A randomized, prospective, two-center comparison of sirolimus-eluting stent and zotarolimus-eluting stent in acute ST-elevation myocardial infarction： The SEZE trial

Background The zotarolimus-eluting stent has shown larger in-stent late lumen loss compared to sirolimus-eluting stents in previous studies. However, this has not been thoroughly evaluated in ST elevation myocardial infarction. Methods This was a prospective, randomized, controlled trial evaluating angiographic outcomes in patients presenting with ST elevation myocardial infarction, treated with zotarolimus-eluting stents or sirolimus-eluting stents. From March 2007 to February 2009, 122 patients were randomized to zotarolimus-eluting stents or sirolimus-eluting stents in a 1：1 fashion. The primary endpoint was 9-month in-stent late lumen loss confirmed by coronary angiography, and secondary endpoints were percent diameter stenosis, binary restenosis rate, major adverse cardiac events （a composite of cardiac death, non-fatal myocardial infarction, and target vessel revascularization）, and late-acquired incomplete stent apposition. Results Angiographic in-stent late lumen loss was significantly higher in the zotarolimus-eluting stent group compared to the sirolimus-eluting stent group （（0.49±0.65） mm vs. （0.10±0.46） mm, P=0.001）. Percent diameter stenosis at 9-month follow-up was also larger in the zotarolimus-eluting stent group （（30.0±17.9）% vs. （17.6±14.0）%, P 〈0.001）. In-segment analysis showed similar findings. There were no significant differences in binary restenosis rate, major adverse cardiac events, and late-acquired incomplete stent apposition. Conclusions Compared to sirolimus-eluting stents, the zotarolimus-eluting stent is associated with significantly higher in-stent late lumen loss at 9-month angiographic follow-up in the treatment of ST elevation myocardial infarction. Although there was no significant difference in 1-year clinical outcomes, the clinical implication of increased late lumen loss should be further studied.

Assessment of Geotechnical Properties of Lateritic Gravels from South-Cameroon Road Network

The southern road network covers the eastern, central and southern administrative regions;its linear network of national and provincial roads is about 5 620 km. It was the subject of a survey for the identification of lateritic quarries. To this effect, 144 sites have been discovered. The collected lateritic gravels were subjected to identification analyzes (Granulometry, Atterberg Limits, densimetry, water content) and geotechnical characterization tests (Proctor Modified, CBR at 95% OPM). The obtained results show that the grain size has a great percentage of gravels (>60%) followed by sand. Stones silt and clay particles are poorly represented. The samples show a high plasticity in conformity with Casagrande’s plasticity diagram. The density values of the solid particles (γ s) are quite stable in the studied area and vary between 26 and 29 KN/m3 for a natural water content comprised between 10% and 20%. The modified proctor test gives an optimal water value (Wop %) comprised between 8% and 12%. Dry density values (γ d) varied between 18 and 30 KN/m3. The bearing capacity of the materials is good (30 < CBR < 90). These results permit the recommendation of the studied materials as foundation layers of roads for traffic classes between T1-T4.

Determinants of the Frequency of Prenatal Consultations among Women Giving Birth in the Maternity Wards of the Commune of Agbangnizoun (zou), Benin

Antenatal care is a safe way to prevent pregnancy-related health problems, difficult deliveries and even further to anticipate low birth weight [1]. But pregnant women, especially in Africa, do not attend antenatal clinics as recommended by the WHO. The general objective of this study was to study the norm in terms of Antenatal Consultation (ANC) frequency as well as the determinants of the frequency among women who gave birth in the maternity hospitals of Agbangnizoun. Our research was a cross-sectional, descriptive, retrospective and quantitative type, which was carried out among 335 women in Agbangnizoun, Benin from 1 February to 3 April 2020 (3 months), in order to find out the general or specific characteristics of women that we associate with the frequency of prenatal consultation. At the end of the study, we found that 60%, 68% and 15% of the surveyed population had received ANC in the first, second and third trimesters, respectively. Ethnicity, income level of spouses over the three trimesters as well as the level of their knowledge and overall practice of ANC are determinants of the frequency of prenatal consultation.

