Navigated laser in diabetic macular edema: the impact of reduced injection burden on patients and physicians-who wins and who loses?

We inquired the impact of reduced therapy discontinuation in diabetic macular edema(DME) on physician's revenue considering anti-vascular endothelial growth factor(VEGF) monotherapy and its combination with Navilas treatment. Data were collected on injection frequency, treatment discontinuation and reimbursement fees for DME treatment with anti-VEGF compared to anti-VEGF in combination with navigated laser. Based on these data an economic model was built to compare physicians revenue over a 5y period using either therapy for 4 European countries and the USA. Due to patients' higher therapy adherence, physicians using navigated laser therapy with anti-VEGF generate similar or higher revenues compared to VEGF monotherapy in all analyzed countries. The use of Navilas decreases the patient's injection burden at the same clinical outcome, while the physician's revenue remained stable or increased. Therewith, therapy discontinuation in DME can be reduced using the combination therapy with Navilas.

Background Diffusion-weighted imaging(DWI)of synovitis has been suggested as a possible non-invasive alternative to contrast-enhanced T1w imaging(ce-T1w).We aimed to study DWI for diagnosing synovitis in the knee joint of pediatric patients,to quantify inter-observer agreement on DWI and ce-T1w and to calculate quantitative measures of synovial diffusivity and *** Forty consecutive patients with known or suspected arthritis of the knee(25 girls,median age 12 years)underwent routine 1.5T MRI with ce-T1w and transverse DWI with b values 50 and 800 s/*** apparent diffusion coefficient(ADC)values and signal intensity of inflamed synovium,joint effusion and muscle were measured with regions of interest ***-contrast T1 w images(diagnostic standard)and diffusion-weighted images at b=800 s/mm2 with ADC map were separately rated by three independent and blinded readers with different levels of expertise for the presence and degree of synovitis along with the level of diagnostic *** Thirty-one(78%)patients showed at least some synovial contrast enhancement,17(43%)children were diagnosed with synovitis on *** by the 1st reader on ce-T1w and on DWI for synovitis showed very good agreement(kappa=0.90).Inter-observer agreement on DWI ranged from moderate to substantial with kappa values between 0.68 and 0.79(all P<0.001).Agreement and diagnostic confidence were generally lower in patients with mild and without synovial enhancement,compared to patients with *** yielded higher signal of inflamed synovium *** tissue,but lower signal *** effusion,compared to ce-T1 w(all P<0.001).Conclusions Diffusion-weighted imaging is a promising,though reader-dependent alternative to contrast-enhanced imaging in patients with arthritis of the knee,based on our preliminary *** holds potential for increasing patient safety and comfort.

Optical inline analysis and monitoring of particle size and shape distributions for multiple applications: Scientific and industrial relevance

Particles occur in almost all processes in chemical and life sciences. The particle size and shape influence the process performance and product quality, and in turn they are influenced by the flow behavior of the particles during production. Monitoring and controlling such characteristics in multiphase systems to obtain sufficient qualities will greatly facilitate the achievement of reproducible and defined distributions. So far, obtaining this information inline has been challenging, because existing instruments lack measurement precision, being unable to process overlapping signals from different particle phases in highly concentrated multiphase systems. However, recent advances in photo-optics made it possible to monitor such features(particle size distribution(PSD), aspect ratio and particle concentration) with advanced image analysis(IA) in real-time. New analysis workflows as well as single feature extractions from the images using multiple image analysis algorithms allowed the precise real-time measurements of size, shape and concentration of particle collectives even separated from each other in three phase systems. The performances, advantages and drawbacks with other non-photo-optical methods for assessing the particle size distribution are compared and discussed.

Short-Term and Long-Term Effects of Natural and Artificial Carbonaceous Substrates on Greenhouse Gas Fluxes

