBS) is a common disorder and many pat.ent. fail t. find adequat. relief from convent.onal t.erapies for t.eir sympt.ms. t.is st.dy t.st. t.e claim t.at.acupunct.re is effect.ve for a majorit. of t.ese ***: A prospect.ve, blinded, sham acupunct.recont.olled t.ial of t.adit.onal Chinese acupunct.re was performed at.a single post.raduat. t.aching hospit.l in Europe. Sixt. pat.ent. wit. well-est.blished IBS were recruit.d. t.e blinded comparat.r was sham acupunct.re administ.red by t.e second of t.o acupunct.rist. who alone was aware of t.e randomizat.on, and who ot.erwise followed t.e prescript.on of t.e first. t.e primary endpoint.was a defined fall in t.e sympt.m score at.13 wk (by int.nt.on t. t.eat.. t.e prior expect.t.on was a 30%placebo response, and a response rat. of 70% from acupunct.re, for which t.e st.dy was adequat.ly ***: Pat.ent. in t.eat.d and sham groups improved significant.y during t.e st.dy-mean improvement.in scores being equal (minus 1.9) and significant.for bot. (P＜0.05;one-t.iled t.t.st.. t.ere was a small numeric but.nonsignificant.difference bet.een t.e response rat. in pat.ent. receiving acupunct.re (40.7%) and sham t.eat.ent.(31.2%). Several secondary end-point. marginally favored act.ve t.eat.ent. but.an improved sympt.m score of any degree of magnit.de occurred more oft.n wit. sham t.erapy (65.6% vs 59.2%).