Gestational trophoblastic disease (GTD) develops from abnormal cellular proliferation of trophoblasts following fertilization. This includes complete and partial hydatidiform mole (HM) and gestational trophoblastic neoplasia (GTN). The aim of this study was to report the epidemiological, clinical and therapeutic profile of gestational trophoblastic neoplasia (GTN) over period of ten years in the department of Oncology Radiotherapy at the University Hospital Joseph Ravoahangy Andrianavalona (HJRA) Antananarivo Madagascar. Medical records of women diagnosed with GTD in the department of Oncology Radiotherapy at HJRA from January 1st, 2007 to September 2017 were retrospectively reviewed. Only patients with the FIGO diagnosis GTN were included, while those with the histological diagnosis of hydatidiform mole (HM), also sometimes classified as GTD, were not included in this study. Also excluded were all cases with incomplete or missing data. Twenty four patients were included. Median age of patients at the time of diagnosis was 37 years (range 18 - 60). Most patients developed GTN following molar pregnancy (75%), had disease duration from antecedent pregnancy of less than 6 months (58.20%), and had the pre-treatment hCG level more than 10,000 IU/L (58.27%). Undifferentiated Uterine Sarcoma: A Rare Cause of Pelvic Pain in Young Woman

We report a case of undifferentiated uterine sarcoma occurring in a 20-year-old woman presenting as a pelvic pain with a mass discovered at the physical examination. The tumor ruptured into the abdominal cavity. A total hysterectomy was performed. On the pathological examination, the uterine corpus presents a proliferation of markedly atypical spindle cells admixed with round cells and pleomorphic cells. The tumor deeply infiltrated the myometrium. The tumor was very necrotizing. There were more than 50 mitoses per 10 fields at high magnification. The isthmus was invaded by the tumor. The diagnosis of undifferentiated uterine sarcoma was confirmed by immunohistochemistry. The tumor shows negativity for the following markers: CKAE1/AE3, h-caldesmon, α-SMA, progesterone receptors, desmin, CD 10, alpha foeto-protein, CD 3 and CD 20.

microRNAs是一类短的,高度保守的非编码RNA,可调节基因的表达。MiR-3031作为一个功能性miRNA,参与调节奶山羊乳腺发育和泌乳等多种重要的生物学过程。本试验旨在研究POU2F3和miR-3031在奶山羊乳腺上皮细胞中的靶向关系及其功能,为奶山羊乳腺发育和泌乳提供理论依据。通过RT-qPCR,Western blot和构建双荧光素酶报告载体验证miR-3031与POU2F3的靶向调节作用。生物信息学分析表明,POU2F3的3’UTR区包含miR-3031的结合位点。同时,miR-3031降低了PsiCHECKTM2-POU2F3 3’UTR-wt的双荧光素酶活性。RT-qPCR和Western blot结果显示,与对照组NC相比,miR-3031 mimics显著降低POU2F3的mRNA和蛋白水平的表达;结果表明POU2F3是miR-3031的靶基因。转染miR-3031 24h、48h后CCK8检测miR-3031对奶山羊乳腺上皮细胞活力的影响。结果显示,miR-3031 mimic显著提高奶山羊乳腺上皮细胞的活力。转染siPOU2F3 24h后EDU检测奶山羊乳腺上皮细胞增殖的情况,结果显示,干扰POU2F3显著促进奶山羊乳腺上皮细胞的增殖。将siPOU2F3和si-NC转染到乳腺上皮细胞中,进一步研究POU2F3对奶山羊乳腺上皮细胞凋亡的作用机理。流式结果表明,siPOU2F3显著抑制奶山羊乳腺上皮细胞凋亡。同时,siPOU2F3显著抑制Bcl-2和Bax蛋白的表达,上调Bcl-2/Bax的比率;同时,检测AKT和mTOR的磷酸化水平。结果表明,siPOU2F3显著上调了AKT、mTOR的磷酸化水平和PTEN的蛋白水平以及显著下调PI3K蛋白的表达。综上所述,miR-3031靶向POU2F3通过AKT/mTOR信号通路促进奶山羊乳腺上皮细胞增殖,抑制凋亡。

