First endoscopic procedure for diagnosis and staging of mediastinal lymphadenopathy

AIM:To compare a first diagnostic procedure of transbronchial needle aspiration(TBNA)with selection of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA)or TBNA for mediastinal ***:Sixty-eight consecutive patients with mediastinal lymphadenopathy on computed tomography(CT),who required cytopathological diagnosis,were *** first 34 underwent a sequential approach in which TBNA was performed first,followed by EUS-FNA if TBNA was *** next 34 underwent a selective approach where either TBNA or EUS-FNA was selected as the first procedure based on the CT ***:The diagnostic yield of TBNA as the first diagnostic procedure in the sequential approach was 62%.In the selective approach,the diagnostic yield of the first procedure was 71%.There was no significant difference in the overall diagnostic yield,but there were significantly fewer combined procedures with the selective ***:Selecting either EUS-FNA or TBNA as the first diagnostic procedure achieved a comparable diagnostic yield with significantly fewer procedures than performing TBNA first in all patients.

High prevalence of osteoporosis in patients with gastric adenocarcinoma following gastrectomy

AIM： To evaluate the prevalence and predictive factors of osteoporosis in patients with gastric adenocarcinoma alter gastrectomy.METhoDS： The study included 133 patients diagnosed with gastric adenocarcinoma but who did not undergo prior diagnostic work-up for osteoporosis. Bone mineral density （BMD） was measured by dual-energy X-ray absorptiometry （DXA） and vertebral deformity was assessed by plain X-rays. We evaluated the effects of age, sex, body mass index （BMI）, anemia, back pain, vertebral deformity, tumor staging, reconstruction type, and past medical history to determine predictive factors of osteoporosis in these patients.RESULTS： The prevalence of osteoporosis in the lumbar spine was 38.3% （male, 28.9%; female, 54.0%）, and 15.0% in the femoral neck （male, 10.8%; female, 22.0%）. The vertebral deformity rate was 46.6% （male, 43.4%; female, 52.0%）. Age, BMI and hemoglobin correlated with BMD （P 〈 0.01）. In males, anemia and age 〉 64 years were independent predictive factors of osteoporosis in multivariate analysis. In females, back pain was an independent factor for osteoporosis.CONCLUSION： The results of this study revealed that prevalence of osteoporosis and vertebral bone deformity rate were high in gastric cancer patients, regardless of post-gastrectomy duration and operation type. Early diagnosis and a proper management plan must be established in these patients.

Alginate is a biopolymer that has exceptional gelling properties, which allow easy gel formation under safe and mild conditions. Consequently, it is often used to encapsulate a variety of cargos, such as cells, enzymes, and lipids, and is typically employed as a model to study hydrogel-based encapsulation sys- tems. Since the first use of alginate in the encapsulation field in the 1970s, many methods have been developed to produce alginate hydrogel particles of different sizes, structures, and morphologies. This review provides an overview of the current progress in the fabrication of alginate hydrogels with vari- ous particle designs, including a discussion of dispersion techniques to pre-template alginate particles, gelation mechanisms, considerations in selecting suitable fabrication methods, and future directions.

Effects of Diisopropylfluorophosphate on Brain Acetylcholinesterase, Butyryicholinesterase, and Neurotoxic Esterase in Rats

The inhibition and the recovery of brain AChE, BuChE, and NTE activities after acute and subacute administration of DFP were studied in the rat. DFP displayed different specificities in inhibiting these enzymes; inhibition was greatest for BuChE followed by AChE and NTE. Recovery was most rapid for BuChE followed by NTE and AChE. The recovery rates of AChE and BuChE following acute and subacute treatment were similar. however, the recovery rate of NTE in subacutely treated rats was significantly faster than that in acutely treated rats. The results suggest that DFP inhibits these three enzymes and the rates of regeneration of these enzymes are significantly different. (c)1989 Academic Press, Inc.

Does the discrepancy in histologic differentiation between a forceps biopsy and an endoscopic specimen necessitate additional surgery in early gastric cancer?

