Factors affecting failed trial of labor and countermeasures:A retrospective analysis

BACKGROUND Vaginal delivery is the ideal mode of delivery for the termination of a ***,the cesarean section rate in China is much higher than the published by the World Health Organization in the Lancet in *** To retrospectively analyze the factors related to failed trial of labor and the clinical indications for cesarean section conversion,explore how to promote the trial of labor success rate,and determine the feasibility of reducing the rate of conversion to cesarean *** A retrospective analysis was performed on 9240 maternal women who met vaginal delivery conditions and required a trial of labor from January 2016 to December 2018 at our *** them,8164 pregnant women who had a successful trial of labor were used as a control group,and 1076 pregnant women who had a failed trial of labor and converted to an emergency cesarean section were used as an observation *** patients’clinical data during hospitalization were collected for comparative analysis,the related factors of the failed trial of labor were discussed,and reasonable prevention and resolution strategies were proposed to increase the success rate of trial of *** The analysis revealed that advanced age(≥35 years old),macrosomia(≥4000 g),delayed pregnancy(≥41 wk),use of uterine contraction drugs,primipara,and fever during labor were associated with conversion to an emergency cesarean section in the failed trial of *** regression analysis showed that age,gestational age,primipara,use of uterine contraction drugs,fever duringbirth,and newborn weight led to a higher probability of conversion to an emergency cesarean section in the failed trial of *** analysis indicated that the following clinical indications were associated with the conversion to cesarean section in the failed trial of labor:Fetal distress(44.3%),social factors(12.8%),malpresentation(face presentation,persistent occipitoposterior position,and persistent occipitotransv

Precipitation Behavior of V-N Microalloyed Steels during Normalizing

The precipitation behavior of V-N microalloyed steel during normalizing process was studied by physicochemical phase analysis and transmission electron microscopy（TEM）. The effect of precipitation behavior on mechanical properties was investigated by theoretical calculations. The results showed that 32.9% of V（C,N） precipitates remained undissolved in the austenite during the soaking step of the normalizing process. These precipitates prevented the growth of the austenite grains. During the subsequent cooling process, the dissolved V（C,N） re-precipitated and played a role in precipitation strengthening. The undissolved V（C,N） induced intragranular ferrite nucleation and refined the ferrite grains. Consequently, compared with hot-rolled steel, the normalized steel exhibited increased grain-refining strengthening but diminished precipitation strengthening, leading to an improvement of the impact energy at the expense of about 40 MPa yield strength.

Effect of intravariceal sclerotherapy combined with esophageal mucosal sclerotherapy using small-volume sclerosant for cirrhotic patients with high variceal pressure

AIM:To explore the effect of intravariceal-mucosal sclerotherapy using small dose of sclerosant on the recurrence of esophageal ***:We randomly assigned 38 cirrhotic patients with previous variceal bleeding and high variceal pressure(> 15.2 mm Hg) to receive endoscopic variceal ligation(EVL) and combined intravariceal and esophageal mucosal sclerotherapy(combined group) using small-volume *** end-points of the study were rebleeding and recurrence of esophageal ***:During a median follow-up period of 16 mo,varices recurred in 1 patient in the combined group as compared with 7 patients in the EVL group(P =0.045).Rebleeding occurred in 3 patients in the EVL group as compared with 1 patient in the combined group(P=0.687).No patient died in the two *** significant differences were observed between the two groups with respect to serious adverse ***:Intravariceal-mucosal sclerotherapy using small dose of sclerosant is more effective than EVL in decreasing the incidence of variceal recurrence for cirrhotic patients.

In a single-pixel fast imaging setup,the data collected by the single-pixel detector needs to be processed by a computer,but the speed of the latter will affect the image reconstruction *** we propose two kinds of setups which are able to transform non-visible into visible light imaging,wherein their computing process is replaced by a camera integration *** image captured by the camera has a low contrast,so here we present an algorithm that can realize a high quality image in near-infrared to visible cross-waveband *** scheme is verified both by simulation and in actual *** setups demonstrate the great potential for single-pixel imaging and high-speed cross-waveband imaging for future practical applications.

