2020—2040年中国5种亚型慢性肾病发病趋势预测研究

背景慢性肾脏病(CKD)是我国乃至全球不可忽视的公共卫生问题。目前,国内关于不同亚型CKD发病趋势预测的相关研究鲜见报道。目的预测2020—2040年中国5种亚型CKD的发病趋势,为CKD的防控提供参考。方法 2023年4—5月,收集全球疾病负担研究(GBD)数据库1990—2019年我国5种亚型CKD的年龄标准化发病率(ASIR)和发病人数。采用变化率(%)和平均年度变化百分比(AAPC)描述和分析我国5种亚型CKD的发病现状。运用Prophet模型预测我国2020—2040年5种亚型CKD的ASIR和发病人数。结果 1990—2019年我国5种亚型CKD的ASIR和发病人数均呈现上升趋势,其中高血压肾病的上升趋势最为明显(AAPC=0.75,P<0.05)。2019年男性2型糖尿病肾病、1型糖尿病肾病、肾小球肾炎肾病和高血压肾病的ASIR和发病人数均高于女性,而女性其他类型肾病的ASIR和发病人数高于男性。2型糖尿病肾病、高血压肾病和其他类型肾病在65~74岁年龄组的发病人数较高。1型糖尿病肾病和肾小球肾炎肾病的发病人数多集中于<5岁年龄组。本研究预测结果表明,预计到2040年,2型糖尿病肾病的ASIR和发病人数分别为23.27/10^(5)(80%UI=20.64/10^(5)~26.08/10^(5))和755 375(80%UI=702 827~811 409)例,1型糖尿病肾病的ASIR和发病人数分别为0.60/10^(5)(80%UI=0.47/10^(5)~0.73/10^(5))和10 625(80%UI=9 519~11 787)例,肾小球肾炎肾病的ASIR和发病人数分别为3.88/10^(5)(80%UI=3.01/10^(5)~4.79/10^(5))和87 050(80%UI=74 470~100 460)例,高血压肾病的ASIR和发病人数分别为15.35/10^(5)(80%UI=13.53/10^(5)~17.29/10^(5))和470 214(80%UI=437 598~504 817)例,其他类型肾病的ASIR和发病人数分别为127.68/10^(5)(80%UI=102.41/10^(5)~154.68/10^(5))和3 901 317(80%UI=3 622 415~4 198 720)例。结论 1990—2019年我国5种亚型CKD的ASIR和发病人数均呈现上升趋势。2020—2040年中国2型糖尿病肾病、高血压肾病和其他类型肾病的ASIR和发病人数仍然呈现上升趋势,虽然1型糖尿病肾病和肾小球肾炎肾病的发病人数逐年增长,但总体的ASIR呈现下降趋势。未来应针对不同亚型的CKD制定相应的防控策略。

Summary of main observation and conclusion A titanium-promoted intramolecular photoenolization/Diels-Alder (PEDA) reaction was developed to construct the core skeleton of aromatic polycyclic terpenoids bearing an all-carbon quaternary center on the benzylic position. Titanium(Ⅳ) isopropoxide [Ti(Oi-Pr)4] plays a key role during the photo cycloaddition, which may help to accelerate the in teraction between dienophile and the stereo-hi ndered diene species as well as control the diastereoselectivity. This photolysis provides a new solution for the stereospecific formation core structures of aromatic abietane diterpenoids and sesquiterpenoids, which have multiple functional groups for the further transformations. As a synthetic application, it was successfully used in the synthesis of indolosesquiterpenoid mycoleptodiscin A.

以某大功率多缸柴油机为研究对象,采用EXCITE Power Unit软件建立隔振器−机体−曲轴的整机隔振系统弹流润滑仿真模型,对比分析机脚布置位置和隔振器刚度对主轴承润滑特性的影响。仿真结果表明:当机脚支撑跨度增加时,支撑位置的轴承孔位移和倾斜程度增加,主轴承粗糙接触压力升高,此时轴承合力降低,摩擦损失功和平均热载改善;当隔振器刚度提升时,主轴承润滑特性趋于改善,采用刚度方案二(刚度为7920 N/mm)时可获得较优润滑指标,继续提升刚度的收益有限。

Last Deglacial Soft-Sediment Deformation at Shawan on the Eastern Tibetan Plateau and Implications for Deformation Processes and Seismic Magnitudes

The eastern margin of the Tibetan Plateau is characterized by frequent earthquakes; however, research of paleo-earthquakes in the area has been limited^ owing to the alpine topography and strong erosion. Detailed investigations of soft-sediment deformation(SSD) structures are valuable for understanding the trigger mechanisms, deformation processes, and the magnitudes of earthquakes that generate such structures, and help us to understand tectonic activity in the region. To assess tectonic activity during the late Quaternary, we studied a well-exposed sequence of Shawan lacustrine sediments, 7.0 m thick, near Lake Diexi in the upper reaches of the Minjiang River. Deformation is recorded by both ductile structures(load casts, flame structures,pseudonodules, ball-and-pillow structures, and liquefied convolute structures) and brittle structures(liquefied breccia, and microfaults). Taking into account the geodynamic setting of the area and its known tectonic activity, these SSD structures can be interpreted in terms of seismic shocks. The types and forms of the structures,the maximum liquefaction distances, and the thicknesses of the horizons with SSD structures in the Shawan section indicate that they record six strong earthquakes of magnitude 6-7 and one with magnitude >7. A recent study showed that the Songpinggou fault is the seismogenic structure of the 1933 Ms7.5 Diexi earthquake. The Shawan section is located close to the junction of the Songpinggou and Minjiang faults, and records seven earthquakes with magnitudes of ?7. We infer,therefore, that the SSD structures in the Shawan section document deglacial activity along the Songpinggou fault.