Key Issues for Water Quality Monitoring in the Zambezi River Basin in Mozambique in the Context of Mining Development

Zambezi River Basin is located in Southern Africa. It is essential for the economy of its eight riparian countries. The delta and almost 11% of its total catchment area are located in Mozambique. The main stream of the river receives a run-off contribution of 1800 m3/s in Mozambique and reaches the ocean with a flow of about 4100 m3/s. Coal mining is developing fast in the Zambezi River Basin in Mozambique due to favourable geological conditions. The coal mining and other activities have an impact on the surface and groundwater quality of the river basin. ARA-Zambeze is the river basin organization responsible for the water quality monitoring in Zambezi River Basin in Mozambique. In spite of limited resources ARA-Zambeze performs surveillance monitoring, while the institutions influencing the water quality of the river are doing operational monitoring. In this paper a monitoring system for the Zambezi River Basin in Mozambique is proposed. The system includes two alternative monitoring procedures. One procedure considers improvement of the current situation, which is characterized by multiple actors doing monitoring in isolation, by standardizing the analytical methods and improving data sharing through a web-based reporting system. The other proposed monitoring procedure considers a centralized approach, by having a consulting company doing the monitoring for the whole river basin in Mozambique. The second option has the advantages of improving consistency and comparability of the data, thereby allowing for more accurate trend analyses. It is concluded that the best way forward is to implement the first procedure and slowly move to the second.

Appendicular Abscess Revealing Gastrointestinal Stromal Tumor of the Appendix

Gastrointestinal stromal tumor (GIST) is a rare tumor of the digestive tract, but it is the most common of the mesenchymal tumors. The location is often gastric and small intestine. GIST of the appendix is extremely rare. We report one case in a 48-year-old man. He complained of severe pain in the right iliac region. The clinical presentation was typically like acute appendicitis. Abdominal ultrasound was performed and suspected an appendicular abscess. Surgical excision of the appendix was performed and histological diagnosis was “gastrointestinal stromal tumor of the appendix”.

Endometriosis of the Sigmoid Colon Mimicking Malignant Lesion

Endometriosis is defined as the presence of endometrial tissue outside the uterus. It is a benign pathology involving 15% of women of reproductive age. The sigmoid site represents 7% of endometriosis but it is the most frequent area in the digestive tract. We report two cases of colonic endometriosis in which the initial diagnosis suspected a malignant disease. Respectively, the patients were 31 and 48 years old. The first patient complained about abdominal pain with symptoms of intestinal obstruction and was classified after radiological examination as an intrasigmoid tumor with bladder adhesion. The second patient was undergone for a radiological examination because of a bowel obstruction syndrom and a mass in the sigmoid colon was suspected. In both cases, sigmoid colon resection was performed and histological diagnosis was sigmoid endometriosis. Sigmoid endometriosis is rarely diagnosed before surgical excision. Suspicious bowel obstruction and malignant origin lead to large surgical excision. Histological examination is important to establish the diagnosis.

Primary Breast Tuberculosis Mimicking Carcinomatous Mastitis: A Case Report

The breast is a rare localization of extra-pulmonary tuberculosis, even in highly endemic countries. It is most often primitive. The clinical and radiological features are sometimes misleading. It poses diagnostic problems especially with cancer. We report a case of breast tuberculosis having clinical and radiological presentations as a breast carcinoma. Through this observation and a review of the literature, we provide an update on the epidemiological characteristics and the means of diagnosis of this lesion. It was a 52-year-old woman with inflammatory lump of the left breast, adherent to the skin. Ultrasonography and mammography revealed a homogeneous, poorly demarcated nodule of approximately 4 cm of diameter, classified ACR4. Surgical excision was performed. The histological diagnosis was breast tuberculosis. The diagnosis of tuberculous mastitis is made on bacteriological and pathological examinations. Histological examination helps for diagnostic confirmation while excluding a malignant lesion of the breast.