Comparison on conjunctival sac bacterial flora of the seniors with dry eye in Ganzi autonomous prefecture

AIM:To compare the bacterial flora in palpebral conjunctiva of xerophthalmia seniors of Tibetan,Yi and Han,and analyze the differences and similarities of the bacteria.·METHODS:The test subjects were selected from 2Tibetan,2 Yi and 3 Han populated places,*** 222 seniors(444 eyes) with dry eye were *** was collected from the palpebral conjunctiva of the subjects and then inoculated onto a blood agar *** 48h of incubation,the bacteria were examined for the differences and similarities between different ethnics.·RESULTS:There was no significant difference(P >0.05)of Gram stain characterization,dominant bacteria and number of the bacterial species present in oxrophthalmia patients among Tibetan,Yi and Han *** bacteria presented in all groups include staphylococcus epidermidis,corynebacterium,micrococcus luteu,int-racellular bacteria sphingomonas,pseudomonas *** bacteria detected from the two of three ethnic groups were staphylococcus aureus,staphylococcus haemolyticus,escherichia coli,kytococcus sedentarius,streptococcus angina,micrococcus lylae,and staphy-lococcus *** incidence rate of bacteria-associated dry eye in Tibetan population was significantly lower than that of Han and Yi population.·CONCLUSION:There is no significant difference in the bacteria flora of palpebral conjunctiva observed among dry eye elder populations of Tibetan,Yi and Han *** of staphylococcus epidermidis,corynebacterium,micrococcus luteu,intracellular bacteria sphingomonas,pseudomonas aeruginosa,staphylococcus aureus,staphylococcus haemolyticus,escherichia coli,kytococcus sedentarius,streptococcus angina,micrococcus lylae and staphylococcus heads are common bacteria flora of the three nationalities inhibiting in this area.

The fatigue resistance of casting polyurethane(CPU)is crucial in various sectors,such as construction,healthcare,and the automotive *** its importance,no studies have reported on the fatigue threshold of *** study employed an advanced Intrinsic Strength Analyzer(ISA)to evaluate the fatigue threshold of CPUs,systematically exploring the effects of three types of isocyanates(PPDI,NDI,TDI)that contribute to hard segment structures based on the cutting *** multiple advanced characterization techniques(XRD,TEM,DSC,AFM),the results indicate that PPDI-based polyurethane exhibits the highest fatigue threshold(182.89 J/m^(2))due to a highest phase separation and a densely packed spherulitic structure,although the hydrogen bonding degree is the lowest(48.3%).Conversely,NDI-based polyurethane,despite having the high hydrogen bonding degree(53.6%),exhibits moderate fatigue performance(122.52 J/m^(2)),likely due to a more scattered ***-based polyurethane,with the highest hydrogen bonding degree(59.1%)but absence of spherulitic structure,shows the lowest fatigue threshold(46.43 J/m^(2)).Compared to common rubbers(NR,NBR,EPDM,BR),the superior fatigue performance of CPU is attributed to its well-organized microstructure,polyurethane possesses a higher fatigue threshold due to its high phase separation degree and orderly and dense spherulitic structure which enhances energy dissipation and reduces crack propagation.

β-2 Adrenergic receptor gene polymorphism and response to propranolol in cirrhosis

AIM: To evaluate the association of β-2 adrenergic receptor(β2-AR) gene polymorphism with response of variceal pressure to propranolol in ***: Sixty-four non-related cirrhotic patients participated in this study and accepted variceal pressure measurement before and after propranolol administration. Polymorphism of the β 2-AR gene was determined by directly sequencing of the polymerase chain reaction products from the DNA samples that were prepared from the ***: The prevalence of Gly16-Glu/Gln27 and Arg16-Gln27 homozygotes, and compound heterozygotes was 29.7%, 10.9%, and 59.4%, *** with cirrhosis with Gly16-Glu/Gln27 homozygotes had a greater decrease of variceal pressure after propranolol administration than those with Arg16-Gln27 homozygotes or with compound heterozygotes(22.4% ± 2.1%, 13.1% ± 2.7% and 12.5% ± 3.1%,respectively, P < 0.01).CONCLUSION: The variceal pressure response to propranolol was associated with polymorphism of β 2-AR gene. Patients with the Gly16-Glu/Gln27 homozygotes probably benefit from propranolol therapy.

Background:To investigate the potential advantage of using low-dose clomiphene citrate to promote ovulation in women with overweight or ***:The patients with overweight were divided randomly into regular clomiphene citrate group(clomiphene citrate 50 mg/d,n=83)and low-dose clomiphene citrate group(clomiphene citrate 25 mg/d,n=105).All patients received treatment from the 5th day of their menstrual ***:Luteinizing hormone levels and estradiol levels were more increased in the low-dose group compared with the regular group(P0.05).Conclusion:The low-dose clomiphene citrate treatment significantly reduced the number of mature follicles and increased the thickness of endometrium on the human chorionic gonadotropin day with less side *** low-dose clomiphene citrate also achieved the better ovulation rate.