To investigate the clinicopathological variables in early gastric cancer (EGC) patients in relation to differentiation discrepancy. METhoDSThe data of 265 specimens from 240 patients with EGC, who had undergone radical operation at Hallym University Sacred Heart hospital from 2010 to 2015, were retrospectively analyzed. We evaluated clinical, endoscopic, and histopathological data according to histological discrepancy. RESULTSClinically significant discrepancy rate showed the difference in differentiated type (well and moderately differentiated) and undifferentiated type (poorly differentiated and signet ring cell) between endoscopic biopsies and postoperative specimens was 9.4% (25/265). There were no differences in tumor location, size, gross pattern, and number of biopsies. Specimens having histological discrepancy showed more submucosal invasion (72.0% vs 49.6%, P = 0.033) and lymph node involvement (24.0% vs 7.9%, P = 0.009) than specimens having non-discrepancy. The rate of a positive epidermal growth factor receptor status was higher in specimens having discrepancy than in specimens having non-discrepancy (81.0% vs 55.4%, P = 0.035). CONCLUSIONThe discordance of histologic differentiation is associated with higher submucosal invasion and lymph node metastases in EGC. Patients have histological discrepancy may require additional surgical treatments.

Synthesis of Al-doped ZnO Nanorods via Microemulsion Method and Their Application as a CO Gas Sensor

Aluminum doped ZnO(AZO) nanorods were synthesized by microemulsion method with different types of *** phase and the morphology of the above nanorods were investigated by scanning electron microscopy(SEM) and X-ray diffraction(XRD).SEM observations showed that the ZnO nanorods had diameters around about 50—200 nm and lengths up to several *** CO gas sensing properties of the AZO nanorods were tested at operating temperatures of 200,300,350 and 400 ℃.It was found that AZO nanorods based sensor exhibited the highest sensitivity to CO at 350℃.

Loose-fit graphitic encapsulation of silicon nanowire for one-dimensional Si anode design

Silicon nanowires（SiNWs） encapsulated with graphene-like carbon sheath（GS） having a void space in between（SiNW@V@GS） are demonstrated for the improved electrochemical performance of Si anode in lithium ion battery. The Si NW@V@GS structure was synthesized by a scalable fabrication method including four successive reactions： metal-catalyzed CVD growth of Si NWs, controlled thermal oxidation, and deposition of the graphitic layer, to form Si NW@SiO2@GS and additional chemical etching of sacrificial SiO2 layer between Si NWs and carbon sheath. During the synthetic process, the thickness of the void spacing was controlled by adjusting the oxidation-dependent process. The well-controlled void space and crystalline graphitic carbon sheath of the SiNW@V@GS structure enable good reversible capacity of1444 m Ahg^（-1） and cycling stability of 85% over 150 cycles.

A comparison of short-term postoperative outcomes including nutritional status between gastrectomy with simultaneous cholecystectomy and gastrectomy only in patients with gastric cancer

Objective: We aimed to evaluate the effect of simultaneous cholecystectomy on the short-term postoperative outcomes and nutritional status in patients with gastric ***: We retrospectively reviewed data from 4,820 patients with gastric cancer who underwent gastrectomy from January 2011 to December 2016. Patients who underwent only gastrectomy(N=4,578) were matched to those who underwent simultaneous cholecystectomy during gastrectomy(N=242) at a 1:1 ratio using propensity score matching analysis. The nutritional status and inflammatory responses preoperatively and postoperatively and postoperative outcomes were compared between the ***: The simultaneous cholecystectomy group showed more intraoperative blood loss and a longer operative time than the gastrectomy only group [150.0(100.0, 200.0) m L vs. 100.0(100.0, 200.0) m L, P=0.006;176.0(150.0,210.0) min vs. 155.0(128.0, 188.0) min, Phocyte count, total protein level, albumin level, fasting glucose level,and prognostic nutritional index excluding the cholesterol level were not significantly different between the groups,and their changing patterns were similar. Although the cholesterol level was significantly lower in the simultaneous cholecystectomy group than in the gastrectomy only group at all follow-up points, the mean value of the decreased cholesterol level was within normal ***: In gastric cancer patients with gallbladder disease, simultaneous cholecystectomy is safe and not associated with additional nutritional loss.
