Frailty and Liver resection: where do we stand?

As the world population is continuously aging, the number of older patients requiring liver surgery is also on the rise. Data have shown that age should not be a limiting factor for liver resection, as it cannot accurately predict postoperative outcomes. Instead, frailty can serve as a more reliable measure of the patient's overall health and functional reserves. Several frailty assessment tools have been implemented for preoperative risk stratification before liver surgery, and higher scores have commonly been associated with postoperative morbidity, mortality, and length of hospital stay. However, no consensus has been reached on the most useful screening tool. Future studies should focus on comparing the currently available assessment tools, constructing a liver resection-specific tool, and assessing the role of frailty assessment tools in preoperative patient optimization.

Obstetrical and gynecologic challenges in the liver transplant patient

An increasing number of childbearing agewomen undergo liver transplantation(LT)in the United *** in this patient subgroup poses a significant challenge regarding the plans for future fertility,particularly in terms of immunosuppression and optimal timing of *** LT is only rarely performed as the outcome is commonly dismal for the mother or more commonly the *** the other hand,the outcomes of pregnancy in LT recipients are favorable,and children born to LT recipients are relatively *** on pregnancy should start before LT and continue after LT up until pregnancy,while all pregnant LT recipients must be managed by amultidisciplinary team,including both an obstetrician and a transplant ***,an interval of at least 1-2 years after successful LT is recommended before considering ***-induced hypertension,pre-eclampsia,and gestational diabetes mellitus are reported more commonly during the pregnancies of LT recipients than in the pregnancies of non-transplant *** adverse fetal outcomes,such asmiscarriage,abortion,stillbirth,or ectopic pregnancy,may occur more often than in the non-transplant population,early planning or delivery either through a planned induction of labor or cesarean section is critical to minimize the risk of *** significant long-term physical or phycological abnormalities have been reported in children born to LT recipients.

Global dissemination of minimally invasive living donor hepatectomy: What are the barriers?

Minimally invasive donor hepatectomy(MIDH)is a relatively novel procedure that can potentially increase donor safety and contribute to faster rehabilitation of *** an initial period in which donor safety was not effectively validated,MIDH currently seems to provide improved results,provided that it is conducted by experienced *** selection criteria are crucial to achieve better outcomes in terms of complications,blood loss,operative time,and hospital *** a pure laparoscopic technique,various approaches have been recommended such as hand-assisted,laparoscopic-assisted,and robotic *** latter has shown equal outcomes compared to open and laparoscopic approaches.A steep learning curve seems to exist in MIDH,mainly due to the fragility of the liver parenchyma and the experience needed for adequate control of *** review investigated the challenges and the opportunities of MIDH and the barriers to its global *** need expertise in liver transplantation,hepatobiliary surgery,and minimally invasive techniques to perform *** can be categorized into surgeon-related,institutionalrelated,and *** robust data and the creation of international registries are needed for further evaluation of the technique and the acceptance from more centers worldwide.

Children infected by severe acute respiratory syndrome coronavirus 2(SARSCoV-2)seem to have a better prognosis than ***,pediatric solid organ transplantation(SOT)has been significantly affected by the unprecedented coronavirus disease 2019(COVID-19)pandemic during the pre-,peri-,and posttransplant ***,immunosuppression constitutes a real challenge for transplant clinicians as increased immunosuppression may prolong disease recovery,while its decrease can contribute to more severe *** date,most pediatric SOT recipients infected by SARS-CoV-2 experience mild disease with only scarce reports of life-threatening *** a consequence,after an initial drop during the early phase of the pandemic,pediatric SOTs are now performed with the same frequency as during the pre-pandemic *** review summarizes the currently available evidence regarding pediatric SOT during the COVID-19 pandemic.

Liver transplantation is the only potentially curative option for unresectable *** introduction of platinum-based chemotherapy drastically improved the survival outcomes of patients with ***,the use of neoadjuvant chemotherapy and the optimal number of cycles required in patients listed for liver transplantation,as well as the potential use of adjuvant chemotherapy,remain ***,the shortage of donor liver grafts,along with the lack of clear consensus on the management of metastatic hepatoblastoma,makes the decision on whether to proceed to liver transplantation even more complex and *** advances may optimize intraoperative imaging of both the primary tumor and metastatic sites,thus facilitating complete *** improvements,along with the wider use of social media platforms to increase public awareness,could potentially pave the way for more optimal implementation of liver transplantation for the treatment of patients with unresectable hepatoblastoma.

Role of innovative 3D printing models in the management of hepatobiliary malignancies

Three-dimensional (3D) printing has recently emerged as a new technique in various liver-related surgical fields. There are currently only a few systematic reviews that summarize the evidence of its impact. In order to construct a systematic literature review of the applications and effects of 3D printing in liver surgery, we searched the PubMed, Embase and ScienceDirect databases for relevant titles, according to the PRISMA statement guidelines. We retrieved 162 titles, of which 32 met the inclusion criteria and are reported. The leading application of 3D printing in liver surgery is for preoperative planning. 3D printing techniques seem to be beneficial for preoperative planning and educational tools, despite their cost and time requirements, but this conclusion must be confirmed by additional randomized controlled trials.

Management of biliary atresia:To transplant or not to transplant

Kasai procedure(KP)and liver transplantation(LT)represent the only therapeutic options for patients with biliary atresia(BA),the most common indication for LT in the pediatric ***,KP represents by no means a radical option but rather a bridging one,as nearly all patients will finally require a liver *** and more experts in the field of transplant surgery propose that maybe it is time for a paradigm change in BA treatment and abandon KP as transplantation seems *** of organs yet makes this option currently not feasible,so it seems useful to find ways to maximize the efficacy of *** previous decades,multiple studies tried to identify these factors which opt for better results,but in general,outcomes of KP have not improved to the level that was *** review provides the framework of conditions which favor native liver survival after KP and the ones which optimize a positive LT *** of transition of care at the right time are also presented,as transplantation plays a key role in the surgical treatment of *** studies and further organization in the transplant field will allow for greater organ availability and better outcomes to be achieved for BA patients.

Detection of human papillomavirus DNA in esophageal carcinoma in Greece

AIM:To detect human papillomavirus(HPV)in theesophageal mucosa and the possible relationship with esophageal cancer in ***:Forty-nine patients underwent esophagogastroduodenoscopy(EGD)and esophageal biopsy at a university hospital that acts as a referral center for Northern *** of these patients(14 male and 5 female)had esophageal squamous cell carcinoma(ESCC)and 30(15 male and 15 female)did not have any reported esophageal *** assessment was followed by polymerase chain reaction analysis of all the *** demographic data(age,sex,and place of birth)and information regarding smoking habits,alcohol consumption or sexual habits were collected.A method of statistical interference,verification of hypotheses based on homogeneity and independentχ2 test,was ***:From the 49 patients that underwent EGD and biopsy,19 had ESCC and 30 had normal esophageal mucosa,with a mean age of 65.2 *** the prevalence of oncogenic risk factors for esophageal carcinoma,an interesting conclusion was that 78%of the patients used tobacco and almost one-third had multiple sexual partners,whereas only 20%of the patients consumed alcohol,which was not statistically significant,when compared to the control *** the ESCC group,the only two positive samples were among the male patients(2/14 male patients with ESCC,14.5%).No HPV was identified in the control *** predominant HPV types identified were 11 and 31,which have a low malignancy *** presence of HPV DNA in the ESCC group was not statistically significant,95%confidence interval(χ2=3.292,P=0.07).CONCLUSION:This is the first relevant study in Greece,and despite the lack of statistical significance,the issue of HPV infection and ESCC does merit further investigation.