The calcite mylonites in the Xar Moron-Changchun shear zone show a significance dextral shearing characteristics. The asymmetric(σ-structure) calcite/quartz grains or aggregates, asymmetry of calcite c-axes fabric diagrams and the oblique foliation of recrystallized calcite grains correspond to a top-to-E shearing. Mineral deformation behaviors, twin morphology, C-axis EBSD fabrics, and quartz grain size-frequency diagrams demonstrate that the ductile shear zone was developed under conditions of greenschist facies, with the range of deformation temperatures from 200 to 300°C. These subgrains of host grains and surrounding recrystallized grains, strong undulose extinction, and slightly curved grain boundaries are probably results of intracrystalline deformation and dynamic recrystallization implying that the deformation took place within the dislocation-creep regime at shallow crustal levels. The calculated paleo-strain rates are between 10-7.87s-1 and 10-11.49s-1 with differential stresses of 32.63-63.94 MPa lying at the higher bound of typical strain rates in shear zones at crustal levels, and may indicate a relatively rapid deformation. The S-L-calcite tectonites have undergone a component of uplift which led to subhorizontal lifting in an already non-coaxial compressional deformation regime with a bulk pure shear-dominated general shear. This E-W large-scale dextral strike-slip movement is a consequence of the eastward extrusion of the Xing’an-Mongolian Orogenic Belt, and results from far-field forces associated with Late Triassic convergence domains after the final closure of the Paleo-Asian Ocean.

Systemic interleukin-9 in inflammatory bowel disease: Association with mucosal healing in ulcerative colitis

To evaluate circulating IL9 in inflammatory bowel disease and disease-associated anemia/cachexia and assess its potential as a mucosal healing *** IL9 as well as other cytokines (IL1β, IL6, IL13, IFNγ, TNFα, and VEGF-A) were determined in 293 individuals: 97 patients with Crohn’s disease (CD) and 74 with ulcerative colitis (UC) and in 122 apparently healthy controls. The clinical activity of CD and UC was expressed in terms of the Crohn’s Disease Activity Index (CDAI) and the Mayo Scoring System (MDAI), respectively, and the severity of bowel inflammation in UC patients was assessed using Mayo endoscopic score. Cytokine concentrations were measured by a flow cytometry-based method using Luminex xMAP® technology. High-sensitive C-reactive protein concentrations (hsCRP) were determined in CD and UC patients using the enhanced immunoturbidimetric *** IL9 was significantly lower in healthy individuals [9 pg/mL (95%CI: 8.2-10)] than in patients with inflammatory bowel disease (IBD): both inactive [14.3 pg/mL (11.9-19.9)] and active [27.6 pg/mL (24.5-32), P < 0.0001]. Cytokine concentrations were significantly higher in active CD [27.4 pg/mL (23.4-32.2)] and in active UC [32.7 pg/mL (27-38.9)] compared to inactive diseases [15.9 pg/mL (10.8-23.4) in CD and 19.4 pg/mL (13.9-27.1) in UC, P = 0.001]. IL9 correlated weakly with CDAI (ρ = 0.32, P = 0.003) and MDAI (ρ = 0.35, P = 0.002) and strongly with endoscopic inflammation in UC (ρ = 0.74, P < 0.0001). As a negative marker of mucosal healing (MH), IL9 had an accuracy superior to hsCRP and IL6 [97% (P < 0.0001), 67% (P = 0.071), and 55% (P = 0.525), respectively]. IL9 was significantly higher in cachectic IBD patients [30.25 pg/mL (24.4-37.5) vs 21.88 pg/mL (18-26.5), P = 0.026] and negatively correlated with hemoglobin concentrations (ρ = -0.27, P < 0.001). Impact of OCT scan-patterns in identifying morphological features of lamellar macular holes and macular pseudoholes

AIM:To evaluate the impact of the optical coherence tomography(OCT)scan patterns on the detection of the features associated with lamellar macular hole(LMH)and macular pseudohole(MPH).METHODS:This is a retrospective analysis of 100 consecutive eyes with LMH(n=41)and MPH(n=59)having at least three of the following OCT features,which include mandatory criteria for the diagnosis of LMH and MPH:Epiretinal membrane,epiretinal proliferation,verticalization,intraretinal cystoid spaces,foveoschisis,irregular foveal contour,foveal cavity with undermined edges,and ellipsoid line *** outcome measurement was the detection frequency of the features in three different OCT scan patterns:1)volume scan;2)six radial scans(R6);and 3)vertical and horizontal radial scans(R2).RESULTS:Of the total eight features,the maximal detection frequency was found as 4.45±1.45,4.35±1.47,and 3.70±1.59,by the volume,R6 and R2,respectively.R2 was inferior to the other patterns in detection of the total features(P<0.001),whereas R6 and volume patterns were found comparable(P=0.312).CONCLUSION:The physician should be aware that the selection of the OCT-scan pattern may influence the detection of mandatory morphological criteria for the diagnosis of LMH and MPH.
