BACKGROUND Pancreaticobiliary maljunction(PBM) is an uncommon congenital anomaly of the pancreatic and biliary ductal system, defined as a union of the pancreatic and biliary ducts located outside the duodenal wall. According to the Komi classification of PBM, the common bile duct(CBD) directly fuses with the ventral pancreatic duct in all types. Pancreas divisum(PD) occurs when the ventral and dorsal ducts of the embryonic pancreas fail to fuse during the second month of fetal development. The coexistence of PBM and PD is an infrequent ***, we report an unusual variant of PBM associated with PD in a pediatric patient, in whom an anomalous communication existed between the CBD and dorsal pancreatic *** SUMMARY A boy aged 4 years and 2 mo was hospitalized for abdominal pain with nausea and jaundice for 5 d. Abdominal ultrasound showed cholecystitis with cholestasis in the gallbladder, dilated middle-upper CBD, and a strong echo in the lower CBD, indicating biliary stones. The diagnosis was extrahepatic biliary obstruction caused by biliary stones, which is an indication for endoscopic retrograde cholangiopancreatography(ERCP). ERCP was performed to remove biliary stones. During the ERCP, we found a rare communication between the CBD and dorsal pancreatic duct. After clearing the CBD with a balloon, an 8.5 Fr 4-cm pigtail plastic pancreatic stent was placed in the biliary duct through the major papilla. Six months later, his biliary stent was removed after he had no symptoms and normal laboratory tests. In the following 4-year period, the child grew up normally with no more attacks of abdominal *** We consider that ERCP is effective and safe in pediatric patients with PBM combined with PD, and can be the initial therapy to manage such cases,especially when it is combined with aberrant communication between the CBD and dorsal pancreatic duct.

BACKGROUND Post-transplant dyslipidemia(PTDL)is a common complication in liver recipients and can cause morbidity and threaten graft *** crosstalk between metabolic inflammation and dyslipidemia has been recently ***,the role of grafts’and recipients’metabolic status in the development of PTDL has not been *** To investigate the association of recipients’metabolic inflammation status with PTDL and construct a predictive *** A total of 396 adult patients who received primary liver transplantation between 2015 and 2017 were *** and cytokines were analyzed using recipients’pre-transplant peripheral blood in a training set(n=72).An integrated prediction model was established according to the clinical risk factors and metabolic inflammation compounds and further verified in a validation set(n=144).RESULTS The serum lipid profile took 3 mo to reach homeostasis after liver transplantation.A total of 278(70.2%)liver recipients developed PTDL during a follow-up period of 1.78(1.00,2.97)*** PTDL group showed a significantly lower tumorfree survival and overall survival than the non-PTDL group in patients with hepatocellular carcinoma(n=169).The metabolomic analysis showed that metabolic features discriminating between the PTDL and non-PTDL groups were associated with lipid and glucose metabolism-associated *** metabolites and cytokines differentially expressed between the two groups,interleukin-12(p70)showed the best diagnostic accuracy and significantly increased the predictive value when it was incorporated into the clinical model in both training and validation *** Recipients’pre-transplant serum interleukin-12(p70)level is associated with the risk of PTDL and has potential clinical value for predicting PTDL.

Background: New-onset hyperglycemia(NOH) is a common phenomenon after liver transplantation(LT),but its impact on clinical outcomes has not yet been fully assessed. We aimed to evaluate the etiology and prognosis of NOH within 1 month after ***: The data of 3339 adult patients who underwent primary LT from donation after citizen death between January 2010 and June 2016 were extracted from China Liver Transplant Registry database and analyzed. NOH was defined as fasting blood glucose ≥7.0 mmol/L confirmed on at least two occasions within the first post-transplant month with or without hypoglycemic ***: Of 3339 liver recipients, 1416(42.4%) developed NOH. Recipients with NOH had higher incidence of post-transplant complications such as graft and kidney failure, infection, biliary stricture, cholangitis,and tumor recurrence in a glucose concentration-dependent manner as compared to non-NOH recipients(P 10 min, cold ischemic time >10 h, anhepatic time >60 min, recipient model for end-stage liver disease score >30, moderate ascites and corticosteroid usage(P < 0.05). Liver enzymes(alanine aminotransferase and gammaglutamyltranspeptidase) on post-transplant day 7 significantly correlated with NOH(P < 0.001).Conclusions: NOH leads to increased morbidity and mortality in liver recipients. Close surveillance and tight control of blood glucose are desiderated immediately following LT particularly in those with delayed graft function and receiving corticosteroid. Strategic targeting graft ischemic injury may help maintain glucose homeostasis.

OBJECTIVE To discuss the effects of clinico-pathological features on lymph node metastasis （LNM） in undifferentiated EGC （early gastric cancer）, as well as identify the appropriate medical management. METHODS From January 1999 to June 2011, 352 patients were treated for undifferentiated EGC in our hospital. All patients had undergone gastrectomy with regional lymphadenectomy. We used univariate and multivariate associated with lymph node entiated EGC. analyses to determine the features metastasis in patients with undiffer- RESULTS Signet ring cell carcinoma （SRC） was more common in patients with undifferentiated EGC than other undifferentiated carcinoma （UDC）. SRC had a tendency to be confined to the mucosa, with a smaller size than other UDC. The incidence of LNM for SRC was lower than that for other UDC. Multivariate analysis showed that LNM was associated with the sex, tumor size, depth of invasion, lymphovascular invasion, and histological type. CONCLUSION Complete endoscopic resection is suitable for SRC- type intramucosal EGC, which is less than 2 cm in diameter without lymphovascular invasion in the postoperative histological examination.

Rheumatic heart disease is still highly prevalent in .developing countries where the incidence of coronaryartery disease （CAD） is also arising. Consequently the occurrence of coexistent CAD and mitral stenosis （MS） dominantly caused by rheumatic fever is more frequent in these countries. Comorbidities such as pulmonary and renal dysfunction occur frequently in these aging populations. Off-pump surgery is widely applied in patients with concomitant CAD and severe comorbidities due to lower incidence of postoperative morbidity. Closed mitral commissurotomy （CMC） is a well established treatment for patients with mitral stenosis. We report here a case of moderate stenosis combined with seriously diffuse 3-vessel CAD and severe chronic obstructive pulmonary disease （COPD） in an elder male treated with off-pump coronary artery bypass （OPCAB） plus CMC.