The adductor muscle scar（AMS） is the fixation point of adductor muscle to the shell. It is an important organicinorganic interface and stress distribution area. Despite recent advances, our understanding of the structure and composition of the AMS remain limited. Here, we report study on the AMS of three bivalves： Mytilus coruscus,Chlamys farreri and Ruditapes philippinarum. Results showed that there were significant differences among their AMS structures. Both M. coruscus and C. farreri were found to have a columnar layer above the nacreous platelet shell structure at the AMS and this layer was more organized in M. coruscus. There was no distinguishable twolayer structure in R. philippinarum. Atomic force microscopy（AFM） and Fourier transform infrared spectroscopy（FT-IR） results showed that the AMS was much smoother than the nacreous inner shell in all the three species and the AMS had minor different compositions from the nacreous shell layer. SDS-PAGE（sodium dodecyl-sulfate polyacrylamide gel electophoresis） study of the proteins isolated from the interface indicated that there was a 70 k Da protein which seemed to be specifically located to the highly organized columnar AMS structure in Mytilus coruscus. Further analysis of this protein showed it contained high level of Asx（Asp＋Asn）, Glx（Glu＋Gln） and *** special structure and composition of the AMS might play important roles in the stability, adhesion and function at this stress distribution site.

A1203-SiC-C iron trough castables were prepared using high alumina bauxite clinker GL-90 （〉 1 mm ） or homogenized bauxite GL-88 to replace the traditional brown corundum aggregates （〉 1 mm ） to reduce the cost of iron trough castables. Effects of the two bauxite aggregates on properties of iron trough eastables were investigated. The results show that the two kinds of bauxite affect flowability, bulk density, strength and slag corrosion resistance of trough castables differently. Compared with homogenized bauxite, high alumina bauxite clinker is more suitable to substitute brown corundum. The castables prepared from the bauxite clinker have similar performances with those prepared from brown corundum, especially used in medium or small blast furnaces with weak thermal impact and lower temperature hot metal. However, in large blast furnaces with severe thermal impact and high temperature hot metal, the performances of the two castables are significantly different. So it is suggested to use the brown corundum based castables in the front gyral zone of the slag skimmer.

Perovskite solar cells(PSCs) have demonstrated excellent photovoltaic performance which currently rival the long-standing silicon solar cells’ efficiency. However, the relatively poor device operational stability of PSCs still limits their future commercialization. Binary sulfide is a category of materials with promising optoelectrical properties, which shows the potential to improve both the efficiency and stability of *** we demonstrate that the inorganic tin monosulfide(Sn S) can be an efficient dopant in 2,2’,7,7’-tet rakis(N,N-di-p-methoxy-phenylamine)-9,9’-spirobifluorene(spiro-OMe TAD) to form a composite hole transport layer(HTL) for PSCs. Sn S nanoparticles(NPs) synthesized through a simple chemical precipitation method exhibit good crystallization and suitable band matching with the perovskites. The introduction of Sn S NPs in Spiro-OMTAD HTLs enhanced charge extraction, reduced trap state density, and shallowed trap state energy level of the devices based on the composite HTLs. Therefore, the resulting solar cells employing Sn S-doped spiro-OMe TAD HTLs delivered an improved stabilized power output efficiency of 21.75% as well as enhanced long-term stability and operational stability. Our results provide a simple method to modify the conventional spiro-OMe TAD and obtain PSCs with both high efficiency and good stability.

The correlation between Epstein-Barr virus(EBV)infection and nasopharyngeal carcinoma(NPC)risk has been extensively *** continual monitoring of EBV-IgAs provides a promising approach of NPC screening in its early *** this study,we successfully synthesized a single-atom nanozyme(SANzyme)through the application of iron-porphyrin based metal organic framework(MOF-FeP).The MOF-FeP possesses precisely-defined electronic and geometric structures that accurately mimic highly-evolved catalytic site of natural *** peroxidase-like activity of MOF-FeP enables it to catalyze the chemiluminescence of luminol *** integrating MOF-FeP into a traditional strip,we created a rapid and highly-sensitive evaluation tool for detecting ***,the MOF-FeP strip enables the simultaneous detection of three EBV-IgAs,greatly improving the accuracy of EBV-associated NPC *** sensitivities of the MOF-FeP strip(75.56%–93.30%)surpass those of current enzyme-linked immunosorbent assay(ELISA)methods(64.44%–82.22%).This test takes only 16 min to perform as opposed to the customary 1–2 h required for standard ***,the MOF-FeP strip is suitable for whole blood samples,thereby significantly simplifying the sample preparation and detection *** conclusion,the MOF-FeP strip combines the simplicity of traditional strip with the high catalytic activity of *** innovative MOF-FeP strip offers a new point-of-care strategy for EBV-IgAs detection,which is expected to markedly facilitate early screening for EBV-associated diseases.
